ABSTRACT

O número de pessoas utilizando substâncias ilícitas de forma recreativa aumenta a cada ano, chamando a atenção de estudiosos de diversas áreas do conhecimento. Com isso, a demanda de exames toxicológicos exigida para trabalhadores, vítimas de crimes e esportistas também tem crescido. A amostra biológica mais utilizada para análises toxicológicas continua sendo a urina, visto que sua obtenção é menos invasiva, possibilita coletar grande volume de amostra e pode-se detectar substâncias até dias após ter ocorrido a exposição ou consumo. Entretanto, estas amostras necessitam de um grande volume físico para serem armazenadas e transportadas aos laboratórios, devendo ser mantidas em temperatura baixa e controlada para conservação. Outro ponto a se considerar é a quantidade de amostra insuficientemente coletada, ou extravasamento do conteúdo, contaminando outras amostras e muitas vezes, inviabilizando a análise. Uma alternativa recente para tais problemas é utilizar a técnica chamada de dried urine spots (DUS), onde poucos microlitros de urina são colocados em um papel absorvente e secos sob temperatura ambiente, preservando de agentes degradantes os componentes presentes na urina. Assim, o objetivo deste trabalho é avaliar a estabilidade das substâncias do presente estudo em alta temperatura, temperatura ambiente e em temperaturas de 4°C e -20°C. Para este fim, foi necessário desenvolver, validar e aplicar métodos de extração e determinação de anfetaminas e produtos de biotransformação de cocaína e tetraidrocanabinol carboxílico (THCCOOH) em amostras dried urine spot, utilizando cromatografia líquida acoplada à espectrometria de massas. Os picos foram identificados por UPLC-ESI-MS/MS, com tempo total de 5 mins utilizando fase A- água, formiato de amônio e 0,1% ácido fórmico, e B- metanol: acetonitrila (6:4) + 0,1% de ácido fórmico. A extração foi feita utilizando acetonitrila: metanol: acetona (1:1:1) +ácido fórmico 0,1%. Não foi possível iniciar a validação de THCCOOH, visto uma possível complexação do analito com o papel. Para as outras substâncias, o método cromatográfico desenvolvido se mostrou eficiente e seletivo, com LOD e LOQ de 10 ng/mL para todos os analitos, sendo linear até 1000 ng/mL, atendeu as especificações de precisão e exatidão e carryover. As amostras permaneceram estáveis ao longo de 32 dias nas temperaturas estudadas, demonstrando a segurança em se utilizar a técnica de DUS para armazenamento e transporte de amostras biológicas dentro da faixa de temperatura do estudo até 32 dias


The number of people using illegal substances in a recreational way increases each year, drawing the attention of scholars from different areas of knowledge. As a result, the demand for workplaces drug tests, toxicological tests for victims of crimes and dopping has also grown. The biological sample most used for toxicological tests remains urine, since obtaining it is less invasive, it is possible to collect a large volume of sample and it is possible to detect substances up to days after exposure or consumption has occurred. However, these samples require a large physical volume to be stored and transported to the laboratories, and must be kept at a low temperature for conservation. Another point to consider is the amount of sample insufficiently collected, or leakage of the content, causing contamination of other samples and often making the analysis unfeasible. A recent alternative to such problems is to use "dried urine spots" (DUS), where few microliters of urine are placed on absorbent paper and dried at room temperature, preserving the components present in the urine from degrading agents. Thus, the objective of this work is to evaluate the stability of the substances in this study at high temperature, room temperature and at temperatures of 4°C and -20°C. For this purpose, it was necessary to develop, validate and apply methods of extraction and determination of amphetamines and biotransformation products of cocaine and carboxylic tetrahydrocannabinol (THCCOOH) in dried urine spot samples, using liquid chromatography coupled to mass spectrometry (LC-MS). The peaks were identified liquid chromatography coupled to a mass spectrometer (UPLC-ESI-MS/MS), with a total time of 5 mins using phase A- water, ammonium formate and 0.1% formic acid, and B- methanol: acetonitrile (6:4) + 0.1% formic acid. Extraction was done using acetonitrile: methanol: acetone (1:1:1) + 0.1% formic acid. It was not possible to perform the validation of THCCOOH, given a possible complexation of the analyte with the paper. To the others substances, the chromatographic method developed proved to be efficient and selective, with LOD and LOQ of 10 ng/mL for all analytes, being linear up to 1000 ng/mL, meeting the specifications of precision and accuracy and carryover. The samples remained stable for 32 days at the temperatures studied, demonstrating the safety of using the DUS technique for storage and transport of biological samples until 32 days on temperature range studied

ABSTRACT

Amphetamines are chemical synthetic drugs that are becoming increasingly popular in China. As a common sample in the inspection of poisons, hair has the advantages of easy storage, good stability, and long detection time compared with traditional human body fluid samples (blood, urine), thus possesses an unique application value in the field of forensic toxicology analysis. By now, methods for detecting amphetamines in human hair have been widely used, and validity of the results has been recognized and adopted by the court. This paper reviews domestic and foreign research progress of the detection of amphetamines in hair samples, including the pretreatment and analytic methods.

ABSTRACT

Amphetamines abuse is defined as a chronic recurrent encephalopathy, and it is a global public health problem which seriously threatens the health of human and the social stability. Long-term abuse and addiction of amphetamines leads to structural and functional changes of specific encephalic regions. Further researches on these encephalic regions, the network of brain and biological information may be helpful to understanding drug abuse mechanism and possible therapeutic measures. Recently, a series of functional imaging techniques, including magnetic resonance imaging (MRI), functional magnetic resonance imaging (f MRI), magnetic resonance spectroscopy (MRS), diffusion tensor imaging (DTI), and positron emission tomography (PET), were used to detect different brain structural changes of the volume and density of encephalic regions, functional changes of cerebral blood flow and brain cognition. The results showed functional imaging techniques play significant roles to detect different structural and functional changes of the brain. Based on these results, the researchers aim to clarify the mechanisms of drug abuse. That is the main focus of this review.

ABSTRACT

Abstract: The consumption of amphetamines and its derivatives, either recreationally or for working reasons, is widespread, currently being the second most consumed drug worldwide. An often overlooked aspect of this drug has been its toxicity over the myocardium. In the following text we describe the case of a young male patient, without any relevant previous conditions, except alcohol ingestion in moderate quantity along with 3 years of using amphetamine-type stimulants and cocaine, whose main clinical manifestation was sudden onset dyspnea with minimal effort, which progressed to resting dyspnea after the ingestion of several tablets of clobenzorex, which was initially hospitalized with a diagnostic of pneumonia and finally diagnosed with a dilatated cardiomyopathy. Substance abuse should be a warning flag on patients presenting with dyspnea in order to think of a possible drug related cardiomyopathy, avoiding that way undue delays on appropriate diagnosis and treatment.


Resumen: El consumo de anfetaminas y sus derivados, ya sea recreativo o por razones de trabajo, es generalizado, siendo actualmente la segunda droga más consumida en el mundo. Un aspecto a menudo pasado por alto sobre esta droga, ha sido su toxicidad sobre el miocardio. En el siguiente texto, se describe el caso de un paciente varón joven, sin condiciones previas relevantes, excepto la ingesta de alcohol en cantidad moderada junto con tres años de uso de estimulantes de tipo anfetamínico y cocaína, cuya manifestación clínica principal fue disnea de inicio repentino con esfuerzo mínimo, que evolucionó a disnea de reposo tras la ingestión de varias tabletas de clobenzorex, que inicialmente fue hospitalizado con un diagnóstico de neumonía y finalmente diagnosticado con miocardiopatía dilatada. El abuso de sustancias debe ser un indicador de advertencia en los pacientes que presentan disnea para pensar en una posible cardiomiopatía relacionada con drogas, evitando así retrasos indebidos en el diagnóstico y tratamiento adecuados.

ABSTRACT

Objetivos: Abordar o perfil dos caminhoneiros que trafegam na BR153, o uso de drogas e as principais substâncias psicoativas consumidas por caminhoneiros e relacionar o consumo dessas substâncias aos aspectos de qualidade de vida e sócio demográficos dos motoristas de caminhão. Métodos: Foram abordados 161 motoristas de caminhão nas proximidades da cidade de Anápolis-Goiás na BR-153. Para avaliar os dados sócio demográficos assim como o uso de drogas entre os caminhoneiros, foi utilizado questionário semi - estruturado, Teste de Triagem do Envolvimento com Álcool, Cigarro e Outras Drogas (ASSIST) modificado. A análise dos dados foi conduzida pelo programa estatístico SPSS-22. Resultados: A maioria dos caminhoneiros abordados era do sexo masculino (99,4%), autodeclarados brancos (60,9%), do estado de Goiás (43,8%) e percorreram em média 639 km/dia. Sobre o uso de anfetaminas (rebite), 32,9% admitiram terem feito uso dessa droga nos últimos três meses. Sobre a aquisição dessa substância, 38,3% relataram não encontrar dificuldades para adquirir a anfetamina e os postos de gasolina foi o local mais citado de acordo com os participantes. Conclusão: O uso de drogas por caminhoneiros se mostrou bem presente na rodovia BR-153, e isso reflete a falta de fiscalização e conscientização destes profissionais. Além disso, os postos de gasolina, que usualmente são os locais de descanso para essa categoria, se configuram como um dos principais centros para agravar as condições de saúde dos mesmos. (AU)


Objectives: To approach truck drivers profile in a highways near Anápolis- Goiás, drug use and the main psychoactive substances used by truckers and relate the consumption of these substances to the social and demographic aspects of truck drivers. Methods: 161 truck drivers were assessed. To approach the social and demographic data as well as drug consumption among truck drivers a semi - structured questionnaire, Involvement Screening Test with Alcohol, Cigarette and Other Drugs (ASSIST) modified was used. Data analysis was conducted using SPSS22. Results: Most covered truck drivers were male (99.4%), self-declared white (60.9%), from the state of Goiás (43.8%) and traveled an average of 639 km (400 miles) / day. On the use of amphetamines, 32.9% admitted having used this drug in the last three months. On the acquisition of that substance, 38.3% reported not find it difficult to acquire amphetamine. The gas stations were the site over quoted according to participants. Conclusion: The use of drugs by truck drivers proved very present in Goiás highways, and this reflects the lack of oversight and awareness of these professionals. In addition, the gas stations, which usually are the rest local for this category are characterized as one of the main centers for serious health conditions. (AU)

ABSTRACT

Objective To develop the method for simultaneous identification and quantification of morphine,6-monoactylmorphine,codeine,heroin,ketamine,3,4-Methylenedioxyamphetamine (MDA),3,4-methylenedioxymethamphetamine (MDMA),amphetamine and methylamphetamine in human plasma.Methods UPLC-MS/MS was adopted to analyze plasma with protein precipitated using 10% trichloroacetic acid.Plasma samples were separated on ACQUITY UPLC HSS C18 column with aqueous solution (0.01% formic acid)-methanol (0.01% formic acid) as the mobile phase,and at a flow rate of 0.3 mL·min-1.The multiple reaction monitoring (MRM) mode was performed combined with the positive ion mode for quantification in four sections.Results The retention time,the characteristic fragment ions,and the relative abundance ratio of the molecular ion peak could be used to identify these nine compounds sensitively.The liner calibration curve of morphine,codeine,heroin,ketamine,6-monoacetylmorphine,MDA,MDMA,amphetamine and methamphetamine were obtained in the concentration range of 5.00×10-3~5.00 (r=0.9934),1.00× 10-2~ 10.00 (r=0.9905),1.00× 10-2~ 10.00 (r=0.9929),2.50× 10-3 ~2.50 (r=0.9960),5.00× 10-3 ~ 5.00 (r=0.9925),5.00× 10-4 ~ 5.00 (r=0.9910),5.00× 10-4 ~ 5.00 (r=0.9924),5.00× 10-4 ~ 5.00 (r=0.9920) and 5.00×10-4~5.00 μg·mL-1 (r=0.9900) respectively.The lowest detection limit was 1.00,1.00,1.00,0.50,0.50,0.10,0.10,0.10 and 0.10 ng· mL-1 respectively.The relative standard deviation (RSD) of the inter-day and intra-day were less than 18%.The relative recovery was more than 60%,and the RSD were less than 15%.Conclusion The method is accurate,sensitive and suitable for identification and quantification of 9 drugs of opiates,ketamine and amphetamines in human plasma.

ABSTRACT

OBJECTIVE To test whether the occupational conditions of professional truck drivers are associated with amphetamine use after demographic characteristics and ones regarding mental health and drug use are controlled for.METHODS Cross-sectional study, with a non-probabilistic sample of 684 male truck drivers, which was collected in three highways in Sao Paulo between years 2012 and 2013. Demographic and occupational information was collected, as well as data on drug use and mental health (sleep quality, emotional stress, and psychiatric disorders). A logistic regression model was developed to identify factors associated with amphetamine use. Odds ratio (OR; 95%CI) was defined as the measure for association. The significance level was established as p < 0.05.RESULTS The studied sample was found to have an average age of 36.7 (SD = 7.8) years, as well as low education (8.6 [SD = 2.3] years); 29.0% of drivers reported having used amphetamines within the twelve months prior to their interviews. After demographic and occupational variables had been controlled for, the factors which indicated amphetamine use among truck drivers were the following: being younger than 38 years (OR = 3.69), having spent less than nine years at school (OR = 1.76), being autonomous (OR = 1.65), working night shifts or irregular schedules (OR = 2.05), working over 12 hours daily (OR = 2.14), and drinking alcohol (OR = 1.74).CONCLUSIONS Occupational aspects are closely related to amphetamine use among truck drivers, which reinforces the importance of closely following the application of law (Resting Act ("Lei do Descanso"); Law 12,619/2012) which regulates the workload and hours of those professionals. Our results show the need for increased strictness on the trade and prescription of amphetamines in Brazil.


OBJETIVO Testar se as condições ocupacionais de motoristas profissionais de caminhão estariam associadas ao uso de anfetaminas após o controle do efeito de características demográficas, de saúde mental e uso de drogas.MÉTODOS Estudo transversal, com amostra não-probabilística de 684 motoristas de caminhão, do sexo masculino, recrutada em três rodovias do estado de São Paulo, durante os anos de 2012 e 2013. Foram coletadas informações demográficas, ocupacionais, sobre uso de drogas e sobre saúde mental (qualidade de sono, estresse emocional e transtornos psiquiátricos). Foi desenvolvido um modelo de regressão logística para identificar os fatores associados ao uso de anfetaminas. A razão de chances (OR; IC95%) foi definida como a medida de associação e o nível de significância considerado foi p < 0,05.RESULTADOS A amostra estudada apresentou idade média de 36,7 (DP = 7,8) anos e baixa escolaridade (8,6 [DP = 2,3] anos). O uso de anfetaminas nos doze meses prévios à entrevista foi autorrelatado por 29,0% dos motoristas. Após o controle do efeito de variáveis demográficas e ocupacionais, os fatores preditores do uso de anfetaminas entre os motoristas de caminhão foram: ter menos de 38 anos (OR = 3,69), menos de nove anos de escolaridade (OR = 1,76), ser autônomo (OR = 1,65), trabalhar em turno noturno ou turno irregular (OR = 2,05), trabalhar mais de 12 horas diárias (OR = 2,14) e usar álcool (OR = 1,74).CONCLUSÕES Os aspectos ocupacionais estão intimamente associados ao uso de anfetaminas entre motoristas de caminhão, reforçando a importância da fiscalização da lei (Lei do Descanso; Lei 12.619/2012) que regulamenta a carga horária de trabalho desses profissionais. Os resultados mostram a necessidade de haver maior rigor sobre a comercialização e prescrição das anfetaminas no Brasil.

ABSTRACT

The use of amphetamines in Brazil is common among truck drivers, which may be an important factor in the occurrence of traffic accidents. This article seeks to estimate the prevalence of amphetamine use among truck drivers. Drivers (N = 134) were stopped on two different highways in Sao Paulo state and they were asked to answer a questionnaire and provide a urine sample for toxicological analysis. All data were analyzed on Stata 8.0. All participants were males with low levels of schooling, whose mean age was 40.8 years. The presence of amphetamines was detected in 10.8% of all urine samples collected, being commonly justified in order to make truck drivers able to maintain their state of awareness. Amphetamine use was detected among truck drivers on Sao Paulo highways. The problem is that when the stimulant effects wear off, sleepiness due to sleep deprivation reduces concentration and good driver performance, making drivers vulnerable to traffic accidents and the related effects.


No Brasil, é comum o uso de anfetaminas por motoristas de caminhão, o que pode culminar na ocorrência de acidentes de trânsito. O objetivo deste artigo é estimar a prevalência do uso de anfetaminas entre caminhoneiros. Motoristas (N = 134) foram abordados em duas rodovias do Estado de São Paulo e solicitados a responder um questionário, assim como a fornecer uma amostra de urina para realização de análises toxicológicas. Todos os dados foram analisados em Stata 8.0. Todos os participantes eram do sexo masculino, de idade média de 40,8 anos e de baixa escolaridade. A presença de anfetaminas foi detectada em 10,8% das amostras de urina, cujo uso foi justificado para manter a vigília durante o trabalho. O uso de anfetaminas foi detectado entre caminhoneiros em rodovias de São Paulo. Cessado o efeito estimulante, a sonolência advinda de uma possível privação de sono diminui a atenção e o bom desempenho na direção, predispondo o condutor aos acidentes de trânsito e seus custos relacionados.

El consumo y dependencia de sustancias, es un serio problema social con alta morbilidad materno-fetal. El aumento de la oferta y el contexto social favorecedor, permitió que la difusión del consumo sustancias tóxicas ilícitas, conlleve una incidencia creciente en gestantes consumidoras de sustancias y aumento de recién nacidos afectados por las prácticas tóxicas de sus madres. La evaluación del riesgo cuando la gestación es expuesta a las drogas es difícil, los resultados pueden estar sesgados por el consumo concomitante de otros tóxicos o por factores sicológicos y socio-sanitarios desfavorables. Aunque tampoco se definió un patrón específico de anomalías congénitas, se considera que el abuso de drogas, en general, comporta mayor riego de desenlace anómalo del embarazo, por un incremento del riesgo de malformaciones congénitas, debido a la probable teratogenicidad de algunas sustancias o de la morbilidad perinatal afectando el crecimiento fetal o el normal desarrollo del embarazo. También existen posibles repercusiones a largo plazo en la capacidad de aprendizaje y comportamiento de los niños expuestos intraútero, aunque no demostró efectivamente. Por lo tanto, todo embarazo en el que se detecto un hábito tóxico se debe considerar de mayor riesgo, tomando las medidas oportunas para lograr que las pacientes se alejen de éstas prácticas, apoyadas por un equipo multidisciplinario, idealmente antes del inicio del embarazo, lo que implica la adopción de medidas profilácticas de información y concienciación de las mujeres en edad fértil y de apoyo durante el embarazo y la lactancia para el abandono de la dependencia


Illicit drug use and dependence is a serious social problem with high maternal and fetal morbidity. Supply increase and propitious social context allowed that the diffusion of the use of illicit toxic substances entails a growing incidence in pregnant women who use illicit drugs and an increase of newborns affected by the practices of their mothers. Risk evaluation is difficult when gestation is exposed to illicit drugs because the results could be biased by the concomitant consumption of others toxic substances or by psychological and socio-sanitary unfavorable factors. Though a specific pattern of congenital anomalies has not been defined, in general it is considered that drug abuse has an increased risk of anomalous outcomes in pregnancies. This risk is caused by an increase in the risk of congenital malformations due to the probable teratogenicity of some substances or the perinatal morbility affecting the fetal growth or normal pregnancy development. Although it has not been demonstrated effectively, there are also possible long-term repercussions in the learning capacity and behavior of the children exposed intra uterus. In conclusion, all pregnancies exposed to illicit drugs must be considered high risk pregnancies and measures should be taken so that the patients avoid these practices supported by a multidisciplinary team. Ideally, this team should start working before pregnancy implying the adoption of preventive measures such as information and public awareness of women in fertile age and support during pregnancy and maternal lactation
