Chinese Acupuncture & Moxibustion ; (12): 709-712, 2019.

ABSTRACT

OBJECTIVE@#To observe the efficacy differences between acupuncture combined with grain-moxibustion and acupuncture on acute urinary retention after epidural anaesthesia for anorectal diseases.@*METHODS@#A total of 60 patients were randomized into an acupuncture combined with grain-moxibustion group and an acupuncture group, 31 cases in each one. In the acupuncture group, acupuncture was applied at Zhongji (CV 3), Guanyuan (CV 4), Qihai (CV 6), Shuidao (ST 28), Pangguangshu (BL 28), Sanyinjiao (SP 6) and Yinlingquan (SP 9). In the acupuncture combined with grain-moxibustion group, grain-moxibustion was given at Zhongji (CV 3), Guanyuan (CV 4), Qihai (CV 6) and Shuidao (ST 28) on the basis of acupuncture. Those who failed to urinate 60 min after the first treatment received the second treatment. 30 min after the second treatment, the lower abdominal symptom scores before and after treatment as well as the bladder residual urine volume after the first urination after treatment were compared between the two groups. In addition, the clinical efficacy and security were evaluated.@*RESULTS@#Compared before treatment, the symptom scores after treatment were reduced in the two groups (<0.05), and the score in the acupuncture combined with grain-moxibustion group was lower than that in the acupuncture group after treatment (<0.05). The bladder residual urine volume in the acupuncture combined with grain-moxibustion group was (26.71±17.01) mL, which was lower than (35.32±20.76) mL in the acupuncture group (<0.05). The total effective rate was 93.5% (29/31) in the acupuncture combined with grain-moxibustion group, which was superior to 71.0% (22/31) in the acupuncture group (<0.05).@*CONCLUSION@#The efficacy of acupuncture combined with grain-moxibustion is superior to simple acupuncture on acute urinary retention after epidural anaesthesia for anorectal diseases, which is safe and reliable.

ABSTRACT

OBJECTIVE@#To explore and quantify the intervention effect of auricular point sticking on perioperative psychological stress in patients with anorectal diseases.@*METHODS@#Eighty patients who underwent anorectal surgery were randomly divided into an observation group (40 cases) and a control group (40 cases). The routine preoperative guidance, preoperative visits, and informed of the postoperative condition were received in the control group. On the basis of the treatment in the control group, auricular point sticking was immediately applied at Shenmen (TF), Shen (CO), Wei (CO), Gan (CO), Pi (CO), Pizhixia (AT), E (AT), Nie (AT) and Zhen (AT) in the observation group.The patients were pressed by themselves, 3 to 5 min per point each time, 5 times a day, and the contralateral auricular points were replaced every 2 or 3 days until 1 week after surgery. The Hamilton anxiety scale (HAMA), Hamilton depression scale (HAMD), and Pittsburgh sleep quality index (PSQI) scores were compared between the two groups before and 7 days after surgery.@*RESULTS@#There was no significant difference in the total HAMA scores between after and before surgery in the observation group (>0.05). The total HAMA score in the control group was higher than that before surgery (0.05). The scores of somatic anxiety factor in the two groups were higher than those before surgery (0.05). The total score of PSQI in the two groups was lower than that before surgery (<0.05), and the total score of PSQI in the observation group was lower than that in the control group (<0.05).@*CONCLUSION@#Auricular point sticking can effectively improve some psychological stress problems during perioperative period in patients with anorectal diseases.

ABSTRACT

Background: It is difficult to evaluate the long-term outcome of colorectal cancer screening project in a short period because of the long course of disease. Aims: To explore the epidemiological characteristics and mortality trends of non-neoplastic anorectal diseases among registered population in Pudong New Area, Shanghai from 1995 to 2017, so as to reflect on other aspects or indirectly the efficacy of colorectal cancer screening project. Methods: According to the Mortality Registration System of household registered residents in Pudong New Area, the crude mortality rate, standardized mortality rate, annual percentage change (APC) and other indicators were used to analyze the mortality trends of non-neoplastic anorectal diseases (K50-K52, K55-K63); the contribution of demographic and non-demographic factors to disease death before and after the implementation of colorectal cancer screening project was calculated by the differential decomposition method. Results: From 1995 to 2017, the total crude mortality rate of non-neoplastic anorectal diseases in Pudong New Area was 2.79/100 000, and the standardized mortality rate was 1.23/100 000; the crude mortality rate showed an upward trend (APC=1.67, P=0.009), whereas the standardized mortality rate showed a downward trend (APC=-3.45, P<0.001). For population aged ≥65 years, the crude mortality rate showed an upward trend from 1995 to 2009, and a downward trend from 2009 to 2017, respectively (APC=2.19, P=0.044; APC=-6.85, P=0.007). Death predominantly occurred in population aged ≥80 years. Before and after the screening project (1995-2012 vs. 2013-2017), the added value of demographic and non-demographic factors on total mortality was 1.24/100 000 and -1.33/100 000, respectively. With 1995 as the baseline, the contribution of non-demographic factors to the added value of mortality after screening was -2.25/100 000, with a contribution rate of 48.94%, which was superior to that before screening (-0.92/100 000, with a contribution rate of 45.37%). Conclusions: Aging of the population is the main cause of increased crude mortality rate of non-neoplastic anorectal diseases in Pudong New Area. Screening for colorectal cancer is an effective intervention to reduce the mortality of anorectal diseases and deserves further promotion.

ABSTRACT

Objective To investigate the clinical effect of compound Chinese medicine fumigation granules on surgical treatment of anorectal disease patients and the impact of postoperative complications and wound healing.Methods 78 cases of patients anorectal surgery from June 2013 to February 2016 in our hospital anorectal were admitted,and were randomly divided into observation group and control group,39 cases in each group,using anti-inflammatory and conventional dressing treatment after operation in both two groups,the observation group was treated with compound Chinese medicine fumigation granules,treatment for 10 days continuous.The changes of wound healing and related clinical symptoms were observed and compared between the two groups.Results Third,7,14 days after operation,VAS scores in observation group were lower than the control group,the differences were statistically significant(P<0.05).Tirst days after operation,there was no significant difference in perianal edema,anal bulge,anal dampness and bleeding symptom score between the observation group and the control group; third,7 days after surgery,the perianal edema,anal bulge,anal wet,bleeding symptom score in observation group were lower than the control group,the differences were statistically significant(P<0.05); fifth,14 days after operation,wound patients smoked wash reduction rate in observation group was higher than the control group,the differences were statistically significant(P<0.05); observation group average wound healing time was(16.3±2.8)days,the control group was(18.4±3.3)days,the differences were statistically significant(P<0.05).Conclusion After surgical treatment of anorectal disease,use the compound Chinese medicine fumigation particles for treatment can help relieve the clinical symptoms of patients,reduce pain,promote wound healing.

ABSTRACT

Objective To understand the prevalence and distribution characteristics of symptoms of anorectal diseases in Chinese residents(older than 18) as to provide reference for prevention,care and treatment of anorectal diseases.Methods Stratified sampling was used according to the economic level in every province in the country,and each province selected two cities.Face-toface interviews were used to collect data in 68 906 interviewers and SPSS 17.0 was performed to analyze the data.Results Of 68 906 respondents,25 634 (37.20 %) reported to suffer from at least one symptoms of anorectal diseases.The prevalence of the top 5 main symptoms of anorectal diseases were prolapse of anus neoplasms(14.55%),anal sore(16.53%),anal pendant expansion (14.98 %),perianal abscess (5.88 %),hematochezia (22.52 %).Each symptom frequency difference was statistically significant (x2=7 727.6,P<0.001).Populations which proned to suffer from symptoms of anorectal disease had the following characteristics:living in city,male,ageing 25-54 years old,having specialist qualifications,lacking of physical.Conclusion The prevalence of symptoms of anorectal diseases among Chinese residents was high,and each symptom had its distribution characteristics.More targeted interventions should be taken in anorectal disease prone people.

ABSTRACT

Objective To learn the prevalence and risk factors of anorectal diseases among urban population aged 18 -80 years in Lanxi.Methods Typical sampling was adopted to select one urban area from Lanxi City.Face -to -face interviews and physical examinations were carried out to collect data during June 2015 to September 2015.Results A total of 3 327 residents were recruited,of which 2 931 were involved in both questionnaires and physical examinations.Totally, 1 554 participants were detected one or more anorectal diseases.The prevalence rate was 53.02% (standardized rate 45.61%).Hemorrhoids cases was accounted to the most,and the prevalence rate was 49.47% (standardized rate 42.41%).Male was associated lower risk of hemorrhoid (OR =0.37,95%CI:0.32 -0.44).Compared to 18 -30 age group,older age were associated with increasing risks of hemorrhoid significantly despite of 31 - age group.Results for different groups were 41 -(OR =1.66,95%CI:1.05 -2.62),51 -(OR =1.94,95%CI:1.23 -3.07),61 -(OR =1.92,95%CI:1.19 -3.09)and 71 -80 (OR =2.62,95%CI:1.57 -4.39).Only junior high school (OR =1.30,95%CI:1.01 -1.66)and senior high school (OR =1.57,95%CI:1.19 -2.07)were significantly associated with high risk of hemorrhoid.Conclusion The prevalence of anorectal diseases in Lanxi urban area is high.Gender,age and education level might be independent risk factors of hemorrhoid.Therefore,preventive strategies should vary across different population with different characteristics.

Introducción: La aparición de la ecografía endoanal 360º significó un cambio importante en la Coloproctología de los últimos 20 años.1 Ha ganado popularidad por ser mínimamente invasivo, indoloro y costo/efectivo si se compara con otros estudios por imágenes. Además, es habitualmente realizada por un Coloproctólogo y puede formar parte del examen en la consulta. Objetivo: Comunicar la experiencia de nuestro grupo en el diagnóstico de diferentes patologías anorrectales mediante la utilización del método. Población y métodos: Entre noviembre de 2010 y abril de 2014 realizamos 978 ecografías endoanales de 360º. Las variables analizadas fueron: estudios realizados según diagnósticos de sospecha; variación en la cantidad de estudios solicitados y realizados durante el periodo establecido; especialidad del médico que solicita el estudio. Resultados: En cuanto al diagnóstico sospechado por el médico derivante, de los 978 estudios, 422 (43,1%) correspondieron a fístulas perianales, 311 (31,79%) ecografías fueron realizadas por diagnóstico de incontinencia anal, 93 (9,5%) como evaluación esfinteriana preoperatoria, 48 (4,9%) por proctalgia crónica, 7 (0,71%) fístulas ano-recto-vaginales, y 20 (2,04%) por otras sospechas diagnósticas (disinergias del piso pelviano, control postoperatorio, etc.). En cuanto a la variación de la cantidad de estudios en el tiempo, se realizaron 8 procedimientos en 2010, 146 durante 2011, 254 en 2012, 402 ecografías en 2013 y 168 durante los primeros meses de 2014. En cuanto a la especialidad acreditada por los médicos que prescribieron el estudio, el 84,1% (823) correspondía a cirujanos colorrectales, el 6,64% (65) a cirujanos generales, el 3,6% (36) a gastroenterólogos, el 2,4% (24) a ginecólogos y el 2,04% (20) a otros. Fueron excluidas del presente trabajo las ecografías realizadas para estadificar o reestadificar los tumores del recto inferior y el ano (22 estudios)...


Background: The appeareance of the 360º endoanal ultrasonography as a diagnostic method an improvement for coloproctology in the last 20 years.(1) It has been gaining popularity in virtue of being minimally invasive, painless and cost/effective, especially when compared to other methods. Furthermore, it is mainly realized by coloproctologists, making it almost part of the physical exam at the consult. Objective: To communicate our experience at the diagnostic of different anorectal pathologies. Population and methods: Between November 2010 and April 2014, we performed 978 studies with the 360º endoanal ultrasound. All of the studies were done with the same equipment and by the same operator. ABSTRACT

Objective:To observe the clinical effects of Zuoyu üo. 1(ZY1)in the prevention of concomitant concurrent complica-tions after operation for anorectal diseases. Methods:164 patients who had received operation for anorectal diseases were randomly di-vided into 2 groups. Patients in the treatment group were treated with sitz baths in ZY1 and the conventional wound dressing method. Patients in the control group were treated with sitz baths in warm water and the conventional wound dressing method. Results:The effi-ciency rate of the treatment group was significantly higher than that of the control group(53. 57% vs. 27. 5%,P<0. 05),as well as the total effectiveness(95. 23% vs. 88. 75%,P<0. 05). The comparison showed that better curative effects existed in the treatment group on distress,bleeding,and edema ratings and wound healing time. Conclusion:Sitz bath in ZY1 after operation for anorectal dis-eases reduces complications and shortens the wound healing time without any side effect. It is thus extensively promising in clinical ap-plications.

ABSTRACT

OBJECTIVE To discuss the necessity of physical examination and laboratory diagnosis before operation of patients with anorectal diseases,and analyze the occupational harm of the staff.METHODS The results of physical examination and laboratory diagnosis before operation in 320 cases were reviewed,and summerized the various kinds of infectious diseases.RESULTS Forty-eight(15.00%) cases accompanied with variety classes of infectious diseases,and 39(12.19%)cases of them accompanied with transfusion-transmitted disease.CONCLUSIONS It is important to think highly of the current situation of anorectal diseases accompanied with infectious diseases,evaluate reasonably the occupational diagnosis to avoid occupational harm.
