ABSTRACT

Background: The triangle of Brocq and Mouchet is an arteriovenous triangle between the anterior interventricularartery, circumflex artery and the great cardiac vein. The vascular triangle has been classified into 4 types viz.closed, superiorly opened, inferiorly opened and completely opened or absent. Polyurethane foam is a newmaterial used in corrosion casting. It is economical, easy to carry out and does not require special apparatus.Context and purpose of the study:a) To study the relationship of the Great cardiac vein (GCV) in the triangle of Brocq and Mouchet in human heartsusing PU foam for corrosion cast.b) To study the relationship of GCV in relation to the arteries forming the triangle.Results: The triangle was present in 98% of the hearts. Relationship of GCV in triangle of Brocq and Mouchet:Inferiorly open type of Triangle was the most common (87.5%). Next common type was, closed type (6.7%). Inrelation to GCV relation with the arteries in the triangle, it was to the left of AIA in 87.51% and crosses circumflexartery, either superficially or deep.The GCV crossing anterior interventricular artery was found in few specimens. It crossed the artery superficiallyor deeply. The intertwining type of variation was observed in few casts, and could not be placed under anyclassification as suggested by previous authors.Conclusion: The topography of the Great cardiac vein and its variations are significant in relation to the arteriesin the region. It is relevant in interventional cardiology, radiology, and catheter-based procedures.

ABSTRACT

The septal perforating arteries of the heart usually branch off from the anterior and inferior interventricular arteries and supply the interventricular septum and the conduction system therein. Since the septal perforating arteries are not directly visible from the outside of the heart, their anatomy and variations might be overlooked. However, the septal perforating arteries have their unique anatomy that needs to be recognized to avoid the damage of the vessels especially during common cardiac procedures such as the coronary artery bypass graft, percutaneous coronary intervention, and aortic valve replacement. A better understanding of these important arteries will help physicians to enhance the overall cardiac care for their patients. Therefore, this article discusses the anatomy, the relationship to the conduction system of the heart and the clinical significance of the septal perforating arteries.

ABSTRACT

We present a unique, unreported variation of the left coronary artery. During dissection classes for first-year medical students, we observed the absence of left coronary artery in an adult male cadaver aged approximately 78 years. The left aortic sinus was dilated and it gave origin to anterior interventricular and left marginal arteries independently. Left marginal artery was large and the circumflex artery arose from it. There were two independent opening for anterior interventricular and left marginal arteries in the left posterior aortic sinus. No variations were found in the origin and branching pattern of right coronary artery and the walls and chambers of the heart.

ABSTRACT

Dual left anterior interventricular coronary artery (also called left anterior descending artery, hereafter referred as LAD) is a rare congenital coronary artery anomaly. Notably, type IV dual LAD has never been reported in Koreans. During a routine dissection, a new variant of type IV dual LAD was found in a 57-year-old Korean male cadaver, whose cause of death was unknown. One LAD originated from the right coronary artery, coursed through the anterior interventricular sulcus, but did not reach the apex cordis. The other LAD arose from the left coronary artery, never entered the anterior interventricular sulcus through its course, but reached the apex cordis, where it met the posterior interventricular branch of the right coronary artery. This is the first report on a new variant of type IV dual LAD in Koreans, which is of clinical importance during procedures containing the coronary artery.
