ABSTRACT

Objective To investigate whether prophylactic antifungal treatment will reduce the incidence of fungal infection(FI)and the mortality in severe acute pancreatitis(SAP).Methods Forty consecutive patients with SAP admitted from January 2008 to December 2009 were divided into prevention group and control group with 20 cases each by random digits table.The patients in prevention group underwent routine therapy plus intravenous fluconazol as prophylactic antifungal treatment,while routine therapy only in control group.The length of hospital stay,the time when FI occurred,the incidence of FI and mortality were compared with each other.Results Compared with control group,the length of hospital stay wag shorter[(40.3±15.4)d vs.(46.5±18.5)d],the time when FI occurred was slewer[(24.5±14.3)d vs. (14.5±12.7)d],the incidence of FI[15.0%(3/20)vs.35.0%(7/20)]and the mortality [10.0%(2/20)vs. 25.0%(5/20)]were lower in prevention group,there were significant differences between the two groups(P<0.05).Conclusion Prophylactic antifungal treatment can shorten the hospital stay,delay the FI occurrence,reduce the incidence of FI and the mortality in patients with SAP.

ABSTRACT

16 ?g/mL, and ≤0.03 ?g/mL , respectively. The MICs did not show significant differences among the ungerminated conidia and the germinated conidia for all the isolates tested except A.versicolor and Phialophora verrucosa. Conclusions Terbinafine is effective against the isolates of Aspergillus spp., dematiaceous fungi and dermatophytes in vitro except Pseudallescheria boydii and Scedosporium apiospermum. The MIC of terbinafine obtained with ungerminated conidia may reflect the antifungal activity of terbinafine against germinated conidia and hyphae of some filamentous fungi in vitro.

ABSTRACT

Objectives To evaluate the application of M38-P method recommend by NCCLS to dematiaceous fungi, and detect the susceptibility of itraconazole, fluconazole, miconazole, econazole, flucytosine, clotrimazole, nystatin, amphotericin B and terbinafine against dematiaceous fungi. Methods Using M38-P method forty-five strains of dematiaceous fungi were studied, which included 2 strains of Bipolaris spp., 1 strain of Drechslera spp., 1 strain of Curvularia spp.,14 strains of C. carrionii, 8 strains of F. pedrosoi, 3 strains of F. compacta and 16 strains of P. verrucosa. The media containing the fungi in the concentrations of (0.4 ~ 5) ? 106 CFU/mL were incubated at 35℃ for 5 ~ 7 days. Results The tested dematiaceous fungi were highly susceptible to itraconazole and terbinafine. Some of them were resistant to itraconzole, fluconazole, flucytosine and amphotericin B. Conclusions The data suggest that M38-P standards could be applied as the susceptibility testing of dematiaceous fungi, and it is necessary to monitor the resistant strains in antifungal treatment.

ABSTRACT

Five non-anesthetized dogs were intravenously infused with 4% pyrithione at a constant rate of 2 ml per minute for 5 min.During the course of treatment,experimental animals had salivation, nausea, vomiting, urination and defecation. In according with the accumulation of infused drug,the symptoms got worse and worse progressively. Finally, convulsion and respiratory failure appeared and death occured due to heart arrest. One hour after infusion and just before death, the concentration of pyrithione in the blood was 341? 180?g/ml and 657?158?g/ml respectively.The average accumulative lethal dose of pyrithione by intravenous infusion was 0.77?0.07 g/kg for dogs.
