ABSTRACT

Background: The patella plays a major role in flexion and extension of the knee. The patella morphology can be a predisposing factor in patella instability. The congruency between the patella and femur affects the kinematics of the patellofemoral joint, contact mechanics of the patellar cartilage, and strain in the underlying bone, whereby higher stresses and strains potentially contribute to cartilage wear and anterior knee pain. Aim and Objective: The present study was conducted to assess the morphology of facets of patella. Materials and Methods: The present study was conducted to assess the morphology of facets of patella. Ninety-six patella of unknown sex from bone store of Department of Anatomy were included in the study. The patellar morphology and morphometry were investigated. Statistical analysis was performed using the SPSS (v 12.01) software package (SPSS Inc., Chicago, Illinois). Statistical significance was set at P < 0.05. Results: In the present study; 96 patella of unknown sex from bone store of Department of Anatomy were included in the study. The mean size of the right lateral facet was 23.34 mm and left lateral facet patella was 23.06. P value was non-significant. The shape of lateral facet was concave in 93.9% in the left patella and 100% in the right patella. The shape of lateral facet was flat in 6.1% in the left patella and 0% in the right patella. The mean size of the right medial facet was 17.68 mm and left medial facet patella was 17.39 mm. P value was nonsignificant. The shape of medial facet was concave in 2% in the left patella and 10.6% in the right patella. The shape of medial facet was convex in 69.4% in the left patella and 61.7% in the right patella. The shape of medial facet was flat in 28.6% in the left patella and 27.6% in the right patella. P value was non-significant. Conclusion: The present study concluded that the mean size of the right lateral facet was more left lateral facet patella. The shape of lateral facet was concave in 93.9% in the left patella and 100% in the right patella. The shape of lateral facet was flat in 6.1% in the left patella and 0% in the right patella. The mean size of right medial facet was more than left medial facet patella. The shape of medial facet was concave in 2% in the left patella and 10.6% in the right patella. The shape of medial facet was convex in 69.4% in the left patella and 61.7% in the right patella. The shape of medial facet was flat in 28.6% in the left patella and 27.6% in the right patella.

ABSTRACT

Patterns and variant morphometries of calcaneal articular facets on talus are concerned before performing of joint ankle surgery, including used as a sex determination. Types of talar facets have been documented in many populations except in Thai race. Therefore, this study attempted to classify the types of talus facets and to measure the facet lengths on dried tali of Thais. The 372 dried tali (204 males, 168 females) from Khon Kaen University Bone Collection were observed for variant types and measured for their facet lengths. The facets were classified into 6 types: type I, tree facet are separated (1.88 %); type II (A), the anterior and middle facets are partially connected with predominant ridge (34.68 %); type II (B), the anterior and middle facets are partially connected with slight ridge (32.53 %); type III, the anterior and middle facets are fully fused to form a single facet (2.96 %); type IV, the anterior and middle facets are partially separated by a ridge and partly by a groove (27.42 %); type V, all facets are continuous fused to form a single facet (0.54 %). In addition, the morphometric lengths of AP (anterior to posterior process) and ML1&2 (medial to lateral process) in male are significantly greater than those of female. The AP, ML, and ML2 of male are 56.71±0.16, 41.63±0.18, and 37.85±0.36 mm, while of female are 51.21±0.12, 37.74±0.16, and 33.85±0.28 mm, respectively. This incidence can be used as ankle surgery consideration and an anthropological marker for sex determination of unidentified talus.


Los patrones y las variantes morfométricas de las facetas articulares calcáneas en el talus deben considerarse antes de la realización de la cirugía del tobillo a nivel articular, incluido su uso para la determinación del sexo. Se han documentado los tipos de facetas en el talus en muchas poblaciones, excepto en la raza tailandesa. Por lo tanto, este estudio clasificó los tipos de facetas del talus y se midieron las longitudes de las facetas del talus, en muestras secas de indiviuos tailandeses. Se analizaron 372 talus secos (204 de hombres, 168 de mujeres) de la colección de huesos de la Universidad Khon Kaen, se observaron distintos tipos de variantes y se midieron las longitudes de las facetas. Las facetas se clasificaron en 6 tipos: tipo I, faceta de árbol separadas (1,88 %); tipo II (A), facetas anterior y media parcialmente conectadas con la cresta predominante (34,68 %); tipo II (B), facetas anterior y media están parcialmente conectadas con una ligera cresta (32,53 %); tipo III, facetas anterior y media están completamente fusionadas para formar una sola faceta (2,96 %); tipo IV, facetas anterior y media están parcialmente separadas por una cresta y en parte por una ranura (27,42 %); tipo V, todas las facetas se fusionan continuamente para formar una sola faceta (0,54 %). Además, las longitudes morfométricas del proceso anterior o posterior (AP) y del proceso medial a lateral (ML1 y ML2) en el varón son significativamente mayores que las de la mujer. El AP, ML1 y ML2 de los hombres son 56,71 ± 0,16, 41,63 ± 0,18 y 37,85 ± 0,36 mm, mientras que las mujeres son 51,21 ± 0,12, 37,74 ± 0,16 y 33,85 ± 0,28 mm, respectivamente. Esta incidencia se puede utilizar como una consideración en la cirugía de tobillo y un marcador antropológico para determinar el sexo del talus no identificado.

ABSTRACT

Introduction: Calcaneous is largest of seven tarsal bones of foot and forms prominence of heel. Most of the timesan osteophytic outgrowth (calcaneal spur) has been observed in the plantar and dorsal aspect of the foot.Theaim of this study is to analyse the incidence of calcaneal spur in relation to morphological variations of talararticular facets of calcaneus in the state of Odisha, India.Materials and Methods: The material for the study consisted of 107 dry (56 right and 51 left), adult calcanei ofunknown sex obtained from Department of Anatomy. The calcaneal spurs were studied in detail and classifiedaccording to types of calcaneus.Results: The incidence of type 1 calcanei was predominant (66.36%) with least incidence being type 4(1.86%) inthe present study. Maximum incidence of calcaneal spurs were found in Type I calcanei (17.8%) followed by10.28% in Type 2, 4.7% in Type 3 and 0.9% in type 4.One out of 107 calcanei had presented with spur on thesustentaculum tali (0.9%). The incidence and type of calcaneal spurs were compared with those of previousstudies and etiology of heel pain has been discussed.Conclusion: Calcaneal spurs are related to type of calcanei with the highest frequency in Type 1 and least in Type4. Other factors, which contribute toward increase in incidence of spurs, are increasing age and weight, concurrentorthopedic diseases, and heel pain.

ABSTRACT

Talus (astragalus) is the second largest tarsal bone. It has no muscular or tendinous attachment. It takes part in the formation of various articulations in the form of talocrural , talocalcaneonavicular or subtalar joints. The prior knowledge of the anatomical set of talus and its various articulations holds significance in operating procedures. Knowing anatomy of talus not only helps in delineating underlying pathology but also helps in treatment. Three hundred dry adult tali (150- right and 150-left) were studied. Each talus was examined for the presence of various patterns of articular facets. They were classified into five groups. Type I was found in 39% cases. Type II in 43.7% cases, Type III in 6% cases, Type IV in 5.3% cases and Type V was in 6% cases; type V has two subtypes. Subtype ‘A’ was found in 5% cases & subtype ‘B’ was found in 1% cases. In the present study, highest incidence of type II tali was 43.7 % and type IV had lowest incidence of 5.3%. These variations and their incidences can be used as an anthropological marker for racial and regional differences in unidentified bones. Further, studies on various other population is indicated using larger sample size to make comparative studies more meaningful.

ABSTRACT

Background: Hypermobility of Atlanto-Occipital joints give rise to symptoms related to vascular compromise viz syncope, vertigo, transient impairment of visual field, altered consciousness. Surgical management of congenital anomalies of craniovertebral junction requires a precise identification of underlying pathophysiological condition. For this reason variation in superior articular facets of atlas vertebrae in terms of its morphology should be known. Method: The morphology of 100 atlas vertebrae had been studied and observations of total 200 facets were noted. Measurements were taken with a vernier callipers and a compass. Result :In the present study, typical oval, elongated or kidney shaped outline was found in 74.5% facets. Constrictions or notches were present on 48% facets. Complete separation (4.5%) is suggestive of tendency towards possible restriction of movement at atlanto-occipital joint as part of evolutionary changes. Roughly circular pressure facets had been observed in 78% of vertebrae indicating possibly greater pressure at this site during movement at atlanto-occipital joint.Conclusion: Knowledge of this variation in craniovertebral junction is of significant importance for Neurosurgeons and Orthopaedic surgeons doing various surgeries in this region.

ABSTRACT

Tali are one of the essential components for weight bearing and play an important role in the foot action. The three calcaneal articular facets of the talus are believed to have diverse morphological variations. However, so far, analysis of the articular facets has not been performed in Korean. In the present study, we classified calcaneal articular facets of the talus and measured several parameters of this bone in Korean adults. Seventy six dry tali (male, 47; female, 29; right, 38; left 38) obtained from forty six (male, 28; female, 18) Korean adult cadavers were used for the investigation. The average age of the cadavers was 64-year-old. Types of calcaneal articular facets of the talus were classified as follows: Type A, a type with three separated facets; Type B, types in which anterior and middle facets are connected, but distinguished by a ridge. Type B was subdivided into B1 (having a notch that separates the two facets partially) and B2 (without a definite notch, thus the two facets appear to be continuous smoothly); Type C, a type with combined anterior and middle facets. Type B was found to be the most common type (60.5%), and the incidence of its subtype B1 (28.9%) and B2 (31.6%) was similar to each other. Type C was noted in 30.3% of the cases, and type A was ranked the lowest (9.3%). Compared to the opposite gender, type C occurred more frequently in males, while type B1 was more prevalent in females. The length of the talus was somewhat longer in males (55.9 mm) than in females (52.8 mm). However, there was no difference between the two sexes in the width of the talus. We could confirm that morphology of calcaneal articular facets of the talus is different between males and females. Also, our results indicate that characteristics of the facets of Korean differ from those of other races. The characteristics of calcaneal articular facets of the talus disclosed by the present study may provide valuable information for the understanding of motor mechanics of the foot in Korean.
