ABSTRACT

Introducción: las úlceras por presión prolongan la estancia hospitalaria, generan un impacto negativo en la persona hospitalizada y elevan costos de atención, por lo que constituyen un problema de salud pública y un indicador de la calidad del cuidado de enfermería. Objetivo: identificar el cumplimiento de las actividades de enfermería en la prevención de úlceras por presión. Método: estudio descriptivo, transversal, en tres instituciones de salud de Barranquilla durante 2010. Universo 97 enfermeras y auxiliares de enfermería. Se utilizó el instrumento cumplimiento de criterios básicos en la prevención de ulceras por presión a pacientes hospitalizado, formato F1PUPPH/05. Se conformó una base de datos en Microsoft Excel. Para evaluar el comportamiento de las variables del estudio se utilizó la estadística descriptiva (medidas de tendencia central y variabilidad, normalidad y detección de datos atípicos y ausentes). Resultados: los criterios de cumplimiento en la prevención de ulceras por presión más empleados fueron: ejecuta el plan de cuidados, utiliza los elementos disponibles en la prevención con un 54,53 por ciento y registra factores de riesgo con un 53,60 por ciento, los menos empleados: valora el riesgo utilizando las escalas reconocidas y revalora y ajusta de acuerdo al estado del paciente, con un 58,76 por ciento cada uno. El 45,36 por ciento de la muestra alcanza el nivel de No cumplimiento de las acciones de prevención y el 35,05 por ciento obtuvo un nivel de excelente. Conclusiones: la debilidad en la competencia de la aplicación de escalas de valoración de riesgo de úlceras por presión confirma la importancia del cumplimiento de las actividades de prevención como indicador de calidad del cuidado de enfermería, así mismo genera un aporte al currículo, investigación y práctica de enfermería(AU)


Introduction: pressure ulcers lengthen hospital stay, produce a negative impact on the hospitalized person and raise care costs, a reason why they constitute a public health concern and an indicator of the quality of nursing care. Objective: to identify the accomplishment of nursing activities in the prevention of pressure ulcers. Method: descriptive cross-sectional study carried out at three health facilities of Barranquilla during 2010. Target group: 97 nurses and nursing assistants. We used the instrument accomplishment of basic criteria for pressure ulcer prevention in hospitalized patients (F1-PUPPH/05). We created a database in Microsoft Excel. To assess the behavior of the studied variables, we used descriptive statistics (central tendency and variability measures, normality and detection of atypical and absent data). Results: the most used accomplishment criteria for pressure ulcer prevention were the care plan execution; the use of elements available in prevention, with a 54.53 percent; and risk factors recording, with a 53.60 percent. The least used were risk assessing by using the recognized scores; and reassessment and adjustment according to the patient's state, with 58.76 percent each. 45.36 percent of the sample reaches the level of unaccomplishment of the prevention actions and 35.05 percent obtained a level of excellent. Conclusions: competence weakness in applying pressure ulcer risk assessment scores confirms the importance of prevention activities accomplishment as a nursing care quality indicator, and produces a contribution to the resume, research and nursing practice(AU)

ABSTRACT

PURPOSE: This study directly compared clinical assessment scores and short-term systemic complications after total knee arthroplasty (TKA) between a group of patients aged 80 or older (141 patients) and another group of patients aged between 65 and 70 years (616 patients) with advanced osteoarthritis. MATERIALS AND METHODS: We retrospectively investigated 757 osteoarthritic patients who underwent primary TKA from January 2007 to January 2011 with a follow-up of 1 year. The surgery was performed using an extramedullary alignment guide instrument without invasion of the intramedullary canal to decrease embolic load and blood loss. RESULTS: At 1 year after surgery, the mean Knee Society knee score was improved in both groups (from 63.6 to 83.2 in octogenarians and from 68.3 to 89.0 in the younger group) and the level of satisfaction was excellent in both groups (8 in octogenarians and 8.3 in the younger group), even though there was no notable change in function score in the octogenarians (from 61.0 to 61.9 in the octogenarians and from 62.3 to 73.6 in the younger group). The total incidence of systemic complications (3.4% vs. 1.2%, p=0.400) and surgical complications (2.1% vs. 0.5%, p=0.229) showed no significant difference between groups. CONCLUSIONS: TKA yielded favorable clinical outcomes with a comparatively low postoperative complication rate in octogenarians despite the negligible functional improvement.

ABSTRACT

Objective To investigate the coagulation function,the related factors and prognosis of preterm infants with periventricular hemorrhage-intraventricular hemorrhage (PVH-IVH).Methods Eighty-one preterm infants with PVH-IVH and 81 infants with non-PVH-IVH as a control were enrolled in the study.The changes in activated thromboplastin time(PT),activated partial thromboplastin time (APTT),Fibrinogen and D-dimer were determined by enzyme-linked immunosorbent assay.Clinical factors which were related to PVH-IVH in infants were analyzed with statistical analysis and prognosis was assessed with neonatal behavioral neurological assessment (NBNA).Results The average of gestational age in PVH-IVH group was(30.9 ± 1.79)weeks,in control group were (31.36 ± 1.39)weeks,and there was significant difference between the 2 groups (F =6.52,P =0.012).There were significant differences in APTI,and D-dimer between PVH-IVH and non-PVH-IVH infants(all P < 0.05).Multivariate Logistic regression analysis showed that the related factors included gestational age,neonatal respiratory distress syndrome(NRDS),hyperoxia,and mechanical ventilation(OR =3.98,1.72,2.99,5.65,all P < 0.05).The NBNA scores of the 2 groups were also significantly different(t =9.16,P < 0.05).Conclusions The occurrence of PVH-IVH is very complex resulted from gestational age,NRDS,hyperoxia,mechanical ventilation and other factors.In order to avoid the occurrence of intracranial hemorrhage and improve the life quality of preterm infants,the NRDS was prevented as far as possible,coagulation abnormalities were managed positively and the mechanical ventilation was shortened.

ABSTRACT

Background and Objectives: Recent amendments in syllabus from 2012 batch for learning physiotherapy ethics for 4 years & passing internal assessment in final year has prompted to undertake innovation with e module as against traditional lecture method which is currently in practice. e- Learning is teaching using electronic media which developed rapidly and power point presentations are being extensively used and hence pilot tested on 2012 final year exam appearing physiotherapy students although traditional lecture method continued for first and second year students. Aim of the study: To study the response of final B.P.Th. students on e module of ethics. Materials & Methods: Needs assessment on innovation was done. This was followed by e-module preparation. Module prepared according to syllabus in power point format. Module incorporated expert opinion from physiotherapy & M.E.T. faculty before final module was prepared which took one month and included one assignment. Module was posted on emails of all 25 final year students. 3 months time was given before evaluation by feedback forms, assignment completion & internal assessment scores. Results: All 25 students completed assignment & scored marks between 50-76% in internals. Students found that Module was easy to learn & follow, created self learning and could be read according to their convenience. Conclusions: it was possible to implement e modular pattern of learning although developing the e module required time and effort. e module enhanced self directed learning and can be implemented for all batches.
