ABSTRACT

Objective: To investigate the psychological health and influencing factors of women on assisted reproduction treatment during the outbreak of the coronavirus disease 2019 (COVID-19). Methods: A questionnaire survey was conducted on 599 women undergoing fertility treatment at the Assisted Reproductive Center of Northwest Women's and Children's Hospital. The basic demographic characteristics and fertility treatment progress of the patients were collected. The psychosocial questionnaire of emergency public health events was used to evaluate the psychological health status of women receiving assisted reproduction treatment during the epidemic. Nonparametric test was used to compare the psychological health status in different groups during the epidemic. Univariate and multivariate analyses of fear dimension and total scores were performed using the generalized linear model. Results: The women were mainly aged between 30 and 35 years old, and the type of assisted reproduction was mainly IVF. During the epidemic, the score of the fear dimension was the highest in the psychological health evaluation of fertility treatment women, with a median value of 0.83. The score of the fear dimension was different among women with different education level, occupation, residence and family monthly income (P<0.001, P=0.049, P=0.001, P<0.001). Women who had higher education level (professionals) and higher family income, and lived in cities had higher scores of the fear dimension. The total scores were different between women of different occupations and places of residence (P=0.047, P=0.010). The total scores of professionals and urban residents were higher. College education or above (β=0.23, 95% CI: 0.07-0.39), urban residence (β=0.12, 95% CI: 0.01-0.24), and higher family income (5 000-9 999 yuan, β=0.19, 95% CI: 0.05-0.34; ≥10 000 yuan, β=0.20, 95% CI: 0.03-0.37) increased the scores of the fear dimension of women receiving fertility treatment. Conclusion: The psychological status of women on fertility treatment during the epidemic of the COVID-19 is mainly manifested as fear. The fear of these urban women with high education level and good family economy is the most serious. Promotion of psychological health during the epidemic should be strengthened to reduce the psychological harm of the epidemic.

ABSTRACT

Objective The evaluation index system of quality of assisted fertility treatment care was constructed to provide the basis for scientific evaluation of quality of care in reproductive center. Methods Based on the structure-process-outcome (SPO) quality model, evaluation index system for fertility treatment care in the reproductive center was constructed by literature query, field process tracking, Delphi expert consultation and analytic hierarchy process (AHP). Results The effective recovery rate ( RR ) of the two rounds of the questionnaire was 100% . The expert authority coefficient Cr values were 0.813, 0.845 respectively. The W value of the coordination coefficient was 0.331, 0.372, both of which were statistically significant (P<0.01). Finally, an evaluation index system of quality of assisted fertility treatment care with three dimensions, 14 secondary indicators and 47 items was formed. Yaaph software was used to determine the weight coefficients of primary and secondary indicators, and the consistency test ( Cr<0.1) was performed, and the variation coefficients of all tertiary items were <0.20. Conclusions The weight of the quality index system of assisted reproductive treatment care in reproductive center is reasonable, the indicators were clear, systematic and objective, and worthy of clinical promotion.

El número de procedimientos de fertilización in vitro y otras técnicas de fertilización asistida avanzadas por año se incrementan vertiginosamente desde el año 1978 hasta la actualidad debido a múltiples razones médicas, ambientales, económicas, profesionales y culturales propias de la sociedad actual. Actualmente, la anestesia total intravenosa es la técnica de elección para estos procedimientos debido a que los fármacos de uso actual brindan un óptimo perfil de seguridad clínica, biológica y genética con una mínima afectación en la tasa de fecundación, división celular, tasa de implantación y embarazo. Se reportó el manejo anestésico de una paciente de 34 años de edad con antecedentes de infertilidad, que acudió al Servicio de Anestesiología del Hospital Lenin para la obtención de ovocitos por punción transvaginal guiada por ultrasonido. Se seleccionó la técnica anestésica total intravenosa con la asociación de midazolam/fentanyl/propofol en perfusión continua, se mantuvo ventilación espontánea hasta el término del procedimiento.


The annual amount of in vitro fertilization procedures and other advanced assisted fertilization techniques have been increasing since 1978 until now due to many reasons such as medical, environment, economics, professionals and cultural issues of our society. Today, total intravenous anesthetic (TIVA) is the best option for fertilization procedures because current intravenous agents offer optimal profile about clinical, biological and genetical security with minimal damage over fecundation rate, cellular division, implantation and pregnancy rate. The anesthetic management of a patient by assisted fertility was reported in this article. The anesthetic management of a 34-year-old with a history of infertility, who was attended at Anesthesiology Department of the Lenin Hospital for obtaining oocytes by transvaginal ultrasound-guided puncture was reported. Total intravenous anesthesia was prescribed with midazolam/fentanyl/propofol association; spontaneous ventilation was to mantain until the end of the surgical procedure.

ABSTRACT

OBJECTIVE: To measure Irish opinion on a range of assisted human reproduction (AHR) treatments. METHODS: A nationally representative sample of Irish adults (n=1,003) were anonymously sampled by telephone survey. RESULTS: Most participants (77%) agreed that any fertility services offered internationally should also be available in Ireland, although only a small minority of the general Irish population had personal familiarity with AHR or infertility. This sample finds substantial agreement (63%) that the Government of Ireland should introduce legislation covering AHR. The range of support for gamete donation in Ireland ranged from 53% to 83%, depending on how donor privacy and disclosure policies are presented. For example, donation where the donor agrees to be contacted by the child born following donation, and anonymous donation where donor privacy is completely protected by law were supported by 68% and 66%, respectively. The least popular (53%) donor gamete treatment type appeared to be donation where the donor consents to be involved in the future life of any child born as a result of donor fertility treatment. Respondents in social class ABC1 (58%), age 18 to 24 (62%), age 25 to 34 (60%), or without children (61%) were more likely to favour this donor treatment policy in our sample. CONCLUSION: This is the first nationwide assessment of Irish public opinion on the advanced reproductive technologies since 2005. Access to a wide range of AHR treatment was supported by all subgroups studied. Public opinion concerning specific types of AHR treatment varied, yet general support for the need for national AHR legislation was reported by 63% of this national sample. Contemporary views on AHR remain largely consistent with the Commission for Assisted Human Reproduction recommendations from 2005, although further research is needed to clarify exactly how popular opinion on these issues has changed. It appears that legislation allowing for the full range of donation options (and not mandating disclosure of donor identity at a stipulated age) would better align with current Irish public opinion.
