ABSTRACT

The Stomach is affected by many conditions,a thourough knowledge of the stomach regarding its shape,position,blood supply,nerve supply,lymphatic drainage,mucular pattern,relations with other organs and anatomical variations which will aid the surgeon to avoid complications during surgeries. Shape and position of the stomach can vary greatly with or without any physiological disturbances. However some of its rare shapes may result in the formation of volvulus or may increase the risk of gastric ulcer. Variant shapes of the stomach may be of congenital occurrence or are acquired later on in life. So as examined after death the shape of stomach is usually fixed at some stage of digestive process – exibiting different shapes physiologically . Large globular portion on the left and narrow tubular part on the right and transition between two regions is usually gradual. 50 stomachs were studied for their size and shape during the period of 2017–2019 in Andhra Medical College ,Visakhapatnam in department of Anatomy. The shape and sizes of the stomachs were noted. Sthenic or normal type/ J Shape Hypersthenic / Steer Horn , Hyposthenic or Asthenic type , Cascade.

ABSTRACT

This paper is based on the theory of"internal diseases will be presented externally"in Traditional Chinese Medicine, combined with the idea of syndrome differentiation and grasping the main symptoms, and proposes the concept of"virtual special disease". It is extracted from ancient books and expert experience, and it is believed that it is widespread and can be tested by clinical practice. The application of virtual syndrome to diagnosis can achieve the purpose of simplifying the complexity TCM syndrome differentiation. On the one hand, it provides a new exploration for enriching the TCM syndrome differentiation system; on the other hand, it provides new ideas for supplementing the biological research of syndromes and developing the diagnosis of integrated traditional Chinese and Western medicine.

ABSTRACT

BACKGROUND AND OBJECTIVES: The purpose of this study was to investigate acoustic (acoustic voice quality index, AVQI) and auditory-perceptual evaluation (grade, rough, breathy, asthenic, strained; GRBAS and consensus auditory-perceptual evaluation of voice; CAPE-V) as a predictor of voice recovery after laryngeal microsurgery (LMS) in patients with vocal polyp. SUBJECTS AND METHODS: A total of 68 patients with vocal polyp participated in this study. Voice samples were analyzed for AVQI by Praat and auditory-perceptual ratings were performed by three speech language pathologists. Voice handicap index-10 (VHI-10) was evaluated by patients themselves. RESULTS: Decreased AVQI, VHI-10, overall severity (OS), and increased smoothed cepstral peak prominence (CPPS) values were measured and statistically significant changes were noted after LMS. The ratio of Grade 0 and Grade 1 was increased. AVQI was correlated with CPPS, Grade, and OS, but not with VHI-10. The voice recovery of pedunculated polyp appeared in all vocal polyp sizes after LMS, but statistically significant differences were found only in small and medium sizes. After LMS of sessile polyps, AVQI, VHI-10, and OS decreased whereas CPPS increased; however, statistically significant difference was confirmed only in VHI-10 and OS. The receiver operating characteristic curve analysis showed the following results: AVQI [cutoff=5.5, sensitivity=61.8%, specificity=76.5%, area under the curve (AUC)=0.712], CPPS (cutoff=5.0, sensitivity=89.7%, specificity=48.5%, AUC=0.743), VHI-10 (cutoff=13.0, sensitivity=77.9%, specificity=89.7%, AUC=0.893), Grade (cutoff=2.0, sensitivity=70.6%, specificity=69.1%, AUC=0.728), and OS (cutoff=51.0, sensitivity=86.8%, specificity=66.2%, AUC=0.833). CONCLUSION: Acoustic and auditory-perceptual variables showed significant positive changes and predicted recovery of voice. Objective]To study Xin'an physician Wu Cheng asthenic diseases related theory of ideas in Bujuji, in order to more comprehensively understand asthenic diseases,to broaden the thinking, enhance the clinical curative effect. [Method]From the academic source of asthenic diseases , syndrome differentiation and treatment,prescription medication,clinical academic value, make research on Wu's thought about asthenic diseases treatment. [Result]Wu Cheng combined with their own perception and experience of clinical evidence, first put forward“external damage caused by asthenic diseases”;Aiming at the asthenic diseases of the treatment,Wu Cheng creatively put forward“support solution method”, “supplementary care method”, “treatment of spleen yin”;Wu Cheng's life was exploring the asthenic diseases,the theory of asthenic diseases had good clinical academic value,not only for the development of the cause of disease theory,but also enriching the theory of health prevention.[Conclusion]Wu Cheng Bujuji rich in content and its syndrome differentiation of asthenic diseases, with much experience, as today, treatment based on the thought of clinical asthenic diseases treatment still has important guiding significance.

ABSTRACT

The patent eustachian tubes (PET) of five patients with a chief complaint of aural fullness were successfully treated with kigikenchuto. A Kampo extract preparation of ogikenchuto and tokikenchuto (daily dose of 9 and 7.5 g, respectively) was applied as a representative prescription for kigikenchuto. The five patients had also been diagnosed with definitive PET on otological examination. Case 1 was a 73-year-old male suffering from aural fullness and autophonia after a common cold. These ear symptoms improved on lying down and resting. He had a history of admission for cerebral hemorrhage with hypertension and weight loss. Bilateral stiffness of the abdominal rectus muscle and an intolerable tickling sensation were noted. After taking kigikenchuto for two weeks, his symptoms improved. An additional study of four patients treated with kigikenchuto, including Case 2, a 75-year-old female, Case 3, a 33-year-old female, Case 4, a 78-year-old female, and Case 5, a 21-year-old male, showed effective cures. Kigikenchuto is a <i>qi</i>-blood supplementing derivative of shokenchuto, a minor center-reinforcing Kampo medicine for asthenic disease. Based on these results, we suggested that kigikenchuto serves as a useful formulation to treat PET. Life guidance to promote health is also essential for cure.

ABSTRACT

We retrospectively investigated the prevalence of influenza virus type A infection and the efficacy of Kampo medicine as a prophylactic for handicapped residents at our social welfare institution, during the 2004/5 flu season. Upon studying a nationwide surveillance report for said season, we supposed that it had been difficult to prevent mass infection at our welfare institution with the usual preventive methods, because type B had prevailed for most of the season, and because of the differing type A (AH 3) antigenicity which prevailed late-season, for which there were no type A vaccines.43 of 90 (47.8%) residents given flu vaccinations twice, and 25 of 110 (22.7%) of staff members given flu vaccinations once contracted type A influenza between March and May of 2005. Interestingly, residents who had been administered Hozai for either short-term health problems or chronic disease, demonstrated a low influenza prevalence, there being a statistically significant difference (p<0.05) between our “Hozai” and “non-Hozai” groups. Furthermore Juzentaihoto, known to enhance immune function, seemed beneficial because of its low associated morbidity rate, and only 2 of 8 residents administered it contracted flu. No difference was observed, however, between generalized “Kampo treatment” and “non-Kampo treatment” groups.We consider that certain Kampo medicines may be useful for the prevention of influenza infection, in institutionalized handicapped residents with short-term health problems. Immunological enhancements, and the influence of Hozai are discussed here.

ABSTRACT

I report on three rebound cases caused by topical steroids, which were successfully treated by Japanese herbal medicine. A drug for clearing away the heat was selected, because I thought skin erythema and hotness was due to the heat. A herbal drug that nourishes Yin and is anti-inflamatory was also selected, because I thought erythematosquamous eruption was due to asthenic heat syndrome. I selected a drug to promote diuresis, because the tendency of edema, exudation, and lichenification was due to disorders of the body's fluid metabolism. One of the cases with constitutional symptoms such as sensitivity to the cold and general malaise received Kampo treatment following diagnosis from the tongue, pulse and abdominal diagnosis by pulsation and skilled questioning of the patient.<br>Case 1 was a 19-year-old female with sensitivity to the cold and malaise. She improved after one week of treatment with Juzen-taiho-to, Sanmotsu-ogon-to and Chorei-to. Case 2 was a 19-year-old male with hyperfunctioning condition. He was almost cured after a month of treatment with Gorin-san, Sanmotsu-ogon-to and Chorei-to. Case 3 was a 25-year-old male with disorders of the body's fluid metabolism in skin and Yindeficiency in the body. He improved after one month of treatment with Rokumi-gan, Sanmotsu-ogon-to and Chorei-to.
