ABSTRACT

Aim: To evaluate the prevalence and distribution of tooth wear due to dental attrition among 12-year-old adolescents according to their gender, reports of tooth grinding during sleep/while awake (bruxism), and sleep features related to sleep-disordered breathing. Methods: A cross-sectional study was conducted with 172 adolescents from southeast Brazil. Parents/caregivers answered questions about sociodemographic characteristics; reports of adolescents' tooth grinding during sleep, and sleep features (e.g. sleep duration and position, snoring, drooling on the pillow) in the previous two weeks. Adolescents self-reported the occurrence of tooth clenching/grinding while awake in the previous two weeks. Tooth wear due to dental attrition was assessed by a previously calibrated researcher, using a validated 5-point analogical ordinal occlusal/incisal tooth wear grading scale, with scores ranging from 0 (no wear) to 4 (loss of crown height ≥ 2/3), depending on tooth wear severity. Descriptive statistics and the Mann-Whitney test (p < 0.05) were performed to identify differences in tooth wear due to the distribution of dental attrition, according to adolescents' characteristics. Results: Most adolescents were female (58.0%) and 81.6% of the participants presented tooth wear due to dental attrition within the enamel. Adolescents who snored had a higher number of incisors with tooth wear due to dental attrition (p = 0.035). Females showed a higher number of canines with tooth wear due to dental attrition (p = 0.020). Adolescents whose parents reported tooth grinding during sleep presented a higher number of bicuspids with tooth wear due to dental attrition (p = 0.001). Conclusion:Tooth wear due to dental attrition within the enamel was observed in most adolescents. The distribution, depending on specific groups of teeth, was higher among female adolescents, adolescents' whose parents reported tooth grinding, and adolescents who snored during sleep.


Objetivos: Avaliar a prevalência e distribuição de DDA entre adolescentes de 12 anos de acordo com o sexo, relato de ranger de dentes durante o sono e vigília (bruxismo) e características relacionadas a desordens respiratórias do sono.Método: Desenvolveu-se um estudo transversal com 172 adolescentes do sudeste do Brasil. Seus pais/responsáveis responderam um questionário contendo informações sociodemográficas, relato sobre os filhos rangerem os dentes durante o sono e hábitos do sono dos filhos (duração do sono, posição que dorme, roncar, babar no travesseiro) nas duas últimas semanas. Adolescentes relataram ocorrência de ranger/apertar de dentes durante a vigília nas duas últimas semanas. DDA foi avaliado por um pesquisador previamente calibrado através de uma escala analógica ordinal de cinco pontos de desgaste dentário para faces oclusal/incisal previamente validada, com escores variando de 0 (sem desgaste) a 4 (perda da coroa em altura ≥ 2/3), dependendo da gravidade do desgaste. Análises descritivas e teste de Mann-Whitney (p < 0,05) foram realizados para identificar as diferenças na distribuição de DDA de acordo com as características dos adolescentes.Resultados: A maioria dos adolescentes eram meninas (58,0%) e 81,6% dos participantes apresentaram DDA em esmalte em algum elemento dentário. Adolescentes que rocavam apresentavam mais incisivos com DDA (p = 0.035). Meninas apresentaram maior número de caninos com DDA (p = 0.020). Adolescentes que rangiam os dentes durante a noite apresentaram maior número de pré-molares com DDA (p = 0.001).Conclusão: Desgaste dentário por atrição em esmalte foi observado na maioria dos adolescentes. A distribuição, dependendo de grupos de dentes específicos, foi maior entre meninas, adolescentes que roncavam durante o sono e que rangiam os dentes durante o sono.

@#Introduction: There is a strong body of evidence advocating parent training programmes in the management of behavioral problems in children. In these programmes, attrition is a major challenge. We aimed to examine the sociodemographic and clinical profiles, parental motivation, and pre-intervention severity of children’s behavioural problems as rated by mothers who completed (PC) and mothers who dropped out (PD) of a parent training programme. We also aimed to determine whether there was any change in their children’s behaviour scores before and after intervention. We hypothesized that parent and child sociodemographic and clinical profiles, parental motivation, and pre-intervention severity of children’s behavioural problems would be different between PC and PD, and that children of PC would experience significant improvements in their behaviour. Methods: Data from a randomised controlled trial (RCT) involving 35 mothers and children aged 6–12 years old with behavioural problems who were enrolled in a parent training programme were examined. Child behaviour was measured using the Strength and Difficulties Questionnaire. Results: No significant differences were found between PC and PD in terms of the sociodemographic and clinical profiles, parental motivation, and pre- or post-intervention child behaviour. After programme completion, PC (n=27) reported improvements in all the behaviour subscales; total difficulties (p<0.001), emotional problem (p=0.004), conduct problem (p=0.001), hyperactivity symptoms (p<0.001), peer problems (p=0.036), and prosocial behaviour (p=0.001). Conclusion: The parent training programme produces significant child behaviour improvements in mothers who complete the programme. Nonetheless, the study has some limitations which restrict the generalizability of these findings.

Objective:To investigate and analyze the loss of personnel in health institutions, and put forward the suggestions.Methods:The staff turnover in various health institutions was analyzed based on a continuous monitoring data from 2012 to 2019.Results:In 2012-2019, the turnover rate of health institutions had generally remained at about 1.5%-2.0%. In 2015-2019, the natural attrition rate was 0.44%-0.70% and the job-changing rate was 0.06%-0.12%. Among all kinds of institutions, the loss of staff in township health centers and Centers for Disease Control and Prevention were the most serious.Conclusions:Some new characteristics have emerged in the flow of health personnel, which have gradually adapted to the demand for health services, and the role of market mechanisms in the allocation of health human resources has gradually emerged.For the institutions with serious attrition, it is suggested to strengthen policy integration, guide the rational flow of health personnel.

RESUMEN Objetivo: Identificar la relación de los riesgos laborales de tipo psicosocial y el desgaste psíquico con respuestas de estrés en 985 trabajadores de una administración pública mexicana. Materiales y métodos: El diseño del estudio es no experimental, transversal y cuantitativo. Se aplicó la batería para el estudio de las condiciones de trabajo de carácter psicosocial CTCPSMAC, validada para población iberoamericana, con Alpha de Crombach 0.927 y población mexicana 0,931. Se realizó el análisis de datos a través del programa IBM SPSS Statistics 21 mediante un análisis bivariado y regresión logística binaria. Resultados: Características personales, relaciones interpersonales en el trabajo y carga y ritmo de trabajo están asociados a respuestas cognitivo-emocional, conductual y fisiológica. Ambientes laborales y amortiguadores del riesgo psíquico están asociados a las respuestas conductual y isiológica. Concepción de las tareas del puesto de trabajo está asociada a las respuestas cognitivo-emocional y fisiológicas. La exposición a una mala interrelación trabajo con problemas familiares o sociales y características de la empresa, está asociada a respuesta conductual. Conclusiones: La investigación mostró que en la organización pública objeto de estudio existe una relación signiicativa entre los factores de riesgo psicosocial del entorno laboral y las respuestas de estrés por parte de los trabajadores que integran la misma, poniendo en riesgo la salud de estos, por lo que es importante considerar el cumplimiento de la Norma Oficial Mexicana NOM-035-STPS-2018, que recién entró en vigencia.


ABSTRACT Objective: The purpose of the present study is to identify the relationship of occupational risks of psychosocial type and psychic attrition with stress responses in 985 Mexican public administration workers. Materials and methods: The study design is non-experimental, cross-sectional, and quantitative. The CTCPSMAC questionnaire, used to study the work conditions of a psychosocial nature, was applied, validated for an Ibero-American population, with a 0.927 Crombach's Alpha; and a 0.931 value for a Mexican population. Data analysis was performed through the IBM SPSS Statistics 21 program, using a bivariate analysis and binary logistic regression. Results: Personal characteristics, interpersonal relationships at work, and work load and rhythm are associated with cognitive-emotional, behavioral, and physiological responses. Work environments and psychic risk buffers are associated with behavioral and physiological responses. Conception of job duties is associated with cognitive-emotional and physiological responses. Exposure to a bad work interrelation with family or social problems, and company characteristics is associated with behavioral response. Conclusions: The research showed that, in the public organization under study, there is a significant relationship between the psychosocial risk factors of the work environment and the stress responses by the workers who integrate it, putting their health at risk,thus, it is important to consider compliance with the Official Mexican Standard NOM-035-STPS-2018, which has just been enacted.

ABSTRACT

O termo Lesões Cervicais Não Cariosas (LCNCs) refere-se à perda de tecido dentário duro na Junção Cemento-Esmalte (JCE), cuja etiologia não está relacionada com o envolvimento bacteriano. A origem e a progressão desses defeitos cervicais são consideradas multifatoriais, sendo atribuídas a três fatores principais, sendo eles: abfração, biocorrosão e abrasão. Tais fatores podem estar combinados e associados a eventos de força excessiva aplicada durante a escovação, juntamente com a abrasividade de dentifrícios, hábitos alimentares e/ou parafuncionais. Essas lesões podem ter morfologias diferentes de acordo com seu fator etiológico principal, podendo se apresentar em forma de cunha, oval ou arredondada. O objetivo deste trabalho foi realizar uma revisão de literatura a fim de apresentar os desafios do Cirurgião-Dentista na descoberta dos diferentes fatores etiológicos das LCNCs. O desafio em questão se dá pelo estudo da combinação desses diversos fatores, bem como a realização de um diagnóstico preciso, fazendo-se necessário o conhecimento adequado da etiologia a fim de prevenir futuras novas lesões, estagnar as existentes e assim, capacitar o Cirurgião-Dentista a realizar um tratamento eficaz e longínquo das LCNCs. No entanto, ainda existem muitas controvérsias na literatura, tornando necessária a elaboração de mais estudos para elucidar a etiologia das LCNCs


The term noncarious cervical lesions (NCCL) refers to the loss of hard tooth tissue at the Cemento-Enamel Junction (CEJ), whose etiology is not related to bacterial involvement. The origin and progression of these cervical defects are considered multifactorial, being attributed to three main factors: abfraction, biocorrosion and abrasion. Such factors may be combined and associated with excessive force events applied during brushing, along with the abrasiveness of dentifrices, eating and/or parafunctional habits. These lesions may have different morphologies according to their main etiological factor, and may be wedge-shaped, oval or rounded. Our aim in this paper was to perform a literature review in order to present the challenges of the dental surgeon in identifying the different etiological factors of NCCL. Such challenge regards the study of the combination of these factors, as well as the achievement of an accurate diagnosis, requiring adequate knowledge of the etiology in order to prevent future lesions, stagnate existing ones, thus enabling the Dental Surgeon to treat NCCL effectively in the long term. However, there are still many controversies in the literature, making it necessary to develop more studies to elucidate the etiology of NCCL

ABSTRACT

El objetivo de este artículo es presentar una alternativa de tratamiento rehabilitador para pacientes jóvenes con gran pérdida de estructura dental, vinculada a lesiones de origen no bacteriano. Se presenta el caso clínico de un paciente de sexo masculino, de 39 años de edad, que acudió a la Cátedra de Odontología Integral Adultos de la Facultad de Odontología de la Universidad de Buenos Aires (FOUBA) relatando síntomas compatibles con hipersensibilidad dentaria y fatiga de los músculos masticadores. Al mismo tiempo, manifestó disconformidad con el aspecto estético de su sonrisa. Teniendo en cuenta la gran pérdida de sustancia en sus piezas dentarias producida por hábitos parafuncionales (bruxismo), se realizó una rehabilitación oral adhesiva con cerámicas utilizando el protocolo de abordaje terapéutico sugerido por la Cátedra. @#<p style="text-align: justify;"><strong>Background.</strong> Attrition in residency training can lead to lower workplace morale and increased costs. Finding associated factors can help revise admissions criteria or identify at-risk residents.</p><p style="text-align: justify;"><strong>Objective.</strong> We aimed to determine factors associated with attrition among residents in pediatrics.</p><p style="text-align: justify;"><strong>Methods.</strong> We applied a mixed cross-sectional (survey) and retrospective cohort (records review) study design. Residents who began training in the Philippine General Hospital in 2012-2018 were included. Our primary outcomes were non-completion of training within three years (attrition), completion beyond three years or ongoing training at a delayed year level (off-cycle), and the composite of attrition or off-cycle. Fisher's exact probability test and t-test were used to compare the non-attrition group versus the attrition group, and the non-attrition group versus the attrition or off-cycle group.</p><p style="text-align: justify;"><strong>Results.</strong> The overall attrition rate and off-cycle rate among 162 residents were 7.41% and 4.32%, respectively. The survey response rate was 73.00%. Four factors were significantly associated with attrition: higher age at entry into the program (p = 0.030), advanced degree (p = 0.009), longer interval from internship completion to start of residency training (p = 0.017), and a lower case presentation score (p = 0.048). The proportion of respondents older than 29 years was significantly higher in the attrition group than the non-attrition group (40.00% vs 0.94%, p = 0.031). Higher age at entry was also significantly associated with the composite outcome (attrition or off-cycle).</p><p style="text-align: justify;"><strong>Conclusion.</strong> Older age at entry, advanced degree, a longer interval from internship, and lower-case presentation scores were associated with attrition among residents in pediatrics from a single center.</p>

ABSTRACT

Introducción: Varias enfermedades neurodegenerativas están asociadas a la ocurrencia de acortamiento de los telómeros, y los convierten en biomarcadores y dianas terapéuticas potenciales. Objetivo: Reflejar la relevancia del acortamiento de los telómeros para enfermedades neurodegenerativas, y destacar sus implicaciones Material y métodos: Se realizó una revisión bibliográfica durante los meses de septiembre de 2019 a enero de 2020. Fueron consultadas bases de datos de referencia, con el uso de descriptores y operadores booleanos. La estrategia de búsqueda avanzada para la selección de los artículos fue empleada, teniendo en cuenta la calidad metodológica o validez de los estudios. Desarrollo: Fueron identificadas evidencias sólidas de asociación entre el acortamiento de los telómeros y las enfermedades de Alzheimer y Huntington, que sugieren un papel relevante de la biología de los telómeros en la fisiopatología de estas enfermedades. Las evidencias disponibles hasta el momento no permiten establecer la relevancia de la biología de los telómeros en la fisiopatología de la Enfermedad de Parkinson o de la esclerosis lateral amiotrófica. Se obtuvieron evidencias de la utilidad de terapias orientadas a la prevención del acortamiento de los telómeros para el tratamiento de enfermedades neurodegenerativas. Conclusiones: El acortamiento de los telómeros es de relevancia fisiopatológica y clínica para las enfermedades de Alzheimer y Huntington, mientras que existen evidencias insuficientes para establecer su importancia en la Enfermedad de Parkinson y la esclerosis lateral amiotrófica. El uso de estrategias para estimular la actividad de la telomerasa tiene potenciales aplicaciones terapéuticas en el contexto de enfermedades neurodegenerativas(AU)


Introduction: Several neurodegenerative disorders are associated with telomere attrition, turning telomeres into potential biomarkers and potential therapeutic targets. Objective: To assess the relevance of telomere attrition for neurodegenerative disorders, highlighting its therapeutic implications. Material and methods: A literature review was carried out from September 2019 to January 2020. Several databases were searched by using descriptors and Boolean operators. Advanced search strategy was used for the selection of articles, taking into account the methodological quality and validity of the studies. Results: Strong evidence for an association between telomere attrition and Alzheimer and Huntington diseases was obtained, suggesting a potential importance of telomere biology in the physiopathology of these diseases. Current evidence does not allow establishing the relevance of telomere attrition in the physiopathology of Parkinson´s disease or Amyotrophic Lateral Sclerosis. Evidence was obtained for the usefulness of therapies for the prevention of telomere attrition in the treatment of neurodegenerative disorders. Conclusions: Telomere attrition has physiopathological and clinical relevance in Alzheimer´s and Huntington´s diseases, though current evidence is not enough to establish its role in Parkinson's disease and Amyotrophic Lateral Sclerosis. Strategies that enhance telomerase activity have therapeutic potential in the context of neurodegenerative disorders(AU)

ABSTRACT

O objetivo deste estudo foi identificar fatores potenciais de impacto sobre a desistência em um ensaio clínico randomizado. Para participar das 12 semanas de intervenção, foram selecionados adolescentes com sobrepeso ou obesidade (IMC percentil ≥ 85), dos sexos masculino e feminino, com média de idade 16,44±1,09. Utilizou-se regressão logística binária para responder pelo desfecho de não conclusão. As regressões foram realizadas por blocos e as variáveis estudadas foram características demográficas e antropométricas, marcadores metabólicos, aspectos motivacionais, funcionamento psicológico e percepção das práticas alimentares parentais. Foi constatada uma taxa de desistência de 48,8%. A maior chance de desistência foi explicada no modelo final de regressão pela autoeficácia para mudança de estilo de vida (OR: 0,732; IC95%: 0,581-0,921; p=0,008), pelo suporte familiar (OR: 0,807; IC95%: 0,685-0,950; p=0,010), pela HbA1c (OR: 11,366; IC95%: 3,123-37,702; p=0,010) e por fatores relacionados às práticas alimentares parentais. Concluiu-se que a família e os aspectos relacionados à motivação para mudança podem ser determinantes de desistência em programas para perda de peso para adolescentes.


The number of overweight or obese adolescents has increased, but the rate of dropping out in programs for this population is high. The objective of this study was to identify potential factors of impact on dropping out in a randomized clinical trial. To take part in a 12-week intervention, overweight or obese adolescents (BMI percentile ≥ 85), males and females, with mean age 17.99±1.09 were selected. Binary logistic regression was used to account for the non-completion outcome. Regressions were performed by blocks and the variables studied were demographic and anthropometric traits, metabolic markers, motivational aspects, psychological functioning, and perception of parental food practices. The drop-out rate was 48.8%. The highest chance of withdrawal was explained in the final regression model by self-efficacy to change lifestyle (OR: 0.732; IC95%: 0.581-0.921; p=0.008), by family support (OR: 0.807; IC95%: 0.685-0.950; p=0.010), by HbA1c (OR: 11.366; IC95%: 3.123-37.702; p=0.010), and by factors associated with parental food practices. It was concluded that the family and aspects related to motivation for change can be determinants of dropping-out in weight-loss programs for teenagers.


El objetivo de este estudio fue identificar factores potenciales del impacto sobre la desistencia en un ensayo clínico aleatorizado. Se seleccionaron para participar en 12 semanas de intervención, adolescentes con sobrepeso u obesidad (IMC percentil ≥ 85), del sexo masculino y femenino con media de edad 16,44±1,09. Se utilizó regresión logística binaria para responder por el resultado de conclusión incompleta. Las regresiones fueron realizadas por bloques y las variables estudiadas fueron características demográficas y antropométricas, marcadores metabólicos, aspectos motivacionales, funcionamiento psicológico y percepción de las prácticas alimentarias parentales. La tasa de desistimiento fue de 48,8%. La mayor probabilidad de desistimiento fue explicada en el modelo final de regresión por auto-eficacia para el cambio de estilo de vida (OR: 0,732, IC95%: 0,581-0,921, p=0,008), por el soporte familiar (OR: 0,807, IC95%: 0,685-0,950; p=0,010), por la HbA1c (OR: 11,366, IC95%: 3,123-37,702; p=0,010) y por factores relacionados con las prácticas alimentarias parentales. Se concluyó que la familia y los aspectos relacionados con la motivación para el cambio pueden ser determinantes de la desistencia de programas para la pérdida de peso para los adolescentes.

OBJECTIVE@#To explore the preliminary application of "biocopy function" in digital technology of temporary crown in severe tooth attrition, so as to reduce the difficulty of final restoration adjustment in severe tooth wear cases, and save clinical time.@*METHODS@#Twelve patients aged between 40 and 65 years with severe tooth attrition were recruited in this study. The experimental group (6 cases, 3 females and 3 males) used the method of digital copy of temporary restoration shape when making the final restoration, and the control group (6 cases, 3 females and 3 males) used the traditional method to make the final restoration. The mean time of central occlusal adjustment and protrusive and lateral occlusal adjustment of each crown, the number of follow-up visits of the patients were recorded and statistically analyzed. The occlusal comfort of the first and the third months after wearing the prosthesis was evaluated by the patients, and the difference of the occlusal comfort of the final prosthesis made by the two methods was compared (the full score was 10 points). The occlusal adjustment time and occlusal comfort score were used as measurement data. Single sample t test was used. The number of follow-up visits was count data. Rank sum test was used for non-normal continuous variables.@*RESULTS@#The mean occlusal time of each crown adjustment in the experimental group was significantly lower than that in the control group (P < 0.01), and the median number of follow-up visits in the experimental group was lower than that in the control group (P < 0.01). The average occlusal comfort score of the experimental group was higher than that of the control group at the first time (P < 0.01). After three months of crown wearing, the median occlusal comfort score of the experimental group was higher than that of the control group (P < 0.05).@*CONCLUSION@#In the case of severe tooth attrition, it is feasible to use the "biocopy function" in the design software to design the final prosthesis, which is conducive to reduce the difficulty of adjustment and achieve predictable functional and aesthetic effects, which is of great significance to save clinical time and improve the accuracy of restoration.

RESUMO Introdução: As lesões cervicais não cariosas são lesões dentárias, que apresentam etiologia multifatorial, sem o envolvimento de bactérias. Objetivo: Descrever as características clínicas, etiologia e tratamento das lesões cervicais não-cariosas. Métodos: Realizou-se uma revisão bibliográfica de estudos publicados nos últimos 5 anos (2014-2018) por meio da busca nas bases de dados: PubMED/Medline, Lilacs, Science Direct, SciELO (Scientific Eletronic Library) e Google Acadêmico. Para a pesquisa foram utilizados os seguintes descritores: "lesões cervicais não cariosas (non-carious cervical lesions)", "abrasão dentária (dental abrasion)", "erosão dentária (dental erosion)", "abfração dentária (dental abfraction)" e "atrição dentária (dental atrittion). Após criteriosa filtragem, foram selecionados 26 artigos e 2 livros para inclusão no estudo. Análise e integração das informações: As lesões cervicais não cariosas comumente classificadas em: abrasão, abfração, erosão e atrição. Essas lesões podem apresentar diversas formas, apesar de serem incluídas em uma classe genérica de denominação. São lesões que causam a perda gradativa dos tecidos mineralizados dentários, podendo trazer inúmeras consequências ao dente acometido. Conclusão: Para um correto diagnóstico e decisão de tratamento, essas lesões devem ser vistas sob seu aspecto etiológico multifatorial. Diversas possibilidades terapêuticas podem ser utilizadas no tratamento dessas lesões sendo necessário que o clínico conheça os principais fatores etiológicos e características clínicas que as diferenciem(AU)


RESUMEN Introducción: Las lesiones cervicales no cariosas son lesiones dentales, que presentan etiología multifactorial, sin la participación de bacterias. Objetivo: Describir las características clínicas, etiología y tratamiento de las lesiones cervicales no cariosas. Métodos: Se realizó una revisión bibliográfica de estudios publicados en los últimos 5 años (2014-2018) a través de la búsqueda en las bases de datos: PubMED / Medline, Lilacs, Science Direct, SciELO (Scientific Eletronic Library) y Google Académico. Para la investigación se utilizaron los siguientes descriptores: "lesiones cervicales no cariosas", "abrasión dental", "erosión dental", "abfracción dental" y " " atrición dental. Después de un cuidadoso filtrado, se seleccionaron 26 artículos y 2 libros para su inclusión en el estudio. Análisis e integración de las informaciones: Las lesiones cervicales no cariosas comúnmente clasificadas en: abrasión, abfración, erosión y atrición. Estas lesiones pueden presentar diversas formas, aunque se incluyen en una clase genérica de denominación. Son lesiones que causan la pérdida gradual de los tejidos mineralizados dentales, pudiendo traer innumerables consecuencias al diente acometido. Conclusiones: Para un correcto diagnóstico y decisión de tratamiento, estas lesiones deben ser vistas bajo su aspecto etiológico multifactorial. Diversas posibilidades terapéuticas pueden ser utilizadas en el tratamiento de esas lesiones, por lo que resulta necesario que el clínico conozca los principales factores etiológicos y características clínicas que las diferencien(AU)


ABSTRACT Introduction: Non-carious cervical lesions are dental lesions of a multifactorial etiology, without the involvement of bacteria. Objective: Describe the clinical characteristics, etiology and treatment of non-carious cervical lesions Methods: A bibliographic review was conducted of studies published in the last five years (2014-2018) by searching the databases PubMED / Medline, Lilacs, Science Direct, SciELO (Scientific Electronic Library) and Google Scholar. The following descriptors were used: "non-carious cervical lesions", "dental abrasion", "dental erosion", "dental abfraction" and "dental attrition. ABSTRACT Non-Carious Cervical Lesions (NCCL) are a current problem of multifactorial origin that is associated with the loss of vertical occlusal dimension, hypersensitivity, loss of teeth, fractures, and many other pathologies that affect the masticatory function of the oral cavity. Identification of the etiological factors of NCCL comprises a key piece for the solution of the problem. Based on clinical scientific evidence related with the diagnosis, a restorative treatment must be planned according to the loss of dental structure in order to return function and aesthetics. The purpose of this case report was to describe a 54-year-old male patient who was diagnosed with generalized NCCL combined with occlusal vertical dimension loss. The protocol of the treatment process is described.


RESUMEN Las lesiones cervicales no cariosas (LCNC) son una problemática actual de origen multifactorial, la cual se ve asociada a la pérdida de dimensión vertical oclusal, hipersensibilidad, pérdida de dientes, fracturas y muchas otras patologías que afectan la función masticatoria de la cavidad bucal. La identificación de los factores etiológicos de las LCNC son piezas claves para la solución del problema. Basados en la evidencia científica clínica relacionado con el diagnóstico, se debe planificar un plan de tratamiento restaurativo que depende de la pérdida de la estructura dentaria para devolver la función y estética. El objetivo de este reporte de caso es describir a un paciente masculino de 54 años a quien se le diagnosticó LCNC generalizadas combinado con una pérdida de dimensión vertical oclusal. El protocolo de plan de tratamiento es descrito paso a paso.

ABSTRACT: Pica is a condition associated with the chronic, compulsive consumption of a particular substance or material that is not considered food. Culturally it is not accepted by society and its nutritional value is non-existent. When this behavior is repeated consistently for more than one month, it is considered to be Pica. This eating disorder does not have a clear etiology, but is associated with; orality in children, psychotic disorders, nutritional deficiencies, psychosocial problems, autism, family problems, OCD, stress, abuse, family separations and other traumatic events. In order to detect it, it is necessary to perform a correct anamnesis, clinical examination, radiographic examination, complementary examinations such as blood count, as well as a relationship of trust between the dentist and the patient are necessary. All of them will be able to guide us and make us come out of suspicion that this harmful behavior exists, since patients often hide it.


RESUMEN: Pica es una condición asociada al consumo compulsivo y crónico de alguna sustancia o material en particular, que no es considerada alimento. Culturalmente no es aceptado por la sociedad y su valor nutricional es inexistente. Cuando esta conducta se repite consiste-ntemente durante más de un mes, se considera como Pica. Este desorden de comportamiento alimenticio no tiene una etiología clara, pero se asocia a la oralidad en niños, trastornos psicóticos, deficiencias nutricionales, problemas psicosociales, autismo, problemas familiares, TOC, estrés, abuso, separaciones familiares y otros eventos traumáticos. Para detectarla, es necesaria una correcta anamnesis, examen clínico, radiográfico, exámenes complementarios como hemograma, además de una relación de confianza rapport odontólogo- paciente. El conjunto de ellos nos podrán guiar y salir de sospecha que esta conducta dañina existe, ya que los pacientes muchas veces lo esconden.

Background: In most countries’ nurses are in short supply, but not as acute as in developing countries. An advanced world fulfils its role by attracting nurses from other countries, while developing countries cannot compete with higher wages. Aims & Objective: The current study was undertaken to analyse the reason for attrition and strategies for retention among staff nurses working at Saveetha Medical College and Hospital, Chennai. Material & Methods: The subjects from the hospital about 50 nurses were selected for this study by simple random sampling method. The research design was organized and analyzed in terms of both descriptive and inferential statistics. Results: The study findings showed that a vast majority of nurses i.e. 35 samples (70%) had high attrition rate and 15 samples (30%) had moderate attrition rate. The attrition rate may vary in association with demographic variables results of the research showed that recommended counselling and social assistance will help and improve the quality of life of nurses and allow them to retain in the same hospital. Conclusion: The nurse administrator should motivate the staff nurses by conducting programs for their good lifestyle. Many service education programs should be organized for nurses to update their knowledge.

Abstract: The objective of the study was to analyze the surface area (SA) of the wear caused by simulated chewing on human enamel and opposing restorative material, namely: composite resin (CR), porcelain fused to metal (PFM), lithium disilicate (LD), or monolithic zirconia (MZr). Forty-eight premolars were selected as enamel specimens and divided randomly into 4 groups (n = 48; n =12) used as antagonists in chewing simulation (250,000 loading cycles) against one of the four selected test materials. Enamel and material specimens were scanned and evaluated under digital microscope, and wear SA (mm2) were recorded. Descriptive statistics, paired t-test, one-way ANOVA, and post-hoc Tukey-HSD tests were used for statistics (p < 0.05). The smallest and largest SA were exhibited by enamel against LD (0.80 mm2) and PFM (1.74 mm2), respectively. PFM (3.48 mm2) showed the largest SA and CR (2.28 mm2) showed the smallest SA. Paired t-test for SA values showed significant difference (p < 0.05) in all wear comparisons between materials and enamel antagonists. The wear of materials were greater than that of their respective enamel antagonists (p < 0.05). One-way ANOVA of the logarithmic means of wear SA revealed significant differences (P<0.05). Post-hoc Tukey test revealed significance for PFM (p < 0.05) with other materials. Wear of all test materials was greater compared to the wear of enamel antagonists. PFM and LD caused the largest and the smallest enamel wear, respectively. CR, LD, and MZr are more resistant than PFM to wear after simulated chewing against enamel.

ABSTRACT

Objetivo: identificar los factores de riesgo de la atrición dental severa en pacientes que acudieron a instituciones de atención odontológica, públicas y privadas, de la ciudad de Cuenca, Ecuador. Materiales y métodos: fueron analizados 237 pacientes adultos, 79 casos con atrición dental grados 2, 3 y 4, según el índice de Smith y Knight, y 158 contro- les con atrición grados 0 y 1. Se realizó un examen clínico y fotográfico para determinar la relación entre la atrición dental y factores como edad, sexo, procedencia, número de dientes residuales, pH salival y bruxismo. Resultados: los pacientes mayores de 25 años (OR= 2,47; IC= 1,41 ­ 4,35; X2= 10,21; p= 0,001), con menos de 20 dientes en boca (OR= 4,95; IC= 1,47 ­ 16,62; X2= 7,97; p= 0,004 ) y con bruxismo (OR= 2,64; IC= 1,45 ­ 4,81; X2= 10,49; p= 0,001) tienen más probabilidades de presentar atrición dental severa. Luego de una regresión logística binaria se confirmó dicha relación: pacientes mayores de 25 años (OR= 2,03; IC= 1,08 ­ 3,818) con menos de 20 dientes en boca (OR= 3,90; IC= 1,07 ­ 14,19) y con bruxismo (OR= 3,22; IC= 1,70 ­ 6,10); sin embargo, se observó una capacidad predictora muy baja de la variables ajustadas según el R­cuadrado de Cox y Snell (0,123) y el R­cuadrado de Nagelkerke (0,172). Conclusiones: edad avanzada, menor cantidad de dientes en boca y bruxismo aumentaron la posibilidad de presentar atrición dental severa en los pacientes analizados. Mientras que el sexo masculino, la procedencia rural y el pH salival crítico (menor a 6) no se comportaron como factores de riesgo.

RESUMEN La dimensión vertical de oclusión (DVO) está representada por la distancia entre dos puntos, que no se pierde; sin embargo, puede verse aumentada o disminuida. Depende en buena medida de la dentición presente. Entidades patológicas como la atrición severa pueden influir su condición. La ruta clínica para llegar a la determinación de realizar un incremento de DVO es un proceso meticuloso, parte de un diagnóstico certero, cuantificación de la misma, así como la vía para realizar el procedimiento. Existen múltiples métodos para la terapéutica del incremento de DVO: dispositivos removibles o fijos. La elección del plan de tratamiento será con base en las particularidades de cada paciente. No existe tratamiento universal sino uno ideal para cada situación específica. Este caso describe las consideraciones previas al aumento de DVO en un paciente masculino de 68 años con atrición generalizada. Se evaluó la magnitud de la reducción de DVO, perfil facial y estética mediante análisis fotográfico y video; el esquema oclusal y la estructura dental presente. La literatura propone calcular con diferentes métodos el restablecimiento de la DVO; antes de elegir alguno se requiere definir y especular qué tanto se encuentra disminuida, para ello no basta un análisis estático o de simple apreciación, debe considerarse el aspecto facial, así como la dinámica mandibular. El incremento de la DVO en una sola intención, mediante restauraciones provisionales, mantenidas dentro de los parámetros de comodidad, es un excelente recurso para ejecutarla, el paciente experimenta una fase de adaptación favorable y sin complicaciones sintomáticas.


ABSTRACT Occlusal vertical dimension (OVD) is represented by the distance between two points, which is not lost, however, can be increased or decreased. Depends to a good extent on the present dentition. Pathological entities such as severe attrition can influence their condition. Clinical route to arrive at the determination to perform an increase of OVD is a meticulous process, part of a correct diagnosis, quantifi cation of the same, as well as the way to perform the procedure. There are multiple methods for this treatment: removable or fi xed devices. Choice of treatment will be based on the particularities of each patient. There is no universal treatment but an ideal for each specific situation. This case describes the considerations prior to the increase of OVD in a 68-year-old male patient with generalized attrition. Magnitude of OVD reduction, facial and aesthetic profi le was evaluated by means of photographic and video analysis; the occlusal scheme and the dental structure present. Literature consulted proposes to calculate with different methods the reestablishment of OVD, before choosing one it is necessary to defi ne and to speculate how much it is diminished, for it is not enough a static analysis or of simple appreciation must consider the facial aspect, as well as the mandibular dynamics. Increasing OVD in a single intention, through provisional restorations, maintained within the parameters of comfort, is an excellent resource to execute it, the patient undergoes a phase of favorable adaptation and without symptomatic complications.

Background: Smokeless tobacco use in the Indian subcontinent is a part of many religious and cultural rituals and has gained a degree of social acceptance. The deleterious effects of smokeless tobacco are not as well-known as those produced by smoking. Objectives: The study was carried out to assess the influence of tobacco chewing on the oral health of adult patients attending the dental outpatients department of Khordha district headquarter, Odisha. Methods: A hospital-based cross-sectional study was conducted among 25–64-year-old patients attending the dental outpatient department of Gopabandhu Khordha district headquarter hospital. A total of 512 study participants, who were age and sex matched, were stratified into four age groups such as 25–34 years, 35–44 years, 45–54 years, and 55–64 years old. Oral health status of the participants was assessed using modified WHO Oral Health Assessment Form (2013). Pearson's Chi-square test, binary and multinomial logistic regression was performed to determine the relationship between oral health problems and tobacco chewing. Results: Among the tobacco chewers, 59.8% had gingival bleeding, 40.6% had periodontal pockets, 30.1% had loss of attachment, 48.4% had attrition, and 4.3% had potentially malignant disorders. Compared to the nonchewers, these oral problems were significantly higher among the chewers. Whereas dental caries experience was significantly lower among the chewers (40.6%) compared to the nonchewers (54.7%). Compared to the nonchewers, chewers had 1.71 times increased odds for gingival bleeding, 1.71 times increased odds for periodontal pockets, 2.39 times increased odds for loss of attachment, and 2.49 times increased odds for attrition, which were statistically significant. Conclusion: Hence, the study revealed that tobacco chewing definitely had an influence on oral health, with statistically significant increase in oral health problems in chewers compared to nonchewers. Moreover, loss of attachment and potentially malignant disorders increased significantly with the frequency of tobacco chewing. Periodontal pockets, attrition, and loss of attachment significantly increased with the duration of the chewing habit.

Introducción: El desgaste dental es un proceso insidioso y acumulativo que lleva a la destrucción del esmalte y la dentina; conocido como lesiones dentales no cariosas y son: Atrición, Abrasión, Abfracción y Erosión. En Cuba estas lesiones han sido poco estudiadas. Objetivo: Identificar las lesiones dentales no cariosas y sus formas clínicas en pacientes atendidos en la Clínica Estomatológica Siboney. Métodos: Se estudiaron 876 pacientes escogidos al azar de la población que fue atendida durante un período de 10 meses en dicha clínica estomatológica. A los mismos se les realizó un examen estomatológico para determinar si eran portadores o no de tales lesiones y cuál o cuáles variedades de las mismas presentaban. Resultados: Se comprobó que 102 de los 876 pacientes examinados eran portadores de lesiones dentales no cariosas para un 11,43 por ciento; de los cuales el 42,15 por ciento presentaron un solo tipo de lesión; 50 pacientes mostraron dos tipos de lesiones, para un 49,01 por ciento, en todas las combinaciones posibles, siendo la combinación más frecuente Atrición y Abfracción seguida por la Atrición y la Abrasión. Por último, nueve pacientes presentaron tres tipos de lesiones, para un 8,82 por ciento; en este caso la combinación más frecuente fue Atrición, Abfracción y Erosión. Conclusiones: El porcentaje de pacientes con lesiones dentales no cariosas fue relativamente elevado; pudiendo presentarse una lesión aislada o combinaciones de dos o tres; siendo las formas clínicas más frecuentes: Abfracción, en el caso de una lesión; Atrición y Abfracción en el de dos y Atrición, Abrasión y Erosión, en el de tres(AU)


Introduction: Dental wear is an insidious and cumulative process that leads to the destruction of enamel and dentin; Known as non-carious dental injuries and are: Attrition, Abrasion, Abrasion and Erosion. In Cuba these injuries have been little studied. Objective: To identify non-carious dental lesions and their clinical forms in patients treated at the Siboney Stomatology Clinic. Methods: We studied 876 patients chosen at random from the population that was attended during a period of 10 months in said stomatological clinic. They underwent a stomatological examination to determine if they were carriers or not of such injuries and which one or which varieties of them they presented. Results: It was verified that 102 of the 876 patients examined were carriers of non-carious dental lesions for 11.43 percent; of which 42.15 percent presented a single type of injury; 50 patients showed two types of injuries, for 49.01 percent, in all possible combinations, with the most frequent combination being Attrition and Abfraction followed by Attrition and Abrasion. Finally, nine patients presented three types of injuries, for 8.82 percent; in this case the most frequent combination was Attrition, Abrasion and Erosion. La colocación de restauraciones directas a base de resina en el área de premolares y molares permite realizar tratamientos complejos en pacientes afectados por atrición dental severa y/o bruxismo. Hoy en día gracias a la técnica de grabado ácido, la fuerza de adhesión de la resina al esmalte disminuye la microfi ltración y sensibilidad postoperatoria. El objetivo de este artículo es describir la rehabilitación provisional mediante restauraciones adhesivas de resina compuesta en un paciente con atrición dental severa, el cual recibirá posteriormente tratamiento de ortodoncia por discrepancia de las arcadas. Al término de este tratamiento se colocarán restauraciones onlay de disilicato de litio (AU)


The placement of resin-based direct restorations in the area of premolars and molars allows the treatment of complex cases in patients aff ected by dental attrition and/or bruxism. Nowadays, due to the acid etching technique, the bond strength of the resin to the enamel will reduce the micro-leakage and post-operative sensitivity. The aim of this article is to describe the provisional rehabilitation with resin adhesive restorations, in a patient with a severe dental attrition, which will receive orthodontic treatment due to discrepancies in the arches. At the end of this treatment, ceramic restorations will be placed (AU)
