ABSTRACT

Background: Warts, the most common manifestation caused by human papillomaviruses. Treatment is needed because of risk of transmission, cosmetic point of view, multiple, painful, and disfigurement caused by warts. Aims and Objectives: In this study, two techniques of autologous therapy are compared in terms of safety and efficacy. Falknor’s needling and autoimplantation, both are minimally invasive procedures with the aim of treating one wart and inducing immunity against the viral infected cells. Materials and Methods: Forty patients of clinically diagnosed cutaneous warts were randomly divided into two groups. In Group A, Falknor’s needling was performed on a single lesion in each patient. In Group B, autoimplantation was done by harvesting a single lesion and then implanting the tissue in subcutaneous tissue of the patient at other body site. Results: Data were analyzed using SPSS software v. 23 (IBM Statistics, Chicago, USA) and Microsoft Office 2007. Both the modalities showed excellent response (P = 0.504) in the treatment of warts with Grade 4 improvement in 85% (17 patients each) in both the modalities, with 5% of the patients showing Grade 2 and Grade 3 improvement each in needling group. Conclusion: Both the modalities of autologous therapy are simple, easy to perform, safe, and cheap modalities with excellent results in the treatment of cutaneous warts.

ABSTRACT

Background: Verrucae are benign tumors involving skin and mucous membrane that are caused by human papillomavirus (HPV). Several treatment modalities are available for treating extensive warts, but no single treatment is 100% effective. An ideal treatment should be effective, safe, with less morbidity and provide long-lasting immunity against HPV. This can only be achieved by stimulation of the immune system against the virus. The auto-implantation of warts is one such simple technique where the wart tissue is harvested as a donor and implanted into the uninvolved skin, thereby causing immune stimulation against the virus and resulting in the resolution of extensive and recalcitrant verrucae. Aim: The aim of the study was to evaluate the safety and efficacy of auto-implantation therapy in the treatment of extensiveand recalcitrant verruca vulgaris. Materials and methods: A hospital-based prospective interventional study was carried out on 50 clinically diagnosed patients of extensive verruca vulgaris. Homologous auto-implantation was done over the uninvolved flexor aspect of the forearm skin after harvesting wart tissue up to the level of the papillary dermis. Patients were assessed monthly and resolution after 3 months was considered as complete clearance. Results were analyzed using SPSS version 20.0. Results: Out of 50 patients, 41 patients came for regular follow up whereas 9 patients didn’t turn up for the follow-up. Out of 41 verrucavulgaris patients, 34 showed resolution within 3 months, accounting for a total clearance rate of 82.9%.

ABSTRACT

Objective To compare the effects of intramuscular or subcutaneous forearm parathyroid autotransplantation after total parathyroidectomy on patients with renal hyperparathyroidism.Methods From Jan.2012 to Dec.2013,total parathyroidectomy was carried out in 90 patients with renal hyperparathyroidism.According to the location of their parathyroid autograft,patients were divided into intramuscular group (n =39)and subcutaneous group(n =51).One patient in the intramuscular group and two in the subcutaneous group were excluded for the high levels of intact parathyroid hormone (iPTH) the day after operation.iPHT was analyzed 2 weeks,1 month,3 and 6 months after surgery.Results The time of autotransplantation was significantly shorter in subcutaneous group than in intramuscular group (11.46 (2.63) min vs 22.12 (3.78) min;t =0.632,P < 0.05).iPTH levels were significantly lower in subcutaneous group than in intramuscular group 2 weeks after operation (P < 0.05).There was no significant difference between the 2 groups regarding iPTH levels at 1 month,3 or 6 months after surgery.In the follow-up one patient in intramuscular group and one in subcutaneous group had graft-dependent hyperparathyroidism.Conclusion As compared to intramuscular parathyroid autotransplantation,subcutaneous parathyroid autotransplantation has advantages of simpler to operate,shorter autoimplantation time and easier to autograftectomy.

ABSTRACT

La reimplantación dentaria ha sido uno de los procedimientos más exigente de rigor y precisión en cuanto a las condiciones que deben existir para lograr el éxito esperado. El propósito de esta presentaciòn es mostrar la evolución y tratamiento del auto-implante del incisivo lateral superior izquierdo, 72 horas después de avulsionado. Se trata de paciente masculino de 18 años de edad, con antecedentes de salud. Acude a consulta luego de tres días de avulsionado el incisivo lateral superior izquierdo por trauma. Se le explicó al paciente el posible fracaso del tratamiento por el tiempo transcurrido, puesto que la inmediatez con que sea tratado favorece el éxito. Se le realizó el reimplante dentario y ferulizó. Se realizó el tratamiento endodóntico al diente en dos sesiones de una semana entre ellas. Se realizaron evoluciones trimestrales y luego semestrales. Casi cinco años después, la evidencia radiográfica no muestra complicaciones existentes y clínicamente presenta una apariencia normal. Los resultados obtenidos fueron satisfactorios y después de cinco años el paciente se mantiene con su diente en boca completamente asintomático(AU)


The dental re-implantation is one of the procedures demanding most rigor and precision in terms of the conditions required to achieve the expected success. Our purpose is to present the evolution and treatment of a case of auto-implantation of the upper left lateral incisor 72 hours after avulsion. The patient was a male 18 year-old with a history of ill health presents three days after avulsion of the upper left lateral incisor as a result of trauma. The patient was informed of the possible failure of the treatment due to the time elapsed. In cases such as this one, immediate treatment is essential to achieve success. Dental re-implantation and ferulization were performed. The tooth was treated endodontically on two visits one week apart. Checkups were conducted which were quarterly at first and then biannual. Almost five years later, radiographic evidence shows no complications and clinical appearance is normal. The results were satisfactory. The patient has preserved his tooth for five years and his status is totally asymptomatic(AU)

Background: In the treatment of multiple warts, there is no single treatment that is 100% effective and different modalities of treatment need to be combined. Aim: To evaluate the efficacy of homologous autoimplantation therapy in the treatment of multiple warts. Methods: A total of 60 patients of multiple verruca vulgaris and palmo-plantar warts were enrolled. Homologous autoimplantation was done after harvesting full-depth wart tissue. Patients were followed upto a period of 6 months. Resolution of warts within a period of 3 months after procedure was considered successful. Results: All the 60 patients were available for follow-up. A total of 28 patients of verruca vulgaris (70%) and 16 patients of palmo-plantar warts (80%) showed resolution of warts within 3 months, accounting for a total clearance rate of 73.3%. Majority of the responders (91%) showed resolution of warts within 2 months. Conclusion: This study demonstrates that homologous autoimplantation could be an effective, simple modality of treatment for multiple warts.
