ABSTRACT

Objective To explore the method that can block the infection route from stomach to lung so as to reduce the incidence of ventilator-associated pneumonia (VAP) in neonates.Methods Neonates with mechanical ventilation were randomly divided into two groups,the treatment group(40 cases) were given probiotics (Bifidobacteria) by nasogastric feeding,control group(40 cases) were given nothing.Prospective survey was performed on the incidence of VAP,gastric juice pH,gastric bacterial colonization,the homology between gastric bacterial colonization and VAP pathogen.Results The VAP incidence of treatment group and control group was 12.5% (5/40) and 47.5% (19/40) respectively,there was significant difference between the two groups ( x2 =10.21,P < 0.01 ).The onset time of VAP in treatment group [ ( 5.4 ±2.1 ) d] was later than that of control group[ (4.3 ± 1.0) d] ( t =3.2,P <0.01 ).The ratio of treatment group that gastric juice dropped to pH≤3 (80.0%,32/40) was higher than that of control group(47.5%,19/40) ( x2 =11.5,P < 0.01 ) ; and the ratio of gastric bacterial colonization in treatment group ( 30.0%,12/40) was lower than that in control group(80.0%,32/40) ( x2 =16.9,P <0.01 ).The homology ratio between VAP pathogen and colonization bacteria in treatment group (40.0%,2/5 ) was lower than that in control group (78.9 %,15/19 ) ( x2 =8.7,P < 0.01 ).Conclusion Probiotics can reduce gastric juice pH of neonates and inhibit gastric bacterial colonization.The onset time of VAP can be delayed,the incidence of VAP can be reduced effectively.

ABSTRACT

Objective To investigate whether antiseptic central venous catheters (CVC) modified with chlorhexidine acetate and silver sulfadiazine can be beneficial in reducing bacterial colonization and catheter-related infection. Methods Prospective controlled non-blinded randomized clinical trial was conducted. Seventy adult inpatients with CVC from intensive care unit of General Hospital of Chinese People's Armed Police Force during June 2007-June 2009 were enrolled. Their baseline characteristics, APACHE Ⅱ score and therapeutic interventions were comparable. Patients were randomly received either an antiseptic CVC ( antiseptic group, n = 28) or a standard two-lumen CVC ( control group, n = 42 ). Microbiological evaluation was done after CVC removal. A catheter bacterial colonization ( CBC) was considered if bacterial growth of > 15 CFU was found by semi-quantitative roll-plate technique from a proximal or distal catheter segment. A catheter-related infection ( CRI) was defined as a colonized catheter with local signs of inflammation. A catheter-related bloodstream infection ( CR-BSI) was defined as a colonized catheter with isolation of the same organism from the patient' s blood with accompanying clinical signs of infection. SPSS 11.5 software was used for statistical analysis. Kaplan Meier curve was used to evaluate the association between CVC retention time and bacterial colonization or infection, and Log-rank test was performed to compare between the groups. Results CVC was removed from 3.6% (1/28) patients of antiseptic group and 21.4% (9/42) patients of control group because of infection (x2 = 5. 143, P 2 weeks, the colonization and infection will increase significantly in both standard or antiseptic CVC, so to shorten the insertion time is an effective measure to decrease the CVC-related infection.

ABSTRACT

OBJECTIVE To study the cause of bacterial infection of ventilator-associated pneumonia(VAP) in intensive care unit(ICU) and summarize effective methods to prevent and control the infection.METHODS Epidemiologic study on 300 patients with VAP in ICU from Dec 1,2003 to Jul 13,2006.Preventing and controlling strategy was as follows.RESULTS Pathogenic bacteria of VAP in ICU mostly were multidrug-resistant ones,of which the G-were 56.3%,G+ were 23% and fungi were 13.7%.CONCLUSIONS To control VAP in ICU proper technique and method are important.Management of hospital infection and related training of staff in ICU are the basic way.

Object To explore the bacteriostatic and anti inflammation activities of ultrasonically nebulized Houttuynai cordata Thunb in patients after pneumonectomy Methods The patients were randomized into 2 groups Patients of the experimental group, inhaled ultrasonically nebulized H cordata, while patients in the control group inhaled ultrasonically nebulized vapors without the addition of H cordata Results Total leucocyte counts on the third and fourth day in the experimental group were significantly lower than that in the control group Neutrophil ratio in experimental group was also lower in all the 5 days of treatment although nonsignificantly Meanwhile, occurrence of Gram negative bacilli colonization in experimental group occurred less than that in the control group But incidence of fungi colonization appeared higher Conclusion Inhalation of ultrasonically nebulized H cordata can ease the inflammation and inhibit the colonization of Gram negative bacilli in respiratory tract
