ABSTRACT

Enterococci are Gram-positive cocci that are normal inhabitants of the urogenital and intestinal tracts of human beings and animals. Of the pathogenic species of Enterococci, Enterococcus avium is an infrequent cause of human infections. This report is an atypical case of bed-sore infection caused by E. avium in an elderly male with multiple comorbidities. Although the patient was given antibiotics along with surgical debridement, he passed away. This case calls attention to the pathogenic prospect of E. avium in clinical settings

ABSTRACT

Background: Development of a bedsore is always avoidable still its worldwide prevalence fluctuates between 0%-56%. Bedsores are maximally observed in patients who are on long term care, in intensive care, immuno-compromised or on home care.Methods: In a tertiary care center-JK hospital, Bhopal, a longitudinal observational study was done for a duration of 3 years, on all registered in-patients, to understand the trend of bedsore rates on monthly basis.Results: In our study, even though, on yearly basis, average of rates is showing a declining trend in favorable manner yet higher statistical analysis (ANOVA test p value = 0.054) has decrypted the non-significant results.Conclusions: Timely and appropriate prophylactic interventions are needed to overcome the increased bedsore rates. Checklist based surveillance and monitoring should be a pressing priority. Cochrane reviews are also suggestive of incorporation of organizational changes, risk assessment tools, wound care teams and education to reduce the incidence of pressure ulcers.

ABSTRACT

Objective To explore the clinical effects and application value of VSD combined with continuous hypoperfusion of ozone water in treating deep bedsore.Methods 40 patients with deep bedsore admitted to our hospital from June 2014 to December 2016 were randomly divided into the observation group (n=20) and the control group (n=20).The control group was treated with VSD while the observation group was treated with VSD combined with continuous hypoperfusion of ozone water.Results Compared with the control group, the wound healing time in the observation group was shortened significantly (P<0.001).The positive rate of bacteriology inspection about the wounds was decreased significantly (P<0.05).The density of capillary blood under microscope was increased significantly (P<0.05).Conclusion The clinical effects of VSD combined with continuous hypoperfusion of ozone water in treating deep bedsore is better than VSD alone apparently.It is worth clinically popularizing.

ABSTRACT

Objective To design an inflatable bed aerator to replace the traditional inflator pump.Methods The aerator used MCS-51 SCM micro control unit as the core component and compressed air as the air source,which was composed of the modules for main control,power supply,air pressure detection,liquid crystal display,button as well as real-time clock and alarming.Results The aerator gained the same efficacy as the traditional inflator pump,while the noise was decreased to the patient and medical staffs,which behaved particularly well for the bedsore prevention of the critical patient.Conclusion The aerator has simple structure,easy operation and high stability,which can be used for the inflation of two air cushions simultaneously and for timely alarming against failures,and thus is worthy promoting practically.

ABSTRACT

Background: Bedsores are injuries to skin and underlying tissues caused by prolonged pressure. Although, some methods like frequently changing position and using particular tools are used for preventing and caring of pressure ulcers, their prevention would still be as a vital urgency. Olive oil is an herbal product with potential preventive effects to prevent bedsores because of its numerous medical properties. The major aim of this study was to determine whether topical olive oil can prevent the risk of developing bedsores in I.C.U. patients. Methods: This clinical trial was accomplished in 2014 with 60 patients who were selected by simple random sampling method into intervention and control groups. During three weeks, the control group had received routine skin care, while the intervention group had received topical Olive oil in addition to the routine care. Statistical analysis, including Chi-square, T-test and Fisher’s tests, were performed with SPSS version 20 software. Results: The intervention group had contained 19 male and 11 female, with an average age of 60.46 ± 18.06 and the control group had contained 20 male, 10 female, with an average age of 50.96 ± 21.38. Five patients (16%) who received routine care plus topical olive oil had developed bedsore after an average of 18.73 ± 5.36 days and twelve patients (40%) who received routine care only had developed bedsore after an average of 15.46 ± 7.40 days. The risks of developing bedsores between two groups were statistically significant (P=0.03). Conclusions: Our study has revealed potential effects of topical olive oil to prevent bedsores in I.C.U. patients.

ABSTRACT

Objective To investigate the effect of health education in the community of high risk group of pressure sore , and to provide a reference for the formulation of community health education strategies .Methods 148 subjects with NORTON score discharge assessment results less than or equal to 14 points were randomly divided into observation group and control group with 74 cases in each group.The control group received routine discharge instructions.The observation group in the conventional discharge guidance on the implementation of the home, regular and telephone guide.The self-made pressure ulcer prevention knowledge questionnaire were com-pared between the two groups at the time of discharge and intervention for caregivers to grasp the situation of pressure ulcer prevention knowledge after 3 months;The incidence rate of discharged patients in the two groups after the comparison of pressure ulcer were com-pared.Results After the intervention, the observation group significantly increased the pressure ulcer prevention, which was signifi-cantly higher than before the intervention and the control group after intervention (all p<0.05);In the observation group was discharged after 3 months, 6 months and 9 months were no bedsore.Control group 3 months after discharge pressure sore the incidence rate was 8. 11%.6 months after discharge pressure ulcer incidence rate was 16.22%.9 months after discharge pressure ulcer incidence was 22. 97%.The observation group of 3 months, 6 months, 9 months after hospital discharge rate of pressure ulcer was significantly lower than that of control group (all p<0.05).Conclusion Health education is helpful to improve caregivers knowledge related to pressure ulcer prevention, reduce the risk of high-risk population community bedsore, and is worthy of popularization and application.

ABSTRACT

Objective To study antibacterial effect of formulas, borneol, honey, gentamicin in wet honey ice bedsores.Methods The third-stage infective bedsore model in rabbit were estabished with Staphylococcus aureus ( S.aureus) , Escherichia coli ( E.coli) , and Pseudomonas aeruginosa ( P. aeruginosa).The rabbits with bedsore were respectively compressed with gauze of honey+borneol, honey+gentamicin,borneol+gentamicin,borneol+gentamicin+honey, vaseline as control group.The strain identification and colony counts were observed before and after treatment.ResuIts The colony count of S.aureus, E.coli and P.aeruginosa in honey+borneol, honey+gentamicin, borneol+gentamicin, borneol+gentamicin+honey post-treatment was significantly higher than that of pre-treatment, respectively (P<0.05).The colony count of three strains in above four formulas post-treatment was significantly lower than that in vaseline group, respectively (P <0.05).The colony count of three strains in borneol +gentamicin +honey (original formula) post-treatment was significantly lower than that in the other formulas, respectively (P<0.05).ConcIusion The antibacterial effect of borneol+gentamicin+honey (original formula) is the best.

ABSTRACT

Objective To explore the clinical effect of application of hospital information system in bedsore nursing process analysis.Methods To combine process management and continuous quality improvement organically,establish a complete set of the pressure ulcer management monitoring process,which was completed,networking,continuous for improvement and this process was used for the implementation of high-risk bedsore warning screening and bedsore informing,hospitalized patients with pressure ulcers online report system and level 3 monitoring system,bedsore nursing consultation system and pressure ulcer nursing intervention.Results The application of informatization in the pressure ulcer management process could effectively reduce the incidence of pressure ulcers,prompt bedsore nursing management level to be more standardized,scientific,and modernization.At the same time,it improved the patients' satisfaction degree with nursing work,meanwhile it let managers found that carrying out of pressure sores extended nursing service was imperative.Conclusions Application of hospital information system in bedsore nursing process analysis is satisfying,which is worthy of further development.

ABSTRACT

Background: To evaluate efficacy and safety of ampucare, a polyherbal product, in patients with bedsore. Methods: One hundred patients, either sex, more than 18 years of age, with bedsore were divided in to two groups of 50 each. Group-I- Served as control- povidone iodine solution was applied locally on the bedsore, once daily. Group- II- Treatment group- ampucare lotion was applied locally, once daily. Primary end point was time to wound healing and secondary end point included reduction in wound surface area at day-7 and at treatment completion. Percent patients cured, improved, or treatment failure were noted. Depth of wound, % healing and adverse effects were recorded. Results: Application of ampucare in patients with bedsore resulted in rapid healing as compared to control group. Reduction in wound surface area was significant (p<0.01) in group-II. Maximum gain in thickness of granular tissue was observed in this group. In treatment group 68% patients showed excellent response as compared to 60 % in control group. Ampucare was well tolerated. Conclusions: Ampucare treatment markedly accelerates wound healing in patients with bedsore.

ABSTRACT

Objective To study the effect of cream of bedsore on the treatment of severe bedsore.Method One hundred and twenty patients were randomly divided into treatment group and control groups Ⅰ and Ⅱ,respecially 40 patients,given systemic treatment.Results The decubitus wound healing of treatment group was significantly better than the other two groups,the total effective rate being 85%,P < 0.01.In the treatment group,pathogenic bacteria culture positive rate on decubitus wound was significantly lower than the two control groups,inhibition rate being 78%,P < 0.05.Staphylococcus aureus,Pseudomonas aeruginosa,Escherichia coli and so on were inhibited.Conclusions In the treatment group,bedsore healing ointment was significantly better than Western medicine used in the control groups.It can improve the microcirculation of the blood perfusion,promote the establishment of collateral circulation,increase the affected area nutrition,improve the local tissue ischemia,hypoxia-induced metabolic disorders,and remove the necrotic tissue,and promote tissue repair and regeneration,the granulation growing,and the effect of epithelial healing.

ABSTRACT

Objective To observe clinical effect of treating bedsore with myijinhuang powder. Methods 35 patients with bedsore were randomly divided into a control group and a treatment group. The treatment group was treated with ruyijinhuang powder, and the control group was treated with routine western medicine. The clinical effect was observed in both groups. Results The clinical total effective rate in the treatment group was significantly higher than that of the control group (P<0.05) . Conclusion The treatment of bedsore with ruyihuangjin powder has good effect.

ABSTRACT

Objective To study the nursing of bedsore prevention in patients with unstable pelvic fracture. Methods Bedsore forewarning risk assessment went on in 39 patients with unstable pelvic fracture by Braden scoring method. Results No bedsore was observed in these 39 patients with unstable pelvic fracture.The nursing result turned out to be satisfactory. Conclusions The incidence rate of bed-sore can be degraded in patients with unstable pelvic fracture by forewarning risk assessment with Braden scoring method and early nursing intervention.

ABSTRACT

Objective To determine the curative effect of a trigeminy treatment including gen-tamycin, albumin and local oxygen therapy on treatment of ulcerative bedsore. Methods 50 cases of hospitalized patients in our hospital with ulcerative bedsore during January,2005 to December,2007 were selected. The patients were randomly classified into the experimental group and the control group with 25 patients in each group. The patients in the experimental group were treated by trigeminy treat-ment after debridement, while the patients in the control group were treated by covering with gen-tamycin and bandaging with dressing after debridement.The therapeutic effect of the two groups was compared by χ2 test. Results After the treatment, 6, 10, 6 and 3 patients in the experimental group, were healing, excellence, utility and inefficacy, respectively. On the contrast in the patients of the con-trol group, 2, 8, 5 and 10 were judged as healing, excellence, utility and inefficacy, respectively. The experimental group showed better effect than the control group. Conclusions The trigeminy treat-ment is able to remarkably increase clinical treatment effect of ulcerative bedsore.

ABSTRACT

Necrotizing fasciitis is a progressive soft tissue infection involving the superficial fascia which can be a lethal disease without proper treatment. The predisposing factors of necrotizing fasciitis are diabetes mellitus, alcohol abuse, drug abuse, obesity, malnutrition and immunocompromised state. Most of the recently reported cases of necrotizing fasciitis were caused by perianal disease. We treated a young woman with necrotizing fasciitis secondary to infected bedsore who had been held in hospital for 15 years due to schizophrenia and epilepsy. Here, we emphasize the specific concern when treating patients with bedsore.

@#ObjectiveTo investigate the tossing and turning time of the stuporous patients while using the bedsore mat blowing out gas.Methods45 cases were as the control group and treated with routine nursing. 117 cases were as the test group, in them, 17 cases were tossed and turned once every 2 hours, 50 cases tossed and turned once every 4 hours and 50 cases tossed and turned once every 6 hours. The patients of the test group were nursed with the bedsore mat blowing out gas as well as routine nursing. The rates of bedsore of two groups were compared.ResultsThe bedsore rate of the control group was 4.44%, 2 hours group was 0%, 4 hours group was 4% and 6 hours group was 22%; there was no significant difference among the control group, 2 hours group and 4 hours group, but there was a significant difference between 2 hours group, 4 hours group and 6 hours group (P<0.01).Conclusion4 hours is suitable for the stuporous patients to toss and turn while using the bedsore mat blowing out gas.

ABSTRACT

OBJECTIVE: To investigate the characteristics, predictors, and consequences of pressure ulcers and to provide prospective epidemiologic data. METHOD: The prospective data of 100 patients with pressure ulcers were collected who were admitted to the department of rehabilitation medicine of 4 hospitals from 2002 June to 2003 September. We have collected the informations on clinical features of pressure ulcer prospectively. RESULTS: Quadriplegia/Tetraplegia was the most commonly involved type of injury, followed by hemiplegia, paraplegia. Most patients developed pressure ulcer before they were transferred to the rehabilitation unit. The average Braden scale scores was 13.06+/-3.46 and were 16 point or below in 80% of patients, and this point was considered as the cut-off score of the patients with high risk. Sacrum was the most common site of the ulcers and most ulcers were 2nd and 3rd stage. The ulcers were treated with conservative (82%) or surgical management (18%). The more severe and larger ulcers required the surgical management. CONCLUSION: The results of this prospective study on clinical features would be helpful for the understandings, prevention and management of pressure ulcers.
