O aumento da mortalidade na categoria dos transtornos mentais e comportamentais - face à redução progressiva das internações hospitalares promovida pela reforma psiquiátrica em curso - tem aquecido o debate entre os proponentes da reforma e parte da comunidade psiquiátrica brasileira. Neste artigo, analisa-se os argumentos dos reformistas.


El incremento de la mortalidad en la categoría desordenes mentales y del comportamiento - concerniente a la reducción progresiva de camas psiquiátricas - tiene acalorado la discusión entre los que proponen la reforma y parte de la comunidad psiquiátrica brasileña. En este artículo se analizan los argumentos de los reformistas.


L'augmentation de la mortalité dans la catégorie des désordres mentaux et comportementaux - face à la réduction progressive des lits psychiatriques préconisée par la réforme psychiatrique en cours - a attisé le débat entre les réformistes et une partie de la communauté psychiatrique brésilienne. Cet article analyse les arguments des réformistes.


The rising death rate in the category of mental and behavioral disorders - related to the progressive reduction in the number of psychiatric beds - is bringing new elements to the discussion between reformists and some sectors of the Brazilian psychiatric community. In this paper, the arguments of the reformists are analyzed.

ABSTRACT

OBJECTIVE: The aims of this study were to (1) validate the inter-rater reliability of the BEHAVE-AD, Korean version, to (2) analyze the quantitative relationship between severity of Alzheimer's disease (AD) and mean scores on each of the BEHAVE-AD categories and mean total BEHAVE-AD score. DESIGN: Cross-sectional study of geriatric patients with AD evaluated at a mental hospital for the elderly. SAMPLE: Fifty-two consecutive patients diagnosed with probable Alzheimer's disease (AD) according to NINCDS-ADRDA diagnostic criteria. RESULTS: In reliability study, significant correlations were obtained for all BEHAVE-AD symptoms category scores and for mean total BEHAVE-AD scores. Analysis of BEHAVE-AD scores as a function of disease severity demonstrated a non-linear relationship between severity of behavioral symptoms and the global and cognitive advance of AD. Score analysis of the BEHAVE-AD indicates that these behavioral disturbances become most severe in the moderate and moderately severe stages of AD. CONCLUSION: All the results proved the cross-cultural validity and reliability of the BEHAVE-AD, Korean version. Furthermore, these results have implications for the methodology of pharmacological trials of putative cognitive enhancer compounds in AD.

ABSTRACT

Behavior disturbances in dementia have many causes, and it is difficult to judge the effectiveness of any method as well as to classify and choose the right agents or methods of treatment. Because agitated behavior disturbances are intermittent phenomena, it is not certain that the change of symptom is response of treatment or incidental. We should help elderly people with risk of dementia, including the preventive intervention and various treatment strategies for the last stages of life. To the comprehensive treatment of behavioral disturbance of patients with dementia, adequate medical resources are essential through organized medical delivery system as well as environmental modification, family management and pharmacotherapy.
