ABSTRACT

Vivax malaria is in general described and considered as benign as it less likely causes severe illness, compared to malaria caused by Plasmodium falciparum (P.falciparum)species. Of late, there have been increasing evidences of Plasmodium Vivax (P.vivax) too causing severe disease and leading to poor outcomes.. We report a case of severe P vivax malaria in a 12 year old child complicated by Acute respiratory distress syndrome(ARDS)

ABSTRACT

Introduction: Fibrous dysplasia is an uncommon, benign pathology that affects the bones and is characterized by the gradual replacement of bone tissue by a fibrous one, creating aesthetic and functional bone disorders. Histologically, it presents an excessive proliferation of the mesenchymal cells that form the bone. Objective: To know the aspects related to decision making, regarding surgical treatment or the option of carrying out clinical and radiographic controls of fibrous dysplasia. Case presentation: We report two cases of pediatric patients who attended the Children´s Hospital Napoleón Franco Pareja (HINFP) in Cartagena-Colombia presenting fibrous dysplasia that produced unilateral facial asymmetry compromising the aesthetics of patients, but reporting no painful symptoms. We describe the characteristics of the pathology and the basic useful criteria in making therapeutic decisions, with either conservative radical surgical approach or only performing a clinical and radiographic control of the lesion. Conclusions: Clinical criteria such as facial asymmetry, aesthetic deformity, and functional commitment are basic aspects in the therapeutic option for patients with in fibrous dysplasia(AU)

ABSTRACT

Diabetic mastopathy affects premenopausal women with longstanding type 1 diabetes mellitus. The diagnosis is based on clinical findings (uni or bilateral hardened, palpable mass) associated with radiological (increase in breast density), sonographic (marked posterior acoustic shadowing), and histopathological (fibrosis and perivascular and periductal lymphocytic infiltration) findings. This disease may clinically simulate a breast carcinoma. The case of a patient with diabetic mastopathy is reported.

ABSTRACT

Objective To study the value of laparoscopic cholecystectomy (LC) for treating gallbladder benign diseases in aged patients. Methods Laparoscopy cholecystectomy (3-port or 4-port) was performed in 2335 senile patients with gallbladder benign diseases from December 1992 to May 2005. Results Five patients were found as having the Mirizzi syndrome and were given a subtotal cholecystectomy. In 18 patients with a frozen Callot’s triangle, the anterior wall of the gallbladder was resected and the mucous membrane on the posterior wall was electrocauterized. LC was completed smoothly in the remaining 2312 patients. No conversions to open surgery were needed. The operation time was 8~55 min (mean, 19 min), and the intraoperative blood loss was 1~ 50 ml (mean, 8 ml). An abdominal drainage tube was placed in 213 patients, including double-cannula lavage in 56 patients. The length of hospital stay was 4~16 d (mean, 6 d). Follow-up examinations for 10 days ~13 years in 1859 patients showed no biliary stenosis, gallbladder pouch, or residual stones. Conclusions Laparoscopic cholecystectomy is safe and feasible in aged patients, being the first choice of operative method. Proper perioperative management, careful prevention co-morbidities, and control of operation time as short as possible are important for senile patients.

ABSTRACT

BACKGROUND: Perioperative morbidity and mortality following a pancreaticoduodenectomy have decreased markedly over the last two decades, so many surgical centers advocate expanding the indications for a pancreaticoduodenectomy to include benign lesions other than periampullary malignancies. METHODS: Between 1990 and 1997, 22 patients underwent a pancreaticoduodenectomy (9 classical Whipple's operations, 10 pylorus-preserving pancreaticoduodenectomies and 3 duodenum preserving resection of the head of the pancreas) for benign disease from among a total of 306 patients receiving pancreaticoduodenectomy. The postoperative morbidity and mortality were analyzed retrospectively. RESULTS: The sites of the lesions were the pancreatic head (n=12), the common bile duct (n=5), the duodenum (n=3) and the ampulla of Vater (n=2). The most frequent indication for pancreaticoduodenectomy was a suspicion of malignancy. There was no immediate postoperative mortality, and perioperative complications occurred in 50% of the patients (11/22 patients). During the follow-up period (mean follow-up: 23.8 month), there were no complications or mortalities related to either disease or the operation. CONCLUSIONS: This study suggests that a pancreaticoduodenectomy should be recommended for periampullary benign disease if malignancy cannot be excluded or if the lesion cannot be removed without the procedure. However, perioperative morbidity and mortality are still higher for a pancreaticoduodenectomy than for other procedures, so patient selection and perioperative management should be careful, also, a more conservative procedure than the classical Whipple's operation should be considered for benign lesions, if possible, for better quality of life.
