ABSTRACT

El presente artículo presenta las recomendaciones de un grupo de expertos venezolanos reunidos en Caracas en Octubre de 2009 por la Sociedad Venezolana de Puericultura y Pediatría en relación al tratamiento farmacológico del asma infantil. Se presentan los objetivos del tratamiento, los medicamentos recomendados, la terapia inicial y de mantenimiento y el tipo de dispositivo para la administración de drogas por vía inhalatoria, de acuerdo a la edad del paciente.


This article presents the recommendations made by a group of Venezuelan experts during a consensus meeting held in Caracas in October, 2009, under the auspices of the Venezuelan Society of Pediatrics, on the pharmacological treatment of childhood asthma. Goals of treatment, recommended medications, initial and maintenance therapy, and devices utilized for delivery of inhaled drugs according to patient’s age are presented.

ABSTRACT

O objetivo do trabalho foi avaliar o desempenho e as características quantitativas de carcaça de suínos machos castrados, mantidos em ambiente de conforto térmico e alimentados com dietas contendo diferentes níveis de ractopamina. Foram utilizados 48 animais, com peso médio inicial de 67,0±2,7kg, distribuídos em delineamento de blocos ao acaso composto por quatro níveis de ractopamina (0, 5, 10 e 20ppm) e seis repetições, com dois animais cada. O período experimental teve duração de 28 dias. A temperatura do ar obtida foi de 20,9±2,9°C, a umidade relativa foi de 86,7±7,3 por cento, e o Índice de Temperatura de Globo e Umidade foi de 69,6±4,0. O peso final dos animais e o consumo de ração não foram influenciados (P>0,05) pelos tratamentos. Os níveis de ractopamina influenciaram (P<0,05) o ganho de peso diário e a conversão alimentar dos suínos. Observou-se redução linear (P<0,05) da espessura de toucinho e aumento da profundidade de músculo e da porcentagem de carne magra na carcaça de acordo com o aumento dos níveis de ractopamina na dieta. O peso e o rendimento de carcaça quente e a área de olho de lombo não foram influenciados (P>0,05) pelos tratamentos. Concluiu-se que o nível ótimo de inclusão de ractopamina na dieta de suínos machos castrados e mantidos em ambiente termoneutro é de 20ppm.


The objective of this study was to evaluate the performance and the quantitative characteristics of barrow's carcass maintained under thermoneutral environment and fed with diets containing different ractopamine levels. Forty-eight barrows were used, with initial weight of 67.0±2.7kg, distributed in a randomized blocks design with four ractopamine levels (0, 5, 10 and 20ppm) and six replicates with two animals each. The experimental period lasted 28 days. The average temperature recorded was 20.9±2.9°C, relative humidity was 86.7±7.3 percent and BGHI was 69.6±4.0. The final weight of the animals and feed intake were not influenced (P>0.05) by treatments. The ractopamine levels influenced (P<0.05) the daily weight gain and feed conversion. There was a linear reduction (P<0.05) of backfat thickness, increase of muscle depth and the carcass lean meat percentage in accordance with the increasing of ractopamine levels in the diet. The weight, carcass yield and loin eye area were not influenced (P>0.05) by treatments. It was concluded that the ractopamine optimal level of inclusion in the diet of barrows kept under thermoneutral environment is 20ppm.

ABSTRACT

O objetivo do trabalho foi avaliar o desempenho e as características quantitativas de carcaça de suínos machos imunocastrados, castrados e fêmeas, na fase de terminação, alimentados com dietas contendo 10ppm de ractopamina. Foram utilizados 120 animais, com peso inicial de 99,1±5,3kg, distribuídos em delineamento de blocos ao acaso, em esquema fatorial 2 x 3 (dois níveis de ractopamina, 0 e 10ppm e três sexos), cinco repetições com quatro animais cada. A inclusão de 10ppm de ractopamina na dieta melhorou (P<0,05) a conversão alimentar dos suínos, independentemente do sexo. A inclusão de ractopamina prejudicou (P<0,05) o ganho de peso dos machos imunocastrados e aumentou (P<0,05) o ganho de peso dos machos castrados e das fêmeas, sem, no entanto, afetar (P>0,05) as características quantitativas de carcaça dos suínos. O sexo foi determinante para o desempenho e as características quantitativas de carcaça; as fêmeas apresentaram rendimento de carcaça quente superior (P<0,05) aos machos imunocastrados, que apresentaram rendimento superior (P<0,05) aos machos castrados. Os machos imunocastrados e as fêmeas apresentaram maior (P<0,05) percentual de carne magra em relação aos machos castrados. Os resultados desta pesquisa evidenciam a necessidade de estudos visando à adequação dos níveis nutricionais das dietas contendo ractopamina para suínos imunocastrados.


The objective of this study was to evaluate the performance and the quantitative characteristics of carcass of immunocastrated, barrows and females in the finishing phase fed with diets containing 10ppm of ractopamine. One hundred and twenty animals were used, with initial weight of 99.1±5.3kg, distributed in a randomized blocks factorial design in 2 x 3 (two ractopamine levels, 0 and 10ppm and three sex), five replicates of four animals each. The inclusion of 10ppm of ractopamine in the diet improved (P<0.05) feed conversion in pigs, independent of sex. The inclusion of ractopamine damaged (P<0.05) the weight gain of the immunocastrated and increased (P<0.05) the weight gain of the barrows and the females, without, however, affect (P>0.05) quantitative carcass characteristics of pigs. The sex was determining factor in performance and carcass quantitative traits and the females had higher (P<0.05) hot carcass yield for immunocastrated male and they have yield above (P<0.05) the barrows. Immunocastrated males and females had higher (P<0.05) percentage of lean meat for barrows. The results of this study highlight the need for studies to adjust the nutrient levels of diets containing ractopamine to immunocastrated pigs.

ABSTRACT

Nos países em que é comercializada, a administração precoce de levosimendana deve ser considerada em pacientes que permanecem sintomáticos e com dispnéia em repouso apesar da terapia inicial, principalmente aqueles com história de insuficiência cardíaca crônica ou em tratamento prolongado com betabloqueadores. Pacientes hipotensos ou com isquemia ativa não são os melhores candidatos para receber infusão de levosimendana e precisam, primeiro, ter esses problemas tratados.


In countries where it is available, early levosimendan infusion can be considered for patients who remain symptomatic with dyspnea at rest despite initial therapy, particularly those with a history of chronic heart failure or chronically treated with beta-blockers. Hypotensive patients or patients with active ischemia are not the best candidates for levosimendan administration and should have these problems addressed first.

ABSTRACT

Objetivo: Evaluar la eficacia de salbutamol y dexametasona nebulizados comparados con salbutamol solo en pacientes con bronquiolitis aguda. Material y métodos: Estudio clínico controlado, aleatorizado, en pacientes de 1 a 18 meses de edad con diagnóstico de bronquiolitis aguda y evolución menor de tres días. Se asignaron dos grupos: 1) salbutamol (n = 24) o 2) salbutamol más dexametasona (n = 25) administrado por micronebulizaciones cada cuatro horas por 24 horas. Se midieron las frecuencias cardiaca y respiratoria, la gravedad del cuadro mediante el Respiratory Distress Assigment Index, el Silvermann-Andersen, saturación de O2 y tiempo de desaparición de la dificultad respiratoria y frecuencia de altas en 24 horas. Resultados: No se encontraron diferencias significativas entre los grupos de estudio, excepto en la frecuencia de altas hospitalarias en las primeras 24 horas, 75 vs 96 % p < 0.04 respectivamente. Conclusiones: La aplicación de salbutamol más dexametasona es más rápida para controlar la dificultad respiratoria en niños con bronquiolitis aguda en comparación con la aplicación de salbutamol solo.


OBJECTIVE: Asess the efficacy of nebulized salbutamol and dexamethasone compared with nebulized salbutamol, in patients with bronchiolitis. MATERIAL AND METHODS: A blinded clinical trial was performed with 49 patients between 1-18 months diagnosed with bronchiolitis with three days or less of disease evolution. Participant's parents signed an informed consent and patients did not receive prior medication. Patients were randomly assigned to two groups: nebulized salbutamol or salbutamol plus dexamethasone, which they received every four hours during twenty-four hours. We measured heart and respiratory rate; respiratory distress index, oxygen saturation, and Silverman Andersen scores. RESULTS: No significant differences were found between groups for treatment response; but the frequency of hospital release at 24 hrs was significant among groups; 75 vs 96 % respectively (p = 0.04). CONCLUSIONS: The administration of salbutamol plus dexamethasone is more effective in the control of respiratory distress in children with bronchiolitis compared with the use of salbutamol alone.
