ABSTRACT

La pandemia de COVID-19 ha puesto en jaque a los sistemas de salud en el mundo; la vinculación entre la investigación biomédica y la práctica asistencial ha probado ser un requisito fundamental para dar respuesta a la misma de manera eficiente y rápida. En este sentido, los biobancos se constituyen como un componente clave ya que favorecen el almacenamiento de grandes volúmenes de muestras biológicas gestionadas en base a criterios que garanticen su óptima calidad, armonización y seguridad, respetando requisitos éticos y legales que aseguran los derechos de los ciudadanos. La cesión de estas muestras a distintos grupos de investigación promueve el desarrollo de nuevas herramientas diagnósticas y terapéuticas y vacunas. Frente a la llegada del SARS-CoV-2 a la Argentina, el Biobanco de Enfermedades Infecciosas estableció rápidamente la colección COVID-19 constituida por muestras de plasma, suero y células mononucleares de sangre periférica de personas cursando la enfermedad o recuperadas. En solo seis meses se enrolaron 825 donantes, lo que significa alrededor de 14.000 viales de material biológico almacenados y a disposición de los investigadores que lo soliciten. A tal efecto, se realizaron seis actos de cesión a diversos grupos pertenecientes a instituciones de investigación, mientras que tres se encuentran en evaluación. Las muestras cedidas han permitido, por ejemplo, el desarrollo de kits serológicos de producción nacional; lo que pone de manifiesto que el rápido establecimiento de esta colección, bajo un sistema de gestión eficiente, constituye una herramienta muy valiosa en la respuesta a esta nueva enfermedad


The COVID-19 pandemic has driven an unprecedented health crisis. Cooperation between biomedical research and healthcare practice has been shown to be a fundamental requirement to provide an efficient and timely response. In this regard, biobanks are key components since they allow the storage of large volumes of biological samples with guaranteed optimum quality, harmonization and safety, ensuring ethical and legal requirements which protect citizen rights. The transfer of these samples to different research groups fosters the development of new diagnostic and therapeutic tools as well as vaccines. Upon SARS-CoV-2 arrival to Argentina, the Biobank of Infectious Diseases rapidly established the COVID-19 collection comprised by plasma, serum and peripheral blood mononuclear cells samples obtained from people within the acute phase of the infection or who have already recovered. In only 6 months, 825 donors were enrolled, representing around 14,000 vials of biological material stored and available to researchers who might require it. In this line, 6 transfer agreements have been already performed to different groups belonging to national research institutions, while 3 are under evaluation. The transferred samples have allowed, for instance, the development of nationally produced serologic kits, which shows that the rapid establishment of this collection, under an efficient management system, represents a highly valuable tool in the response to this new disease.

ABSTRACT

Knowledge about reproduction of white-lipped peccary is of great importance to assist with the conservation of this species and enable its rational use in captivity. This study aimed to evaluate the effect of ACP-103®, ACP-116® and BTS semen extenders on sperm viability during cooling of Tayassu pecari semen. Five ejaculates from four adult males were chilled. The animals were submitted to the protocols of sedation and anesthesia for semen collection by the electroejaculation method. After collection, the semen was macro- and microscopically assessed and diluted to reach 35x106 spermatozoa/mL in each of the three different extenders tested. The fresh-extended semen was packed in a BotuFLEX® thermal box to keep samples at 15°C for 24 hours. After cooling, the following semen parameters were analyzed: sperm motility, functional and structural integrity of sperm membranes, mitochondrial activity, chromatin condensation, and the thermoresistance test was performed. The parameters sperm motility, structural and functional integrity of sperm membranes, mitochondrial activity, and chromatin condensation were preserved after use of the extenders tested, and were similar to those of in natura semen (p>0.05). Curvilinear velocity (VCL) (p<0.05) was the only parameter with reduced values after cooling regardless of the extender used. The percentage of sperm with normal morphology was greater in samples cooled using the BTS extender (p<0.05). The ACP-103®, ACP-116® and BTS extenders can be used for the cooling and preservation of white-lipped peccary semen at 15°C for 24 hours.(AU)


Para auxiliar na conservação da espécie e permitir o uso racional do queixada em cativeiro é de grande importância o conhecimento sobre a reprodução da espécie. Objetivou-se avaliar o efeito dos diluidores de sêmen ACP-103®, ACP-116® e BTS na viabilidade espermática durante a refrigeração do sêmen do Tayassu pecari. Foram refrigerados cinco ejaculados provenientes de quatro machos adultos. Os animais foram contidos com auxílio de puçá e submetidos ao protocolo de sedação e anestesia para realização da coleta de sêmen pelo método da eletroejaculação. Depois da coleta, o sêmen foi avaliado macro e microscopicamente e diluído para atingir 35x106 espermatozoides/mL em cada um dos três diferentes diluidores testados. O sêmen diluído foi acondicionado em caixa térmica BotuFLEX® para manter as amostras a 15°C por um período de 24 horas. Depois da refrigeração, os espermatozoides foram avaliados quanto aos parâmetros de movimento espermático, integridade funcional e estrutural das membranas espermáticas, atividade mitocondrial, condensação da cromatina e teste de termorresistência. Os diluidores testados preservaram as características cinéticas, a integridade estrutural e funcional das membranas espermáticas, a atividade mitocondrial e a condensação da cromatina semelhante ao sêmen in natura (P>0,05). O único parâmetro que reduziu com o processo de refrigeração independente do diluidor utilizado foi a Velocidade Curvilinear (VCL) (P<0,05). Foi observado aumento do percentual de espermatozoides morfologicamente normais nas amostras refrigeradas em BTS (P<0,05). Os diluidores ACP-103®, ACP-116® e BTS podem refrigerar e conservar o sêmen de queixada a 15°C por 24 horas.(AU)

Background: Human biological material has become an important resource for biomedical research. Tumor Biobanks are facilities that collect, store and distribute samples of tumor and normal tissue for further use in basic and translational cancer research. mRNA-translation has been demonstrated to modulate protein levels and is considered a fundamental post-transcriptional mechanism of gene expression regulation. Thus, determining translation efficiencies of individual mRNAs in human tumors may add another layer of information that contributes to the understanding of tumorigenic pathways. To analyze the RNAs actively engaged in translation, RNAs associated with ribosomes (polysomes) are isolated, identified and compared to total RNA. However, the application of this technique in human tumors depends on the stability of the polysomal structure under Biobank storage conditions that usually consists of ultra-low temperature. Since the effect of freezing on the stability of the polysomal structure in stored tumor samples is not known, it is essential to evaluate this factor in the frozen samples, validating the use of biobank samples in studies of translational efficiency. Methods: Xenograft tumors were divided in two parts, half was subject to immediate processing, and half was frozen for posterior analysis. Both parts were subject to polysomal separation, RNA extraction and identification through RNAseq. Results: It was possible to successfully extract and identify total and polysomal RNA from both fresh and frozen tumoral tissue. The quantification of the polysome profile indicated no difference in the translational efficiency estimated in fresh versus frozen tissue. Gene expression data from the fresh versus frozen tissues were compared and the correlation between the polysome associated fresh x frozen (R = 0,89) and total fresh x frozen (0,90) mRNAs was calculated. No difference was identified between the two conditions. Conclusions: We demonstrated that tissue freezing does not affect the polysomal structure, consequently validating the viability of the use of biobank stored tissue for polysome associated RNA analysis (AU)

ABSTRACT

El proceso de recolección de material biológico humano, necesita cada vez más la organización de la información para la gestión de las muestras y la integración de los datos relacionados con el participante o donante, provenientes de diferentes fuentes. Con el avance de la computación, esto permitirá identificar posibles interacciones sociodemográficas, genéticas, ambientales, entre otras con determinada enfermedad. Las estructuras de datos, los sistemas de codificación y los sistemas de metadatos, se han convertido en un desafío para la organización de los biobancos. La gestión, integración, seguridad, privacidad y análisis de los datos, son retos importantes para los investigadores y la informática. La normalización de los datos, la armonización e interoperabilidad de sistemas informáticos de biobancos permitirán el óptimo uso del material biológico, convirtiéndose en no solo un gran recurso para estudios epidemiológicos y clínicos a gran escala, sino también en bases para nuevas pruebas de diagnóstico e intervenciones terapéuticas personalizadas.


The collection, processing and storage of biological samples need a system for not only organizing and managing the patient samples but also integrating data records from different sources related to these patients. Along with computer advancement, these integration processes will allow to identify possible relationships between sociodemographic, genetic and environmental factors with specific diseases. Therefore, data structures, coding and metadata systems, have become essential elements for controlling biobanks. In fact, management, integration, security, privacy and data analysis are current challenges for scientists and computer administrators. The standardization of data, harmonization and interoperability of biobank computer systems will help to have an optimum use of biological material. As a result, these advances will turn into a great resource for large-scale epidemiological and clinical studies as well as the basis for new diagnostic tests and personalized therapies.

ABSTRACT

Los Biobancos con fines de Investigación están aumentando en número e importancia, con un gran potencial que contribuye a la generación de conocimiento pertinente para la solución de problemas en salud. El reclutamiento de donantes es vital para su éxito. Sin embargo, este proceso tiene implicaciones importantes de carácter ético, legal y social. El consentimiento informado requerido para la participación del donante impone desafíos éticos importantes en torno a la privacidad, confidencialidad, uso secundario de muestras y datos para estudios futuros, retorno de resultados, intercambio de datos, distribución de beneficios entre otras. Diferentes modelos de consentimiento informado han sido propuestos para responder a algunos de estos retos éticos . Sin embargo, no se ha llegado a un acuerdo, por lo tanto la discusión continúa abierta. Este trabajo pretende contribuir con la discusión, considerando fundamental el significado y el valor del proceso de consentimiento informado para la participación en investigación biomédica. Se discuten los diferentes tipos de consentimiento que se están utilizando actualmente evaluando el nivel de control y autodeterminación por parte del donante.


Biobanks with Research purposes are increasing in number and importance, with great potential that contributes to the generation of knowledge relevant to solving health problems. For the Biobanks to be successful, donor recruitment is essential. However, this process has important ethical, legal and social implications. Informed consent required for the participation of the donor imposes significant ethical challenges about privacy, confidentiality, secondary use of samples and data for future studies, return of results, data exchange, and distribution of the benefits among others. Different templates of informed consent have been proposed to address some of these ethical challenges. However, an agreement has not been reached, and the discussion remains open. This work aims to contribute to the discussion, considering the meaning and value of the informed consent process for participation in biomedical research, and examines the different types of consents currently used evaluating the level of control and self-determination displayed by the donor.

ABSTRACT

La investigación biomédica enfocada a la preservación de la salud y manejo de la enfermedad requiere hoy del trabajo articulado bidireccional entre básicos y clínicos, lo cual ha generado una nueva tendencia, denominada investigación traslacional. Este tipo de investigación se fundamenta en poder brindar una atención en salud oportuna, pertinente, eficaz y personalizada, para lo cual se requieren muestras biológicas e información clínica asociada, garantizando a su vez seguridad, calidad y confidencialidad para los donantes. Promover la investigación traslacional y la aplicación de los avances del conocimiento y de la tecnología derivados de la investigación y la innovación, requiere del apoyo de infraestructuras de fácil acceso que faciliten la rápida demostración experimental de una hipótesis o la comprobación de un modelo simulado previamente. Dentro de las diversas plataformas biomédicas y de salud existentes, los Biobancos, en sus diferentes modalidades, se constituyen en una de las más atractivas plataformas a la hora de contribuir a establecer puentes entre la investigación básica y clínica, con la práctica asistencial. La necesidad de contar con muestras biológicas humanas de alta calidad y, al mismo tiempo, la obligación de preservar los derechos de los donantes, ha elevado la gestión de los Biobancos a la categoría de disciplina científico-técnica, con una complejidad particular que involucra múltiples aspectos entre los cuales se incluyen aspectos científicos, técnicos, éticos, jurídicos y sociales. Esta serie de artículos, que serán publicados en los diferentes números de la revista durante el 2016, tienen como objetivo hacer una revisión crítica de los aspectos más relevantes en torno a la gestión de los Biobancos con fines de investigación y proponer una serie de guías de manejo del material biológico humano a conservar las cuales se han desarrollado en el marco del Programa Cardiecol.


Biomedical research aimed at the preservation of health and disease management requires nowadays a bidirectional and articulated collaboration between basic and clinical work, which has generated a new trend called translational research. This type of research is based on the ability to provide timely, relevant, effective and personalized healthcare, for which biological samples, and associated clinical information are required, while ensuring safety, quality and confidentiality for donors. In order to promote translational research and the application of the advances in knowledge and technology from research and innovation, the support of accessible infrastructure is required to facilitate the rapid experimental demonstration of an hypothesis or testing a previously simulated model. Among the diverse biomedical and healthcare existing platforms, the Biobanks, in their various forms constitute one of the most attractive platforms contributing to establish bridges between the basic and clinical research with the clinical practice. The need for human biological samples of high quality, and at the same time, the obligation to preserve the rights of donors, has raised the Biobanks' management to the scientific and technical category, with the added particular complexity of involving multiple aspects including scientific, ethical, legal, and social factors. This series of articles that will be published in different issues of the magazine in 2016, aims to make a critical review of the most relevant aspects regarding the management of Biobanks for research, and to propose a series of guidelines for the management of human biological material as developed by the program Cardiecol.

Objetivo: El presente estudio evaluó el impacto de variables pre-analíticas sobre las concentraciones séricas de la homocisteína y su posible aplicación en biobancos con fines de investigación. Metodología: En diez adultos voluntarios auto declarados sanos, se tomaron muestras de sangre periférica bajo diferentes condiciones de ayuno, posición de toma de la muestra (supino versus sentada) y diferentes intervalos de tiempo entre la toma y la separación definitiva de componentes. Todas las alícuotas fueron almacenadas a - 800C en el biobanco hasta el momento de ser procesadas. La medición de homocisteína se hizo por duplicado en Immulite® 2000. Se realizó análisis de concordancia por medio de coeficiente de Lin (σ) y MANOVA. Resultados: La medición de homocisteína es altamente reproducible (σ=0.908, IC95% 0.861 a 0.955), sin que el ayuno o el tiempo de centrifugación de la muestra afecte su concentración. Sin embargo, la posición al momento de la toma de muestra, implica una reducción media de 14.2% (IC95% 8.4% a 20.0%) en la concentración de homocisteína en posición decúbito supino versus la toma en posición sentado. Conclusión: La homocisteína es un biomarcador estable, sin que su valor se vea alterado por variables pre-analíticas como los tiempos entre toma de muestra, centrifugación y separación de componentes (almacenamiento temporal a 4°C hasta 24 horas). Sin embargo, la postura del participante al momento de la toma de muestra produce una variabilidad significativa. Estos hallazgos reiteran el papel de un biobanco en la estandarización de los procesos de toma, manipulación, almacenamiento y gestión con criterios de excelencia. Objective: This study evaluated the impact of pre-analytical variables on serum concentrations of homocysteine, and its possible applications in biobanks for research purposes. Methods: Peripheral blood samples from ten self-reported healthy volunteers were obtained under different conditions of fasting and body position (supine and sitting). Blood samples were temporarily stored for different time intervals between its collection and its centrifugation. All aliquots were stored at -80ºC in our biobank until processing. Duplicate Homocysteine measurements were performed in Immulite® 2000. A concordance analysis was performed using Lin coefficient (σ) and MANOVA. Results: The measurement of homocysteine is highly reproducible (σ = 0.908, 95% CI 0861-0955) and fasting or time before centrifugation does not affect its concentration. However, we found an average reduction of 14.2% (95% CI 8.4% to 20.0%) for the Hcy levels in samples obtained when the subjects were in supine position as compared to the levels in sitting position. Conclusion: Homocysteine levels are stable, when the blood samples remain without centrifugation, up to 24 hours at 4ºC. However, body position of the participant at the time of sampling can affect Hcy levels. ABSTRACT

La recolección y conservación de muestras biológicas con fines investigativos es una actividad ligada desde tiempo atrás con la historia y evolución de la ciencia y la medicina; hoy en día, la disponibilidad de bancos de conservación de muestras adecuadamente validadas, está asociada con el desarrollo presente y futuro de la investigación científica. En general se entiende que un banco de conservación de muestras es un espacio físico que permite mantener en condiciones ideales una serie de muestras biológicas de manera organizada, observando los más altos estándares de calidad, apegados a la normatividad ética y legal establecida para estos casos y cuyo fin es la investigación biomédica tendiente a generar nuevo conocimiento, así como a diversas aplicaciones diagnósticas y terapéuticas. Los bancos de conservación de muestras constan, más allá de su infraestructura física, de una base de datos que permite conocer en tiempo real y al detalle el estado y condición de cada muestra, además de protocolos y normas claras y perfectamente establecidas para la utilización de las muestras para los proyectos de investigación que las requieran. La mayoría de los bancos de conservación de muestras nacieron de la necesidad de almacenar muestras biológicas remanentes de alguna investigación biomédica o poblacional, de diferentes campañas de salud, o de muestras recolectadas con propósitos forenses y de criminalística. Posteriormente, con el mejoramiento de las técnicas de conservación de muestras y con el advenimiento de las microtécnicas de análisis molecular, quedó patente la necesidad de institucionalizarlos hasta convertirlos en lo que son hoy, el punto de partida para cualquier proceso investigativo y de generación de nuevo conocimiento, una herramienta científica de primer orden con un potencial de desarrollo virtualmente ilimitado. Este escrito pretende recrear algunos de los aspectos más importantes del desarrollo de los biobancos, así como resaltar su importancia para la consolidación de una cultura investigativa de calidad, seria y sostenida en el tiempo, que redunde en el desarrollo de la comunidad científica en particular y de la sociedad en general. The collection and storage of biological samples for research purposes is an activity linked for some time with the history and development of science and medicine. Nowadays, the availability of properly validated sample preservation banks is associated with the current and future development of scientific research. It is generally understood that a sample conservation bank is a physical space that allows to keep in ideal conditions a number of biological samples in an organized manner, observing the highest standards of quality, according to the ethical and legal norms established for these cases and whose aim is biomedical research, tending to generate new knowledge, as well as diverse diagnostic and therapeutic applications. Beyond their physical infrastructure, biobanks consist of a database that allows to know in real time and in detail the status and condition of each sample, as well as protocols and clear and well established guidelines for the use of samples required for research projects. Most sample conservation banks were born of the need to store remaining biological samples of some biomedical or population research, of different health campaigns, or of samples collected for forensic purposes and criminalistics. Later, with the improvement of sample preservation techniques and the advent of molecular analysis microtechnics, the need to institutionalize them and make them what they are today became evident: the starting point for any research process and of new knowledge generation, a scientific tool of the first order with a virtually unlimited growth potential. This paper aims to recreate some of the most important aspects of the development of biobanks as well as to highlight its importance for the consolidation of a serious and sustained over time research culture of quality, that benefits the development of the scientific community in particular, and of society in general. Este artigo apresenta a justificativa e o processo de elaboração do marco normativo do Hospital de Clínicas de Porto Alegre (HCPA) para o armazenamento e utilização de materiais biológicos humanos e suas informações associadas em atividades de pesquisa. Um grupo de trabalho multiprofissional se reuniu e discutiu todos os aspectos referentes a esta questão que envolve as atividades de biobanco e de biorrepositórios. Como produto deste trabalho é apresentada a Normativa Institucional.


This article presents the rationale and framework for developing the Hospital de Clinicas de Porto Alegre (HCPA) normative for storage and use of human biological material and their associated information on research activities. A multidisciplinary task force was established and all issues related to biobanks and biorepositories activities in periodic meetings were discussed; the final result is presented as an Institutional Normative.

ABSTRACT

As atividades de pesquisa em saúde cada vez mais necessitam do armazenamento de materiais biológicos em biorepositórios e em biobancos. O estabelecimento de políticas institucionais para lidar com aspectos técnicos, éticos, legais e sociais é imprescindível para garantir a sua adequação. O Hospital de Clínicas de Porto Alegre está com um projeto de desenvolvimento em execução para propiciar que estas atividades sejam harmônicas e integradas.


Health research increasingly requires the storage of biological samples in biorepositories and in biobanks. The establishment of institutional policies to deal with technical, ethical, legal and social issues is essential to ensure the adequacy of these activities. Hospital Clínicas de Porto Alegre has a development project to bring harmony and integration to these activities.
