ABSTRACT

Neste trabalho a palavra equidade vai ressoar em direção ao funcionamento das instituições, em particular as de psicanálise, e à escuta analítica do traumático social, com ênfase no racismo. As instituições tendem a ser conservadoras em sua natureza, mas são compostas por pessoas que se renovam no decorrer do tempo e podem abrir-se para mudanças. Ao se tornarem permeáveis aos sofrimentos e mal-estar que advém do campo social e comunitário podem adotar posicionamentos que favoreçam seu envolvimento nessas problemáticas. É destacado como a escuta analítica pode encontrar pontos cegos quando os/as analistas/terapeutas se fecham em bolhas de ressonância mútua, não se sensibilizando e nem se colocando na pele do outro diferente de si mesmo ou de seu grupo social. Torna-se fundamental perceber como a cultura racista onde fomos criados impregna nosso inconsciente, através dos Ideais do Eu, com a crença de que o branco seria a medida do que é universal e belo e, ao mesmo tempo, negando nossa realidade racista.(AU)


In this paper the word equity will resonate towards the functioning of institutions, in particular those of psychoanalysis, and the listening to the social trauma, with an emphasis on racism. Institutions tend to be conservative in nature, but they are made of people who renew themselves over time and can be open to change. By becoming permeable to the sufferings that come from the social and community field they may adopt positions that favor their envolvement in these issues. It highlights that analytical listening can find blind spots when analysts/therapists close themselves in bubbles of mutual resonance, not putting themselves in the shoes (skin) of others, diferents from themselves or their social group. It becomes fundamental to understand how the racist culture where we were raised impregnates our unconscious, through the ideals of the Ego, with the belief that white people would be the measure of what is universal and beautiful and, at the same time, denying our racist reality.


En este trabajo la palabra equidad resonará hacia el funcionamiento de las instituciones, en particular las del psicoanálisis, y a la escucha analítica del trauma social, com énfasis en el racismo. Las instituciones tienden a ser de naturaleza conservadora, pero están formadas por personas que se renuevan con el tiempo y pueden abrirse a cambios. Al volverse permeable a los sufrimientos y malestares que vienen del campo social y de la comunidad pueden adoptar posiciones que favorezcan su implicación en estos problemas. Se destaca que la escucha analítica puede encontrar puntos ciegos cuando los analistas/terapeutas se cierran en burbujas de resonancia mutua, no sensibilizarse y no ponerse en la piel de otra persona que no sea uno mismo o su próprio grupo social. ABSTRACT

Background: There are “blind spots” on chest computed tomography (CT) where pulmonary nodules can easily be overlooked. The number of missed pulmonary nodules can be minimized by instituting a training program with particular focus on the depiction of nodules at blind spots. Purpose: The purpose of this study was to assess the variation in lung nodule detection in chest CT based on location, attenuation characteristics, and reader experience. Materials and Methods: We selected 18 noncalcified lung nodules (6–8 mm) suspicious of primary and metastatic lung cancer with solid (n = 7), pure ground-glass (6), and part-solid ground-glass (5) attenuation from 12 chest CT scans. These nodules were randomly inserted in chest CT of 34 patients in lung hila, 1st costochondral junction, branching vessels, paramediastinal lungs, lung apices, juxta-diaphragm, and middle and outer thirds of the lungs. Two residents and two chest imaging clinical fellows evaluated the CT images twice, over a 4-month interval. Before the second reading session, the readers were trained and made aware of the potential blind spots. Chi-square test was used to assess statistical significance. Results: Pretraining session: Fellows detected significantly more part-solid ground-glass nodules compared to residents (P = 0.008). A substantial number of nodules adjacent to branching vessels and posterior mediastinum were missed. Posttraining session: There was a significant increase in detectability independent of attenuation and location of nodules for all readers (P < 0.0008). Conclusion: Dedicated chest CT training improves detection of lung nodules, especially the part-solid ground-glass nodules. Detection of nodules adjacent to branching vessels and the posterior mediastinal lungs is difficult even for fellowship-trained radiologists

ABSTRACT

OBJECTIVE: To use historiography in the analysis of the fictional writings of a Jamaican novelist to identify aspects of psychopathology of Jamaican people. METHOD: Each of 12 stories of "The World is a High Hill" by novelist Erna Brodber was assigned an explanatory title and a 'psychic centrality'. A narrative qualitative analysis of the fourteen main themes of each story was created using a Lickert scale, calculating the psychopathological penetrance or weighted significance of each theme. RESULTS: The four main psychic centrality containments that emerged from this analysis were the black/white racial paradox (n = 4, 33%) and the partisan/political paradox (n = 1, 8%), the sexual/ duplicity paradox (n = 5, 43%) and the social/spiritual paradox (n = 2, 17%). Five of fourteen themes reached maximal penetrance: family (92%), representation of generations with families (92%), issues of intimacy (92%), sex (75%) and issues of dependency (67%). Seven themes - personal and social conflicts (64%), issues of child development (53%), sexual identity (50%), pregnancy (48%), and political (42%), racial (36%), and religious (33%) conflicts reached moderate penetrance. The two themes of migration (30%) and homosexuality (14%) reached minimal penetrance. CONCLUSIONS: The analysis reveals a profound and practical historiographic representation of the contemporary scotoma that currently paralyses many Jamaicans as a product of the enslavement of Africans in the New World, and mirrors the clinical syndrome of personality disorder revealed from contemporary Jamaican medical research.


OBJETIVO: Utilizar la historiografía en el análisis de la narrativa de ficción de una novelista jamaicana, para identificar aspectos de la psicopatología del pueblo jamaicano. MÉTODO: A cada uno de los 12 cuentos de El mundo es una alta colina ("TheWorld is A High Hill") de la novelista Erna Brodber, le fue asignado un título explicativo y una "centralidad psíquica'. Se hizo un análisis cualitativo narrativo de los catorce temas principales de cada cuento, usando una escala de Likert para calcular la penetrancia psicopatológica o significación ponderada de cada tema. RESULTADOS: Las cuatro contenciones principales de la centralidad psíquica surgidas de este análisis fueron: la paradoja racial blanco/negro (n = 4, 33%), la paradoja política/partidista (n = 1, 8%), la paradoja sexual/duplicidad (n = 5, 43%), y la paradoja social/espiritual (n = 2, 17%). Cinco de los catorce temas alcanzaron máxima penetrancia: la familia (92%), la representación de generaciones con familias (92%), los problemas de la intimidad (92%), el sexo (75%) y los problemas de dependencia (67%). Siete temas - conflictos personales y sociales (64%), problemas de desarrollo infantil (53%), identidad sexual (50%), embarazo (48%), y los conflictos políticos (42%), raciales (36%) y religiosos (33%) - alcanzaron una penetrancia moderada. Dos temas - la migración (30%) y la homosexualidad (14%) - alcanzaron penetrancia mínima. CONCLUSIONES: El análisis revela una representación historiográfica profunda y práctica del escotoma contemporáneo que paraliza actualmente a muchos jamaicanos como resultado de la esclavitud de los africanos en el Nuevo Mundo. Asimismo, refleja el síndrome clínico del trastorno de la personalidad revelado por la investigación médica jamaicana contemporánea.
