ABSTRACT

La transfusión de sangre y sus productos constituyen un método terapéutico de probada valía cuando es utilizado con oportunidad en situaciones médicas específicas, de ahí que su acceso seguro y universal sea un servicio esencial dentro de la cobertura de salud. Se realizó revisión de fuentes documentales para saber acerca de la disponibilidad actual de estos productos biológicos en diferentes regiones del mundo y en Cuba, que es el propósito de este trabajo. Se demuestra que los países desarrollados con sistemas de salud bien estructurados y servicios de transfusiones de sangre basados en la donación voluntaria de sangre, poseen generalmente mejor disponibilidad para hacer frente a sus necesidades transfusionales que los países en desarrollo. Cuba, aunque alcanzó los principales propósitos que la Organización Mundial de la Salud recomienda para los programas nacionales de transfusiones de sangre, posee significativas amenazas que pueden poner en peligro la continuidad de esas conquistas; es necesario entonces revisar los eslabones que conforman la cadena de la transfusión de sangre, desde la promoción, el reclutamiento, y la selección de individuos como donantes, hasta el uso clínico de este importante y poderoso recurso terapéutico(AU)


Blood transfusion and blood products are a proven effective therapeutic method when they are timely used in specific medical situations, hence their safe and universal access is an essential service within health coverage. A review of documentary sources was made to find out the current availability of these biological products in various regions of the world and in Cuba, which is the objective of this paper. It was proven that the developed countries, with well-structured health systems and transfusion services based on voluntary blood donation, generally have better availability to meet their transfusion needs than the developing countries. Although Cuba has attained the main targets recommended by the World Health Organization for the national blood transfusion programs, it faces significant threats that could endanger the continuity of these achievements(AU)

ABSTRACT

Objective To study the effect of massive blood transfusion program(MTP) in early massive blood transfusion and preventing coagulopathy.Methods Fifty cases of massive blood transfusion were clinically collected and performed the statistical analysis and processing before and after intervention according to the blood transfusion scheme of MTP.The statistical comparison was performed by using the data before and after intervention.Results PLT was decreased with the increase of transfusing of RBC.After transfusing 15 U of RBC,the PT test,INR and APTT were increased significant(P<0.05).After transfusing 20 U of RBC,the Fib was decreased significant(P<0.05).The MTP had statistical difference between before and after intervention(P<0.01).Conclusion The MTP application has an important significance to prevent the occurrence of coagulopathy and evaluate the coagulation status.
