ABSTRACT

OBJECTIVE@#To investigate the evolution of blood separation results by gel extraction of multiple myeloma (MM) patients, and to evaluate the clinical value of abnormal blood separation results for the evaluation of disease and prognosis.@*METHODS@#The clinical data of 5 patients diagnosed newly MM patients with abnormal blood separation of gel collection vessels in our hospital were retrospectively analyzed, and the changes of blood separation results and blood index levels were followed up with the improvement of treatment effect, and the correlation of different blood index levels was analyzed.@*RESULTS@#In 5 patients with newly diagnosed MM, the blood separation result showed floating phenomenon after centrifugation, which divided into three layers and the order from top to bottom is separator gel, serum, and red blood cells(RBC). With partial remission of clinical symptoms, the blood separation results were still abnormal, which were divided into three layers from top to bottom: serum, RBC and separator gel. Finally, with complete remission of the disease, blood separation results returned to normal, from top to bottom: serum, separator gel, RBC. With the blood separation results from abnormal to normal, the blood routine indicators: Hb, Hct levels gradually increased, neutrophil/lymphocyte ratio (NLR), monocyte/lymphocyte ratio (MLR) gradually decreased; biochemical indexes: TP, GLB, Ig and β2-MG levels gradually decreased. Tumor load related indicators: serum IL-6, TNF-α, IL-17 levels gradually decreased, and IL-35 levels gradually increased; and the differences were statistically significant (P<0.05). Pearson correlation analysis showed that serum β2-MG was positively correlated with IL-6, TNF-α and IL-17 levels (r=0.710, 0.756, 0.581, P<0.05), and negatively correlated with IL-35 level (r=-0565, P<0.05).@*CONCLUSION@#Abnormal blood separation exists in MM patients, and there are significant differences in blood, tumor load and immune balance related indexes in patients with different blood separation results, which provides partial experimental basis for evaluation of disease, efficacy and prognosis with different blood separation results.

ABSTRACT

【Objective】 To optimize the bottom and top(B&T) method for nucleated cell separation in cord blood in order to reduce the disturbance and improve the nucleated cell yield. 【Methods】 A new device comprised of two plastic plates and connection units was developed. B&T Cord blood bag was put between the plates, which were then centrifuged and put into the serum expresser together to go through serum and red blood cells separation simultaneously. After centrifugation parameter adjustment, the new Plates-B&T method was compared with traditional method in parallel. 【Results】 New Plates-B&T method could avoid buffy coat movement and disturbance. Compared to conventional PRP method, Plates-B&T method reduced centrifugation and separation time from(29.12±2.79)min to(14.91±2.12)min(P<0.05). Using new method, the recovery rate of total nucleated cell(TNC) was (89.04±5.19)%, the removal rate of red blood cell(RBC) was(55.56±22.57) %, and hematocrit(HCT) in buffy coat was(0.48±0.09)L/L. RBC removal was enhanced significantly, especially for cord blood samples with medium-low volume. 【Conclusion】 Based on B&T method, we established a new device and method, which increased the efficiency and quality of cord blood nucleated cell separation.

ABSTRACT

BACKGROUND: Buffy coat method is one of the blood components processing methods widely used in many countries including Europe and Canada. For the first time in Korea, we evaluated the qualities of blood components manufactured by buffy coat method. METHODS: We collected 400 mL whole bloods using the quadruple top and bottom blood bag from thirty-five donors. Whole bloods were processed into leukoreduced RBC, leukoreduced pooled platelet, and 24 hr frozen plasma by the buffy coat method with blood bags and instruments of Fenwal and Fresenius. The qualities of each blood component were analyzed at each scheduled day, and compared with the standard guidelines of quality control in Korean Red Cross. RESULTS: The volume and hemoglobin of RBCs were lower than the standard guidelines. Comparing with the standard of apheresis platelets, leukoreduced pooled platelets showed higher total platelet yield with the median 3.70x10(11)/unit. Frozen plasma showed increased volume recovery than the standard guideline, but the activity of factor VIII at Day 35 was decreased to 0.66+/-0.14 IU/mL. CONCLUSION: We have found that the yields of pooled platelet and the frozen plasma processed by buffy coat method were higher than the standard guidelines. To introduce the buffy coat method to routine blood component separation process in Korea, further evaluations about the cost-effectiveness of buffy coat method are required.

ABSTRACT

Objective To propose a method & Implementation for automatic blood ingredient separating and transferring.Methods Whole blood could be layered after centrifugal processing,and the color of the layers appeared different certainly.So color sensors could be used to receive different color signals of ingredient blood to control the process of separating whole blood and transfer different blood ingredients to different blood bags.When plasma was extruded out of the whole blood,resistance-strain pressure sensor was used to get the weight,and the air was cleaned out through extrusion movement.Air cleaning automatic control was realized through examining the intensity of pressure in the blood bag by photoconductive resistance.When the process of separating blood finished completely,the glue pipes could be heated automatically.The device adopted main-subsidiary configuration,the master was mostly in charge of separating and transferring blood ingredients of whole blood and the assistor was used to extrude out the air remains in the plasma bag and get the weight of the plasma.Results The assistor was made actually by using a PC as a virtual master to conduct experiments debugging and improve the assistor,and then the precision of electronic weighing reached 0.5g while the air in blood bag was almost cleaned absolutely.A communication protocol was developed and the serial communication between the master and the assistor were realized by VC6.0.Conclusion This method and device can automatically separate and transfer different blood components fast and efficiently,thus meeting the automation needs of the blood station.
