ABSTRACT

El osteosarcoma es el tumor óseo primario, maligno, más común; se presenta más frecuentemente en la segunda década de la vida. Las metáfisis de los huesos largos que crecen rápidamente como el fémur distal y el húmero proximal son comúnmente los más afectados. Clínicamente es característico el dolor óseo persistente por 3 ­ 6 meses. Radiológicamente, presenta imágenes de márgenes no geográficas, con destrucción (lisis) y diferentes tipos de reacción perióstica, incluyendo el triángulo de Codman. El principal tratamiento incluye la quimioterapia y la cirugía, donde según su estadío, se plantea la conservación del miembro, plastia rotacional o amputación. En éste trabajo presentamos un caso clínico de un paciente adolescente con osteosarcoma de fémur tratado con cirugía preservadora del miembro con la técnica de criocirugía (nitrógeno líquido)(AU)


Osteosarcoma is the most common bone malignant tumour, frequently appearing during the 20s. The metaphyses of long bones which have a rapid growth, e.g. knee and shoulder, are the most commonly affected. A clinical characteristic is the persistent pain in the affected bone which may last from 3 to 6 months. Radiologycally, the lytic areas have non-geographical boundaries and present destruction of the bone tissue, apart from having different types of periosteal reactions, including the Codman Triangle. The treatment consists of chemotherapy and surgery. Chemotherapy is applied before and after surgery. The surgery may be limb salvage surgery or complete amputation of the affected member. This work describes a clinical case of an adolescent suffering from a femur osteosarcoma treated through limb salvage surgery under cryosurgery(AU)

ABSTRACT

The aim of this study was to describe surgical features of resection of hemimelic epiphyseal dysplasia of the patella. We already described the clinical and imaging features in another article. The patient was a six-year-old boy with a tumor in his right knee measuring 12 cm longitudinally and 6 cm transversally, which was adhering to the patella and had been slowly growing for two years. Biopsy findings were suggestive of a benign osteochondromatous lesion, without a defined diagnosis. Imaging examinations such as radiography and tomography showed areas of bone formation and radiotransparent areas, while magnetic resonance imaging showed areas of hypo and hypersignal in T1 and T2, of estimated size 8.5 cm longitudinally and 6 cm transversally. The tumor growth was surgically resected and curettage was performed on the epiphyseal nucleus of ossification of the upper and medial centers of the patella, with good patellar remodeling and normal development. The patient did not present any recurrence of the lesion up to the time of reaching skeletal maturity...


Descrever aspectos cirúrgicos da ressecção de displasia epifisária hemimélica da patela. Os aspectos clínicos e de imagem já foram descritos em outro artigo. Paciente masculino, seis anos, com tumor de crescimento lento em joelho direito, por dois anos, com dimensões de 12 cm no eixo longitudinal e 6 cm no transversal, aderido à patela. Biópsia sugeriu lesão osteocondromatosa benigna, sem diagnóstico definido. Exames de imagem, como radiografias e tomografias, com áreas de formação óssea e áreas radiotransparentes e na ressonância magnética áreas de hipo e hiperssinal em T1 e T2, de tamanho estimado em 8,5 cm no eixo longitudinal e 6 cm no transversal. Foram feitas ressecção cirúrgica da tumoração e curetagem do núcleo de ossificação epifisário do polo superior e medial da patela, com boa remodelação patelar e desenvolvimento normal. Paciente não apresentou recidiva da lesão até o término da maturidade esquelética...

ABSTRACT

O grupo de oncologia ortopédica proporciona atendimento médico e psicossocial aos pacientes internados e em tratamento ambulatorial, portadores de tumores músculos-esqueléticos. Nesta pesquisa, nosso objetivo foi conhecer o perfil socioeconômico dos paciente-familiares e as mudanças ocorridas após a constatação do diagnóstico. A amostra foi composta por 25 famílias de pacientes com osteossarcoma de membro inferior. A pesquisa ocorreu no período de setembro a outubro/2005, através de formulários com questões fechadas e abertas, utilizando para análise qualitativa o discurso do sujeito coletivo. O estudo foi prospectivo, descritivo, com abordagem quanti-qualitativa. Dos entrevistados, 68 por cento eram do sexo feminino, 44 por cento destes eram genitoras; 76 por cento estavam trabalhando e 28 por cento possuíam vínculo empregatício. 60 por cento possuía renda entre 2 a 5 salários mínimos. Após o diagnóstico, 92 por cento tiveram aumento de gastos; 80 por cento apresentaram dificuldade com a quimioterapia; 56 por cento relacionam o transporte como dificultador para aderir ao tratamento. 100 por cento desesperaram-se diante da descoberta do câncer. O câncer ocasiona mudanças nos papéis dos membros da família. Existe o receio da recidiva da doença que leva a família a ter medo da morte, fazendo-se necessário um trabalho em conjunto com a equipe multiprofissional.


The orthopaedic oncology group provides medical and psychosocial care to patients, both in hospital and outpatient facilities, with musculoskeletal tumors. With this research we aimed to know the socioeconomical profile of the patients/ families, as well as the changes occurred after such diagnosis is received. The sample was constituted of 25 families of patients with osteosarcoma on lower limbs. The study was conducted between September and October 2005 by means of forms containing open and closed questions, using the collective subject speech for qualitative analysis. The study had a prospective, descriptive design with quanti-qualitative approach. Among the respondents, 68 percent were females including 44 percent of mothers; 76 percent worked, and 28 percent of these had formal jobs. 60 percent had a family income amounting 2-5 minimum wages. After diagnosis, 92 percent faced increased expenses; 80 percent reported challenges with the chemotherapy; 56 percent mention transportation as the major factor affecting compliance to treatment. 100 percent feel desperate when they first know they have cancer. Cancer causes changes in family members' roles. There is always the fear of recurrence, which ultimately lead the family to fear death, thus requiring a joint effort with a multidisciplinary team.
