ABSTRACT

Objetivo: determinar la calidad y efectividad del trabajo realizado en el Centro Mamario del Hospital Universitario Austral, mediante la correlación entre las diferentes categorizaciones del BI-RADS, el resultado de la anatomía patológica y el cálculo del respectivo VPP. Correlacionar los resultados con los obtenidos a nivel internacional. Material y método: se realizó una revisión de todas las biopsias guiadas por ultrasonido, se seleccionaron aquellas pacientes estudiadas con algún método de diagnóstico por imágenes en el Centro Mamario del Hospital Universitario Austral y cuya categorización final de BI-RADS fuera de 4 o 5, entre junio de 2014 y mayo de 2017. Los resultados de la anatomía patológica fueron divididos en 3 categorías (benigno, maligno y alto riesgo) Resultados: de una total de 491 pacientes 223 correspondieron a la subcategoría 4a, 69 pacientes a la subcategoría 4b, 85 pacientes a la subcategoría 4c y 114 pacientes a la categoría 5. En las subcategorías 4a y 4b predominaron las lesiones benignas y en la subcategoría 4c y categoría 5 predominaron las lesiones malignas. Los VPP para cáncer de mama obtenidos fueron de 7%, 48%, 84% y 98% para las subcategorías 4a, 4b, 4c y categoría 5 respectivamente. Conclusiones: la investigación demostró que los resultados de los VPP, la calidad y efectividad del trabajo realizado en el Centro Mamario del Hospital Universitario Austral se encuentran dentro de los parámetros internacionales.


Objetive: to determine the quality and effectiveness of the work carried out in the Breast Center of the Austral University Hospital, through the correlation between the different categorizations of the BI-RADS, the result of the pathological anatomy and the calculation of the respective VPP. Correlate the results with those obtained internationally. Material and method: a review of al ultasound-guided biopsies was performed, those patients studied with some diagnostic imaging method were selected in the Breast Center of the Austral University Hospital and whose final categorization of BI-RADS was 4 or 5, between june 2014 and may 2017. The results of the pathological anatomy were divided into 3 categories (bening, malignant and high risk). Results: from a total of 491 patients, 223 corresponded to subcategory 4a, 69 patients to subcategory 4b, 85 patiens to subcategory 4c and 114 patients to category 5. In subcategories 4a and 4b benign lesions predominated and in subcategory 4c and category 5, malignant lesions predominated. The VPP for breast cáncer obtained were 7%, 48%, 84% and 98% for subcategories 4a, 4b, 4c y category 5 respectively. Conclusions: the investigation showed that the results of the PPV, the quality and effectiveness of the work carried out in the Breast Center of the Autral University Hospital are within the international parameters.

ABSTRACT

Periductal mastitis and mammary duct ectasia are now considered as separate disease entities in the female breast, and these two diseases affect different age groups and have different etiologies and clinical symptoms. These two entities have very rarely been reported in the male breast and they have long been considered as the same disease as that in the female breast without any differentiation. We report here on the radiologic findings of a rare case of periductal mastitis that developed during the course of chemotherapy for lung cancer in a 50-year-old male. On ultrasonography, there was a partially defined mass with adjacent duct dilatation and intraductal hypoechogenicity, and this correlated with an immature abscess with a pus-filled, dilated duct and periductal inflammation on the pathologic examination.

ABSTRACT

PURPOSE: To define and correlate characteristic sonographic and pathologic findings in men with gynecomastia. MATERIALS AND METHODS: We reviewed medical records and sonographic findings of 26 patients with pathologically proven gynecomastia. All of the chief symptoms, physical findings and causes were categorized. Characteristic sonographic patterns of gynecomastia were analyzed and compared with pathologic findings. RESULTS: Gynecomastia was most common in the 20- to 40-year-old age group (50%) 6;palpable mass was found in 62% of all those cases. Gynecomastia was unilateral or asymmetrical in 22 men (85%), and was caused idiopathically in 21 (81%). The characteristic sonographic patterns correlated with pathologic findings were as follows : (1) focal subareolar smooth oval (or triangular) indistinct margined homogeneous hypoechoic(or isoechoic) patterns were found in 14cases (54%). Eight of these (57%) were of the florid type of gynecomastia (67%) ; (2) diffuse homogeneous hyperechoic (or isoechoic) patterns were found in five cases (19%) ; four of these (80%) were of the intermediate or fibrotic type : (3) involuting female breast parenchymal patterns were found in seven cases (27%), of which six(86%) were of the intermediate or fibrotic type. CONCLUSION: The characteristic sonographic patterns of gynecomastia correlate closely with pathologic types related to the duration of the condition. Ultrasonography isa useful primary diagnostic modality for the evaluation of gynecomastia.

ABSTRACT

PURPOSE: The purpose of this study is to evaluate the radiologic features and clinical utility of ultrasonography and mammography in cases of gynecomastia. MATERIALS AND METHODS: This study involved 40 men inwhom gynecomastia had been pathologically diagnosed by surgical incision. In 21 cases, a retrospective analysis of ultrasonographic and mammographic findings was performed. RESULTS: Causative factors of gynecomastia among the 40 pathologically-proven cases were idiopathic or pubertal in 33 cases, related to male hormone deficiency in three cases and to chronic liver disease in four. Bilateral involvement was seen in 14 cases, and unilateral involvementin 26 ; among unilateral cases, right side was involved in 10 cases, and the left side in 16. Mammographically, asubareolar discoid lesion was present in 12 cases, diffuse increased breast density was seen in five cases and dendritic marginated subareolar lesion without microcalcification in one. Ultrasonographically, a round smooth marginated low echogenic lesion in the subareolar region was seen in five cases, a diffuse hyperechogenic pattern without definite mass in two cases and an ill defined low echogenic lesion in one. CONCLUSION: The male breast is small, so in cases of gynecomastia, ultrasonography is an effective diagnostic modality. Mamography will, however, be helpful in the detection of microcalcification in cases of gynecomastia seen on sonography.
