ABSTRACT

Objective:To explore the effect of decision-making assistance during the implantation selection of breast augmentation patients to provide a reference for improving decision-making aids.Methods:From June 2018 to June 2020, the decision-making assistance programs were used in the Department of Plastic and Reconstructive Surgery, Shanghai Ninth People's Hospital for prosthesis selection in 51 breast augmentation female patients, aged 23 to 42 years, with an average of 31 years old. The BREAST-Q scale was applied to assess the postoperative results of augmentation patients. Postoperative breast satisfaction was also evaluated by the physicians.Results:With decision-making assistance, breast augmentation patients' satisfaction with breasts was (80.27±11.45) points, satisfaction with surgical results (83.41±12.29) points, social and psychological status scores (87.24±7.62) points, and sexual life status scores (85.49±7.90) points, physical condition score (73.94±8.98) points. There was no statistical difference in the scores of breast size between physicians and patients ( P>0.05). In the satisfaction score and total score of breast shape and feeling, physicians' scores were higher than patients' self-report scores, and the difference was statistically significant ( P<0.05). Conclusions:The patient self-reported postoperative outcomes are at a high level under the application of decision aid program. We can further improve the decision aid program for breast augmentation patients, adjust patient's surgical expectation, and realize shared decision making.

Resumen Introducción El Breast-Q® módulo reconstrucción mamaria es un instrumento específico para evaluar la calidad de vida asociada a la cirugía mamaria desde el punto de vista del paciente. Objetivo Realizar la traducción y adaptación transcultural del Breast-Q® módulo reconstrucción mamaria Versión 2.0 al español chileno. Materiales y Método Se utilizaron las guías de validación lingüística del MAPI/TRUST Research Institute . El proceso consistió en traducción inglés-español, contra-traducción español-inglés, conciliación y aplicación piloto a 6 pacientes. Resultados Todas las pacientes comprendieron la encuesta y no existieron dudas sobre redacción y parámetros lingüísticos. No se requirieron más modificaciones. Conclusiones El proceso de traducción y adaptación cultural del instrumento fue completado exitosamente. El instrumento se encuentra listo para la validación lingüística.


Introduction The Breast Q Reconstruction Module is a specific instrument for assessing breast surgery related quality of life from the patient's perspective. Aim To carry out a transcultural translation and adaptation of version 2.0 to Chilean Spanish. Materials and Method Linguistic validation guides of the MAPI/TRUST Research Institute were used. The process consisted of English-Spanish translation, Spanish-English back translation, conciliation and pilot application of the scale in 6 patients. Results Patients had good understanding and no doubt about redaction and linguistic parameters. No further modifications were needed. Conclusions Traduction and cultural adaptation of the instrument was completed successfully in Chilean population. The instrument is ready for linguistic validation.

ABSTRACT

Objective@#This is a study to evaluate complication rate, satisfaction and quality of life after breast reconstruction under different classifications. The classification criteria are the timing of operation, surgical procedures, and patients′age.@*Methods@#By reviewing the surgical cases from August 2004 to June 2018, the authors summarized the data of 102 patients with breast reconstruction of the same surgeon in Peking University People′s Hospital and Plastic Surgery Hospital (Institute) CAMS PUMC. Fifty-four patients who met the inclusion criteria and completed the BREAST-Q breast reconstruction module scale, were divided into immediate group or delayed group, according to the timing of operation; divided into abdomen group or other procedures group, according to the surgical procedure; and divided into the young patients′group or senior patients′group according to age. The index were (1) postoperative complication rate, (2)satisfaction with breasts, psychosocial well-being, physical well-being of chest and abdomen, sexual well-being, satisfaction with outcome, satisfaction with information, satisfaction with care (based on the BREAST-Q scale).@*Results@#Complication rate was 9.1%(1/11) in the immediate group, 16.3%(7/43) in the delayed group, 14.7%(5/34) in the abdomen group, 15.0%(3/20) in other procedures group, 13.6%(3/22) in the young patients′group, and 15.6%(5/32) in the senior patients′group. There was no significant difference in the incidence of complications (P>0.05). The postoperative scores of satisfaction with breasts, psychological well-being and sexual well-being were significantly higher in the immediate group than in the delayed group(P=0.026, P<0.001 and P=0.001, respectively). The score of postoperative physical well-being of chest was significantly higher in the abdomen group than other procedures group(P=0.020). The score physical well-being of abdomen was lower compared with the preoperative in the abdomen group (P<0.001). The score of physical well-being of abdomen was lower in the senior patients′group than that of the young patients′group (P=0.010).@*Conclusions@#The complication incidence of breast reconstruction patients was similar among different groups. The breast reconstruction surgery can significantly improve satisfaction and quality of life of patients. To some extent, immediate breast reconstruction can reduce the psychological and physiological effects of breast loss on patients, leading to higher postoperative satisfaction. Breast reconstruction with abdominal flaps can provide patients with better chest health but poor abdomen health. Although elderly patients undergoing breast reconstruction will have poor physical well-being of abdomen, they can still achieve higher satisfaction of operation.

Objetivo Determinar los cambios en calidad de vida en mujeres sometidas a mamoplastia de aumento o mastopexia con prótesis, usando el instrumento validado Breast-Q®. Materiales y métodos Estudio de cohortes prospectivo. Se incluyeron todas las pacientes operadas de mamoplastia de aumento o mastopexia de aumento en el Hospital Clínico de la Universidad de Chile entre octubre de 2013 y mayo de 2014. La calidad de vida (CdV) se midió con el instrumento validado Breast-Q®. Se realizó estadística descriptiva y analítica para evaluar el cambio global en CdV y por dominios considerando estadísticamente significativo un valor p < 0,05. Resultados En el periodo estudiado se operaron 58 pacientes, 33 (56,9%) completaron el instrumento Breast-Q® en el preoperatorio y 25 (75,7%) de ellas en el postoperatorio. La CdV subió de un 49,7% a un 82,5% (p < 0,001); analizando por dominio hubo mejoría en autoestima (52% a 88%, p < 0,001), autoimagen (22% a 80%, p < 0,001) y vida sexual (31% a 86%, p < 0,001). Los síntomas físicos empeoraron desde un 94% a 75% (p < 0,001). Conclusiones El aumento mamario mejora la CdV, específicamente la autoestima, la autoimagen y la vida sexual, pero las molestias físicas aumentan. La satisfacción global fue alta.


Objective To determine the changes in the quality of life of women subjected to augmentation mammoplasty or a prosthesis mastopexy, using the validated tool Breast-Q®. Materials and methods A prospective cohorts study was conducted that included operated on for augmentation mammoplasty or augmentation mastopexy in the Hospital Clínico de la Universidad de Chile (HCUCH) between October 2013 and May 2014. The quality of life (QoL) was measured using the Breast-Q® validated tool. Descriptive and analytic statistics were performed to evaluate the overall change in QoL and by domains, considering a p<.05 as statistically significant. Results A total of 58 patients were operated on, of whom 33 (56.9%) completed the Breast-Q® questionnaire before surgery, and 25 (75.7%) of them in the post-operative period. The quality of life increased from 49.7% to 82.5% (p<.001). When analysed by domain, there was an improvement in Self-esteem (52% to 88%, p<.001), Self-image (22% to 80%, p<.001) and Sex Life (31% to 86%, p<.001). The physical symptoms increased from 75% to 94% (p<.001). Conclusions Breast augmentation improves the quality of life, in particular, self-esteem, self-image, and sex life, but physical discomforts increased. The overall satisfaction was high.

Abstract Background: The results of breast reduction surgery are usually reported in terms of anthropometric assessments. However the impact of surgery in quality of life is seldom informed. Aim: To assess the effects of breast reduction surgery on the quality of life of patients. Material and Methods: The "Breast-QReduction and Mastopexy Module" was prospectively applied to all women subjected to breast reductionsurgery in a University Hospital during one year. Results: Twenty women answered the survey in thepreoperative period and 20 in the postoperative period. The most commonly cutaneous pattern employedthat the inverted T in 10 women, followed by the Wise pattern in six patients. The most commonly usedpedicles were superior in 10 patients, followed by superomedial in seven patients. Mean excision weightwas 923 g (range 150-2850). Mean Breast Q score improved from 45 to 65%. Satisfaction with breastappearance improved from 31 to 83%, self-esteem from 39 to 80%, sexuality from 50 to 81% and physicalsymptoms decreased from 48 to 16%. There was a significant association between the magnitude of excision and improvement in physical symptoms. Conclusions: Quality of life improves notably in womenafter breast reduction surgery.


Resumen Objetivo: Tradicionalmente la evaluación de resultados en reducción mamaria se ha limitado a evaluaciones antropométricas, que si bien nos entregan una evaluación objetiva de un aspecto de los resultados, resultan insuficientes para evaluar el impacto global en calidad de vida (CdV) de la cirugía en nuestros pacientes. Es por esto que el objetivo de este trabajo es determinar el cambio en calidad de vida en mujer es sometidas a reducción mamaria y explorar sus factores determinantes. Materiales y Métodos: Se realizó un estudio de cohorte prospectiva con todas las pacientes que ingresaron al Hospital Clínico de la Universidad de Chile (HCUCH) y se sometieron a reducción mamaria por cualquier técnica desde julio de 2013 a abril de 2014. La calidad de vida (CdV) se midió con el instrumento validado Breast-Q Reduction and Mastopex y Module. Se utilizó estadística descriptiva para caracterizar la muestra y analítica para determinar el cambio en CdV y sus factores pronósticos. Resultados: Entre julio de 2013 y abril de 2014 se operaron 25 pacientes, el seguimiento promedio fue de 7 meses. Veinte pacientes contestaron el Breast-Q preoperatorio y 20 el post operatorio. El patrón cutáneo más empleado fue la T acortada (10; 55,6%) seguido por el patrón de Wise (6; 33,3%); el pedículo más usado fue el superior (10; 55,6%) seguido del supero medial (7; 38,9%). La resección promedio total fue de 923 g, variando entre 150 g y 2.850 g. El puntaje promedio del Breast-Q subió de45% a 65% (p = 0,005). Las pacientes mejoraron su satisfacción con la apariencia mamaria desde un 30,8%a un 82,9% (p < 0,001), su autoestima de 38,7% a 80,1% (p < 0,001), síntomas físicos de 47,8% a 15,5% (p= 0,003) y sexualidad de 50,0% a 81,4% (p < 0,001). La magnitud de la resección se asoció a la mejoría de síntomas físicos (Coeff 0,07; p = 0,023). No se encontraron otras asociaciones significativas entre el cambio en parámetros de calidad de vida y variables independientes. Conclusiones: En este reporte preliminar hemos podido objetivar que existe una mejoría significativa en la calidad de vida de las pacientes sometidas a una reducción mamaria tanto en forma global como también en cada uno de los aspectos evaluados, mejorando significativamente su autoimagen, autoestima, vida sexual y síntomas físicos.

Objective To evaluate the design formulas of the optimal location for splitting pectoralis major in terms of making type Ⅰ or Ⅱ dual-plane implant pocket.Methods Sixty-five patients with micromastia were collected.Breasts were divided into two types according to the soft-tissue pinch thickness of the lower pole:type Ⅰ (thickness>2 cm;34 cases) and type Ⅱ (thickness ≤2 cm;31 cases).The optimal levels at which the pectoralis major (PM) was severed were 3/4 or 2/4 of the distance of new inframammary fold for type Ⅰ or Ⅱ dual-plane pockets,respectively.All patients completed the pre-and post-operative BREAST-Q augmentation modules before and 6 months after surgery.The scores were changed into hundred-mark system by QScore software.Results The recovery processes were well-off.The breasts contour was good.Patients reported higher scores of satisfaction with breasts,psychosocial well-being and sexual well-being after surgery than before surgery (62.0±8.9 vs 27.9± 4.3,65.0± 17.2 vs 17.4±8.3,60.5± 14.2 vs 30.3±5.5,P<0.05).The mean satisfaction score for the overall outcome was 90.6±5.4.However,there was no significant difference in physical well-being between before operation and aftre operation (85.3±9.5 vs 84.7± 10.6).No complications such as severe capsular contracture,or displacement occurred.Conclusions The design formulas make the determining procedure of the optimal location for pectoralis major splitting for two types dual-plane implant pockets easier and more exactly.Our modified design method can provide the implant with the optimal soft tissue coverage,and bring desired and stable breast aesthetic outcomes.The higher satisfaction and quality of life reported by patients indicate that the formulas are feasible and worth to recommend.

ABSTRACT

Background: The Breast Q Reduction and Mastopexy Module evaluate breast related quality of life from the perspective of the patients. Body self-image, social performance, sexuality and physical symptoms are assessed. Aim: To translate the Breast Q Reduction and Mastopexy Module to Spanish and validate the translated version to be used in Chile. Material and Methods: The linguistic validation guidelines of the MAPI/TRUST Research institute were used and the survey was applied to six patients aged 29 +/- 6 years with 14 +/- 2 years of studies. The internal stability of the test was assessed using Cronbach alpha. Results: The mean body mass index of the patients was 25 +/- 1 kg/m². The internal stability of the scale was 97 percent. The scores of the test in the preoperative period ranged from 74 to 125 points. Conclusions: The Breast Q Reduction and Mastopexy Module is a valid instrument to assess the results of reductive mammoplasty among Spanish speaking patients.


Objetivo: El Breast Q Reduction and Mastopexy Module es un instrumento específico para la evaluación de calidad de vida asociado a mamas desde la perspectiva del paciente, publicado originalmente en inglés, el cual cuenta con una evaluación pre y otra postoperatoria. El objetivo del presente estudio es traducir y validar este instrumento al español para su uso en Chile. Material y Método: Se utilizaron las guías de validación lingüística del MAPI/TRUST Research institute. El proceso de validación requirió la traducción ingles-español, contra traducción español-inglés, conciliación inglés-inglés y aplicación piloto de la escala en al menos 5 sujetos. La estabilidad interna se evaluó con el alfa de Cronbach, se utilizó estadística descriptiva y analítica para analizar los resultados. Resultados: Las 6 pacientes evaluadas tuvieron una edad promedio de 29 +/- 16 (SD) años, IMC 25 +/- 1 kg/m² y 14 +/- 2 años de estudio. La estabilidad interna de la escala fue de 97 por ciento. Los resultados obtenidos entre el preoperatorio de las pacientes, variaron de 74 a 125 puntos, con una mediana de 98 puntos. Conclusiones: El instrumento Breast Q Reduction and Mastopexy Module es un instrumento válido, confiable y reproducible para la evaluación de los resultados de la mamoplastía de reducción, desde la perspectiva de los pacientes chilenos hispanoparlantes.
