ABSTRACT

Introduction: There are, at present, no biomarkers topredict to prognosis of Gallbladder cancer. We conducted aprospective exploratory study to evaluate the role to serum CA19-9 and CA 242 as prognostic markers.Material and Methods: We enrolled consecutive patients forthis study and CA 19-9 and 242 were estimated from venoussamples. Association of these markers with clinical variablesand median overall survival (OS) difference between patientswho has raised versus normal levels of these markers wasdetermined.Results: Sixty-two patients were enrolled for this study.Forty-four (71%) patients had elevated CA19-9. Thirty-nine(62.9%) patients had an raised CA242 levels. CA 19-9 wasfound to be significantly associated with the presence ofjaundice (p=0.038) and advanced stage (p=0.009). MedianOS of patients who had elevated CA 19-9 was 5.73 monthscompared to 8.33 months in patients who had normal CA19-9. The difference was not statistically different (p= 0.15).Median OS for patients who had elevated CA 242 was 5.53months, which was inferior to those who had normal levels(9.1 months). This difference approached, but was notstatistically significant (p=0.055).Conclusion: This is the first study to show associationbetween CA 19-9 and stage of disease in GBC. At present,CA 19-9 and CA 242 cannot be recommended as prognosticmarkers. However, role of CA 242 needs to be examined ina larger cohort of patients of GBC to establish its usefulness.

ABSTRACT

Objective To evaluate the sensitivity and specificity of CA242/CA72‐4 ratio in diagnosing early colorectal carcino‐ma .Methods Totally 87 blood specimens of colorectal carcinoma(CRC) were collected .The Chemiluminescence method was adopt‐ed to detect the levels of CA242 and CA72‐4 .Then the receiver‐operating characteristic (ROC) curve was used to analyze the sensi‐tivity and specificity of CA242/CA72 4 ratio in the diagnosis of CRC .Results Among 87 cases of suspected CRC ,56 cases (64 .37% )were eventually diagnosed as CRC ,31 cases (35 .63% ) were excluded from CRC ;the CA242 and CA72‐4 levels and CA242/CA72‐4 ratios in the CRC group were higher than those in the non‐CRC group ,the differences were statistically significant (P<0 .05) ,moreover the CA242/CA72‐4 ratio was more significant(P=0 .013) .The areas under curve (AUC) of CA242 ,CA72‐4 and CA242/CA72‐4 ratio were 0 .754 ,0 .621 and 0 .775 respectively ,in which CA72‐4 had no significant correlation with the early CRC ,while CA242 and CA242/CA72‐4 ratio had significant correlation with the CRC diagnosis(P=0 .013 ,0 .006) and their 95%confidence intervals (CI) were 0 .576-0 .851 and 0 .612 -0 .887;when the CA242/CA72 4 cutoff value was 2 .6(CA242=28 .6 , CA72‐4=11) ,the sensitivity for early diagnosis of CRC was 91 .32% and the specificity was 94 .57% .Conclusion The sensitivity and specificity of CA242/CA72‐4 ratio in the early diagnosis of CRC are higher than those of CA242 or CA72‐4 single index ,espe‐cially w hen the ratio is close to 2 .6 ,high attention should be paid to the possibility of CRC .

ABSTRACT

Objective To investigate the change of serum CA724, CA242 and AFP levels before and after transcatheter artery chemoembolization (TACE) treatment of primary hepatic carcinoma(PHC)patients as well as its clinic significance. Methods Patients of PHC (n=45) and healthy adults (n=40) were enrolled. Serum samples were collected from each healthy people and PHC patients 2 days before TACE,l week and 1 month after TACE. Electrochemiluminescence Immunoassay (ECLI) was used to determine serum CA724, CA242 and AFP levels, and correlations among three indexes as well as their relationships with clinical data were also analysed., Change of serum CA724 and CA242 levels before and after TACE were compared in AFP0.05). CA724 and AFP expressions are associated with tumor size(P<0.05). After one month of therapy, serum CA724 level was obviously decreased in both AFP<400μg/L and AFP≥400μg/L groups. Before and after TACE therapy, there was a positive correlation between the expression of AFP and CA724(r=0.754,P<0.05). Conclusion Serum CA724 can be used as one of the tumor markers to assist the evaluation of curative effect of TACE on PHC.

ABSTRACT

Objective To conduct a methodological evaluation on flowcytomotry fluorescence detection of serum CA242, and to evaluate its value in the diagnosis of pancreatic cancer.Methods The blood samples of 40 cases of pancreatic cancer, 49 cases of other tumors, 48 cases of benign digestive diseases and 159 healthy volunteers were collected.Fowcytometry fluorescence immunoassay was used to detect serum level of CA242, and it was compared with routine ELISA method to measure its sensitivity and specificity.Results The detection limit of CA242 by flowcytometry fluorescence immunoassay was 0.89 U/ml, the linear range was 1~ 500 U/ml after confirmation.The within-batch CV was 3.37%~ 5.30%, between batch CV was 7.43% ~ 9.60%.When compared with routine ELISA, flowcytometry fluorescence immunoassay showed the equation of linear regression is Y =1.0398X-0.947, r =0.9687.Area under ROC curve was 0.811 ± 0.025 (95% CI 0.763 ~0.859), with 18.625 U/ml as the best cutoff value, the specificity was 92.0% and the sensitivity was 62.1%.Conclusions Flowcytometry fluorescence immunoassay for CA242 testing has the advantage of shorter detection time, miltiple sample and testing project detection, which is worth of clinical application.

ABSTRACT

Objective To determine the clinical value of EUS in combination with detection of CA19-9 and CA242 for early diagnosis of pancreatic cancer.Methods General features of high-risk group were determed by EUS and the plasma levels of CA19-9 and CA242 measured by ELISA.Results The serum level of CA19-9 was abnormal in 9.80% (10/102) and that of CA242 in 12.75% (13/102) of the patients.They were both abnormal in 7 cases.Of the 7 cases, 3 were found to suffer from pancreatic cancer by EUS, 1 had the tumor that had been clinically confirmed, 1 had posterior peritoneal tumor and 2 were normal.Among 102 cases detected by EUS, 2 showed low echo in pan-creatic head, 2 semi-cyst, 3 pancreatic cancer and proved by pathology.Accurate diagnotic rate, sensi-tivity and specificity were 90.20% (92/102), 50% (4/8) and 93.62% (88/94), respectively, for serum CA19-9.For the serum CA242, the 3 parameters were 87.25% (89/102), 50% (4/8) and 90.43% (85/94), respectively.For combination of both methods, they were 98.04% (100/102), 100%(8/8) and 97.87%(92/94), respectively.Accurate diagnostic rate of the latter was significantly higher than that of the former 2(P<0.05).Concision Combination of EUS with detection of serum CA19-9 and CA242 is of great vlaue for early screening of pancreatic cancer.

ABSTRACT

Objective To investigate the clinical significance of CA242 combined with TGF-? to diagnose adenocarcinoma of pancreas.Methods Thirty patients were involved in this study,who were hospitalized from Dec.2004 to Sept.2005.ELISA was used both to investigate the expression of CA242 in 30 cases and TGF-? in 20 cases.Twenty healthy volunteers were as controls.Results CA242 was increased in adenocarcinoma of pancreas compared with controls[(312?5)kU/L vs(56?2)kU/L,P0.05.Conclusion The specificity of each of CA242 and TGF-? to diagnose adenocarcinoma of pancreas is high.The sensitivity of TGF-? is higher than that of CA242.CA242 combined with TGF-? can increase the diagnosis rate of adenocarcinoma of pancreas.
