ABSTRACT

【Objective】 To analyze the difference of Ct value of HBsAg-/HBV DNA + in blood samples from different types of voluntary blood donors by double ELISA and HBV DNA (MP6) detection, and to investigate the correlation between Ct value and the frequency of repeated blood donation, the first nucleic acid reactivity and the interval time of previous blood donation, so as to provide reference for laboratory evaluation of the effectiveness of nucleic acid testing(NAT) strategy for repeated blood donors occult hepatitis B virus infection(OBI). 【Methods】 The Ct value and information of blood donors from February 2019 to January 2022 in our laboratory were collected. According to the cumulative number of blood donations, they were divided into two groups:first-time blood donor group (Group A) and repeated blood donor group (Group B). Group B was subdivided into Group C 1( twice of blood donation) and group C 2(three or more times of blood donation) according to the cumulative times of blood donation, and Group D 1(< 1 year), Group D 2(1-3 years), Group D 3(3 years or more) according to the first NAT reactivity and the time of previous blood donation, the difference of Ct value and resolution yeild of HBV DNA in each group was compared. The yeild of HBV DNA in two groups was compared by chi-square test, and the difference of Ct values were compared by Nonparametric test. 【Results】 From February 2019 to January 2022, a total of 270 283 blood donors were tested, including 135 695 in Group A and 134 588 in Group B. The yeild of HBV DNA in Group A was 0.150% (203/135 695), which was higher than that in Group B [0.083% (111/134 588)] (P <0.05).All Ct values were non-normal distribution by normal distribution test, and were expressed as median (quartile), the median values of MP6 and resolution Ct were 37.0(35.9,38.2) and 35.5(33.7,36.9) in Group A, 37.2(36.4,38.1) and 36.5(35.5,37.6) in Group B, respectively. Ct values of MP detection and resolution in Group A, of MP detection and resolution in group B, and of resolution in group A and B were all significant (P<0.05) From the cumulative number of blood donations to compare, the median values of MP detection and resolution Ct were 37.5(36.6,38.3) and 36.5(35.4,37.6) in Group C1,37.1(36.4, 37.9) and 36.6(35.6,37.8) in Group C2, respectively. Significant difference in resolution Ct value between Group A and Group C1, Group A and C2 was noticed(P<0.05), the median values of MP detection Ct in D1, D2 and D3 groups were 37.2(36.3,38), 37.1(36.5,37.9), 37.8(36.6.38.9),respectively, with median resolution CT values at 37.0(35.7,37.8), 35.9(34.8,36.9), 36.9(36.1,37.7), respectively. There was a significant difference in the resolution Ct values between between Group A and D1 and D3 groups (P<0.05), and there was a significant difference between the MP detection and resolution Ct values in D2 Group (P<0.05). The resolution Ct values in D2 and D3 Group were lower than those in D1 Group (P<0.05).The interquartile distribution of Ct values in Group A was wider than that in other groups, and the interquartile distribution of Ct values in Group B was more concentrated. Conclusion The Ct value of HBV DNA detected by nucleic acid in blood donors was correlated with different times of blood donation and different intervals of blood donation. The laboratories of blood station should pay attention to the nucleic acid test results of different types of blood donors to ensure blood safety.

ABSTRACT

Objective To study the effects of different CT values assignment methods on the dose calculation of radiotherapy plan for brain metastases,which will provide a reference for radiotherapy treatment planning based on MR images.Methods A total of 35 patients treated with radiotherapy for brain metastases were selected,with pre-treatment CT and MR simulated positioning performed at the same day.Based on the simulation CT images,three dimensional conformal radiation therapy (3D-CRT) or intensity modulated radiation therapy (IMRT) plans were calculated as the original plan (Plan1).The CT and MR images were rigidly registered and then the main tissues and organs were delineated on CT and MR images.The average CT values of each tissue and organ were calculated.Three groups of pseudo CT were generated by three CT values assignment methods based on the CT images:whole tissue was assigned 140 HU;cavity,bone and other tissues were assigned-700 HU,700 HU and 20 HU,respectively;different tissues and organs were assigned corresponding CT values.The dose distribution of Plan1 was recalculated on three groups of pseudo-CT to obtain Plan2,Plan3 and Plan4,respectively.Finally,the dosimetric difference between Plan1 and other plans (including Plan2,Plan3 and Plan4) were compared.Results The average CT values of bone and cavity were (735.3 ± 68.0) HU and (-723.9 ± 27.0) HU,respectively.The average CT values of soft tissues was mostly distributed from-70 to 70 HU.The dosimetric differences between Plan2,Plan3,Plan4,and Plan1 decreased in turn.The differences of maximum dose of lens were the biggest,which can reach more than 5.0%,1.5%-2.0% and 1.0%-1.5%,respectively,and the differences of other dose parameters were basically less than 2.0%,1.2%and 0.8%,respectively.In the pixelwise dosimetric comparison,the areas with more than 1% difference in the local target cases were mainly distributed in the skin near the field.On the other hand,those in the whole brain target cases were mainly distributed at the bone,cavity,bone and soft tissues junction,and the skin near the field.In addition,the dose calculation error of CT value assignment methods in 3D-CRT plan was slightly larger than that in IMRT plan,and that in whole brain target cases were significantly larger than that in local target cases.Conclusions Different CT value assignment methods have a significant effect on the dose calculation of radiotherapy for brain metastases.When appropriate CT values are given to bone,air cavity and soft tissue,respectively,the deviation of dose calculation can be basically controlled within 1.2%.And by assigning mass CT values to various tissues and organs,the deviation can be further controlled within 0.8%,which can meet the clinical requirements.

ABSTRACT

Objective:To investigate the CT value added value of the mean of critical value in the diagnosis of giant cell tumor clinical sensitivity.Methods: Aretrospective analysis, selected from March 2013 to November 2014, the clinical data of 120 patients with bone tumors treated, they can be divided into two groups at random, control group and the observer, 60 cases in each group, observer for patients with giant cell tumor of bone, the control for the patients with giant cell tumor of bone. Each patient chose three ROI, the added value of the mean values of enhanced CT and CT values mean as observed values, under the experience of observation analysis of patients with giant cell tumor of the added value of CT values mean critical value and its sensitivity in the clinical diagnosis of giant cell tumor of bone.Results: The observation group of patients after CT enhanced scan display area average CT value of HU(108±39)HU was significantly higher than the control group(82±78)HU CT value(t=3.348, P<0.05), statistically significant difference; Through the ROC curve analysis, found that when the added value of CT values mean acuity 96.5 as critical, sensitivity and specificity of diagnosis of giant cell tumors of bone are 90.9% and 90.9% respectively; When the CT value added value of the mean acuity 41.5 HU as the critical point, the sensitivity and specificity of diagnosis of giant cell tumor bone were 98.9%, 48.1% respectively.Conclusion: The bone giant cell average CT value added value of diagnosis of critical value of 96.5HU 41.5HU, and high sensitivity, but the specificity is low, the added value of CT values help determine the existence of the giant cell tumors of bone.

ABSTRACT

Objective To probe the changes of CT values in liver parenchyma in order to evaluate the therapeutic effect of acute pancreatitis.Methods 104 patients with acute pancreatitis which were diagnosed and treated by department of gastroenterology.Ac-cording to pathological results,the patients were divided into mild acute pancreatitis (MAP)group and severe acute pancreatitis (SAP)one.The CT values of liver parenchyma were measured before and after treatment,and the correlations between CT values changes and the amylase in blood and urine were analyzed.Results The CT values of liver parenchyma showed a negative correlation with the pathological severity of acute pancreatitis (r=-0.089,P <0.05).The accuracy using the changes of CT values to evaluate the therapeutic effect was significantly different between the MAP and the SAP group with different sensitivity of 92.2% and 85.7%and specificity of 33.3% and 94.1% respectively.In addition,the changed trend of CT values in liver parenchyma showed negative correlations with triglycerides and blood amylase.Conclusion CT scan is a useful imaging method in evaluating the liver damage and the therapeutic effect in patients with acute pancreatitis in emergency.
