ABSTRACT

Calcified chronic subdural hematoma is relatively common, however, its rupture into the subcortical white matter is very rare. A 37-year-old patient with a large, calcified, chronic subdural hematoma which ruptured intracerebrally forming a frontal lobe hemorrhage is reported. Craniotomy for removal of the hematoma and calcification achieved marked reduction in seizure frequency.

ABSTRACT

The authors report a case of calcified chronic subdural hematoma with arachnoid cyst. A 45-year-old man had a 30-year history of epilepsy. Brain CT revealed lentiform calcified mass lesion with a cyst on the right temporal lobe, and an osteoplastic craniotomy was performed. The mass contained a clay-like substance with shiny crystal. And microscopically, a thick fibrous membrane with a band of calcification was seen.Postoperatively, the frequency of epolepsy decreased. To the author's knowledge, this is the first report to document calcified chronic subdural hematoma associated with arachnoid cyst.

ABSTRACT

Althouth the connective-tissue membrane surrounding a subdural hematoma has been described as hyalinized, calcified partially or in patches, a totally organized, calcified or ossified chronic subdural hematoma in the elderly patient is extremely rate. We report an asymptomatic case of a huge calcified chronic subdural hematoma in a 67-year-old male patient who had no previous trauma history. Computerized tomographic(CT) scan showed an oval-shaped mass with totally calcified rim at the left frontotemporoparietal region. After en-block removal of the calcified mass, postoperative CT scan showed intracerebral hematoma at the deepest portion of the calcified mass that was partially adhesive to the pia mater. The patient suffered from a transient motor aphasia after the operation. Relevant literatures are briefly reviewed.

ABSTRACT

After von Rokitansky's description in 1984 of a calcified chronic subdural hematoma, the number of such lesions has reached so far a total of 108. The majority of patients reported have been children and young adults. On review of the literature, elderly patients reported over the age of sixty five years are only 5, so far as we know. A 68-year-old Korean male with a calcified chronic subdural hematoma is reported. He was admitted to our department because of a status epilepticus. During Korean war (1950), abut 37 years before admission, he had been suffered from a blunt head injury, but mistreated then, and afterwards th had several episodes of convulsion. Neurological examination on admission revealed only a drowsy consciousness and mild papilledema on the left. Plain skull X-ray films and brain CT scan demonstrated a left sided biconvex type of intracranial calcification. A left frentoparietal craniotomy was performed. After opening the dura mater, the thickened outer membrane, the subdural hematoma and the thin bone like plate were removed thoroughly. Content of the subdural hematoma was brown muddy substance. In the eight months following the operation, the generalized seizure did not appear. Postoperative CT scan demonstrated complete re-expansion of the brain parenchyme.
