ABSTRACT

Background: Hemorrhoidal disease is one of the most common anorectal conditions encountered in daily practice for centuries. Conservative management of hemorrhoids (with oral diosmin, calcium dobesilate cream local application) and injection sclerotherapy with 3% polidocanol-both are very commonly practiced, cost effective, and outpatient department-based management of hemorrhoids which bear rare adverse effects. Comparative efficacy of injection sclerotherapy and conservative management in cases of hemorrhoids is still confusing and contradictory to each other according to different studies. Aims and Objectives: This study was conducted to compare the effectiveness between conservative management of hemorrhoids and injection sclerotherapy. Materials and Methods: This institution-based experimental study done over 74 patients of first- and second-degree internal hemorrhoids. Thirty-seven patients were randomly selected and managed with conservative management, and the rest with injection sclerotherapy. Treatment responses were assessed after 3 weeks and in unsatisfactory responses, treatment was repeated for another 3 weeks. Reassessment was done after 6 weeks and 6 months. Results: This study demonstrated that both injection sclerotherapy and conservative management had improved per rectal bleeding after 3 and 6 weeks of treatment, but the former had a significant edge over the counterpart after 6 months of treatment (56.8% vs. 35.1%, P = 0.002). In case of reduction of hemorrhoidal mass – injection sclerotherapy had significantly better outcome in all the follow-up visits, that is, 3 weeks (54.1% vs. 13.5%, P = 0.001), 6 weeks (70.3% vs. 35.1%, P = 0.008) and 6 months (59.5% vs. 27.0%, P = 0.003). Injection sclerotherapy had superior overall treatment outcome after 6 weeks of treatment (73% vs. 45.9%, P = 0.04) and long-term periods (56.85% vs. 27%, P < 0.01). Conclusion: Injection sclerotherapy shows significantly better and long-lasting improvement in symptomatic hemorrhoids in comparison to conservative management which is mainly able to limit per rectal bleeding only for a short period of time.

ABSTRACT

Objective:To explore the effect of epalrestat combined with calcium dobesilate on cardiovascular and cerebrovascular diseases and oxidative stress in diabetes patients with peripheral neuropathy.Methods:In a retrospective analysis, 92 patients with diabetic peripheral neuropathy were admitted to the Department of Endocrinology and Metabolism Hospital of Bozhou People’s Hospital from Jun. 2018 to Jun. 2021, which were divided into observation group and control group according to the different treatment methods, with 46 cases in each group. The control group were treated with epalrestat. On this basis, the observation group were given calcium dobesilate combined treatment. The two groups were compared in terms of the Michigan diabetes neuropathy score (MDNS) Michigan neuropathy screening instrument (MNSI) score, oxidative stress reaction [serum superoxide dismutase (SOD), reduced glutathione (GSH) ], clinical efficacy, complications and adverse reactions Using t-test and chi square test.Result:After treatment, the MDNS and MNSI scores of both groups decreased, and the observation group [ (23.49±3.73), (8.49±1.97) ] were lower than the control group [ (28.49±3.85), (11.53±2.28) ] ( t=8.337, 6.843, P<0.05) ; After treatment, the levels of SOD and GSH in both groups increased compared to those before treatment, and the observation group [ (38.96±4.89), (89.79±6.92) ] were higher than the control group those [ (36.42±4.61), (86.74±6.20) ] ( t=2.563, 2.226, P=0.012, 0.028) ; The clinical efficacy of the observation group was higher than that of the control group ( Z=1.592, P=0.042) ; The incidence of complications in the observation group after 1 year was significantly lower than that in the control group ( χ2=4.389, P=0.036) ; The incidence of adverse reactions in the observation group was slightly lower than that in the control group ( χ2=0.155, P=0.694) . Conclusion:Epalrestat combined with calcium dobesilate is effective in the treatment of diabetes peripheral neuropathy, can effectively reduce complications, and has high treatment safety, which is worthy of clinical application.

ABSTRACT

OBJECTIVES: This study assessed the protective effect of calcium dobesilate against contrast-induced nephropathy (CIN) after coronary angiography (CAG) or percutaneous coronary intervention (PCI) in patients with diabetes and chronic kidney disease (CKD). METHODS: A total of 130 patients with diabetes and CKD estimated glomerular filtration rate: 30-90 mL/min/1.73m2 were enrolled and included in the analysis. They were divided into experimental (n=65) and control groups (n=65). Patients in the experimental group were administered oral calcium dobesilate (500 mg) three times daily for 2 days before and 3 days after the procedure. The serum creatinine (SCr), cystatin C (Cys C), and neutrophil gelatinase-associated lipocalin (NGAL) levels were measured before and after the procedure. RESULTS: The mean SCr level at 24h after the procedure was found to be significantly lower in the experimental group than in the control group (79.1±19.6 μmol/L vs. 87.0±19.3 μmol/L, p=0.023). However, the Cys C and NGAL levels were not significantly different between the two groups at all measurement time points (all p>0.05). The incidence of CIN defined by the SCr level was significantly lower in the experimental group than in the control group (3 [4.6%] vs. 13 [20.0%], p=0.017). However, the incidence of CIN defined by the Cys C level was not statistically different between the two groups (7 [10.8%] vs. 7 [10.8%], p=1.000). CONCLUSIONS: This study revealed that calcium dobesilate has no preventive effect against CIN in patients with diabetes and CKD.

ABSTRACT

@#AIM: To explore the clinical value of fundus retinal argon laser photocoagulation combined with calcium dobesilate in the treatment of diabetic retinopathy(DR).<p>METHODS:A prospective study from August 2017 to August 2019 was conducted. Totally 182 patients(326 eyes)with DR were divided into two groups. The observation group received fundus retinal argon laser photocoagulation combined with calcium dobesilate, and the control group received fundus retinal argon laser photocoagulation. Flash electroretinography was performed before treatment and after treatment, and changes in a and b waves were recorded. Changes in the best corrected visual acuity(BCVA), retinal neovascularization fluorescein leakage area, central macular thickness(CMT)and retinal microcirculation parameters were detected. The absorption time of retinal edema, bleeding and exudation was recorded, and the incidence of postoperative complications was counted.<p>RESULTS: At 6mo after treatment, a-wave and b-wave amplitudes of the two groups were decreased(<i>P</i><0.001). The b-wave amplitude of observation group was larger than that of the control group(<i>P</i><0.05). There were no statistically significant differences in a-wave and b-wave peak time and amplitudes between the two groups(<i>P</i>>0.05). The BCVA of the two groups was increased, and the macular leakage area and CMT were reduced(<i>P</i><0.001). Compared with the control group, the BCVA of the observation group was higher, the macular leakage area and CMT were smaller, peak blood flow velocity and average blood flow velocity of the central retinal artery were higher, and the vascular resistance index and pulsatility index were lower(<i>P</i><0.001). The absorption time of edema, bleeding and exudation was shorter in observation group than in the control group(<i>P</i><0.001). There was no significant difference in the incidence of surgical complications between the two groups(<i>P</i>>0.05).<p>CONCLUSION: Fundus retinal argon laser photocoagulation combined with calcium dobesilate is superior to retinal photocoagulation alone in the treatment of DR. The former can promote the improvement of visual acuity, reduce fluorescein leakage, and promote the absorption of retinal edema, bleeding and exudation, which is conductive to improving retinal microcirculation and inhibiting neovascularization.

ABSTRACT

Objective:To investigate the combined therapy of calcium dobesilate capsules, oral administration of Dahuoluo capsules, and intravenous drip of Danhong injection in treatment of patients with nonproliferative diabetic retinopathy.Methods:106 cases were randomly selected from the patients with nonproliferative diabetic retinopathy admitted in Traditional Chinese and Western Hospital of Dazhou City from Jan. 2019 to Dec. 2019. Based on the random numbers table, the cases were divided into control group and intervention group, with each group 53 patients. They were respectively treated with the therapy of calcium dobesilate capsules alone and the combined therapy of calcium dobesilate capsules, oral administration of Dahuoluo capsules and intravenous drip of Danhong injection, and the therapeutic effects were compared.Results:After treatment, the visual acuity of patients in both groups significantly improved ( P<0.05) , and the visual acuity of patients in the intervention group was higher than that of patients in the control group ( P<0.05) ; after treatment, the peak systolic velocity (PSV) and end diastolic velocity (EDV) of the central retinal artery (CRA) and posterior ciliary artery (PCA) of the patients in both groups significantly increased ( P<0.05) , while RI significantly reduced ( P<0.05) , and PSV and EDV of CRA and PCA in the intervention group were significantly higher than those of the control group ( P<0.05) , while RI was significantly lower than that of the control group ( P<0.05) . After treatment, the therapeutic efficiency of the intervention group was 96.23%, significantly higher than 79.25% in the control group ( χ2=7.102, P<0.05) . The incidence of adverse reactions in the intervention group was 9.43%, which had no significant difference with 3.78% in the control group ( χ2=1.377, P>0.05) . After targeted treatment was given, the adverse reactions disappeared. Conclusion:The combined therapy of calcium dobesilate capsules, oral administration of Dahuoluo capsules, and intravenous drip of Danhong injection was significantly effective in treatment of patients with nonproliferative diabetic retinopathy.

ABSTRACT

Objective:To investigate the clinical efficacy of vascular rehabilitation capsules and calcium dobesilate on non-proliferative diabetic retinopathy and its effect on inflammatory factors.Methods:A total of 80 patients with type 2 diabetes and non-proliferative retinopathy diagnosed in the endocrinology department of First People′s Hospital of Huaihua were randomly divided into the control group ( n=40 cases, 72 eyes) and the observation group ( n=40 cases, 76 eyes). The two groups were given conventional basic treatment. The control group was given calcium dobesilate, and the observation group was added with the combination of vascular rehabilitation capsules and treatment for 24 weeks. After treatment, the clinical therapeutic effects of the two groups were evaluated, and the levels of inflammatory factors interleukin-6 (IL-6) and high-sensitivity C-reactive protein (hs-CRP) were analyzed before and after treatment. The occurrence of adverse reactions was monitored during treatment. Results:The total effective rate of the observation group was higher than that of the control group (92.5% vs 75.0%, χ 2=4.501, P<0.05). There was no significant difference in the levels of IL-6 and hs-CRP in the two groups of patients before treatment [(4.41±0.92)pg/ml vs (4.47±0.84)pg/ml, (16.36±4.02)mg/L vs (16.38±3.26)mg/L]; after 12 weeks and 24 weeks of treatment, the levels of IL-6 and hs-CRP in the observation group were significantly lower than those in the control group [12 h: (2.55±0.64)pg/ml vs (2.94±0.59)pg/ml, (12.21±3.48)mg/L vs (14.33±3.04)mg/L; 24 h: (1.93±0.49)pg/ml vs (2.36±0.45)pg/ml, (9.63±3.05)mg/L vs (11.52±2.15)mg/L, all P<0.05]. During the treatment, no serious adverse events occurred in both groups of patients. Conclusions:Vascular rehabilitation capsules combined with calcium dobesilate is an effective treatment for non proliferative diabetic retinopathy. It can improve the clinical efficacy, safety and reduce inflammatory reaction, which is worthy of further clinical application.

ABSTRACT

We observed the effect of calcium dobesilate (CaD) on apoptosis induced by cisplatin in human proximal tubular epithelial cells (HK-2) and explored the possible mechanism. Based on HK-2 cells apoptosis model induced by cisplatin, CCK-8 method was used to detect the effect of CaD on the proliferation of HK-2. Apoptosis was detected by flow cytometry. Reactive oxygen species (ROS) assay was used to evaluate the level of oxidative stress. The mitochondrial membrane potential was measured by JC-1 method. The expression levels of p53, caspase-3, bcl-2 and bax in cisplatin-induced HK-2 were detected by Western blot. The expression of renal injury factor 1(KIM-1) and neutrophil gelatin-related apolipoprotein (NGAL), markers of acute kidney injury, were detected by ELISA. The results showed that CaD could reduce the oxidative stress level induced by cisplatin and inhibit apoptosis in renal tubular epithelial cells. Cisplatin can up-regulate the protein expressions of p53, caspase-3, bax, KIM-1 and NGAL, and reduce the expression of bcl-2. After using CaD, the protein levels of KIM-1, NGAL, p53, caspase-3 and bax were significantly reduced, while the levels of bcl-2 were increased. This study has shown that CaD can alleviate cisplatin-induced HK-2 injury and inhibit HK-2 apoptosis, which may be related to the regulation of bax/bcl-2/caspase-3 apoptosis signaling pathway.

@#AIM: To analyze the effect of 532nm laser panretinal photocoagulation(PRP)combined with calcium dobesilate on fundus microcirculation and macular thickness in patients with diabetic retinopathy(DR), so as to provide experimental data for their clinical treatment. <p>METHODS: According to the non-randomized clinical concurrent control study and voluntary principle, 174(239 eyes)DR patients were divided into observation group and control group. Both groups were treated with 532nm laser PRP, and the observation group was additionally treated with calcium dobesilate. BCVA(LogMAR)vision, clinical effect, fundus microcirculation, macular thickness and other clinical indexes were compared between the two groups before and after treatment(at 3mo after PRP). The incidence of complications during treatment was statistically analyzed. <p>RESULTS: After treatment, BCVA in both groups were increased, and it was higher in observation group than control group. The clinical curative effect of observation group was better than that of control group(<i>P</i><0.001). After treatment, the resistance index(RI)of central retinal artery(CRA)and osterior ciliary artery(PCA)in both groups were decreased(<i>P</i><0.05), while the peak systolic velocity(PSV)and end diastolic velocity(EDV)were increased(<i>P</i><0.05). RI in observation group was lower than that in control group, while EDV and PSV were higher than those in control group(<i>P</i><0.001). After treatment, the macular thicknesses and other clinical indexes(hemangioma, bleeding spots, gray value of visual field)in both groups were decreased(<i>P</i><0.05), and they were lower in observation group than control group(<i>P</i><0.001). There was no significant difference in the incidence rate of complications between the two groups(0.8% <i>vs </i>2.6%)(χ2=0.344, <i>P</i>=0.557). <p>CONCLUSION: Compared with 532nm PRP in treatment of DR, curative effect of 532nm PRP combined with calcium dobesilate is better. The latter can improve fundus microcirculation and macular thickness more significantly, with high safety.

ABSTRACT

Objective: Haemorrhoids are associated with bleeding, pain, itching, exudation and swelling. So there is an unmet need foreffective treatment by both oral and topical drugs. Hence this study was designed to evaluate the efficacy of oral EuphorbiaProstrata fortified with Calcium Dobesilate and topical cream of Euphorbia Prostrata with Lidocaine. Methods: The study was aprospective, open label, single arm, multicentric study, in 30 patients with hemorrhoids treated with oral FDC EuphorbiaProstrata extract 100 mg plus Calcium Dobesilate 500 mg and topical FDC cream of Euphorbia Prostrata extract 1% w/w plusLidocaine 3% w/w for 14 days.Symptoms like bleeding, pain, itching, exudation and swelling were assessed and scored duringand at the end of the study. Results: There was decline from the baseline value scores right from day 4 i.e. bleeding (0.97±0.15),pain (0.60±0.13), itching (0.47±0.13), exudation (0.13±0.06) and swelling (0.43±0.09); after 7 days bleeding (0.15±0.07), pain(0.12±0.06), itching (0.15±0.07), exudation (0.15±0.07) and swelling (0.19±0.08) and after day 14 the symptoms of bleeding(0.09±0.06), pain (0.08±0.06) and swelling (0.08±0.06) were almost cured while symptoms like itching (0.00±0.00), exudation(0.00±0.00) disappeared completely. There was reduction in the number of patient’s population with symptoms towards the end ofthe study. Conclusion: Oral Euphorbia Prostrata extract 100 mg fortified with Calcium Dobesilate 500 mg and topical cream ofEuphorbia Prostrataextract 1% w/w with Lidocaine 3% w/w showed maximum improvement during first 4 days of therapy andachieved total improvement till the end of therapy thus providing multimodal targeted approach to treat a multimodalhaemorrhoidal disease.

@#AIM: To investigate the clinical value of epalrestat combined with compound anisodine in treatment of non-proliferative diabetic retinopathy(NPDR).<p>METHODS: According to the random remainder method, they were divided into A, B and C three groups. Group A was treated with calcium dobesilate, group B was treated with epalrestat, and group C was treated with epalrestat combined with compound anisodine. All groups were treated for 6mo. The BCVA and fundus color photos, fundus fluorescein angiography(FFA), oscillatory potentials of electroretinogram(ERG-Ops)and optical coherence tomography(OCT)were performed before and after 1mo, 3mo and 6mo of treatment. The therapeutic effect was evaluated. <p>RESULTS: Before treatment, there was no statistical difference among the three groups in BCVA, Ops wave amplitude or central macular thickness(<i>P</i>>0.05). The BCVA was improved, Ops wave amplitude was increased and the central macular thickness was decreased in the three groups after 1mo, 3mo and 6mo of treatment. Compared with the same group before treatment, there were statistically significant differences(<i>P</i><0.05). The improvement of visual acuity at different time and the increase of Ops wave amplitude were better in group B than group A, better in group C than group B, while the decrease of central macular thickness was greater in group B than group A, greater in group C than group B(all <i>P</i><0.05). The total effective rate among the three groups were significant difference(<i>P</i><0.05), and the total effective rate of group C was better than that of group B and group A(<i>P</i><0.0167).<p>CONCLUSION: Epalrestat combined with compound anisodine is effective in treatment of DR. The combined treatment can significantly improve the visual acuity, relieve macular edema and promote the recovery of retinal function.

ABSTRACT

Objective To observe the efficacy of combination therapy of calcium dobesilate dispersible and monosialotetrahexosylganlioside sodium on interleukin-6 (IL-6) and monocyte chemotactic protein-1 (MCP-1) in elderly patients with painful diabetic peripheral neuropathy.Methods From January 2012 to May 2017,in the Second People's Hospital of Lianyungang 70 patients of painful diabetic peripheral neuropathy,aged ≥60 years,were analyzed in this study.They were randomly divided into observation group (35 cases) and control group (35 cases).The observation group was treated with 40mg monosialotetrahexosylganlioside sodium dissolved in 250mL physiological saline,intravenous infusion per day,and oral calcium dobesilate dispersible 0.5g twice a day for two weeks.The control group was treated with methylcobalamin injection 0.5mg per day for two weeks.The clinical treatment effects and levels of IL-6 and MCP-1 were observed and compared between the two groups.Results After two weeks of treatment,the MDNS and MNSI scores of the observation group [(13.09 ± 5.38)points,(2.53 ± 1.19)points] were significantly lower than those of the control group [(18.31 ± 6.13) points,(4.19 ± 1.05) points,t =2.036,2.365,all P < 0.05] and those before treatment [(21.26 ± 4.28) points,(5.40 ± 0.89) points,t =3.251,3.698,all P < 0.05].The VAS-PI scores in the observation group [(6.24 ± 1.25) points,(4.13 ± 1.69) points] were significantly lower than those in the control group[(7.26 ± 1.28) points,(6.34 ± 2.65) points] at the first and second week (t =3.265,5.395,all P < 0.05).The serum levels of inflammatory cytokines IL-6 and MCP-1 in the observation group [(15.16 ±0.88) ng/L,(157.19 ± 11.22) ng,/L] were significantly lower than those in the control group[(17.87 ± 1.19) ng/L,(198.21 ± 12.07)ng/L,t =2.152,1.365,all P <0.05]and those before treatment[(20.26 ± 1.05) ng/L,(260.44 ± 13.63) ng,/L,t =1.235,0.965,all P < 0.05].Conclusion Combination of calcium dobesilate and mono-sialotetrahexosyl ganglioside may alleviate the sensory and pain sensations in patients with painful diabetic peripheral neuropathy,possibly by reducing the level of inflammatory cytokines IL-6 and MCP-1.

ABSTRACT

·AIM: To investigate the effect of calcium dobesilate on vitreous hemorrhage in patients with proliferative diabetic retinopathy ( PDR ) after pan retinal photocoagulation (PRP). ·METHODS:Totally 62 patients (30 cases with binocular lesions, 32 cases with monocular lesions, a total of 92 eyes) with PDR who were treated in our hospital from January 2015 to July 2017 were selected as the subjects. They were divided into the control group ( treated with pan retinal photocoagulation, n = 30, 17 cases with monocular lesions, 13 cases with binocular lesions, a total of 43 eyes ) and the study group ( treated with calcium dobesilate on the basis of treatment for the control group, n=32, 15 cases with monocular lesions, 17 cases with binocular lesions, a total of 49 eyes ). The recovery of visual acuity, blood rheology ( plasma viscosity, hematocrit, erythrocyte deformation index) and the incidence of complications such as vitreous hemorrhage in the two groups after surgery were observed. ·RESULTS: There was no significant difference between the two groups in the rate of excellent and good visual acuity, plasma viscosity, hematocrit or erythrocyte deformability index before treatment ( P>0. 05 ). After treatment, the rate of excellent and good visual acuity in the study group was significantly higher than that in the control group (P<0. 05). After treatment, the plasma viscosity and hematocrit decreased significantly while the erythrocyte deformability index significantly increased only in the study group, and changes of above -mentioned indexes in the study group were more obvious than those in the control group after treatment (P<0. 05). The incidence rate of vitreous hemorrhage and total incidence rate of complications in the study group were significantly lower than those in the control group ( P<0. 05). ·CONCLUSION: The application of calcium dobesilate in patients with PDR after pan retinal photocoagulation can effectively improve the recovery of visual acuity and reduce the incidence of complications such as vitreous hemorrhage. The mechanism may be related to effectively improving the hemodynamics.

·AIM: To study the effect of panretinal photocoagulation ( PRP ) combined with calcium dobesilate on fundus microcirculation and the thickness of the retinal nerve fiber layer (RNFL) in patients with diabetic retinopathy (DR). ·METHODS: Totally 166 cases of DR patients in the Department of Ophthalmology of our hospital in October 2015 to October 2017 were selected. According to the random number table method, 166 cases were divided into the control group ( Group A) and the study group (Group B), with 83 cases in each group. Group A was treated with PRP, and Group B was given calcium dobesilate on the basis of Group A. Observation and comparison between the two groups on clinical curative effect, postoperative complication rate and the changes of best corrected visual acuity ( BCVA ), blood rheology index [ hematocrit ( Hct), erythrocyte sedimentation rate ( ESR ), deformation of erythrocyte ( DE ), plasma viscosity (PV), aggregation of erythrocyte (AE)] and the changes of hemodynamic indexes [resistance index (RI), end diastolic velocity (EDV), peak systole velocity (PSV) of central retinal artery ( CRA )] before and after treatment. Besides, the RNFL thickness of the optic disc, the nasal side, the inferior and the superior side, and the average RNFL thickness of the optic disc center were observed and compared. · RESULTS: After treatment, BCVA in both groups decreased significantly compared with that before treatment (P<0. 05), and there was no significant change between Group A and B (P>0. 05), and the effective rate in Group B was significantly higher than that in Group A (P<0. 05 ). There was no significant difference in the incidence of complications and the rate of loss of visits in the two groups of A and B (P>0. 05). Compared with those before treatment, the EDV and PSV values of two groups increased after treatment (P<0. 05), and RI value decreased (P<0. 05). After treatment, the EDV and PSV values of CRA in Group B were significantly higher than those in Group A (P<0. 05), and the values of RI and Hct, ESR, PV, DE and AE were significantly lower than those of Group A ( P<0. 05 ). After treatment, the average, inferior and superior RNFL of the two groups were all thinner than those before the treatment (P<0. 05). The decrease of average, inferior, superior and nasal RNFL of Group A was more than those in the Group B (P<0. 05). ·CONCLUSION: Retinal photocoagulation combined with calcium dobesilate in the treatment of DR is effective and safe. It can effectively improve the fundus microcirculation and reduce the damage of PRP to DR patients' visual acuity and retinal nerve fiber layer thickness.

ABSTRACT

AIM: To analyze the effect of calcium dobesilate combined with hypoglycemics for patients with diabetic retinopathy (DR) and cataract, and the influence on microcirculation of eye fundus, hypoxia inducible factor-1α (HIF-1α), vascular endothelial growth factor (VEGF) level. METHODS: Totally 98 DR patients with cataract (126 eyes) from January 2013 to December 2015 in our hospital were selected as the research objects, and were divided into two groups randomly(treatment group:64 eyes in 49 patients, control group: 62 eyes in 49 patients). The control group was treated with acarbose tablet and metformin, while treatment group was treated with calcium dobesilate additionally. The clinical effect, the glycemic control effect, serum HIF-1α and VEGF level, eye function and fundus microcirculation of two groups after 12mo were compared. RESULTS: After 12mo, the total effective rates of two groups were 87.5%,61.3% respectively, which indicated significantly difference (P<0.05); the vision of treatment group was significantly higher than that of control group (P<0.05). Two groups' blood glucose level decreased significantly, and no significant difference between two groups(P>0.05). After 1-month treatment, the plasma viscosity, erythrocyte aggregation index, erythrocyte deformability index, erythrocyte sedimentation rate and hematocrit in the treatment group were significantly lower than those in the control group (P<0.05). The PSV and EDV of the posterior ciliary artery and central artery in the treatment group were significantly higher than those in the control group (P<0. 05), and RI was significantly lower than that in the control group (P<0.05). After 12-month treatment, the HIF-1α level of two groups were 35 90士11.36mmol/L, 46.75士12.08mmol/L respectively;the VEGF of two groups level were 89.72士13.61mmol/L, 110.30士16.74mmol/L, respectively, the treatment group's HIF- 1α level and VEGF were significantly lower than control group (P<0.05), and both decreased significantly after treatment (P<0.05). CONCLUSION: Calcium dobesilate combined with hypoglycemics can effectively increase the clinical effect in the treatment of retinopathy diabetic cataract, effectively control blood glucose, improve microcirculation of eye fundus, decrease HIF - 1α, VEGF level, inhibit angiogenesis.

ABSTRACT

·AIM: To study the effect of calcium dobesilate combined with panretinal photocoagulation on severe non-proliferative diabetic retinopathy patients (NPDR). ·METHODS:A total of 92 patients (184 eyes) with severe NPDR were collected in our hospital from December 2014 to April 2017, and divided into laser group and combined treatment group by random number table method. The laser group was treated with panretinal laser photocoagulation, and the combined treatment group received calcium dobesilate combined with retinal laser photocoagulation. The effect of the two groups was compared on the retinal microcirculation parameters. · RESULTS: Seven weeks after treatment, treatment effective rate in combined treatment group was higher than that in laser group (P<0.01). Fundus examination parameter such as Ops OS2, subfoveal choroid thickness (SFCT) in combined treatment group were higher than those in laser group (P<0.01). Retinal microcirculation parameters such as PSV, MV in combined treatment group were higher than the level of the laser group,RI,PI levels were lower than those in laser group (P<0.01). · CONCLUSION: Calcium dobesilate combines with panretinal photocoagulation in severe NPDR patients can effectively improve the overall effect and optimize the fundus structure and retinal microcirculation.

ABSTRACT

·AIM:To investigate the effect of calcium dobesilate in the treatment of non-proliferative diabetic retinopathy (NPDR) after panretinal laser photocoagulation. ·METHODS: A total of 40 patients (62 eyes) with clinically diagnosed severe NPDR were randomly divided into control group (22 cases,31 eyes) and study group (18 cases,31eyes). Two groups were all given pan-retinal photocoagulation treatment,while the study group continued to receive calcium dobesilate for 12mo after treatment. After 12mo, before treatment and 12mo after treatment, the changes of best corrected visual acuity (BCVA),average threshold sensitivity, and retinal nerve fiber layer(RNFL) thickness were compared between two groups. · RESULTS: The response rate was 55% and 84%, respectively in the control group and study group, the difference was statistically significant(P<0.05). Compared to the pre-treatment measurement,the average threshold sensitivity and RNFL were reduced at 12mo after treatment in control group, the difference were statistically significant(P<0. 05). Compared to the pre-treatment measurement,the average threshold sensitivity and RNFL were reduced at 12mo after treatment in study group,the difference were not statistically significant(P>0.05). At 12mo after treatment, the difference of the average threshold sensitivity and RNFL between two groups were statistically significant(P<0.05). ·CONCLUSION: calcium dobesilate could be effective in preventing the retina from damage of retinal photocoagulation in DR. Compared with simple laser photocoagulation, panretinal laser photocoagulation combined with calcium dobesilate can improve curative effect and visual acuity in NPDR patient.

ABSTRACT

AIM:To investigate the clinical effect of compound Xueshuantong capsule combined with calcium dobesilate in the treatment for patients with early diabetic retinopathy ( DR) .?METHODS: According to the random table method, 54 cases of DR patients ( 68 eyes ) in the ophthalmological clinic of our hospital from February 2015 to November 2016 were divided into control group and research group with 27 cases (34 eyes) in each group. Patients in the control group were treated with calcium dobesilate tablets, while patients in the research group were treated with compound xueshuantong capsule based on the control group. The duration of treatment was 5mo in both groups. The total clinical efficiency of the two groups was calculated, and the visual, blood glucose and inflammatory factors of the two groups were detected before and after treatment, in addition the adverse reaction and recurrence rate were observed.?RESULTS:The total clinical efficiency of the patients in the research group was 91% ( 31/34 ) , which was significantly higher than 68% (23/34) of the control group (P<0. 05). Compared with before treatment, the vision, the volume of hemangioma, macular thickness, plaque area, grey value view, blood glucose and inflammatory factor levels of two groups of patients after treatment dropped significantly, meanwhile the vision, blood sugar and inflammation index levels of research group were better than control group, and the differences were statistically significant (P<0. 05). No adverse reactions occurred during the treatment of the two groups. The recurrence rate of research group was 3% ( 1/31 ) , which was significantly lower than that of the control group as 26% (6/23) (P=0. 039).? CONCLUSION: The curative effect of compound xueshuantong capsule combined with calcium dobesilate in the treatment of patients with early DR is significant. It can significantly improve eyesight, blood glucose and inflammatory response and has a good forward curative effect.

ABSTRACT

@#AIM: To investigate the clinical effect of Qi Ming granule combined with calcium dobesilate in treatment of non-proliferative diabetic retinopathy(NPDR).<p>METHODS: Totally 94 patients with I~III stage NPDR in our hospital from January 2014 to May 2016 were collected and randomly divided into control group and observation group, all were unilateral, with 47 eyes in each group. The two groups were given control of blood glucose, diet and exercise therapy and other conventional treatment. The control group were treated with conventional western medicine calcium dobesilate treatment, 0.5g,3 times per day, the observation group combined with Qi Ming granule treatment based on control group treatment. Both groups were treated for 3mo. Two groups were observed before and after treatment for visual sensitivity and changes of serum cytokines \〖vascular endothelial growth factor(VEGF)and insulin-like growth factor-1(IGF-1), pigment epithelium derived factor(PEDG)\〗. Therapeutic effect and safety of the two groups were comprehensively evaluated and compared. <p>RESULTS: The average sensitivity of the observation group after treatment was lower than that before treatment and the control group after treatment, the difference were statistically significant(<i>P</i><0.05).The VEGF,IGF-1,PEDG of observation group after treatment, were significantly improved compared with the control group, the difference were statistically significant(<i>P</i><0.05). There were no obvious adverse reactions in the two groups, the total effective rate was 89% in the observation group, which was higher compared with the control group 72%, and the difference was statistically significant(<i>P</i><0.05). <p>CONCLUSION: Qi Ming granule combined with western medicine in the treatment of DR can effectively improve the function of the retina and ease the progress of the disease, and the safety is good, the clinical treatment effect is remarkable.

Objective To investigate the calcium dobesilate tablets combined with mecobalamin injection in treatment of diabetic peripheral neuropathy and effects on oxidative stress and neurotrophic factor.Methods 82 cases of diabetic peripheral neuropathy(DPN) patients from February 2015 to October 2016 in our hospital were selected and randomly divided into observation group and control group with 41 cases in each group.The control group was treated with mecobalamin injection, the observation group were treated with calcium dobesilate tablets.The course of treatment was two weeks in two groups.Results The total efficiency of the observation group (90.24%) was higher than the control group (68.29%) , the difference was statistically significant ( P<0.05 ) , patients in the observation group after the treatment of common peroneal nerve and median nerve SCV and MCV higher than the control group, the difference was statistically significant (P <0.05), the level of serum malondialdehyde (MDA) in the observation group was lower than that in the control group and the level of superoxide dismutase ( SOD) in the observation group was higher than that in the control group, the difference was statistically significant (P<0.05), the levels of serum NGF and BDNF in the observation group were higher than those in the control group, the difference was statistically significant (P<0.05), the two groups were no obvious adverse reactions.Conclusion Calcium dobesilate tablets and mecobalamin injection curative effect in the treatment of diabetic peripheral neuropathy , and patients could improve oxidative stress and neurotrophic factors, and the safety is good, has the important research significance.

ABSTRACT

Objective To explore the effect of Rhodiola kirilowii Regel combined with calcium dobesilate in the treatment of early diabetic nephropathy.Methods 80 patients with early diabetic nephropathy were selected,and they were divided into treatment group and control group according to the digital table,40 cases in each group.The two groups were given diet control and appropriate exercise and positive controlled blood sugar,blood fat,blood pressure.The control group was given calcium dobesilate,based on this,the treatment group was given Rhodiola kirilowii Regel 10mL into 5% glucose injection 250mL and 3 unit insulin to intravenous drip.The period of treatment was fifteen days.The UAER,Scr and BUN were compared between two groups before treatment and fifteen days after treatment.Results Before treatment,the Scr,BUN and UAER in the control group were (88.00 ± 18.19) μmol/L,(5.98 ± 1.92) mmol/L,(123.31 ± 60.01) μg/min respectively,which in the treatment group were (85.80 ± 18.31) μmol/L,(5.96 ± 1.94) mmol/L,(140.21 ± 62.92) μg/min respectively,there were no statistically significant differences between the two groups(t =-1.191,-0.016,0.432,all P >0.05).After treatment,the Scr,BUN and UAER in the control group were (84.61 ± 11.71) μmol/L,(6.30 ± 1.37) mmol/L,(97.81 ± 49.16) μg/min respectively,which in the treatment group were (75.60 ± 11.44) μmol/L,(5.25 ± 1.24) mmol/L,(39.81 ± 23.43) μg/min.There were no statistically significant differences in Scr and BUN of the two groups compared with before treatment (the control group:t =0.767,-0.657;the treatment group:t =1.947,1.219,all P > 0.05).There was statistically significant difference in UAER of the two groups compared with before treatnent (t =2.850,5.402;P =0.046,0.006),but UAER of the treatment group after treatment decreased significantly.After treatment,there were no statistically significant differences in Scr and BUN between the two groups (t =-1.229,-1.236,all P > 0.05),while there was statistically significant difference in UAER between the two groups (t =-2.394,P =0.044).The effective rate in the treatment group was 87.5 %,which was higher than 57.5 % of the control group (x2 =9.028,P < 0.05).Conclusion Rhodiola kirilowii Regel combined with calcium dobesilate can ameliorate protenuria and improve renal function of the patients with early diabetic nephropathy.
