ABSTRACT

Objective To investigate the improvement role of clotrimazole suppositories and fluconazole combined therapy in sex life of patients with candidal vaginitis.MethodsThe clinical data of 80 cases of patients with candidal vaginitis in our hospital from January 2015 to January 2017 were retrospectively analyzed, these patients were divided into clotrimazole suppositories and fluconazole combined therapy group (combined treatment group, n=40) and fluconazole single therapy group (single treatment group, n=40) according to the treatment methods, the clinical effects, main clinical symptoms and signs improvement time, quality of life of the two groups were statistically analyzed.ResultsThe total treatment efficiency of the combined treatment group 100.0% (40/40) was significantly higher than the single treatment group 87.5% (35/40) (P<0.05), the vaginal itching, pain, burning sensation, abnormal leucorrhea, abnormal discharge improvement time were significantly shorter than the single treatment group (P<0.05), the sexual desire, sexual arousal, lubrication, orgasm and sexual satisfaction scores were significantly higher than the single treatment group (P<0.05), the pain score was significantly lower than the single treatment group (P<0.05).ConclusionClotrimazole suppositories and fluconazole combined therapy can more effectively improve the sex life of patients with candidal vaginitis than fluconazole single therapy.

ABSTRACT

La microbiota vaginal en la edad fértil está dominada por Lactobacillus spp. Su sobrecrecimiento provoca el aumento en la producción de ácido láctico, disminución del pH vaginal y lisis de células del epitelio vaginal, causando vaginosis citolítica, la cual se confunde con vulvovaginitis candidiásica (VVC) por la aparición de signos y síntomas similares. Se determinó la prevalencia de vaginosis citolítica, así como sus características clínicas y epidemiológicas en pacientes que consultaron con clínica sugestiva de VVC durante el período agosto-septiembre del año 2015. Se encontraron 12 pacientes con clínica compatible de VVC. En 11 (91,7%) se realizó el diagnóstico de vaginosis citolítica y en 1 (8,3%) de vulvovaginitis candidiásica. El 63,7% de las pacientes (7) tenían diagnóstico previo de VVC recurrente. Los síntomas más frecuentes fueron prurito vaginal o vulvar y secreción vaginal blanquecina de aspecto grumoso como leche cortada. La clínica iniciaba entre los 4-6 días posteriores a la menstruación, y desaparecía días antes o justo con el inicio de una nueva fase menstrual. En conclusión, se encontró que en las pacientes examinadas la sintomatología de ambas patologías resultó indistinguible, por tanto el diagnóstico clínico no es suficiente y debe recurrirse al diagnóstico de laboratorio para distinguir entre vaginosis citolítica y vulvovaginitis candidiásica.


The vaginal microbiota in fertile age is dominated by Lactobacillus spp. Its overgrowth causes increased production of lactic acid, decreased vaginal pH and lysis of cells of the vaginal epithelium, causing cytolytic vaginosis, which can be confused with vulvovaginal candidiasis (VVC) due to the similar signs and symptoms. The prevalence of cytolytic vaginosis, as well as its clinical and epidemiological characteristics was determined in patients who consulted with suggestive signs and symptoms of VVC during the period August to September of the year 2015. Twelve patients with signs and symptoms compatible with VVC were considered. Of those, 11 (91.7%) were diagnosed as cytolytic vaginosis and 1 (8.3%) corresponded to vulvovaginal candidiasis. From the 12 patients included, 7 (63.7%) had previous diagnosis of recurrent VVC. The most frequent symptoms recorded were vaginal or vulvar pruritus and whitish, cheesy or curd-like vaginal discharge. Signs and symptoms started between 4 to 6 days after menstruation, and disappeared days before or just with the onset of a new menstrual cycle. In conclusion, it was found that in the patients examined, signs and symptoms of both pathologies were indistinguishable. Therefore, the clinical diagnosis is not enough and laboratory studies should be used to distinguish between cytolytic vaginosis and vulvovaginal candidiasis.

ABSTRACT

Objective:To observe the efficacy of A clotrimazole combined with ultrasonic atomization of ozone in the treatment of candidal vaginitis. Methods:Totally 200 cases of candidal vaginitis were divided into the observation group and the control group with 100 cases in each. The control group was given A clotrimazole creams through vaginal administration, qd, while the observation group was treated by ultrasonic atomization of ozone additionally, qd, 15min per time with five times. The clinical efficacy and the recurrence rate after 6 months of the two groups were compared. Before the treatment and in the first and fourth week after the treatment, vaginal secretion bacterial culture was performed and pH value was detected. Results:The cure rate and the total effective rate of the observa-tion group was 88% and 98%, respectively, which was significantly higher than that of the control group (69% and 82%, respective-ly, P0. 05). Conclusion:A clotrimazole combined with ultrasonic atomization of ozone in the treatment of candidal vaginitis can improve the therapeutic effect.

OBJECTIVE:To estimate the cost-effectiveness of4therapeutic regi mens in the treatment of candidal vaginitis.METHODS:182out-patients with candidal vaginitis in the hospital where the authors worked were randomly di?vided into4groups:baofukang suppository group(A),miconazole suppository+miconazole cream group(B),povidone Iodine ointment group(C),and luohuazizhu suppository group(D),medical economy was applied to analyze the cost-effectiveness.RESULTS:The costs in4groups were56.2yuan,69.4yuan,75.3yuan,and53.9yuan respectively;The effective rates were93.3%,91.3%,91.3,and80.0%respectively;The ratios of cost to effectiveness were0.60,0.76,0.82,and0.67re?spectively;Compared with D,the added cost-effectiveness ratio of A,B,C are0.17,1.37and1.89respectively.CONCLU?SION:Baofukang suppository is a preferable drug to treat candidal vaginitis.

ABSTRACT

In order to study the role of prostaglandin E2(PGE_2)on the pathogenesis of candidal vaginitis,the effect of Candida albicans and histamine on the production of PGE_2 by peripheral blood mononuclear cells from patients with candidal vaginitis and normal controls,the effect of PGE2 on germ tube formation of Candida albicans,and the effect of PGE2 on Candida albicans adhering to vaginal ep- ithelial cells were observed.The results showed that:Candida albicans and histamine induced PGE2 synergistically,and the production of PGE2 increased markedly.PGE2 enhanced germ tube formation in Candida albicans,increased the number of Candida albicans adhering to the vaginal epithelial cells, and the pathogenicity of C.albicans.The above results indicate that PGE_2 plays a role in the pathogene- sis of candidal vaginitis.
