ABSTRACT

Se presenta el caso clínico de un paciente de 44 años de edad, con diagnóstico de virus de inmunodeficiencia humana/sida desde el año 2003, que fue ingresado en el Hospital General Docente "Dr. Juan Bruno Zayas Alfonso" de Santiago de Cuba por presentar aumento progresivo del volumen de la región anterior y lateral izquierda del cuello que se extendía hasta la zona anterior de la axila de ese lado, con cianosis y edema en esclavina, disfagia, tos seca irritativa, ronquera, así como astenia marcada, anorexia y pérdida de peso. El cuadro clínico y los resultados de los estudios analíticos e histológicos confirmaron la presencia de un linfoma no Hodgkin de alto grado, unido a candidiasis bucal y esofágica, y síndrome de consunción por el virus de inmunodeficiencia humana.


The case report of a 44 years patient with diagnosis of human immunodeficiency virus /aids since 2003 that was admitted to "Dr. Juan Bruno Zayas Alfonso" Teaching General Hospital in Santiago de Cuba is presented due to a volume progressive increase of the preceding and left lateral region of the neck that extended to the preceding area in the armpit of that side, with cyanosis and shoulder girdle edema, dysphagia, dry irritating cough, hoarseness, as well as marked asthenia, anorexia and weight loss. The clinical pattern and the results of the analytic and histological studies confirmed the presence of a high-grade non Hodgkin lymphoma, along with oral and esophageal candidiosis, and consumption syndrome due to the human immunodeficiency virus.

ABSTRACT

Las infecciones del aparato genital femenino constituyen un problema importante de la práctica ginecoobstétrica. Son causa de morbilidad, mortalidad materna y neonatal. Entre los factores que explican su frecuencia están: aborto provocado, aumento de intervenciones quirúrgicas e infecciones de transmisión sexual. OBJETIVO: Realizar el diagnóstico y determinar la prevalencia de infecciones vaginales en mujeres que asisten al Laboratorio de Microbiología del Hospital Ginecoobstétrico "Ramón González Coro" con la utilización del Juego de diagnóstico de infecciones vaginales Newvagin C-Kure. MÉTODOS: Se examinaron 150 mujeres, de edades entre 17 y 74 años, de mayo a octubre de 2007. Se utilizó examen directo, criterios de Amsel y cols, así como prueba de aglutinación al látex. RESULTADOS: La prevalencia de las infecciones vaginales estudiadas fue de 62 % al examen directo y de 58,6 % con la prueba de aglutinación al látex. La infección vaginal con mayor prevalencia fue vaginosis bacteriana, seguida de candidiosis vaginal y trichomoniosis vaginal. El grupo de edad con mayor prevalencia de estas infecciones fue el de 28-37 años. La manifestación clínica más frecuente fue la leucorrea. El inicio precoz de las relaciones sexuales, la realización del acto sexual sin protección y el uso de métodos anticonceptivos, estuvieron relacionados con la presencia de estas infecciones vaginales. CONCLUSIONES: El uso del Juego de diagnóstico de infecciones vaginales NEWVAGIN C-Kure, en consultorios de médicos de familia, policlínicos comunitarios y consultas de ginecología resulta ventajoso. La realización del diagnóstico por el personal paramédico, su rapidez y la certeza en la indicación del tratamiento médico son los principales factores a considerar


The infections of female genital tract are a significant problem of gynecologic and obstetric practice. They are cause of morbidity and of mother and neonatal mortality. Among factors explaining its frequency are: provoked abortion, increase of surgical interventions and sexual transmitted infections. OBJECTIVES:To make the diagnosis and to determine the vaginal infections prevalence in women served in Microbiology Laboratory of "Ram¾n Gonzßlez Coro" using the NEWVAGIN C-Cure diagnosis kit. METHODS: Authors examined to 150 women aged between 17 and 24 from May to October, 2007. Direct examination, Amsel et al criteria and latex agglutination test were used. RESULTS: Prevalence of study vaginal infections was of 62 percent in direct examination and of 58.6 percent using the latex agglutination test. The more prevalent vaginal infection was the bacterial vaginosis followed by vaginal candidiasis and vaginal trichomoniasis. The more prevalent age group with these infections was that aged 28-37. More frequent clinical manifestation was the leucorrhea. The early onset of sexual intercourse, the intercourse without protection and the use of contraceptive methods were related to presence of these vaginal infections. CONCLUSIONS: The use of NEWVAGIN C-Kure diagnosis kit for vaginal infections in family consulting room, community polyclinics and Gynecology consultations is very advantageous. Making diagnosis by paramedical staff, its speed and accuracy in prescription of medical treatment are the leading factors to be considered

ABSTRACT

HIV infection causes qualitative and quantitative reduction of the T helper (Th) subset of T lymphocytes, facilitating opportunistic infection. One of the common opportunistic infections among HIV/AIDS patients is Candida infection in the oropharynx and esophagus. Detection of increased Candida colonialization is not always easy, CD4 count is a parameter that could be used as reference. The fact is there’s only few laboratory can provide CD4 count. This study is a cross-sectional correlative study to determine the relation between total lymphocyte count as a much more applicable laboratory parameter and the intensity of Candida colonization in the oropharyngeal cavity of patients with HIV infection/AIDS. It was performed in the outpatient ward and inpatient ward of Dr. Cipto Mangunkusumo Hospital, from August 2004 to January 2005. The selected study subjects underwent interview, physical examination and had their blood samples and gargle samples taken. 60 study subjects were recruited, consisting of 52 males (86.7%) and 8 females (13.3%). The mean total lymphocyte count was 1194.53 cells/μL. Oropharyngeal candidiasis was found in 44 subjects (73.3%). There is a trend of higher Candida colonies number with lower total lymphocyte count despite no significant correlation between total lymphocyte count and the number of Candida colonies in the oral cavity of patients with HIV infection/AIDS. There is significant different between total lymphocyte count in positive and negative Candida colonies.

Las infecciones micóticas invasoras se presentan con mayor frecuencia en pacientes con neutropenia posterior al uso de quimioterapia para el tratamiento del cáncer. Se presenta una paciente de 4 años con diagnóstico de leucemia linfoide aguda, quien, luego de la quimioterapia, desarrolló neutropenia febril y diarrea. Recibió tratamiento antibiótico y antimicótico de amplio espectro. Se aisló Candida albicans y se observaron trofozoítos de Entamoeba histolytica en la materia fecal . La paciente desarrolló candidiosis crónica diseminada que fue tratada con anfotericina B y, posteriormente, con fluconazol. Se ilustra la imagen tomográfica conocida como 'ojo de buey' y su correspondiente estudio histopatológico. Candida spp. es la levadura más común en pacientes inmunosuprimidos con tumores hematológicos que reciben quimioterapia. La candidiosis diseminada que se presenta en estos pacientes persiste y se hace evidente clínicamente una vez se resuelve la neutropenia; se denomina candidiosis crónica diseminada. En la paciente se presentaron varios factores de riesgo, como el tratamiento previo con antibióticos de amplio espectro, la colonización del tracto gastrointestinal por Candida y la neutropenia prolongada. El diagnóstico por imagen se hace principalmente por ecografía, tomografía axial computarizada y resonancia magnética. Se han descrito cuatro patrones tanto ecográficos como tomográficos, de los cuales se destacan el tipo 1 ('ruedas entre ruedas') y el tipo 2 ('ojo de buey'), que son característicos de la candidiosis crónica diseminada. El tercer patrón (imágenes hipoecoicas), aunque no es específico de la candidiosis crónica diseminada, es el más comúnmente hallado en ambas técnicas. En la paciente se observaron los patrones 2 y 3, y el diagnóstico se confirmó con el estudio histopatológico.


Invasive fungal infections are more commonly found in patients who develop neutropenia after chemotherapy. A 4-year-old girl with diagnosis of acute lymphoid leukemia developed febrile neutropenia after chemotherapy. Broad spectrum antibiotics and antimycotic therapy were initiated. Candida albicans was isolated and Entamoeba histolytica was observed in stool examination. Chronic disseminated candidiasis had developed and was treated with amphotericin B, initially, and fluconazol. Computed tomography images were obtained that demonstrated a classic 'bull´s eye' pattern; a concurrent histological study confirmed the diagnosis. Candida spp. is the major cause of opportunistic mycosis in immunosuppresed patients receiving chemotherapy for haematologic malignancies. An initial infection results in disseminated candidiasis, which persists and becomes chronic. In the 4-year-old patient, the identified risk factors consisted of a previous therapy with broad spectrum antibiotics, the gastrointestinal tract colonization with Candida albicans and prolonged neutropenia. Imaging diagnoses are made by ultrasonography, computed tomography and magnetic resonance. With ultrasound and tomography, 4 distinct patterns have been described. Pattern 1 ('wheels within wheels') and 2 ('bull´s eye') are important, since they are characteristic of chronic disseminated candidiasis. The third pattern (hypoechoic image) is the most common finding with both techniques. In the current patient, patterns 2 and 3 were seen and the diagnosis was confirmed by histological study.
