ABSTRACT

Introdução: os acidentes de trânsito terrestre (ATT) representam um problema de saúde pública de grande relevância, que tem provocado impacto na morbimortalidade da população brasileira. Assim, torna-se premente a busca de soluções que visem a melhorar a segurança no trânsito e reduzir os ATT, que comprometem e oneram diferentes setores da sociedade e a qualidade de vida das vítimas. Objetivo: estimar tendências acerca da morbimortalidade por ATT na Bahia, entre 2011 e 2021. Metodologia: trata-se de um estudo ecológico, quantitativo, descritivo, realizado mediante análise dos dados populacionais do Departamento de Informática do Sistema Único de Saúde (DATASUS) acerca dos indivíduos que sofreram ATT na Bahia, entre 2011 e 2021. Resultados: foram registrados 2.268 óbitos e 102.490 internações, com maior ocorrência na macrorregião de Salvador, com o sexo masculino, cor parda, faixa etária entre 20 e 29 anos, mais frequente entre os motociclistas, seguidos dos pedestres. Observou-se tendência de aumento no número de internações, nos dias de permanência hospitalar e nos valores dos serviços associados, bem como variabilidade do número de óbitos e redução da taxa de mortalidade. As fraturas dos ossos da tíbia e clavícula, do antebraço e fêmur foram os procedimentos cirúrgicos com maior quantidade de internações relacionadas. Conclusão: os dados evidenciaram uma tendência de aumento do número de traumas, lesões, internações, óbitos e custos decorrentes dos ATT. Destarte, almeja-se que os Resultados do presente estudo subsidiem discussões e políticas públicas que intensifiquem as medidas de segurança no trânsito, com vistas a reduzir a incidência dos ATT e suas consequências socioeconômicas.


Introduction: road traffic accidents (RTA) represent a public health problem of great importance, which has had an impact on the morbidity and mortality of Brazilian population. Thus, the search for solutions that aim to improve traffic safety and reduce ATT becomes urgent, which compromise and burden different sectors of society and the quality of life of victims. Objective ­ To estimate trends in morbidity and mortality from RTA in Bahia, between 2011 and 2021. Methodology: this is an ecological, quantitative, descriptive study carried out by analyzing population data from the Department of Informatics of the Unified Health System (DATASUS) about individuals who suffered RTA in Bahia, between 2011 and 2021. Results: there were 2,268 deaths and 102,490 hospitalizations, with the highest occurrence in the macro-region of Salvador, with males, mixed race, aged between 20 and 29 years, more frequent among motorcyclists, followed by pedestrians. There was a trend towards an increase in the number of hospitalizations, in the days of hospital stay and in the values of associated services, as well as variability in the number of deaths and a reduction in the mortality rate. Fractures of the tibia and clavicle, forearm and femur bones were the surgical procedures with the highest number of related hospitalizations. Conclusion: data showed a trend towards an increase in the number of traumas, injuries, hospitalizations, deaths and costs resulting from RTA. Thus, it is hoped that the Results of the present study support discussions and public policies that intensify traffic safety measures, with a view to reducing the incidence of RTA and their socioeconomic consequences.

ABSTRACT

Introducción: los siniestros viales representan un significativo problema de salud pública y una de las principales causas de morbimortalidad evitables en la Argentina. La vigilancia epidemiológica permite valorar el alcance del problema y orientar respuestas. El objetivo de este estudio es describir las características demográficas, uso de la vía pública, mecanismo de producción, evaluación en Unidad de Emergencias y morbimortalidad de los casos ingresados en el sistema de vigilancia epidemiológica de siniestros viales en el Hospital Italiano de Buenos Aires durante durante el año 2018. Materiales y métodos: estudio observacional de corte transversal. Se analizaron las variables sexo, grupo etario, tiempo de internación, tipo de usuario, mecanismo de producción, gravedad del herido y del siniestro. Resultados: se registraron 404 consultas por siniestros viales. El sexo masculino fue el predominante en el 62,13% (251). El rango etario más frecuente abarcó los 25 a 44 años (187). La forma de traslado mayoritaria fue el automóvil en el 42,57% (172), seguido de motovehículos 29,21% (118). La gran mayoría fueron secundarios a colisiones entre dos vehículos. El 80,20% (324) ingresó deambulando por sus propios medios y el 83% (336) no requirió internación. El 90,6% (366) consultó por heridas leves. De los heridos graves, el 71,05% (27) fue de sexo masculino y el 26,32% (10) mayor de 75 años. La mayoría eran usuarios de automóvil. Ocurrieron 3 óbitos de mujeres mayores de 75 años. Conclusión: se observó una mayor cantidad de siniestros viales en adultos jóvenes, principalmente en varones. En general, usuarios de vehículos que consultaron tras una colisión con otro vehículo. Mayormente, ingresaron por sus propios medios, no requirieron internación de ningún tipo y presentaron heridas leves. Los casos graves correspondieron en su mayoría a varones con gran proporción de adultos mayores. Los casos fatales ocurrieron en mujeres adultas mayores. Contar con información del perfil epidemiológico de los siniestros viales permite la comprensión de esta compleja problemática en el ámbito local y el diseño de estrategias de intervención ajustadas a dicho perfil. Introduction: road accidents represent a significant public health problem and one of the main causes of preventable morbidity and mortality in Argentina. Epidemiological surveillance makes it possible to assess the scope of the problem and guide responses. The objective of this study is to describe the demographic characteristics, use of the public highway, production mechanism, evaluation in the emergency center and morbidity and mortality of the cases admitted to the epidemiological surveillance system of road accidents at the Hospital Italiano de Buenos Aires during 2018. Materials and methods: observational cross-sectional study. The variables sex, age group, length of stay, type of user, production mechanism, severity of the injured and the accident were analyzed. Results: 404 consultations for road accidents were registered. Male sex was predominant in 62.13% (251). The most frequent age range was 25 to 44 years (187). The majority form of transportation was the automobile in 42.57% (172), followed by motorcycle vehicles 29.21% (118). The vast majority were secondary to collisions between two vehicles. 80.20% (324) were admitted wandering by their own means and 83% (336) did not require hospitalization. 90.6% (366) consulted for minor injuries. Of the seriously injured, 71.05% (27) were male and 26.32% (10) were older than 75 years. Most were car users. There were 3 deaths of women older than 75 years. Conclusion: there was a greater number of road accidents in young adults, mainly in men. In general, vehicle users consulted after a collision with another vehicle. Moreover, they entered by their own means, they did not require hospitalization of any kind and presentation of minor injuries. Severe cases correspond mostly to males with a large proportion of older adults. The fatal cases occurred in older adult women. Having information on the epidemiological profile of road accidents allows the understanding of this complex problem at the local level and the design of intervention strategies adjusted to this profile. BACKGROUND: Compared to 10 years ago, the ambient particulate matter 10 (PM10) and carbon monoxide (CO) levels in South Korea have decreased. However, compared to many other OECD countries, these levels are still too high. Concentration of air pollutants such as PM10 is especially higher during winter than during summer. The first step to rationally solving the air pollution problem in Korea is to identify the key air pollution sources during each season. This ecological study was performed to assess the association between the number of days the accepted PM10 and CO thresholds were exceeded and the concentration of potential emission sources in winter season 2015. METHODS: An emission inventory of the PM10 and CO emissions in the 232 administrative South Korean districts in January, 2015, and February, 2015 and December, 2015, and the population density, number of car registrations, number of car accidents, industrial power usage, and presence of a fossil-fuel power plant in each district was established on the basis of official web-page data from the government. For all emission source variables except power plants, the administrative districts were grouped into quartiles. Districts were also divided according to whether a power plant was present or not. Negative binomial regression was performed to assess the associations between the PM10 and CO air pollution (defined as ≥100 g/m3 and ≥ 9 ppm, respectively) and the concentration of each emission source. RESULTS: Compared to the districts with the lowest population density, the districts with the third highest population density associated most strongly with air pollution. This was also observed for industrial power usage. Car accident number and car registration numbers showed a linear relationship with air pollution. Districts with power plants were significantly more likely to have air pollution than districts that lacked a plant. CONCLUSIONS: Greater car numbers, industrial activity, and population density, and the presence of fossil-fuel plants associated with air pollution in the 2015 winter in South Korea. These data highlight the contaminant sources that could be targeted by interventions that aim to reduce air pollution, decrease the incidence of exposure, and limit the impact of pollution on human health. Introdução: os acidentes de trânsito configuram-se como um relevante problema na saúde pública diante de grande impacto no que concerne à morbimortalidade da população e ganha destaque entre os jovens do sexo masculino. Objetivo: descrever o perfil epidemiológico e avaliar a gravidade do trauma de vítimas de acidentes com motocicletas atendidas pelas equipes do Serviço de Atendimento Móvel de Urgência do Rio Grande do Norte (SAMU 192 RN). Métodos: estudo descritivo, transversal, com abordagem quantitativa. Coletou-se dados de 667 fichas de atendimentos de enfermagem do SAMU 192 RN de vítimas de acidentes com motocicletas, no período de janeiro a junho de 2014. Resultados: a faixa entre 25 e 34 anos de idade apresentou 32,5 percento, destacando-se, dentro dessa faixa, os homens, com 26,4 percento de todos os atendimentos. A pontuação máxima (melhor prognóstico) do instrumento, esteve presente em 95,7 percento dos atendimentos. Conclusões: a caracterização do perfil epidemiológico da população enfatizou a presença masculina nos eventos acidentais com motocicletas, além dos jovens entre 15 e 34 anos de idade(AU)


Introducción: los accidentes de tráfico configuran como un problema de salud pública importante, de gran impacto en la morbilidad y mortalidad de la población y se destaca entre los hombres jóvenes. Objetivo: describir el perfil epidemiológico y evaluar la gravedad del trauma de las víctimas de los accidentes de motocicleta atendidos por los equipos de Atención Móvil de Urgencia de Rio Grande do Norte (SAMU 192 RN). Métodos: estudio descriptivo, transversal, con abordaje cuantitativo. Los datos fueron recolectados por 667 fichas de cuidados de enfermería SAMU 192 RN de víctimas de accidentes de motocicleta en el periodo enero-junio de 2014. Resultados: el intervalo entre 25 y 34 años de edad representa 32,5 por ciento, destacando, dentro de esta gama, los hombres, con 26,4 por ciento de todas las asistencias. La puntuación máxima (mejor pronóstico) del instrumento, estuvo presente en 95,7 por ciento de los casos. Conclusiones: la caracterización del perfil epidemiológico de la población destacó la presencia de los hombres en todos los accidentes con motocicletas, además de los jóvenes entre 15 y 34 años de edad(AU)


Introduction: Car accidents constitute an important public health concern, which has a great impact on population morbidity and mortality mainly standing out among young men. Objective: To describe the epidemiological profile and to assess trauma severity in victims of motorcycle accidents assisted by Rio Grande do Norte Emergency Mobile Assistance teams (SAMU 192 RN). Methods: Descriptive, cross-sectional study with a quantitative approach. The data were collected by 667 SAMU 192 RN nursing care records from motorcycle accident victims in the period January-June 2014.Results: The range between 25 and 34 years of age represents 32.5 percent, standing out in this range, men, with 26.4 percent of all assistance. The maximum score (best prognosis) of the instrument was present in 95.7 percent of the cases. Conclusions: The characterization of the epidemiological profile of the population highlighted the presence of men in all motorcycles accidents, in addition to young people aged 15-34 years(AU)

BACKGROUND AND OBJECTIVES: With increasing frequency of car accidents, patients of dizziness caused by car accidents are also increasing. Various types of dizziness or vertigo can occur from car accidents depending on different injury mechanisms. Since accurate diagnosis is important for providing proper treatments, we evaluated clinical characteristics related to vestibular function of patients with dizziness caused car accidents. SUBJECTS AND METHOD: In this retrospective case review study that runs from January 2011 to March 2013, a total of 82 patients with dizziness following car accident were enrolled consecutively. We analyzed the final diagnosis of dizziness according to different mechanisms of injury during car accident through clinical record review. Patients who developed dizziness within one month of car accident were included, excluding those who had temporal bone fracture and previous history of dizziness. RESULTS: Of the different types observed, 36.6% was head injury, 24.4% whiplash injury, 3.7% complex injury, 2.4% others and the rest was unknown. In the final diagnosis, the different types included 36.6% benign paroxysmal positional vertigo (BPPV), 23.2% unclassifiable dizziness, 18.3% cervical vertigo, 7.3% labyrinthine concussion, 3.7% BPPV with labyrinthine concussion and the rest was others. Of the different types of dizziness symptoms, 58.5% was headache, 45.1% was audiologic symptoms, and others included earfullness, tinnitus and hearing disturbance. Tinitogram and pure tone audiogram results show that 2.9% (27 people) of patients have tinnitus and 7.3% (6 people) have hearing disturbance. CONCLUSION: An accurate diagnosis and timely management would be very important in forming a proper approach for post traumatic vertigo patients.

ABSTRACT

La clasificación de Fraser para la rodilla flotante, ha sido desde su publicación, la más difundida y utilizada a nivel mundial para estadificar esta lesión. En la rodilla flotante, el compromiso intraarticular se ha relacionado con mayor tasa de complicaciones y con los resultados funcionales más pobres. Sin embargo, la clasificación de Fraser, no toma en cuenta el compromiso de los tejidos blandos, lo cual también es importante para determinar el tratamiento, complicaciones y pronóstico. Otra lesión asociada a la rodilla flotante y que ha sido ampliamente documentada, es el compromiso de los ligamentos de la rodilla y las fracturas de rótula. Otros sistemas de clasificación propuestos posteriormente a la clasificación de Fraser, no toman en cuenta estos y otros elementos. Consideramos que una clasificación para la rodilla flotante, debe tomar en cuenta cuatro aspectos fundamentales para su diagnótico, tratamiento y pronóstico: morfología de la lesión, compromiso articular, compromiso de las partes blandas y compromiso de los ligamentos de la rodilla y afectación de la rótula. Con esta investigación proponemos un nuevo sistema de clasificación para la rodilla flotante que toma en cuenta estas variables, y que además, es simple y proporciona importante información para su diagnóstico, tratamiento y pronóstico(AU)


The Fraser classification for floating knee has been since its publication, the most widespread and worldwide used to stage this injury. In the floating knee, intra-articular commitment has been linked to higher rates of complications and poorer functional outcomes. However, the classification of Fraser, does not take into account the commitment of the soft tissues, which is also important in determining treatment, complications and prognosis. Another lesion associated with floating knee and has been widely documented, is the commitment of the ligaments of the knee and patella fractures. Other classification systems subsequently proposed to Fraser classification, do not take into account these and other elements. We think that a classification for floating knee should include four key factor for diagnosis, treatment and prognosis: lesion morphology, joint involvement, commitment soft tissue and commitment of knee ligament and patellar involvement. With this research we propose a new classification system for floating knee that takes into account these variables, and furthermore, is simple and provides important information for diagnosis, treatment and prognosis(AU)

ABSTRACT

La rodilla flotante es la asociación simultánea de fracturas ipsilaterales del fémur y la tibia que incluye una combinación de fracturas diafisiarias, metafisiarias e intraarticulares. Su tratamiento representa un reto terapéutico. Se realizó un estudio retrospectivo, analítico, observacional de cohorte hospitalaria conducido en un centro de IV nivel. Se incluyeron todos los pacientes de ambos sexos con edad igual o mayor a 16 años, que ingresaron con diagnóstico de rodilla flotante, desde enero 1980 a diciembre 2010. Se incluyeron 314 pacientes. 71 % de sexo masculino y edad promedio de 28,22 años. El grupo etario de 16-25 años fue el más afectado. 66% fueron abiertas. Los accidents automovilísticos fueron la causa en 92% de los casos. 58% fueron tipo I de Fraser. La técnica quirúrgica que se relacionó con los mejores resultados clínicos y radiológicos al término del seguimiento fue el enclavado endomedular bloqueado tanto para el fémur como para la tibia. La tasa de infección fue 14% y mortalidad del 6%(AU)


A floating knee is the simultaneous ipsilateral association fractures of the femur and tibia which includes a combination of diaphyseal, metaphyseal and intraarticular fractures. lts an high-energy injury caused trauma and is associated with high risk of morbidity and mortality. Treatment of floating knee representa a therapeutic challenge. We performed a prospective, analytic cohort observational hospital conducted a level IV center. We included all patients of both sexes, aged greater than or equal to 16 years, admitted with the diagnosis of knee Floating Hospital Universitario de Los Andes, from January 1980 to December 2010. 314 patients were found. 71 % were male and mean age 28.22 years. The age group 16-25 years was the most affected. 66% were open. The cause was motor vehicle accidents in 92% of cases. 58% were type I Fraser. The surgical technique was associated with better clinical and radiological follow-up was at the end of the locked intramedullary nailing both the femur and tibia for. The infection rate was 14% and mortality of 6%(AU)

ABSTRACT

A 9-year-old boy developed large apical ventricular septal defect with rapid cardiopulmonary deterioration after a blunt chest injury from a car bump. The defect was successfully repaired 10 hours later. The detailed history, investigations and treatment are reported with a review of the literatures.
