ABSTRACT

La diabetes mellitus constituye una entidad con una elevada prevalencia que en los últimos años ha tenido una tendencia ascendente, principalmente en población económicamente activa, lo que implica que cada vez existirán más mujeres en edad fértil con esta enfermedad. Se recomienda que estas mujeres reciban un tipo de atención especializada como la atención preconcepcional, tres meses antes de la concepción como mínimo, para coadyuvar al logro de resultados gestacionales favorables. Es conveniente, entonces, que esta actividad clínica sea protocolizada para normalizar y facilitar la práctica profesional en este campo, con el fin de mejorar la calidad de este servicio asistencial. Este constituye el motivo fundamental de la implementación del protocolo de atención preconcepcional en diabetes, en el Centro de Atención al Diabético del Instituto de Endocrinología, que se presenta en este artículo(AU)


Diabetes mellitus is an entity with a high prevalence that in recent years has had an upward trend, mainly in the economically active population, implying that more and more women of childbearing potential with this disease will exist. It is recommended that these women receive a type of specialized care such as preconception care, at least three months before conception, to help achieve favorable gestational outcomes. It is then desirable that this clinical activity can be registered in order to standardize and facilitate professional practice in this field, in order to improve the quality of this care service. This is the fundamental reason for the implementation of the preconceptional care protocol in diabetes, at the Diabetic Patients Care Center of the Institute of Endocrinology, that is presented in this article(AU)

ABSTRACT

Background@#There has been an increasing trend in reported sexual abuse patients in the Philippines in the past 20 years. Patient evaluation is critical from health care providers and it is imperative to have an acute care protocol that heath care providers can use in managing these patients.@*Objective@#To assess the adherence to the acute care protocol as applied to adult female sexual abuse patients who consulted at the Philippine General Hospital OB Admitting Section from March to August 2019@*Methods@#The study design used was an observational, cross-sectional study via descriptive analysis using a designed questionnaire. Relevant data regarding compliance to the acute care protocol was assessed from patient experience via the questionnaire after undergoing the standard services of the hospital. Data was then documented, tabulated and processed via Microsoft Excel data sheets.@*Results@#In terms of patient perspective, 96.2 % of all cases (n=27) were provided service by OBGYNs and psychiatrists and 100% were assessed by social workers. Although stated in the protocol, legal and/or police assistance was not provided by the hospital during patient consult.@*Conclusion@#The institution was able to adhere to the acute care protocol services of patients in terms of obstetric and gynecologic, psychiatric and social worker services but not legal and/or police assistance.

ABSTRACT

Objective To determine the effectiveness of an oral care protocol on prevention and treatment of chemotherapy-induced oral mucositis in patients with hematological neoplasms. Methods Totally 106 patients with hematological neoplasms undergoing therapies from March to October 2017 were enrolled. Patients who were included from March to June 2017 constituted the control group, others from July to October constituted the intervention group. Patients in control group received the regular care, while patients in intervention group received a 3-week period oral care protocol. The oral health status of the patients was recorded and compared between the two groups. Results On the 7th,14th, 21th day after chemotherapy, the oral mucositis level of I degree,Ⅱdegree,Ⅲdegree andⅣdegree of the intervention group were 11, 13, 17, 1 cases, 28, 16, 0, 0 cases, 9, 3, 0, 0 cases, which were lower than the control group whose degrees were 24, 18, 10, 2 cases, 21, 13, 12, 0 cases, 13, 9, 8, 0 cases, respectively. The difference between the two groups was statistically significant (Z=-2.177,-2.504,-2.113, P<0.05). On the 3th, 7th, 14th and 21th day after chemotherapy, the oral pain scores reported by patients in the intervention group were 2.00(2.50), 3.00(2.75), 2.50(2.00), 0.00 (1.00), which were lower than the control group whose scores, in contrast, were 3.00(4.00), 5.00(3.25), 5.50 (4.00), 2.00(3.25). The difference between the two groups was statistically significant (Z=-2.587,-3.326,-4.925,-3.217, P <0.05). The positive rate of microbial examination in the intervention group was 14.6%(7/48), which was lower than 36.2%(21/58) in the control group. The difference between the two groups was statistically significant(χ2=6.318, P<0.05). Conclusions The oral care protocol established and implemented based on evidence could effectively decrease the incidence of chemotherapy-induced oral mucositis, ease oral pain and reduce inflammation for patients with hematological neoplasms.

Introducción: Las mujeres cardiópatas estuvieron "condenadas" durante muchos años a no poder asumir la maternidad debido al riesgo incrementado de mortalidad materna que el embarazo representaba para ellas, actualmente el 85 porciento o más de estas pacientes sobreviven la edad de 18 años, y muchas, dependiendo del tipo específico de la afectación cardiaca y el grado de corrección que se le pueda realizar, pueden tener una expectativa de vida similar a la población general. Objetivo: describir los resultados perinatales obtenidos en las pacientes embarazadas con diagnóstico de cardiopatía congénita, a partir de la aplicación de un protocolo de trabajo realizado por un grupo multidisciplinario de especialistas. Métodos: se realizó un estudio descriptivo prospectivo transversal, en el que se analizaron factores clínicos y perinatológicos asociados a las gestantes con cardiopatías congénitas, atendidas en el Servicio de Cardiopatía y Embarazo del Hospital Ramón González Coro, en el municipio Plaza, provincia La Habana, en el periodo correspondiente entre el 1ro. de enero de 2000 y el de 31 de diciembre de 2010. Resultados: el 75 porciento, de las pacientes se embarazó antes de los 30 años de edad. El 10,4 porciento, presentó morbilidad de causa obstétrica y entre ellas prevaleció la diabetes gestacional. Hubo agravamiento de la clasificación funcional I al inicio del embarazo a II y III al momento del parto. No hubo recién nacido con diagnóstico de cardiopatía congénita. Conclusión: Con la aplicación del Protocolo de Atención a la Gestante Cardiópata se han logrado resultados perinatales alentadores en la gestante con cardiopatía congénita


Introduction: for many years, the women presenting with heart disease were condemned to not to get pregnant due to the increased risk of maternal mortality caused by it, nowadays the 85 percent or more of these patients survives the 18 years old, and many of them, depending on the type specific of cardiac affection and the degree of a potential correction, may to have a life expectancy similar to general population. Objective: to describe the perinatal results obtained in pregnant patient diagnosed with congenital heart disease, from the implementation of a work protocol conducted by a multidisciplinary group of specialists. Methods: a cross-sectional, prospective and descriptive study was conducted to analyze the clinical and perinatology factors associated with congenital heart diseases seen in the Heart Disease and Pregnancy Service of the Ramón González Coro Hospital of the Plaza municipality of La Habana from January 1, 2000 to December, 31 2010. Results: the 75 percent of patients got pregnants at 30 years old. The 10.4 percent had a morbidity of obstetric cause and among them prevailed the gestational diabetes. There was worsening of functional I classification at onset of pregnancy at II and III at delivery. There was not newborn diagnosed with congenital heart disease. Conclusion: with the implementation of the Protocol of Care to Heart Disease Patient it has been possible to achieve encouraging perinatal results in pregnant presenting with congenital hart disease
