ABSTRACT

Objective:To study the effect of simulated case base on the training of resident’s post competency in the department of emergency. Methods:The study selected 63 residents who had the standardized training in the Department of Emergency, Xuanwu Hospital of Capital Medical University from June 2018 to October 2019, and they were randomly divided into observation group ( n=32) and control group ( n=31). The observation group was trained by emergency simulated case base, while the control group was trained by routine training methods such as theoretical lecturing and single skill training. The differences between the two training methods in the abilities of diagnosis and treatment, medical knowledge, interpersonal communication, practice-based learning and improvement of post competencies were compared by means of theoretical and operational assessments. Two groups were investigated with questionnaires, self-evaluation and satisfaction degree of post competency, and the differences between the two groups were compared. SPSS 19.0 software was used for t test and chi-square test. Results:There was no significant difference in the scores of medical knowledge and skill operation between the observation group [(43.53± 9.26) points and (7.32±1.03) points] and the control group [(42.47±8.64) points and (7.24±1.12) points] ( P>0.05). However, the scores of clinical diagnosis and treatment ability, professional accomplishment, interpersonal communication ability and practice-based learning and improvement were all significantly higher in the observation group than those in the control group ( P<0.05). The questionnaire self-assessment showed that there was no significant difference in scores of medical knowledge and skill operation between the observation group [(4.23±0.46) points and (4.05±0.52) points] and the control group [(4.14±0.38) points and (3.98±0.69) points] ( P>0.05), and the self-evaluation scores of professional accomplishment, interpersonal communication ability, clinical diagnosis and treatment ability, practice-based learning and improvement ability were higher than those in the control group ( P<0.05). The satisfaction with teaching method in the observation group was also higher than that in the control group ( P=0.020). Conclusion:The application of simulated case base can improve post competency in the standardized training of residents in the department of emergency, with good training effect.

ABSTRACT

Objective:To construct the postgraduate teaching case database of rotation in department of cardiology, and to explore its application effect. Methods:The postgraduate teaching case base of rotation in department of cardiology was constructed during April 2020 to December 2020. A total of 32 graduate students rotating in Department of Cardiology of Affiliated Hospital of Guilin Medical University from January 2021 to April 2021 were selected and randomly divided into two groups. The routine group adopted the traditional teaching method, and the research group applied the teaching case base on this basis, both for one month. The examination results, the changes of clinical practice ability before and after the teaching, and the satisfaction with the teaching mode were compared between the two groups. SPSS 22.0 was used for t test and chi-square test. Results:The final results of the theoretical examination and practical operation examination of the two groups were higher than those of the entrance examination ( P<0.05), and the results of the theoretical examination and practical operation examination of the research group were higher than those of the routine group ( P<0.05). The scores of clinical practice ability of inquiry, case analysis, diagnosis, treatment and follow-up in the two groups after teaching were higher than those before teaching ( P<0.05), and the scores of clinical practice ability in the research group after teaching were higher than those in the routine group ( P<0.05). The satisfaction scores on improving examination results, enhancing clinical practice ability, deepening professional understanding and enhancing professional confidence of the teaching mode of the research group were higher than those of the routine group ( P<0.05). Conclusion:The construction and application of postgraduate teaching case base of cardiology rotation can improve the examination results, enhance the clinical practice ability, and achieve the satisfaction of postgraduates.

ABSTRACT

Neuromuscular diseases is one of the difficulties in neurology teaching and clinical practice. We chose the typical cases of Guillain-Barre syndrome and progressive muscular dystrophy as case based study, and evaluated the effect of teaching. The result shows that application of case base study can help medical students to enhance their learning interests and cultivate their good clinical thinking and then to meet the requirements of the teaching syllabus. So it is worth improving and promoting in clinical practice teaching.

ABSTRACT

There are some disadvantages in internal medicine practice teaching now. To find a new teaching model, and implement a teaching reform is the key to improve teaching quality. We try to apply the case introduction to the teaching method of evidence based practice in the department of internal medicine . The new teaching model which combined case base study with evidence-based medicine in internal medicine practice can help put forward questions and guide students to analyze questions by the case. It also helps find evidence and solve questions by evidence-based method. The organic combination of the two, not only can stimulate students' learning enthusiasm and initiative, de-velop the correct clinical thinking, but also can effectively promote students' self-learning ability to update their knowledge. It is worth widely developing in internal medicine practice.
