ABSTRACT

A 60-year-old man with bilateral corneal opacity underwent cataract extraction surgery involving the use of a limbal relaxing incision in his left eye. He had lower lid ectropion and lagophthalmos in both eyes. Eleven days after the surgery, a slit-lamp examination revealed a neurotrophic corneal ulcer with a punch-out epithelial defect and rolled edges at the center of the pre-existing corneal opacity. The patient was treated with sodium hyaluronate, autologous serum, and oral doxycycline. Six weeks after the surgery an improvement in corneal sensation was observed and the neurotrophic corneal ulcer subsequently healed over the course of one year. In this report, we present a case of neurotrophic keratitis that occurred after performing cataract surgery concurrent with a limbal relaxing incision. As such, we suggest that limbal relaxing incisions should be performed cautiously in patients with causative risk factors for corneal hypesthesia.

ABSTRACT

PURPOSE: To evaluate the efficacy of topical 0.1% sodium hyaluronate (HA) and 0.05% cyclosporine A (CsA) on tear film parameters after cataract surgery. METHODS: A total of 109 eyes of 86 patients who underwent cataract surgery were divided into four groups: 74 eyes without dry eye syndrome (groups 1 and 2) and 35 eyes with dry eye syndrome (groups 3 and 4). Groups 1 (37 eyes) and 3 (17 eyes) used 0.1% HA only, while Groups 2 (37 eyes) and 4 (18 eyes) used both 0.1% HA and 0.05% CsA. Tear film break-up time (BUT), Schirmer's test, tear clearance rate (TCR) and keratoepitheliopathy were evaluated before surgery and 1 week, 1 month, 2, 3 and 6 months after surgery. RESULTS: But, Schirmer's test and TCR recovered at postoperative 3 months in groups 1 and 3, after 2 months in group 2 and after 1 month in group 4. Keratoepitheliopathy recovered at postoperative 1 month in groups 1, 2 and 4, after 2 months in group 3. CONCLUSIONS: The combined use of topical 0.1% HA with 0.05% CsA is helpful in restoring tear film parameters after cataract surgery especially in patients with dry eye syndrome.

ABSTRACT

This study was carried out to evaluate corneal astigmatism according to distance between the limbus and internal lip after temporal incision cataract surgery. Twenty-five eyes of patients undergoing routine phacoemulsification with one plate haptic silicone IOL(STAAR)posterior chamber lens implantation with 3.0 mm temporal scleral pocket incision and sutureless method by one surgeon were randomly selected. Corneal astigmatism was evaluated at postoperative first day, first week, second week, first month, second month and analyzed by the Cravy s vector method. Internal lip length 1.0 mm group showed 0.11 diopter aganist-the-rule change of corneal astigmatism at the first week and 0.11 diopter with-therule change at the third month and internal lip length 1.5 mm group showed 0.16 diopter aganist-the-rule change of corneal astigmatism at first week and 0.04 diopter agaist-the-rule change at the third month but these differences were not statistically significant(students t-test:p>0.05). In this study we found that if a distance between the limbus and internal lip has longer than 1.0 mm, it was not an important factor on influencing the surgically induced astigmatism and could get wound stability after sutureless cataract sugery using a watertight scleral tunnel incision.

ABSTRACT

To investigate the change in upper eyelid position after cataract operation, We studied 40 eyes of 40 patients prospectively who underwent phacoemulsification and intraocular lens implantation. Levator function and vertical palpebral lid fissure height were measured preoperatively and at postoperative day 1, 7, 28 and 56 in and the surgical and the fellow eyes. Postoperative mean vertical lid fissure height decreased compared with preoperative mean vertical lid fissure height in both eyes. The postoperative decrease in vertical lid fissure height tended to decrease with times and these trends had statistically significant correlation between both eyes. This suggests that both eyes are affected by factors unassociated with the cataract surgery and it is most apparent in the immediate postoperative period. In conclusion, we recommend not only the surgical eye but the fellow eye should be evaluated preoperatively and postoperatively to understand the eyelid position after cataract operation.

ABSTRACT

@#The tri-plate horizontal mould with nonrigid surgical sponge inserts was developed to establish an industrial system for shaping surgical sponge which is used for cataract operation. The conception on an insertion mechanism of nonrigid flat inserts was firstly suggested by us. The prototype horizontal moulds with tri plate have been designed and manufactured. The experiment results of hundred thousand of times injection shaping have proved that the conception of the design is rational. The structure of shafting mould for nonrigid flats has been confirmed. Present article discussed the design and working mechanism of the insertion in the horizontal injection mould.

ABSTRACT

We performed a epidemiologic study to find a prevalence and risk factors of diabetic retinopathy. We reviewed 130 diabetic patients who had been performed the fundus examination for the first time for retinopathy at the ophthalmic department of Chung Ang university hospital from March 1993 to December 1993. We studied epidemiologic work-up sheet and performed ophthalmic and laboratory examination. The patients' ages ranged from 25 to 81 years, with an average age of 58.9 years. The age, sex, intraocular pressure, fasting blood sugar, post cibos 2 hours (Pc2hrs), age of onset, residence, smoking, alcohol drinking, diet pattern, blood type, treatment method and menstruation status were not significantly correlated with prevalence of diabetic retinopathy statistically(p>0.05). But the duration of diabetes(p<0.01), glycosylated hemoglobin(HbA1C) and cataract operation(p<0.05) were significantly correlated with diabetic retinopathy statisically. The prevalence of diabetic retinopathy was 28.5%(background changes in 21.5%, preproliferative changes in 4.6% and proliferative change in 2.4%). The mean interval between diagnosis of diabetes and the first examination of retinopathy was 6.62 years.

ABSTRACT

The functioning filtering bleb constructed after trabeculectomy in glaucomatous patient might be fail after cataract operation probably due to transient collapse during cataract extraction and inflammatiry reaction of the filtering bleb which result in unsuccessful intraocular pressure control. To compare if the results were identical in cases with the use of encircling band to enlarge the aqueous absorbing scar tissue to trabeculectomy, intraocular pressure was followed for more than 6 months after cataract extraction and posterior chamber lens implantation in 7 eyes that previously underwent silicone tube insertion with the use of encircling band. There were no cases with increased intraocular pressure needed to control postoperatively. Therefore it could be possible to concluded that inflammatory reaction of the filtering bleb or transient collapse during cataract extraction has no negative influences on intraocular pressure in silicone tube implanted eye using encircling band.
