ABSTRACT

OBJECTIVE To analyze the situation and causes of the shortage of drugs in centralized volume-based procurement, and to put forward countermeasures and suggestions for optimizing centralized procurement rules and improving the drug shortage supply guarantee system. METHODS The shortage data of the first five batches of centralized purchasing drugs during January 2019 and December 2021 were collected from the Shortage Database of Chinese medicine economic information; descriptive statistical analysis was performed for shortage frequency, varieties and reasons. RESULTS & CONCLUSIONS After the first five batches of centralized volumetric procurement policy landed, the frequency of drug shortage reported for selected specifications and selected enterprises increased significantly, mainly drug shortage of selected enterprises. The shortage of some varieties from selected enterprises in the short term was obvious, and most of them were commonly used clinical drugs. The occurrence of drug shortages was affected by various links such as the production, circulation and use, and the main reasons were shortage or monopoly of raw materials. In order to ensure the drugs supply, the government should scientifically assess the risk of production interruption of enterprises, further optimize centralized procurement rules, improve the reporting system for drug shortages, and attach importance to the supply of key drugs; in addition, hospitals should take a variety of measures within the scopes of policies to flexibly respond to drug shortages so as to avoid delaying the treatment of patients.

ABSTRACT

Objective:To analyze the structure of supply and demand, the distribution status of essential drugs and low-cost drugs and the influencing factors in the distribution, and provide strategies and suggestions. Methods:The literature research, Delphi method, sampling survey and the other methods were used to analyze the effects of distribution pattern on the drug distribution in Henan, and then optimization measures were studied systematically. Results:The distribution mode of centralized purchasing drugs was the major influen-cing factor in the drug distribution rate and enthusiasm. Conclusion:The distribution of centralized purchasing drugs should stick to the principles of essential drugs priority and reasonable use in order to enhance the distribution enthusiasm of manufacturers. The distribution should base on the two-way choice distribution mode and combine with the mode of centralized distribution and independent choice, mean-while, the information technology should be improved to optimize dynamic management of distribution and promote the effectiveness of drug delivery.

ABSTRACT

OBJECTIVE:To standardize hospital purchasing management for medicinal material. METHODS:The separation of the four powers,including plan,purchase,supply,and payment,was practiced on the basis of centralized purchasing medicinal material in our hospital. SWOT method was used to analyze the strengths,weaknesses,opportunities and threats of the model;im-provement measures were formulated;finally the effects of the model were evaluated. RESULTS:This model played an active role in the professional advantage of purchasing management,made up for the lack of management system and manpower shortage dis-advantage;adapted to policies to seize the opportunities for improvement,reduced the threat of standard deletion and purchasing cost,and promoted reasonable consumption of medicinal materials. And then the purchasing cost of our hospital was reduced after system procedure improvement,fine management of medicinal material and professional level improvement,medicine ratio fell from 42% in 2010 to 37% in 2014;logistics support became stable and standard,and the level of medicinal material supply was improved. CONCLUSIONS:The purchasing model for medicinal material under separation of the four powers provides the refer-ence for the correct course of the standardized,normalized and modernized hospital purchasing management.

Objective To analysis the major problems and the causes in the process of centralized purchasing of national essen -tial drugs, and to give some effective solutions according to the price problems for ensuring the quality and regular supply of national es -sential drugs , decrease patients burden effectively and safeguard the medication safety of patients .Methods The implementation of cen-tralized purchasing of national essential drugs was investigated , the problems which were major and outstanding were analyzed to find out the internal causes and the effective solutions were given .Results The progression of centralized purchasing of national essential drugs was successfully in general .The implementation of the principle of winning of the bidding with the lowest bid price lead to excessive low price, which caused some problems such as lower quality or bad supplied of some national essential drugs .Conclusion The price policy of centralized purchasing of national essential drugs should not be instituted based on the lower price , more winning of biddings .It needed more scientific and overall perspective thoughts .It maybe solve the problems mentioned above by winning of the bidding with the lowest bid price in the interval average price , appropriately loosing the control of difference rate with the results of decreasing the drug price , avoiding cut-throat competition, ensuring the quality and supply of drugs , lightening the patients′burden, releasing the problem of high cost of getting medical treatment .

ABSTRACT

In this paper, a systematical and retrospective review was made on how the drug purchasing system of Chinese healthcare institutions has evolved from decentralized purchasing to centralized purchasing based on four stages as budding stage, initial stage, adjustment stage and deepening stage. In each stage, the working characteris-tics, achievements and problems have been analyzed. Targeting at the problems and challenges confronted with cur-rent drug purchasing, recommendations was made to reform and improve public hospitals’ centralized purchasing sys-tem, such as persisting centralized purchasing online at provincial level, sticking to “four favorable” principles, breaking through the mechanism of compensating medical cost with drug-selling profits, promoting coordinated inter-action reforms in healthcare insurances, distribution and pricing, strengthening comprehensive services and supervi-sion, etc.

ABSTRACT

OBJECTIVE:To provide reference for establishing and perfecting drug supply system at county,township and village levels.METHODS:The exploration and practice was conducted from aspects of establishing institutions,establishing drug list and drug purchasing prices,selecting distribution company,and strengthening pharmaceutical administration,etc.RESULTS:A drug supplier and a drug distribution enterprise should be respectively selected by the special agency established in local Medical Administrative Command,and all the drugs used in township hospitals should be purchased from that drug supplier then distributed by special drug distribution enterprise to the target hospitals.CONCLUSION:It is feasible to establish a consummate centralized drug supply system for township hospitals (community health service centers),based on which,the drug supply system at county,township and village levels can be established.A consummate drug supply system is the key for the smooth running of the system of national essential drugs.

The authors first expound the connotation and extension, characteristics and forms of governmental purchasing and the significance and necessity of centralized drug purchasing. Then they focus their discussion on the specific procedures of centralized drug purchasing. Finally they point out that it is imperative to seek unity of thinking, standardize operations, adopt institutionalized means of management, and make a really good job of centralized drug purchasing.
