ABSTRACT

Four versions of diagnostic criteria for cerebral vascular diseases have been published since 1978 in China. Based on the three previous versions, the 2019 version conformed the new published Chinese classification for cerebral vascular disease and International Classification of Diseases-11, integrated the symptoms, signs and imaging findings, and then formulated the diagnostic criteria for main types of cerebral vascular diseases. It emphasizes the clinical manifestation and neuro-imaging findings, specifies etiology on diagnosis and becomes the most concrete and comprehensive version of diagnostic criteria for cerebral vascular diseases in China.

ABSTRACT

Notoginseng Radix et Rhizoma has the efficacy of dissolving stasis and hemostasis and reducing swelling and easing pain. Panax notoginseng saponins (PNS) is the main active component of Notoginseng Radix et Rhizoma, and the main components include ginsenoside Rb1, Rg1, Re, Rd and notoginsenoside R1. Injections with PNS as the medicinal material basis have become main TCM injections for the treatment of cerebral vascular diseases, with confirmed clinical efficacy. This article reviewed the research progress in pharmacological effects, clinical application and adverse reactions of PNS in treatment of cerebral vascular disease, with a purpose to provide references for further research and clinical application.

ABSTRACT

Objective The original data of CT perfusion(CTP) using 320-row area detector can reconstruct the whole-brain dynamic CT perfusion-derived angiography (CTPa).To confirm whether CTPa could replace CTA in diagnosis of cerebral vascular stenosis and occlusive diseases.Methods Thirty-five patients undergoing CTA and CTPa were screened retrospectively for this study.The image quality,display of fine intracranial vascular,vascular enhancement CT value,SNR and CNR were compared from the same patient.Using DSA imaging as the golden standard,the Sn and Sp for detecting the vessel segments stenosis ≥50% or occlusion were compared for two examinations.Results CTPa showed higher image quality score,vascular enhancement CT value,SNR and CNR than CTA (P<0.001).CTPa was superior to CTA in the display of fine intracranial vascular.The Sn and Sp for detecting the vessel segments stenosis ≥50% or occlusion were 88.57% and 99.26% with CTA,91.42% and 99.75% with CTPa respectively(P> 0.05).Conclusion Compared with CTA,CTPa provides better image quality and comparable vascular pathology.CTPa is expected to replace CTA in diagnosis of cerebral vascular stenosis and occlusive diseases.

ABSTRACT

Objective To evaluate the value of digital subtraction angiography(DSA) in diagnosis and planning endovascular treatment of cerebrovascular diseases. Methods Thirty-one children suspected with ischemic cerebrovascular diseases were treated by urokinase and dextran after they underwent DSA.Results Twenty-four cases (77%) had cerebrovascular abnormalities. Two cases had arteriovenous malformations, 1 case had posterior communicating artery aneurysm,21 cases had cerebrovascular stenosis or occlusions. Shortly after intervention therapy using urokinase,muscle strength increased by one level in 13 cases,two levels in 3 cases,3 levels in 2 cases and no effect in 4 cases. The total effective rate reached 86%. Seven patients with ischemic stroke achieved 100% recanalization. Conclusion DSA is very important in diagnosis of cerebrovascular stenosis and occlusion.

It views the physiological characteristics of cardio-cerebral vascular diseases are deficiency of root and excessiveness of appearance,i.e.root being deficiency of Yin,Yang,Qi and blood;the superficiality being blood stasis,sputum and Qi block.The common treatments are to activate blood circulation and get rid of stasis,warmly activate Yang Qi,protect heart Qi,nourish Qi and relief block,induce resuscitation with aromatics,etc.

ABSTRACT

@#ObjectiveTo study the impact of stroke on elderly's mental status.Methods71 elderly inpatients aged between 60 to 90 who suffered stroke(31 cases) or not(40 cases) were assessed the mental status as a whole, the evaluation of comprehension of linguistic information,and non-verbal reasoning abilities using Mini Mental State Examination(MMSE), Token Test and Raven Test.The relationships between these cognitive aspects and stroke was also analysed.ResultsThere were 68% of elderly suffering dementia symptoms in stroke group which was much higher than Non-stroke group (15%).The correlation between MMSE and Token is more close than the correlation between MMSE and Raven in stroke group. The correlations among MMSE, Token and Raven were significant different between stroke and Non-stroke groups. There is a positive correlation coefficient between age and Token scores in dementing patients; whereas a closer relationship between age, education level and Raven in Non-stroke group was found. This implied the impacts of stroke on the time-course of mental degeneration.Conclusions stroke can interrupt normal mental structure and speed up the process of degeneration of cognitive functions. Semantic dysfunction might manifestation earlier than non-linguistic functions. Token is more sensitive than Raven in detecting dementia symptoms of stroke type.
