ABSTRACT
Objective To investigate the clinical application value of detection of serum beta 2 adrenergic receptor(beta 2-AR) mRNA and nerve tissue growth factor (NGF) mRNA in airway remodeling on children with bronchial asthma.Methods The clinical serum data of 33 cases of children with bronchial asthma in airway remodeling were selected,divided into the pre-treatment group and the post-treatment group,and selected 29 cases of children with respiratory diseases as the control group.The above research subjects were outpatients or inpatients from department of pediatrics in the First People's Hospital of Xianyang City from 2015.3 to 2017.3.The expression of serum beta 2-ARmRNA and NGFmRNA were detected by realtime fluorescence quantitative PCR (Real-time-PCR,RT-PCR).The relationship between the changes of serum 2-ARm-RNA and NGFmRNA and the occurrence of asthma remodeling was compared and analyzed.All the subjects were measured the inner diameter of the airway (L),diameter of the airway (D),the airway wall thickness (T) calculated by T=(D-L)/2,the airway lumen area (AI),total airway cross-sectional area (AO) with High Resolution Computerized Tomography in evaluation of airway remodeling of asthma through the above indexes.The clinical significance of serum beta 2-ARmRNA and NGFmRNA by receiver operating characteristic curve (ROC curve) was evaluated.Results The serum beta 2-ARmRNA and NGFmRNA expression in the post-treatment group were,respectively,up-regulation and down-regulation in comparison with the pre-treatment group,and there was significant difference (t =37.27,36.53,all P＜0.01),which in the post-treatment group were,respectively,down-regulation and up-regulation in comparison with the control group,and there was significant difference (t=7.95,12.67,all P＜0.01).The expression of erum beta 2-ARmRNA and NGFmRNA had a negative correlation in the remodeling group (r=-0.697,P＜0.01),which was associated obviously respectively with L,T and AI (r=0.775,-0.780,0.793,all P＜0.01),(r=-0.750,0.763,-0.779,all P＜0.01).The expression of erum beta 2-ARmRNA and NGFmRNA hada negative correlation in the pre-treatment group (r=-0.703,P＜0.01),which was associated obviously respectively with L,T and AI (r=0.782,-0.797,0.803,all P＜ 0.01),(r =-0.772,0.787,-0.809,all P ＜0.01).According to the statistical results of SPSS,ROC curve analysis showed that the expression levels of serum beta 2-ARmRNA and NGFmRNA in the remodeling group were 0.767 (95 ％ CI:0.658 ～ 0.876,P =0.000);0.744 (95 ％ CI:0.632 ～0.856,P=0.000).The sensitivity and specificity of beta 2-ARmRNA and NGF mRNA at the optimal critical value were 93.9 ％ and 96.2 ％,92.9 ％ and 95.7 ％.Conclusion The expression of serum beta 2-ARmRNA and NGFmRNA can monitor treatment of airway remodeling on children bronchial asthma,and estimate disease development,effectively control the progress of airway inflammation in reversing asthma disease progression to provide the basis for early intervention of children with bronchial asthma.
ABSTRACT
Objective:To study the curative efficacy of budesonide combined with huaiqihuang granules in the treatment of children with asthma and its effects on the serum leukotriene D4(LTD4),nerve growth factor (NGF),tissue inhibitor of metalloproteinase-1 (TIMP-1) levels.Methods:90 cases of children with asthma who were treated from March 2014 to March 2015 in our hospital were selected and divided into the observation group (n=45) and the control group (n=45) according to the random number table.Both groups of children were given bronchial antispasmodic agent,oxygen,antibiotics,glucocorticoid and other conventional treatment after admission,the control group was given budesonide atomization inhalation treatment on the basis of conventional treatment,0.5 mg budesonide were add in 3ml saline for continuous inhalation,twice a day,while the observation group was treated with Huaiqihuang Granules on the basis of the control group,twice a day.After 12 weeks of treatment,the curative effect,disappearance time of wheezing,cough relief,pulmonary rales,wheezing and other symptoms,forced vital capacity (FVC),serum LTD4,NGF,TIMP-1 levels,CD3+,CD4+,CD8+ of both groups were compared.Results:After treatment,the total effective rate of observation group was significantly higher than that of the control group [93.33％(42/45) vs 77.77％(35/45)](P＜0.05);disappearance time of breath,cough,moist rales and wheezes were significantly shorter than those of the control group[(4.32± 1.03)d vs (6.08± 1.24)d,(5.60± 1.12)d vs (7.21± 1.30)d,(3.19± 0.98)d vs (4.98± 1.02)d,(3.25± 1.03)d vs (5.89± 1.35)d](P＜0.05);the FEV1,FEV1/FVC were significantly higher than those of the control group [(92.63± 10.01)L/s vs (78.36± 9.19)L/s,(95.37± 11.72)％ vs (80.19± 10.23)％](P＜0.05);the serum LTD4,NGF,TIMP-1 levels were significantly lower than those of the control group [(7.24± 0.86)ng/mL vs (12.68± 1.01vng/mL,(68.18± 9.01)pg/mL vs (80.78± 10.24)pg/mL,(34.16 ± 5.06)ng/mL vs (49.76 ± 5.47)ng/mL] (P ＜0.05);the CD3+,CD4+ were significantly higher than those of the control group [(66.15± 7.20)％ vs (62.03± 6.85)％,(45.13± 7.90)％ vs (37.42± 7.06)％],CD8+ was significantly lower than that of the control group[(34.16± 5.06)％ vs (49.76± 5.47)％] (P＜0.05).Conclusion:Budesonide combined with Huaiqihuang granules was effective for children with asthma,which could enhance the immune function,improve the lung function,reduce the serum levels of LTD4,NGF,TIMP-1.
ABSTRACT
Objective To investigate the therapeutic effect of inhaled budesonide suspension combined with psychological intervention on children with asthma. Methods 64 children with asthma were enrolled in our hospital from October 2015 to December 2016. They were divided into experimental group and control group, 32 cases in each group. The control group was treated with inhaled budesonide suspension.The experimental group was treated with inhalation with budesonide suspension combined with psychological intervention, the treatment efficiency and symptom improvement were compared and analyzed in the two groups. Results The effective rate of the experimental group was 96.87%, which was higher than that of the control group 84.37%, the difference was statistically significant (P<0.05). The symptoms of cough, wheezing, wheezing and dyspnea in the experimental group was obviously better than that in the control group, the difference was statistically significant (P<0.05). Conclusion The effect of inhalation with budesonide suspension combined with psychological intervention in the treatment of children with asthmais pbvious.It can effectively improve the clinical symptoms, and promote recovery of children.

Objective To analyse effect of different dose of sustained-release theophyline on prealbumin, C-reactive protein and white blood cell count in patients with pediatric asthma.Methods 58 children with acute asthma attack in our hospital were collected.All children were randomly divided into high dose group and low dose group,29 cases in each group.On the basis of conventional treatment, high dose group were treated with high dose theophylline sustained release tablets 5 mg/kg, one times per eight hours,orally,and low dose group was treated with low dose theophylline sustained release tablets,2 mg/kg, one times per eight hours,orally.Two groups were treatment for two weeks.After the treatment, the serum levels of pre albumin, C-reactive protein and veinal blood level of white blood cell count were detected in all children.Results Compared with high dose group post-treatment, the serum level PA was higher in low dose group (P<0.05);the serum level of CRP was lower in low dose group (P<0.05);the veinal blood level of WBC, percentage of neutrophils and percentage of eosinophils was lower in low dose group (P<0.05) .Conclusion Compared with high dose of sustained-release theophyline,the low dose of sustained-release theophyline can significantly reduce the serum CRP level and veinal blood level of WBC, percentage of neutrophils and percentage of eosinophils, improve serum PA level in patients with pediatric asthma, reduce inflammation in patients.

@#ObjectiveTo explore the relationship between ORMDL3 gene polymorphism and Asthma in children.Methods178 cases of children with asthma were divided into mild group and severe group, and 129 cases of children were treated as normal control group who tested blood samples in the PLA General Hospital and its Affiliated Hospital in the recent 4 years. ORMDL3 gene locus rs7216389 genotype was detected, including PCR, restriction endonuclease analysis and bio-statistical aspects of analysis.ResultsThere was significant difference (P<0-05) in 3 genotypes (CC＼TC＼TT). There was no significant difference in TT between the mild group and the control group (P>0-05). The rate of severe group was higher than that in the mild and the control groups(P<0-05). Asthmas in children showed higher T allele frequency.ConclusionORMDL3 gene polymorphism showed a certain relevance with asthma in children.

OBJECTIVE : To prepare the sustained release tablets of salbutamol sulfate,and to establish the method of quality control.METHODS: The sustained release tablets were prepared by using salbutamol sulfate, lactose, sodium carboxymethyl starch, HPMC, EC, and MCC and coated with enteric soluble acrylic resin.HPLC was employed to determine the content of salbutamol sulfate, while at the same time dissolution grade in vitro was determined, and stability was investigated.RESULTS: : The detectable concentration of salbutamol sulfate showed a good linear correlation with peak area in the range of 1.25～20.00?g/ml.The average recovery of salbutamol sulfate was 98.60% (RSD=0.66%).The prepared tablets showed a timing release 5 hours after administration.No evident changes in dissolution were found in all tests of stability.CONCLUSION: The formulation of the preparation is reasonable; the preparation technique is simple and feasible, and the quality is stable and controllable.
