ABSTRACT

Introdução: A falta da projeção mentoniana no terço inferior da face algumas vezes é responsável pela quebra da harmonia do contorno facial. A utilização de implantes aloplásticos, preenchimentos submetidos à ritidoplastia para correção destas deformidades. Neste estudo, propomos uma nova manobra para aumento da projeção mentoniana com uso de um retalho cervical associado à ritidoplastia. Métodos: Foram avaliados 11 pacientes operados no período de 01/2017 a 01/2018, utilizando-se o retalho cervical para projeção mentoniana, e tendo como critério de inclusão somente pacientes que almejavam um aumento mentoniano, sem utilização de próteses, preenchimentos ou abordagem óssea, e que seriam submetidos à ritidoplastia. Resultados: Através da análise cefalométrica evidenciou-se melhora da projeção mentoniana e do contorno cervical, e não houve complicações no pós-operatório imediato ou tardio. Conclusão: O retalho cervical utilizado para aumento mentoniano além de apresentar resultados e aceitação satisfatórios, elimina o uso de materiais sintéticos, redução de custos, segurança e durabilidade, alcançando um contorno mandibular mais refinado e uma projeção mentoniana mais natural.


Introduction: The lack of chin projection in the lower third of the face is sometimes responsible for the breaking of the facial contour harmony. Alloplastic implants, fillers, and osseous advancements have been used to correct these deformities. In this study, we propose a new maneuver to increase chin projection by using a cervical flap associated with rhytidoplasty. Methods: We assessed 11 patients who underwent operations using the cervical flap for chin projection between January 2017 and January 2018. The inclusion criteria were only patients who desired chin augmentation without the use of prosthetics, fillers, or osseous approaches, and those who would undergo rhytidoplasty. Results: A cephalometric analysis revealed improvements in chin projection and cervical contour, and no complications in the immediate or late postoperative period. Conclusion: In addition to presenting satisfactory results and acceptance, the cervical flap used for chin augmentation eliminated the use of synthetic materials, reduced surgical costs, and improved safety and durability, achieving a more refined mandibular contour and natural chin projection.

ABSTRACT

In December 2018, a 29-year-old female patient with lingual artery embolism after hyaluronic acid injection was admitted to the Second Affiliated Hospital of Kunming Medical College. The examination showed swelling tongue, the mucosa on the front and back of the right tongue became white, and the tip of the tongue slightly atrophied. The movement of tongue was disturbed. The patient′s pronunciation was unclear. The right tongue was numb, resulted by the weak circulation. Hyaluronidase was injected into tongue. The vasodilator was used. And hyperbaric oxygen chamber was applied. Thereafter, the blood supply and oxygen concentration of embolized area were increased, and collateral circulation was established. Antibiotic therapy and continuous treatment with neurotrophic drugs (rat nerve growth factor + methylcobalt ammonium) were given to promote the repair of nerve. After treatment, the embolism of lingual artery was significantly improved. Three months later, the patients had mild muscular atrophy on the right side of the tongue, with partial recovery of the right tongue mucosa. Tongue movement, sensory and taste functions recovered. Hyaluronic acid injection needs to be standardized, performed by surgeons who masters the anatomical structures, the injection layer and spot. If the vascular embolism occurs, hyaluronidase should be used as early as possible. It also needs to reduce local tension in both injected and affected area, reduce the pressure of injection on blood vessels, improve local blood circulation, so as to achieve better therapeutic effect.

ABSTRACT

The chin, located in the most prominent part of the lower facial profile, maintains the balance and coordination of the face. The chin size, shape, spatial position and proportion can all give rise to various chin malformations. Chin plasty plays an important role in improving facial appearance. Recent studies indicate that it’s difficult to completely meet the mental aesthetic requirements with a single procedure. The combination of different chin plastic surgery has become a hot spot in plastic surgery. In this paper, the morphological analysis of chin, the surgical progress in Chin plasty, the advantages and adverse effects of these methods, and the synthetic application of various surgeriesis were reviewed.

ABSTRACT

Objective To analyze and discuss the possible reasons of the bone resorption beneath the prostheses after chin augmentation.Methods Twelve patients were admitted to our department for further correction after chin augmentation with materials.The bone resorption was observed through the clinical research and X-ray examination.Results All the patients were underwent the removal of the materials,genioplasty was performed in 8 patients,and two patients were treated by chin augmentation with polyethylene.All the patients were satisfied with their facial contouring.Mild bone resorption was found in seven patients (depth of bone resorption ≤2 mm),in which five patients were used with silicone materials,two patients were performed with expanded polytetrafluoroethylene implants.Moderate bone resorption was seen in three cases.All of them were used with silicone implants (2 mm < depth of bone resorption ≤4 mm).Severe bone resorption happened in two patients (depth of bone resorption >4 mm).One was used with silicone implant,and the other one was carried out with expanded polytetrafluoroethylene implant.Conclusions The imbalance among mentalis muscle,materials and underlying bone might be one of the key reasons.Thus for mild and moderate microgenia cases,chin augmentation with material is suitable,while long-term fellow-up study is necessary.But for the cases of severe mirogenia or microgenia and micrognathia with dentofacial deformity or mentalis muscle hyperactivity,genioplasty might be performed as well to correct their deformities.

ABSTRACT

Objective High-density porous polyethylene(Medpor)has been widely used in chin augmentation in recent years.The study aimed to observe the outcomes of clinical use of high-density porous polyethylene implant in chin augmentation in contour plastic surgery of low face.Methods From 2005 to 2008,149 cases of chin augmentations intraorally with Medpor implants combined with were collected.121 of them received bilateral mandibular angle osteotomy and 28 cases received bilateral mandibular angle osteotomy and bilateral malar reduction simultaneously.They wre all females aged from 17 to 46 years.The patients were followed up for 6 to 36 months(average 24 months)and photographs were taken before and after operations.Clinical outcomes were observed and analysed.Resuits 145 patients(97.3%)were satisfied with their chin and contour of low face.2 patients (1.3%)were not satisfied with the projection of their chin,and 1 of them were admitted to take out the implant and received sliding advancement genioplasty.1 patient(0.7 0A)felt the implants was too large to some extent and was admitted to modify the implant.1 patient(0.7%)received genioplasty 6months after operation because of the deviation of the implant.20 patients(13.4%)felt numbness of the lower lip which gradually disappeared 1 to 3 months postoperatively.Complications such as infection,edema and extrusion,break of the implant were not found during the following period.Conclusions Medpor is an excellent biomaterial with good biocompatibility.Simultaneously chin augmentation with Medpor combined with bilateral mandibular angle osteotomy intraorally shows a good result in contour plastic surgery of low face.
