ABSTRACT

Perimenopause refers to the period around menopause. In Ayurveda it is considered as Rajakshaya, the transition period of body from predominance of Pitta dosha to Vatadosha which is characterized by aggravation of Vata, alteration of Pitta and Kaphadosha. It is included as a Swabhavikavyadhi in Ayurveda. Management of Dhatukshaya, Rasayana, Balya and Rasa- Raktha Prasadana treatments are seen beneficial in this condition. Guduci is one among Rasayana drugs mentioned in Ayurveda. The drug Guducisatva taken for the study is from the classic Yogaratnakara and has Dhatukrith and Vayastapana property. The objective of the study was to evaluate the effect of 1g Guduci Satva along with Ksheera (25ml boiled and lukewarm) in perimenopausal symptoms. Females in the age group 40-50 years with perimenopausal symptoms were selected for the study from the OPD of Prasutitantra-striroga, GAVC Hospital, Tripunithura as per the inclusion criteria. Study tools are clinical case proforma and Greene Climacteric scale score above 30. Drug was given orally in the dose of 1g twice along with 25ml Ksheera one hour before food. Duration of administration was 30 days and assessment was taken on 0th day, 31st day and 60th day. Results were analysed statistically by Wilcoxon's signed rank test and Friedman’s test. The study result is highly significant after treatment with P value <0.001 during treatment and during follow up and is found effective in reducing the symptoms in perimenopause.

Background: Menopause is a biological event characterized by the complexity of factors. On an average one-third of the women’s life consists of the post-menopause years, and health care programs for women do not address concerns beyond reproductive ages. The aim of this study was to describe the magnitude of menopause-related symptoms, the pattern of health care seeking, and associated factors. To assess the prevalence of gynecological disease in post-menopausal women.Methods: This cross-sectional study was done in Karpaga Vinayaga Institute of Medical Sciences and Research Center - obstetrics and gynecology OPD. Over a period, months in the year 2019. Stages of reproductive aging workshop (STRAW) revised criteria and nomenclature are used for the ascertainment of menopause and inclusion of the women in the study. It provided a comprehensive basis for staging since there were more complexities on ascertainment of menopause explained. The STRAW criteria are considered as the gold standard for assessing menopausal stages.Results: A total of 600 participants were included in the study. The proportion of women who had at least one menopause-related symptom was 95.95 (95% CI 93.73-97.54) and 58.3% of women had severe symptoms. Severities of symptoms in psychological, physical, vasomotor, and sexual domains were 56.7%, 70.5%, 49.3%, and 10.2% respectively. Premature menopause (p<0.016), induced nature of menopause (p<0.031), dyslipidemia (p<0.006) and other medical condition (p<0.003) were associated with severity of menopausal related symptoms.Conclusions: A high proportion of women are affected by menopause-related symptoms. Care seeking for all symptoms is not uniform, indicative of a lack of knowledge about the treatable nature of many of these symptoms. Sensitization of both women and the health care system may serve to address this issue of menopause-related symptoms and the possibility of treatment for these.

Introducción: la expresión clínica del síndrome climatérico se asocia con el estilo de vida de la mujer, factores socioculturales, ambientales y laborales, las relaciones familiares y de pareja, la sobrecarga en los roles que desempeña, su personalidad y autoestima. Objetivos: identificar la percepción de intensidad de la sintomatología del síndrome climatérico en un grupo de mujeres de edad mediana y relacionarla con algunos factores socioculturales. Métodos: se realizó una investigación descriptiva transversal en el Centro de Investigaciones Médico-Quirúrgicas desde el 17 de septiembre de 2012 hasta el 31 de mayo de 2013. Se entrevistaron 120 mujeres entre 40 y 59 años de diferentes departamentos asignados al azar, a las que se aplicó la encuesta para medir la intensidad del síndrome climatérico recomendada en el Consenso cubano vigente. Resultados: una tercera parte de la muestra ya había tenido la menopausia. El promedio de edad fue de 48,52 años. Hubo predominio de los síntomas moderados y muy molestos. Las condiciones de la vivienda, el hacinamiento y las disfunciones familiares no resultaron significativas para la sintomatología. Del total de mujeres, 58,1 por ciento refirieron disfunción con su pareja y presentaron sintomatología moderada o intensa. La sintomatología vasomotora fue más intensa en locales de trabajo calurosos. Conclusiones: más de la mitad de las mujeres con disfunción de pareja presentaron síntomas moderados e intensos, sobre todo, genitourinarios. Los síntomas psicológicos son más frecuentes en las mujeres con pérdida de la autoestima y la mitad de ellas los percibe con intensidad moderada o severa(AU)


Introduction: the clinical expression of climacteric syndrome is associated with the lifestyle of women, socio-cultural, environmental and occupational factors, family and marital relationships, overloading in the roles played, personality and self-esteem. Objectives: identify the perception of intensity of the climacteric syndrome symptoms in a group of middle-aged women and relate them with some sociocultural factors. Methods: adescriptive investigation was conducted at the Center for Medical and Surgical Research from 17 September 2012 to 31 May 2013. 120 women were interviewed between 40 and 59 years of different departments randomized. These subjects took a survey to measure the intensity of the climacteric syndrome. The survey used was recommended in the current Cuban Consensus. Results: one third of the sample had their menopause and the average age was 48.52. There was predominance of moderate and very troublesome symptoms. Housing conditions, overcrowding and family dysfunction were not significant for the symptoms. 58.1percent of women who reported dysfunction with her partner showed moderate to severe symptoms. Vasomotor symptoms were more intense in local hot jobs. Conclusions: more than half of women with marital dysfunction presented mostly moderate to severe symptoms, genitourinary. Psychological symptoms are more common in women with loss of self-esteem and half of them perceived moderate or severe intensity(AU)
