ABSTRACT

O presente trabalho parte da constatação no campo da nossa disciplina da existência de um hiato entre a clínica e a teoria. Procura caracterizar a ideia de pensamento clínico inspirado na obra de André Green e outros autores, como uma singular racionalidade, e trata de criar pontes entre a experiência vivida na clínica e certos parâmetros norteadores da teoria (ou teorizações ad hoc) que não tornem a experiência uma mera aplicação técnica, mas que em contrapartida não desabem numa idealização da espontaneidade irrefletida e/ou atuada. A fecundidade da noção de pensamento clínico nos casos limite será explicitada a partir de um relato clínico no qual podem ser investigadas e reconhecidas noções como enquadre, trauma, limite da representação, indiscriminação afeto-representação e traumatismo narcísico-identitário...


This work emerges from the realization in our discipline's field of the existence of a hiatus between the clinical and the theoretical. An attempt is made to characterise the idea of the clinical thought inspired by the work of André Green and other authors, as a binding thought between both. It is about creating bridges between the lived experience in the clinical and certain guiding parameters of the theoretical (or ad hoc theorizations) which do not turn an experience a mere technical application, but which on the other hand will not collapse in an idealization of the unthinking and/or acted idealization. The fertility of the notion of clinical thought in limit cases will be made explicit from a clinical account where notions such as framing, trauma, representation limit, affection-representation indiscrimination and narcissistic-identity trauma can be investigated and recognized...


Este trabajo parte de la constatación, en el ámbito de nuestra discipl ina, de la existencia de una brecha entre la clínica y la teoría. El autor intenta caracterizar la idea de pensamiento clínico inspirado en la obra de André Green y otros autores, como un pensamiento de enlace (una singular racionalidad) entre ambas. La idea gira en torno de construir puentes entre la experiencia vivida en la clínica y ciertos parámetros rectores y algunas teorías (o teorías ad hoc) que no hacen de la experiencia una mera aplicación técnica, pero que tampoco acaban en la idealización de una espontaneidad irreflexiva o una actuación. La fecundidad de la noción de pensamiento clínico en los casos limite (borderline) se pondrá de relieve en un breve informe clínico del cual emanan y pueden ser investigados conceptos como encuadre; trauma; límite de la representación; no discriminación afecto-representación y trauma narcisista-identitario...

ABSTRACT

Clinical tutor system is a unique exploration and practice with characteristics of our school,which has been carried out in the process of clinical course learning and clinical practice among eight-year-program clinical majors.Through analyzing and summarizing the implementation among 2005 and 2006 grade students,we found that the clinical tutor played an active role in cultivating students' clinical thoughts and personal ethics and in improving clinical ability and scientific ability.However,some problem were existed in tutors themselves-specialized major,busy,unclear cultivation objective,etc.Therefore,further improvement of the system was needed in order to better meet the needs of the eight-year-program medical talent training and to provide related references.

André Green instala os fundamentos de seu pensamento sobre o afeto a partir de uma crítica nocional dos textos freudianos e pós-freudianos, mostrando, assim, sua preocupação em se inscrever numa história, sem deixar de insistir no aspecto estrutural da questão. Procura, também, propor uma alternativa ao modelo lacaniano, explorando os recursos consecutivos à exclusão, por Lacan, do afeto no seio de sua teoria. As autoras retomam os principais componentes do afeto segundo Green, em particular seu aspecto insconsciente, para em seguida mostrar que seu trabalho de pensamento clínico lhe permite precisar o funcionamento do afeto. Foram escolhidos três exemplos importantes no pensamento clínico de André Green: a alucinação negativa da mãe, a paixão e os estados de indiscriminação afeto/representação.


André Green establishes the foundations of his thoughts on affection from a critique of the notions of Freudian and Post-Freudian texts, showing, as such, his concern in fitting into a story, while insisting on the structural aspect of the matter. He also proposes an alternative to the Lacanian model, exploring the resources consecutive to the exclusion, by Lacan, of affection in the heart of his theory. The authors resume the main components of affection according to Green, specially its unconscious aspect, in order to show that his work in clinical thought allows him to specify the functioning of affection. Three important examples of André Green's clinical thought were chosen: the mother's negative hallucination, passion and the cases of indiscrimination of affection/representation.


André Green instala los fundamentos de su pensamiento sobre el afecto a partir de una crítica nocional de los textos freudianos y pos freudianos, mostrando, de esta forma, su preocupación para inscribirse en una historia, sin dejar de insistir en el aspecto estructural del tema. Busca también proponer una alternativa al modelo lacaniano, explotando los recursos posteriores a la exclusión, por Lacan, del afecto en el seno de su teoría. Las autoras retoman los principales componentes del afecto de acuerdo con Green, en particular su aspecto inconsciente, para, en seguida, mostrar que su trabajo de pensamiento clínico le permite precisar el funcionamiento del afecto. Fueron escogidos tres ejemplos importantes en el pensamiento clínico de André Green: la alucinación negativa de la madre, la pasión y los estados de indiscriminación afecto/representación.

Este trabalho é parte de uma apresentação feita em 2012, no II Encontro Internacional André Green, realizado na Asociación Psicoanalítica Argentina. O texto retoma alguns conceitos fundamentais de Green, particularmente os conceitos de pulsão, representação e pulsão de morte. Desenvolvem-se, a partir daí, algumas ideias sobre as reformulações da técnica, interpretação, construção, contratransferência imaginativa e silêncio do analista.


'This article is part of a presentation shown in 2012, during the II International André Green Meeting, which took place at the Asociación Psicoanalítica Argentina. The text considers some of Greens basic concepts, particularly the ideas of drive, representation and death drive. From this, ideas on the reformulations of the technique, interpretation, construction, imaginative countertransference and silence of the analyst are developed.


Este trabajo es parte de una presentación realizada en el II Encuentro Internacional André Green del año 2012, en la Asociación Psicoanalítica Argentina. El texto retoma algunos conceptos fundamentales de Green, en particular los conceptos de pulsión, representación y pulsión de muerte. De ahí se desarrollan algunas ideas sobre las reformulaciones de la técnica, interpretación, construcción, contratransferencia imaginativa y silencio del analista.

It has formed a unique clinical thought mode in therapeutical principle and method of treatment of pediatric primary nephrotic syndrome based on syndrome differentiation.In treatment of pediatric kidney diseases based on syndrome differentiation,it must be according to the pathogenesis of "asthenia in origin and asthenia in superficiality",made strengthening vital qi to bank up the root and eliminating pathogenic factor to cure the manifestation as the basic rule,mainly with the method of strengthening vital qi.In clinic,it should determine treatment on the basis of pattern types based on syndrome differentiation,first clear the manifestation and root cause of disease and the excess and deficiency,the root of disease takes asthenia of both the spleen and kidney as core,and the manifestation of disease takes wind,damp and blood stasis as the key.

The vital force of Chinese medicine is rooted in the excellent clinical efficacy,however,the good efficacy,is inseparable from a profound theoretical basis,noble medical ethics,and superb medical skills;Based on these aspects,the characteristics of clinical thinking of the medical master Professor WANG Mian-zhi was analyzed preliminary.The master developed a unique clinical thinking style by integrating the ethics,principles of Chinese medicine and clinical skills,which was based on deep morality and rigorous scholarship.Professor Wang's unique style can be summed up as emphasis on syndrome differentiation,elaborate discussion on treating,fine adjusting formula and herbs according to the evidence based syndrome change;thinking carefully and harmoniously;Thought of integration and innovation,overcoming difficulties.Master Wang's thought is very valuable for the cultivation of Chinese medicine scholars and is beneficial for clinical and research personnel of Chinese Medicine,it is worth being further developed.
