ABSTRACT

Objective::To observe the clinical efficacy of modified Weijingtang and Maxing Shigan Tang combined with half-dose hormones in the treatment of mycoplcasma pneumoniae pneumonia in children caused by toxic heat closing lung. Method::Totally 120 cases were randomly divided into control group and observation group, with 60 cases in each group. In addition to the basic therapy, control group was given Lianhua Qingwen granules + methanilone (10 mg·kg-1, 2 times/day), while observation group was given modified Weijingtang and Maxing Shigan Tang + methanilone (10 mg·kg-1, 1 time/day) for 14 days. Lung function indicators [maximal voluntary ventilation (MVV), time of peak tidal expiratory flow (TPTEF), forced expiratory volume in one second (FEV1), volume peak time expiratory flow (VPTEF)], plasma myocardial enzyme profile [creatine kinase (CK), creatine kinase isoenzyme (CKMB), lactate dehydrogenase (LDH), hydroxybutyric acid dehydrogenase (HBDH)], immune function [immunoglobulin G (IgG), immunoglobulin M (IgM), erythrocyte immune complex (RBC-ICR), erythrocyte C3b receptor (RBC-C3bR)], inflammatory factors [tumor necrosis factor-alpha (TNF-α), gamma-interferon (IFN-γ), interleukin-13 (IL-13), interleukin-17A (IL-17A)], clinical efficacy and adverse reactions were observed before and after treatment. Result::Four cases fell off during the study period. The total effective rate in observation group was 96.6%(57/59), which was higher than that in control group 84.2%(48/57, P<0.05). Compared with control group after treatment, lung function, RBC-C3bR and IL-13 were increased in observation group (P<0.05), while myocardial enzyme spectrum, IgG, IgM, RBC-ICR, TNF-α, IFN-γ and IL-17A were decreased in observation group (P<0.05). The incidence of adverse reactions in observation group was, lower than that in control group(P<0.05). Conclusion::Modified Weijingtang and Maxing Shigan Tang combined with half-dose hormones can improve lung function, myocardial enzyme profile, immune function and inflammatory factor levels of refractory mycoplasma pneumoniae in children caused by toxic heat closing lung, with a incidence of adverse reactions.

ABSTRACT

Objective:To discuss clinical effect of addition and subtraction therapy of Wuhutang combined with Qingjin Jianghuotang to community acquired pneumonia (CAP) in children with syndrome of phlegm heat closing lung, and to study the influence to inflammatory factors. Method:One hundred and forty patients were randomly divided into control group (69 cases) and observation group (71 cases) by random number table. Patients in two group of chidren got comprehensive symptomatic treatment measures of anti-infection, antipyretic, expectorant, antiasthmatic and respiratory support of inflammatory factors. The control group was treated with Lingyang Qingfei granules.1 g/time,3 time/day. Patients in observation group added addition and subtraction therapy of Wuhutang combined with Qingjin Jianghuotang, 1 dose/day. The courses of treatment in two groups were 7 days. And temperature, time of antipyretic, time of complete antipyretic and rate of complete antipyretic at the 7th day after treatment were recorded. And release time and disappearance time of cough, expectoration, disappearance time of pulmonary rales and treatment failure were also recorded. And before and after treatment, scores of syndrome of phlegm heat closing lung were graded, and levels of serum high sensitive C-reactive protein (hs-CRP), procalcitonin (PCT), tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) were detected. Result:Analyzed by rank sum test, effect in observation group was better than that in control group (Z=2.106, P<0.05), and curative effect of traditional Chinese medicine (TCM) syndrome was also better than that in control group (Z=2.119, P<0.05). Time of antipyretic, time of complete antipyretic, release time and disappearance time of cough and expectoration and disappearance time of pulmonary rales were all shorter than those in control group (P<0.01). Rate of complete antipyretic at the 7th day after treatment in observation group was 96.92%(63/65) higher than 82.81%(53/64) in control group (χ2=7.085, P<0.05). Failure rate of treatment was 9.23%(6/65) lower than 23.44%(15/64) in control group (χ2=4.775, P<0.05). And major symptom, physical sign score, minor symptom score, the total score of syndrome of phlegm heat closing lung and levels of hs-CRP, PCT, TNF-α and IL-6 were all lower than those in control group (P<0.01). Conclusion:On the basis of comprehensive anti-infection treatment, addition and subtraction therapy of Wuhutang combined with Qingjin Jianghuotang can control the clinical symptoms, and the advantages of rapid onset, rapid symptom regression, short course of disease can be found, and it can also reduce the inflammatory reaction, control the progress of the disease. The complete antipyretic rate, disease efficacy and TCM syndrome efficacy are better.

ABSTRACT

With the discussion on the origins and the evolution of Star Twelve Points, combined with ancient astronomical stellar map, it is realized that the three-dimensional spatial diagram of Star Twelve Points can be approximately regarded as a Big Dipper map from the side view. Under the direction of image thinking, the function of Big Dipper was compared with the function of Star Twelve Points. Furthermore, according to "the opening-closing-pivoting" theory in and the theory of " cycle in round" proposed by , the mechanisms of Star Twelve Points on adjusting functional activities of and the movement of viscera-meridian--blood is elaborated, providing a new idea for acupuncture clinical treatment of miscellaneous diseases.

Introduction: Freiberg’s disease, the osteochondrosis of themetatarsal seen in our part of the world, usually at late stages,are refractory to the conservative treatment. Objective:Thepurpose of this study was to evaluate the results of dorsalclosing wedge osteotomy and pin fixation besides debridementand synovectomy.Material and Methods: Twenty patients (14 females,6 Males), mean age 19.3 years; range 14 to 26 years wereevaluated and teated by dorsal closing wedge osteotomy andpin fixation besides debridement and synovectomy, betweenseptember 2016 to 2018 at our institution. The presentingsymptoms were pain on walking or sports, limitation of dailyroutine activities, footwear problems, cosmetic deformitiesof forefoot and failed conservative treatment. Three patientshad a history of trauma. According to Smillies’s classificationtwelve patients were type IV and eight were Type V.Meanfollow-up period was 14.85 months(range 6 months to 24months).Results: Patients were assessed by the lessermetatarsophalangeal interphalangeal (LMPI) scale bykitaokaetal. At the final follow-up, scoring had changed from(44-66) with an average 54.65 to (76-100) with an average85.3. There was no case of infection, avascular necrosis,arthritis or pseudoarthrosis.Conclusion: Treatment of freiberg's disease with dorsalclosing-wedge osteotomy shows satisfactory pain relief andimprovement in quality of life

ABSTRACT

Currently, compared to jaw-closing (JC) α-motoneurons, the information on the distribution and morphology of glutamatergic synapses on the jaw-closing (JC) γ-motoneurons, which may help elucidate the mechanism of isometric contraction of the JC muscle, is very limited. This study investigated the distribution and ultrastructural features of vesicular glutamate transporter 1 (VGLUT1)- and VGLUT2-immunopositive (+) axon terminals (boutons) on JC γ-motoneurons by retrograde tracing with horseradish peroxidase, electron microscopic immunocytochemistry, and quantitative analysis. About 35% of the boutons on identified JC γ-motoneurons were VGLUT+, and of those, 99% were VGLUT2+. The fraction of VGLUT1+ boutons of all boutons and the percentage of membrane of JC γ-motoneurons covered by these boutons were significantly lower than those for the JC α-motoneurons, revealed in our previous work. The bouton volume, mitochondrial volume, and active zone area of the VGLUT2+ boutons on the JC γ-motoneurons were uniformly small. These findings suggest that the JC γ-motoneurons, in contrast to the JC α-motoneurons, receive generally weak glutamatergic synaptic input almost exclusively from VGLUT2+ premotoneurons that form direct synapse with motoneurons.

ABSTRACT

STUDY DESIGN: Retrospective case series. PURPOSE: Describe the technique and evaluate the outcome of apical vertebral column resection (VCR) with sagittal rotation and anterior opening and posterior closing (AOPC) maneuver for correction of severe post-tubercular kyphosis (PTK). OVERVIEW OF LITERATURE: The surgical procedures described for the correction of PTK are VCR, pedicle subtraction osteotomy, transpedicular decancellation osteotomy, and closing-opening osteotomy. METHODS: We retrospectively evaluated 21 patients who had been operated on with single stage apical VCR with AOPC maneuver. Radiographs were obtained before surgery and at regular follow-up intervals. These were used to calculate the angle of kyphosis. Back pain was rated using the Visual Analog Scale (VAS) and neurological status was graded using Frankel grading. Radiological outcome was assessed by the improvement in the angle of kyphosis and fusion following surgery. Neurological status was assessed using Frankel grading. RESULTS: The study included eight males and 13 females with a mean age of 21.9 and average follow-up time of 30.4 months. The average number of vertebral bodies destroyed was 2.57. Kyphosis was improved from a mean of 68.42°±13.23° preoperative to 8.71°±4.58° postoperative. The average correction achieved was 87.10%. Preoperative VAS score improved from a mean of 6.38±0.92 preoperative to 1.38±0.49 postoperative. No patients had any sign of neurological deterioration. Seven out of eight patients with a preoperative neurological deficit improved following surgery. Two patients developed superficial wound maceration, one had persistent postoperative hypotension, and the other developed hemothorax. All patients recovered fully without a need for additional surgery. CONCLUSIONS: Single stage simultaneous anterior column lengthening and posterior column shortening is an effective method for surgical correction of severe PTK.

ABSTRACT

Abnormal orofacial functions such as lip incompetency in the period of growth and development can cause morphological anomalies of the craniofacial complex. Therefore, it is crucial to make an early diagnosis based on the evaluation of the myofunctional conditions, and to make appropriate treatment plans.The objectives of this study were to quantitatively evaluate the standard lip closing force (LCF) of each age in the elementary school children, and to evaluate the relationships between LCF and affecting factors.The sample consisted of 765 children who were 7 – 12 years old in Jeonju city. Clinical examination about occlusal conditions and lip competency, and LCF measurement were performed by a single examiner. LCF was measured three times for each children with the LCF measuring device.The LCF was correlated positively with age in both sexes. The distribution of LCF groups was correlated significantly with Angle's classes and lip competency (p = 0.016, 0.004). The proportion of children with high LCFs was greater in the “competent lip” group, whereas the proportion of those with low LCFs was greater in the “incompetent lip” group.

ABSTRACT

Short-term closing live poultry markets have been frequently adopted to control H7N9 epidemic.To evaluate the effect of short-term closing live poultry markets,we analyzed the data of environment surveillance of H7 by RT-PCR,and the human H7N9 cases in Zhongshan City from Feb 2014 to May 2017.RRx =The positive rate during the x-th week after closing / the positive rate of the week before closing days * 100%.Three rounds of short-term closing live poultry markets were administered.Round one:from Feb 10 to 23,2014,the neighboring cities didn't synchronize.The H7 positive rate increased since the trade recovered,and RR1 =0.40 (95%CI:0.28-0.59),RR3 =0.63 (95%CI:0.32-1.24),RR4 =0.83 (95%CI:0.48 -1.46).Two human cases were reported in May,2014.Round two:from Feb 19 to 28,2015,the neighboring cities synchronized as the province policy.The H7 positive rate maintained a level lower than 10% since the trade recovered,and RR1 =0.15 (95%CI:0.07-0.34),RR2=0.21 (95%CI:0.10-0.41),RR3 =0.03 (95%CI:0.00-0.18),RR4 =0.10 (95%CI:0.04-0.27).No more human cases reported in that epidemic season.Round three:from Jan 8 to 21,2017,the neighboring cities didn't synchronize with Zhongshan City.The H7 positive rate had increased since the trade recovered,and RR1 =0.25 (95 % CI:0.09-0.68),RR3 =0.37 (95%CI:0.14-1.00),RR4 =1.07 (95%CI:0.54-2.11).Two human cases were reported in Feb,2017.Results indicated that,if the policy of closing live poultry markets was administered in single city,the environment pollution rate would rise shortly and the risk of human infection would increase once the trade recovered.However,if it was synchronously administered in all the cities in one region,the environment pollution rate could maintain at a low level and the risk of human infection would reduce enormously.

ABSTRACT

"The opening-closing-pivoting theory" recorded in () visualizes the activity of six meridians and indicates the relationship among the six meridians in physiology and pathology. Through the analysis on the classic medical works and modern literature of TCM by the modern medical masters, a specific relationship is discovered among the hand and foot meridians of the same name and among the organs. The is corresponded and interacted among the hand and foot meridians of the same name. Except the exterior and interior relationship among the organs, the related communication is also existing. Both meridians and organs are closely related to "the opening-closing- pivoting" theory. This discovery is the inheritance and development of "the opening-closing-pivoting theory" and plays the important role in the guidance of the acupoint selection of clinical acupuncture analgesia.

ABSTRACT

In , had used Qimen (LR 14) for the most times compared with other acupoints among the limited numbers of acupuncture prescriptions. Qimen (LR 14) is the front- point of the liver meridian, acting on smoothing liver and regulating activity. It is commonly used to treat the disorders caused by retarded circulation of and blood, such as distension and pain of the hypochondriac region and masses. Based on the opening-closing-pivot theory of the six meridians, the specificity of the running course of the meridian and the property of Qimen (LR 14), the mechanisms of Qimen (LR 14) were analyzed on the treatment of febrile diseases, aiming to enhance the considerations and application of this points in clinical practice and contribute to the further understanding of ' s academic thoughts on the syndrome differentiation in the treatment of diseases.

ABSTRACT

Extremely low birth weight infants remain at increased risk of intraventricular hemorrhage from the fragile vascular bed of the germinal matrix; the roles of hypotension (ischemia) and reperfusion (hyperemia) in the development of intraventricular hemorrhage are still debated. Cerebrovascular pressure autoregulation protects the brain by maintaining constant cerebral blood flow despite changes in blood pressure. The ontogeny of cerebrovascular pressure autoregulation has not been well established and uncertainty remains about the optimal arterial blood pressure required to support brain perfusion. Another important aspect of premature cerebral hemodynamics is the critical closing pressure––the arterial blood pressure at which cerebral blood flow ceases. Interestingly, in premature infants, the critical closing pressure approximates the mean arterial blood pressure. Often in this unique population, cerebral blood flow occurs only during systole when the diastolic arterial blood pressure is equal to the critical closing pressure. Moreover, the diastolic closing margin, a metric of cerebral perfusion that normalizes diastolic arterial blood pressure to the critical closing pressure, may be a better measure than arterial blood pressure for defining cerebral perfusion in premature infants. Elevated diastolic closing margin has been associated with intraventricular hemorrhage. This review summarizes the current state of understanding of cerebral hemodynamics in premature infants.

By developing a novel endoscopic succession closing device to overcome the shortcomings of existing devices that cannot deploy several clips at a time, to perform structural analysis on different clamp structures and to validate their performances in tissue closure through finite element analysis. Methods Comparative analyses of three clamp structures, namely, the aligning tooth structure (original, clamp A), the staggered tooth structure (clamp B), a combination structure with page break angle and staggered tooth (clamp C), were performed to analyze pressure and its distribution on tissues when clamping the stomach wall. Displacement of 7.5 mm was then applied on the clamps to simulate the effect of the operating procedures of the device and tissue kick-back. Results The maximum stresses of the clamp A and B were located on the first pair of teeth which was closest to the rotating shaft, with the stress being 10.39 kPa and 10.11 kPa, respectively. The maximum stress (11.35 kPa) of the clamp C was located on the second pair of teeth. For clamp A and B, the longer the distance to shaft, the larger pressure on stomach tissues. While for clamp C, the pressure on device-tissue interface showed little change along the path. Under tensile displacement, clamp A and B slipped off from the tissue when displacements reached to 5 mm and 6.5 mm, respectively, while clamp C did not. Conclusions Clamp with page break angle and staggered tooth can exert the uniform max pressure to tissues and provide a larger contact area away from the rotating shaft, thus improving anti-slippage and performance of the novel endoscopic closing device.

ABSTRACT

In order to study the effect elements of anthers opening-closing movement of Paris polyphylla var. yunnanensis, and its cell morphology regulatory mechanism. Anthers daily opening in the morning and closing in the evening and its corresponding ecological elements changes were recorded. Different light, temperature, humidity experiment and artificial rainfall experiment were designed to observe the effect on anthers opening-closing movement, paraffin sections were made to observe the cell morphology change when the anthers daily opening and closing. The result showed that the movement of anthers daily opening and closing was regulated by ecological elements. The overall trend was high temperature and strong light, low humidity was favorable for anther opening, and low temperature, weak light, high humidity was favorable for anther closing. In this experiment, the effect of these ecological elements on the movement of anthers opening from strong to weak was humidity, temperature, light. The effect of these ecological elements on the movement of anthers closing from strong to weak was light, humidity, temperature. The direct contact of the raindrops causes the rapid closing of the anthers in the rain. Observing the cell morphology change when anthers opening and closing, it was a pollen sac dehydration and water-absorption process, different light, temperature and humidity conditions induce different physiological activities in the cell, which caused the cell osmotic pressure change, eventually resulting in anther opening and closing movement. But anthers closing caused by the rain was a simple physical adjustment process, the raindrops fell on the anthers directly, which caused anthers soaked water and change of the cell osmotic pressure, then resulting in anther closing.

Resumen La erosión tardía de la pared auricular y de la aorta es una causa infrecuente, pero potencialmente mortal en los pacientes que han sido sometidos al cierre percutáneo de un defecto del septum auricular, en especial cuando existe una alta relación entre el tamaño del dispositivo escogido con el tamaño del defecto y la presencia de bordes insuficientes. Presentamos el caso de un hombre de 70 años con un episodio de colapso hemodinámico secundario a taponamiento cardiaco debido a erosión muy tardía de la aurícula derecha por un Amplatzer® implantado 4 años atrás para el cierre de un defecto septal auricular. Conclusión La erosión tardía de los dispositivos de cierre percutáneo para los defectos atriales, aunque infrecuente, deberá ser tenida en cuenta en los pacientes que presentan colapso hemodinámico.


Abstract Late atrial wall and aorta erosion is a rare cause of potential fatallity in patients who have undergone percutaneous closure of an atrial septal defect, especially when a high ratio device to defect is chosen, and an insufficient aortic rim is present. We report the case of a 70 year old male with an episode of hemodinamic collapse secondary to cardiac tamponade due to a late erosion of a 4 years ago implanted Amplatzer® used to close an atrial septal defect. Conclusion Very late wall erosion of an atrial septal closure device, although uncommon, should be suspected in patients presenting with hemodynamic collapse.

ABSTRACT

Background: The range of movements of mandible in Indian individuals was not studied to set a norm. The changes happening before and after prosthodontic treatment were also not studied. Aim: To determine the range of mandibular movements viz, protrusion, working side movement measured at mandibular first molar region both on the right and left sides while performing lateral excursions, non working movement measured at mandibular first molar region both on the right and left sides while performing lateral excursions, right lateral movement measured in the incisor region, left lateral movement measured in the incisor region, passive opening and active opening in normal individuals and prosthodontically treated individuals. To determine morphometric values such as intercondylar distance, distance between intercondylar axis and axis connecting mesioincisal angles of the mandibular central incisors, distance between intercondylar axis and axis connecting mesiobuccal cusps of the mandibular molars and inter mandibular molar distance related to mandible and mandibular arch of the individuals who served as sample in the above study. Material and Methods: The study was conducted on normal individuals and then on prothodontically treated individuals .The sample consisted of 160 normal individuals and 80 treated individuals. The normal individuals were divided into four according to age viz, Group I: 21-30yrs, Group II: 31-40yrs, Group III: 41-50 yrs, Group IV: 51-60 yrs. Similarly the treated individuals were divided into four according to type of treatment viz, Group A- Complete dentures, Group B-Removable partial dentures, Group C-Fixed partial dentures, Group D-single crowns. The morphometric and mandibular movement parameters were first measured in normal individuals and then in prosthodontically treated individuals with digital Vernier caliper directly and using putty index. Result and Conclusion: Conclusions drawn from the study are as follows A norm could be formulated both for morphometrics related to mandible and mandibular arch and for mandibular movements. The values obtained are relevant to individuals belonging to Bangalore and nearby areas. Opening and lateral movements can be measured directly in the incisal region using a pair of Vernier calipers. In order to measure the excursive movements in the posterior region and the protrusion in the anterior region, putty indices can be made which is a simple procedure that can be performed in a dental clinic. Prosthodontic treatment in general has a restrictive effect on mandibular movements. This observation has a limitation that the sample size is only 80. Mandibular movements can be reproduced in a computer using the graphic capabilities of coral draw. More precise data could be obtained if the inclusion of the treated individuals were restricted for the age.

ABSTRACT

La política pública chilena ha optado por materializar el cierre de escuelas rurales argumentando su baja calidad educativa. Esto ha traído una serie de consecuencias para la comunidad en la que se inserta la escuela. Una forma de revertir esta situación, es mejorando la calidad educativa, para lo cual se identificaron y caracterizaron perfiles de escuelas rurales de alto rendimiento académico según sus resultados en la prueba SIMCE 2011. Se utilizó un análisis multivariado, que incluyó análisis por componentes principales y el método de clasification and regression trees. Los resultados evidenciaron dos perfiles de escuelas rurales exitosas, cuyas principales características se relacionan con la disciplina y la percepción de autoeficacia del estudiante. Estos factores se presentan como importantes elementos a potenciar en los planes de mejoramiento educativo, como alternativa al cierre de escuelas.


The Chilean public policy has chosen to materialize the closure of rural schools arguing the low quality of its education. This decision has several consequences for the community in which the school is inserted. This situation can be address by improving the quality of education itself. For this purpose, in this study the profiles of rural schools with high academic performance were identified and characterized based on the results of the SIMCE 2011. A multivariate analysis was used including principal component analysis and the method of classification and regression trees. The results showed two profiles of successful rural schools, the main characteristics of them are related to the discipline and self-efficacy of the students. These factors are presented as milestones for the improvement programs for the education (promoted by the Ministry of Education throughout the country), as an alternative to the closure of the rural schools.

ABSTRACT

Objective To investigate the hemodynamic parameters to stabilize cerebral blood flow within limits of cerebral autoregualtion. Methods We assessed the flow velocity of middle cerebral artery using transcranial Doppler and recorded invasively the blood pressure simultaneously. We then analyzed the curves of cerebral blood flow autoregulation (CBFA) and calculated upper limit of autoregulation (ULA) and lower limit of autoregulation (LLA). The values of critical closing pressue (CCP) and resistance area product (RAP) were calculated according to previous theory. The relationship between CCP, RAP and MABP were analyzed. Results In the process of increasing or decreasing blood pressure, ULA and LLA of normal rats were 148.12 ± 7.49 mmHg or 62.96 ± 3.34 mmHg, respectively. When mean artery blood pressue (MABP) changed within limits of cerebral autoregulation, the cerebral blood flow velocity changed little (increasing:0.65± 0.27 cm/s/10mmHg MABP, decreasing:0.43±0.23cm/s/10 mmHg MABP), while CCP and RAP changed significantly (in?creasing: 4.60 ± 1.06 mmHg/10mmHg MABP and 0.11 ± 0.04/10 mmHg MABP, decreasing: 6.74 ± 0.59 mmHg/10 mmHg MABP and 0.09 ± 0.02/10mmHg MABP). After fixing change of blood flow velocity, CCP and RAP were correlated with MABP more remarkablely, although all blood flow velocity, CCP and RAP were significantly correlated with MABP. Conclusion Within limits of cerebral autoregulation, stable cerebral blood flow is mainly achieved by the change of CCP and RAP against blood pressure changing on normal rats, especially the increasing or de?creasing of CCP.
