ABSTRACT

Objective To observe the change situation of body metabolism and gastrointestinal function of patients with laparoscopic radical resection of colorectal cancer during the perioperative period. Methods 56 patients with colorectal cancer who were treated with radical resection in our hospital from May 2015 to July 2016 were selected as the research subjects.56 surgical patients were divided into control group (conventional open surgery group,28 cases) and observation group (laparoscopic surgery group,28 cases) by the random number table. The indexes of protein metabolism, lipid metabolism indexes and gastrointestinal function of surgical patients in two groups during the perioperative period were detected and compared. Results The protein metabolism indexes, lipid metabolism indexes and gastrointestinal function indexes of surgical patients in two groups before the surgery had no statistical significance (all P > 0.05),while the protein metabolism indexes, lipid metabolism indexes and gastrointestinal function indexes of surgical patients in two groups after the surgery all had significant differences, and they were all better than those of control group (all P < 0.05).Conclusions The body metabolism and gastrointestinal function indexes fluctuations of patients with laparoscopic radical resection of colorectal cancer during the perioperative period are smaller than those of open surgery,so the laparoscopic radical resection is more suitable to the patients with colorectal cancer.

ABSTRACT

Objective To explore the safety and clinical effect of combined laparoscopic-colonoscopy resection of colorectal tumor. Methods A total of 26 patients with early colorectal tumor was treated by combined laparoscopic-colonoscopy resection. To observe the postoperative complications, the mean operative time, mean intraoperative blood loss, mean time of gastrointestinal function recovery, and mean postoperative hospital stay were analyzed. Results All the 26 cases were operated successfully. The mean operative time was 60 ~ 162 (93.7 ± 22.5)min.The mean intraoperative blood loss was 15 ~ 120 (35.9 ± 24.2) ml.The mean time of gastrointestinal function recovery was 48 ~ 120(73.2 ± 14.5)h.The mean postoperative hospital stay was 5 ~ 13 (7.4 ± 1.8) d.No postoperative complications occurred, such as stomach leak, enterobrosis and intestinal obstruction.Follow up 6 ~ 12 months, there were no tumor residue and recurrence. Conclusions The combined laparoscopic-colonoscopy resection was located exactly, reasonable excision scope, minimally invasive, quick recovery and other advantages. It was worth of clinical application.

ABSTRACT

Objective To discuss the clinical effect of restrict rehydration strategy on anastomotic healing of patients with colorectal cancer. Methods 483 cases of colorectal cancer ( from January 2008 to November 2009) were analyzed retrospectively,166 in fluid restriction group and 317 in tradition therapy group.Postoperative early rehabilitations and complications were studied and compared. Results The first time of passing flatus(3.9 d vs 4.4 d),first ambulation(2.9 d vs 3.3 d),oral intake(2.9 d vs 3.6 d),time with use of urinary catheter(4.6 d vs 5.5 d),and drains(2.2 d vs 3.1 d),and postoperative hospital stay ( 8.7 d vs 11.6 d) in fluid restriction group were significantly earlier or less than those in tradition therapy group ( P < 0.01 ),while there were no significant differences in time with use of nasogastric tubes ( 1.1 d vs 1.2 d) between the 2 groups ( P > 0.05 ).There was significant difference in the postoperative complications rate between the two groups ( P < 0.05 ).Conclusions Restrictive fluid regimen could reduce the incidence of common complications for patients after colorectal surgery, and might have a certain promoter action to the anastomotic healing.
