ABSTRACT

Implant prosthodontics is beneficial for edentulous patients in enhancing the support, retention, stability, phonation and so on. Various types of prosthesis supported by implant, including implant retained- or supported-overdenture for the removable type and ceramo-metal and fixed prostheses with processed acrylic teeth for the fixed type, are frequently used. Treatment planning for the prosthesis with implant must be made after considering individual characteristics such as form of residual ridge, soft tissue, interocclusal relationship, economic status. Fixed prosthesis with processed acrylic teeth (also known as 'implant hybrid prosthesis' or 'bone anchored bridge') has the advantages of both removable and fixed prosthesis such as proper soft tissue profile, esthetic outcome, increased masticatory efficiency and psychological stability. The 73-years-old female patient came to the department of prosthodontics, Dental hospital of Yonsei University. She was diagnosed with Kennedy class I partial edentulism in the maxilla and complete edentulism in the mandible. This article reports a satisfactory clinical and esthetic outcome of full mouth rehabilitation using removable partial denture in the maxilla and implant hybrid prosthesis in the mandible.

ABSTRACT

Oral sub mucous fibrosis is an insidious chronic disease and a precancerous condition, affecting any part of the oral cavity and sometimes the pharynx. Management of patient with oral sub mucous fibrosis presents difficulties at all stages, from the preliminary impressions to fabrication of prostheses. This article will present simplified approach for management of a 61year old male edentulous patient with oral sub mucous fibrosis. So that final outcome may be achieved.

ABSTRACT

PURPOSE: In cases when implant supported overdenture is made by using standard size implant, additional procedure such as bone surgery and bone grafting can be required. And it gives burden to doctor and patient in terms of cost. Therefore, it is necessary to find the implant therapy for the edentulous patients in making denture with accordable cost and simple procedure. MATERIALS AND METHODS: Edentulous patients with upper and lower dentures participated in this study. Before the operation, survey about patient's satisfaction to the existing dentures was carried out. Surgical procedures included four small diameter implants installation anterior area and immediate loading. One and three month after the procedure, the same survey about patient's satisfaction was carried out, and radiography was taken. RESULTS: We are doing research to the nine patients. Survival rate is 97.2 percent. The comparison of patient's satisfaction before and after surgery is performed based on oral health impact profile 49. We analyze mainly with masticatory discomfort, retention, aesthetics, social problem, psychological discomfort problems. As a result, satisfaction level is increased at all factors. Retention is the most increased satisfactory factor followed by mastication difficulty, pronunciation, psychological discomfort, social discomfort, aesthetics in order. Marginal bone loss is 0.21 mm at 12 weeks after implant placement. CONCLUSION: This research reveals that the denture supported by mini dental implant increases patient's satisfaction. This study will be continued with more patients for a long time and we are scheduled for taking additional radiography to check whether peri-implant bone resorption occurs or not.

ABSTRACT

Objetivo: Analisar, comparativamente, a prevalência de desordem temporomandibular, em relação aos fatores etiológicos, tais como gênero, faixa etária e condições clínicas apresentadas pelas próteses totais, de acordo com os índices anamnésico (Ai) e clínico (Di) de disfunção, desenvolvidos por Helkimo, em pacientes portadores de próteses totais. Métodos: A amostra foi composta por noventa pacientes institucionalizados, portadores de próteses totais bimaxilares, selecionados aleatoriamente, com idade média de 67,2 anos. Os dados coletados foram tabulados e os testes estatísticos de Kruskal-Wallis e Qui-quadrado aplicados, adotando-se o nível de significância de 5% (p<0,05). Resultados: Foi observada diferença estatisticamente significante da prevalência de desordem temporomandibular em relação às idades das próteses totais atuais, espaços funcionais livres, desgastes das superfícies oclusais dos dentes artificiais e condições de retenção e estabilidade das próteses totais superiores e inferiores, de acordo com ambos os índices. Conclusão: Os pacientes que usavam próteses totais em condições clínicas adequadas apresentaram menos sinais e sintomas de desordem temporomandibular do que os pacientes que usavam próteses totais em condições clínicas insatisfatórias.


Objective: The aim of this study was to make a comparative evaluation of the prevalence of temporomandibular disorder in complete denture wearers, with regard to the etiological factors, such as gender, age, and complete dentures clinical conditions, according to the anamnestic (Ai) and clinical (Di) dysfunction index, developed by Helkimo. Methods: The randomized sample was composed of 90 institutionalized patients and bimaxillary complete denture wearers, with a mean age of 67.2 years, who were included in this study. The collected data were tabulated and the Kruskal-Wallis and Chi-square statistical tests were applied, at the level of significance of 5% (p<0.05). Results: Statistically significant difference in the prevalence of temporomandibular disorder was observed with regard to the ages of the current complete dentures, free-way space, wear of the occlusal surfaces of the artificial teeth, and the conditions of retention and stability of the maxillary and mandibular complete dentures, according to both the indexes. Conclusion: The patients who wore complete dentures in adequate clinical conditions presented fewer signs and symptoms of temporomandibular disorder than the patients who wore complete dentures in poor clinical conditions.

ABSTRACT

STATEMENT OF PROBLEM: Conventional mandibular complete denture lacks in support and stability for edentulous patients to use. PURPOSE: To enhance support and stability of mandibular complete denture, 2 implants were inserted on the inter-mental foramina space of edentulous patients. With 2 magnetic attachments on the inner side of the overdenture, we gave immediate loads and evaluated the stability of the implants, the responses of the supporting tissues, and the satisfaction of the patients. MATERIAL & METHODS: 6 edentulous patients (5 male, 1 female) each recieved 2 SLA implants in the inter-mental foramina region and implants were immediately loaded with overdenture with magnet attachments. To evaluate the implants stability and the peri-implant tissues, clinical exams, RFA tests and radiographic exams were preformed at the 1 week, 2wks, 6wks, 12wks, and 24wks. post-surgery. RESULTS: The mean surgery time was 45.7 +/- 7.7 mins., while the denture delivery time was 45.5 +/- 12.6 mins. Only 2 of 14 implants were failed. Survived implants that remained were clincally and radiographically stable. Mean ISQ values were relatively stable, showing 69.71+/-5.55, 69.00+/-9.48, 67.92+/-7.86, 67.92+/-9.58, 70.08+/-7.61, 71.92+/-6.43 at the 1 wk, 2 wks, 6 wks, 12 wks, 24 wks. follow up check. Crestal bone changes were -1.18mm+/-0.68mm, -1.35+/-0.69mm, -1.47+/-0.68 mm at the 6wks, 12wks, 24wks. follow up check. Bleeding on probing(BOP index) was not significant. CONCLUSION: Mandibular ovedentures with 2 magnetic attachments over two interforaminal implants on edentulous patients for immediate function is a recommendable novel treatment for edentulous patients which shows stability on the implants and supporting tissue.
