ABSTRACT

Objective To investigate the clinical efficacy and safety of visual standard channel combined with visual ultrafine channel PCNL precision puncture in treatment of complex renal calculi. Methods From June 2015 to October 2016, 48 cases of complicated renal calculi were treated with multi-channel lithotripsy with visual standard channel ultrasonic pneumatic lithotripsy combined with visual superfine channel PCNL precision puncture holmium laser lithotripsy. Including 10 cases of staghorn stone, 38 cases of multiple renal stones. Results 110 channels were established in 48 patients. 4 cases of preoperative renal insufficiency with infection in the puncture found in the pus and stones load larger, intraoperative diarrhea and PCNL simple treatment of obstruction site stones; 44 cases to complete one of the surgery: There were single channel established in every one of 5 cases, and double channels established in every one of 24 cases, three channels in established in every one of 15 cases; There were two cases of surgery in 8 cases and there were 12 new channels established. The average time of unilateral first operation was 75 (35 ~ 125) min. The first clearance rate was 79.2% (38/48), and the total clearance rate of postoperative stone was 87.5% (42/48). 6 cases of residual stone combined with ESWL and drug row of stone, followed up for 3 months, 6 cases of stone row net, the total stone clearance rate of 100.0% (48/48). Two consecutive postoperative no sepsis, bleeding, ureteral injury and other serious complications. Conclusions Visual standard channel combined with visual superfine channel PCNL precise puncture for the treatment of complex renal calculi is safe and effective, with high fruiting rate and low complication, which can be popularized in clinical practice.

ABSTRACT

Objective To study the effect and prognosis of multiple channels percutaneous renal ureteral calculi removal in the treatment of patients with complex renal calculi.Methods One hundred and forty-four cases patients with complex renal calculi accepted treatment in Dongda Hospital of Shanxian County from June 2012 to March 2015 for research.According to the random number table method,the patients were divided into the observation group and the control group,72 cases in each group.The single channel type was used in the control group,while multi-channel type operation was use in the observation group.Indicators related to the operation,stones clearance rate,changes in the degree of hydronephrosis,and the major complications after treatment of the two groups were compared.Results The operation time of the observation group was (104.82 ±8.54) min,significantly longer than that of the control group ((90.23 ±7.68) min),but the duration of hospital stay,fistulation tube indwelling time and the residual stone surface area were (6.84±1.25) d,(5.23±2.16) d and (223.16± 113.49) mm2 respectively,significantly less than that of the control group ((8.33±2.13) d,(6.74 ± 2.19) d and (429.54 ± 107.88) mm2 respectively),the differences were statistically significant(P =0.000).The stone removal rate of the observation group was 86.11% (62/72),significantly higher than that of the control group(72.22% (52/72)),the difference was statistically significant (P =0.040).There was no significant difference in terms of the total stone clearance rate and the degree of kidney water after treatment between the two groups (P>0.05).The total complication rate of the observation group was 11.11%(8/72),of the control group was 9.72% (7/72),and the difference was not statistically significant(P >0.05).Conclusion The efficacy of multi-channel operation for complex renal calculi is better,safe and feasible,but should reduce the number of channels as much as possible,so as to reduce the damage to patients effectively.

ABSTRACT

Objective To evaluate the clnical value of percutaneous nephrolithotomy combined with flexible cystoscope in the treatment of complicated renal calculi.Methods Data of 53 cases of complex renal calculi treated with single channel percutaneous nephrolithotomy combined with flexible cystoscope from July 2011 to September 2013 were analyzed retrospectively.There were 31 males and 22 females.Their age ranged from 26 to 74 years with a mean age of 49 years.Thirty-two cases were located on the left side,19 cases right,2 cases bilateral,21 cases of staghorn calculi and 32 cases of multiple stones.All the stones were filled in 2 or more calyces,with 7 cases in upper ureteral stones at the same time and 2 cases in solitary kidney.The longest diameter of stones was between 2.5 cm to 4.6 cm,with an average of 3.7 cm.There were 39 cases with hydronephrosis and the separation of the collection system was 2.0-5.1 cm with an average of 3.8cm.The period which patients suffered from stones was from 1 month to 6 years.Fourteen cases underwent ESWL and 6 cases underwent open operation before percutaneous nephrolithotomy.All patients underwent single channel percutaneous nephrolithotomy under the guidance of color Doppler ultrasound,and the channel of puncture was expanded to 24 F.Results Fifty-three cases (55 sides) were performed successfully in one-stage sugery and the operation time was between 85 to 170 min,with an average of 119 min.Thestone clearance rate was 89% (49/55) and postoperative hemoglobin decreased 3.9-9.2 g/L with an average of 6.1 g/L.The hospitalization time was 8 to 12 days with an average of 9.2 days.Two cases had hemorrhage of about 500 ml after surgery and the renal hemorrhage stopped after clamping of renal fistula without blood transfusion.Two patients suffered from fever on surgery day and 1 patient on one day after surgery,and the body temperature returned to normal level after anti-infection treatment for 2 or 3 days.No renal perforation,fluid pneumothorax or damaged surrounding organs occurred.6 patients had residual stones,among which 2 were treated by two-stage operation and 4 were treated with extracorporeal shock wave lithotripsy.Conclusion Percutaneous nephrolithotomy combined with flexible cystoscope has little trauma and high stone clearance rate in the treatment of complex renal calculi with high clinical value.
