ABSTRACT

Immediate dentin sealing is an essential procedure in dentistry, involving the application of sealing agents at the interface between dentin and the restorative material immediately after caries removal and cavity preparation. This process aims to seal exposed dentinal tubules, providing protection to the dental pulp. The present case involved a 56-year-old male patient who complained of discomfort in tooth number 17. After clinical and radiographic evaluation, a wide, maladapted restoration in the mesio-occlusal portion of the tooth was identified, with no endodontic treatment in the affected element. Following planning and informed consent, the following steps were taken: removal of the carious lesion from tooth number 17, followed by the execution of mesial marginal elevation and immediate dentin sealing. Silicone addition molding of the affected tooth and antagonist, along with bite registration, was performed. A semi-direct restoration in composite resin was fabricated based on the obtained gypsum model. The restoration was cemented in the subsequent appointment, meeting the essential requirements to restore form, function, and aesthetics, resulting in an improvement in the patient's quality of life(AU)

ABSTRACT

Direct composite resin restoration are widely used in dentistry to restore posterior teeth. However, when there is large coronary destruction, that the distance from the isthmus exceeds two- thirds of the intercuspal distance, indirect composite resin restorations are indicated. This study aimed to report the analysis of a dental record of a patient who received an indirect restoration in composite resin in a badly destroyed posterior tooth. Through the analysis of dental records of patients seen in the disciplines of Supervised Internship of the Dentistry Course at FSG Centro Universitário in the year 2023, the dental record of a patient who attended the FSG dental clinic with fractured amalgam restoration with composite resin repair was selected who had painful symptoms. The diagnostic procedure took place through clinical and radiographic examination, which revealed the indication of replacing the unsatisfactory restoration and carrying out an indirect restoration in composite resin. The aesthetic and functional results presented demonstrated the viability of the indirect composite resin restoration technique for rehabilitating posterior teeth with extensive coronal destruction(AU)

ABSTRACT

The choice of restorative technique plays a fundamental role in the effectiveness and duration of rehabilitation treatment. The objective of this case report was to describe the use of the semi-direct technique to create a composite resin restoration in a lower first molar. The patient had an unsatisfactory restoration on tooth 36, which needed to be replaced due to cavity infiltration. The semi-direct technique was chosen due to the amplitude of the cavity, which involved support structures, and the combination of advantages of the direct and indirect approaches. The procedure involved the removal of carious tissue, and the application of calcium hydroxide paste, followed by the application of a thin layer of glass ionomer and, subsequently, fluid resin to seal the dentin. The preparation was carried out following the necessary principles. The element in question was molded with addition silicone and the antagonist arch was molded with alginate. Both models were poured with silicone for semi-rigid models and mounted on toy parts occluders. The semi-direct restoration was made in Filtek Z350 XT composite resin, respecting the anatomy of tooth 36. Pigments were used to improve aesthetic details. After finishing and polishing, the piece was conditioned and cemented with Relyx Ultimate dual cement. It can be concluded that the restorative approach using the semi-direct technique built on a semi-rigid model is a conservative and advantageous therapeutic option for teeth with extensive coronal destruction. This technique allows for effective restoration, ensuring both the aesthetics and adequate function of the affected tooth(AU)

ABSTRACT

Aim: The aim of this study was to verify the color variation between different composite resins and the Vita Classical Shade Guide. Methods: Two-millimeter thickness samples were made (n = 6) from eight commercial brands of composite resin (shade A2): Charisma (Kulzer), Forma (Ultradent), Harmonize (Kerr), Luna (SDI), Opallis (FGM), Oppus Bulk Fill (FGM), Vittra (FGM) and Filtek Z250 XT (3M ESPE). Specimens were stored in distilled water for 7 days and then polished. Color measurements of samples and A2 shade of the Vita Classical Shade Guide were performed using the Vita Easy Shade Advance 4.0 spectrophotometer on a black background. Color variations were calculated using the CIEDE2000 formula, considering values ≥0.81 being noticeable by the human eye and ≥1.77 being clinically unacceptable. Results were statistically analyzed with a 5% significance level. Results: Color variation (ΔE) of composite (E1 ) compared to the Vita Classical Shade Guide (E0 ) was greater than clinically acceptable for all the materials evaluated in this study. Forma (ΔE=2.08 ± sd=0.47) and Filtek Z250 XT (2.50 ± 0.20) had the smallest amount of color variation values found in the results. Harmonize (3.32 ± 0.63) presented values similar to Filtek Z250 XT, but it was worse than Forma. Vittra (3.51 ± 0.28), Charisma (3.80 ± 0.20), Opallis (4.24 ± 0.30) and Luna (5.67 ± 0.20) did not differ among each other and presented higher color variation than Forma, Filtek Z350XT and Harmonize. Oppus Bulk Fill (13.94 ± 1.12) was the composite with the greatest color variation. Conclusions: The findings in this study show that attention should be taken when using the Vita Color Shade Guide for composite shade selection

ABSTRACT

ABSTRACT Advanced Polymerization System (APS) technology in a commercial composite resin enables reduction of the concentration of camphorquinone without altering composite physicochemical properties. Aim: The aim of this study was to evaluate the degree of conversion and mechanical properties of a commercial composite with an advanced polymerization system (APS) and compare it to other composites that do not use this system. Materials and Method: Five groups were analyzed. Group 1 (VT: Vittra APS - FGM); G2 (AU: Aura - SDI); G3 (ES: Quick Sigma Stelite - TOKOYAMA); G4 (FZ: Filtek Z350 XT - 3M ESPE); G5 (OP: Opallis -FGM). Degree of conversion (DC, n=3) was analyzed immediately and after 24h by analysis with FTIR spectroscopy. For Knoop hardness (KHN, n=3), 5 indentations were made at the top and bottom of specimens 2 mm thick. Flexural strength (FS, n=10) was determined by the three-point method in a universal testing machine. Polymerization stress (PS) was determined by light-curing the material (1.0 mm high) between polymethylmethacrylate rods in a universal testing machine. Light curing was performed with a Valo Cordless LED (1,000 mW/cm2 x 20 s: 20J). The results were analyzed using ANOVA and complemented by Tukey's test (α=0.05). Results: The highest DC values (immediate and 24h after) were observed for VT and OP resins, followed by FZ, AU and ES. FZ (top and bottom) had the highest KHN values, similar to VT top. AU, ES, OP and VT had statistically different KHN between their top and bottom surfaces. The highest RF values were observed for FZ, followed by OP/VT, ES and AU. The highest TP values were observed for FZ, OP and VT. Conclusion: The Vittra APS resin with a new polymerization system presents satisfactory performance for the parameters evaluated.

ABSTRACT

Traumatic dental injuries are a worldwide public health problem, among which dental fractures are the most frequent, mainly involving the anterior teeth. The approach of large anterior fractures is a challenge for dental surgeons of any level because, in addition to function, they directly affect the patient's esthetics, requiring a plan that involves different specialties in dentistry. Objective: Report a clinical case of functional and aesthetic rehabilitation of dental fractures in maxillary central incisors involving endodontic treatment, installation of a fiberglass post and direct restoration in composite resin. Case Report: The 17-year-old patient suffered a fracture in the upper central incisors caused by a fall from a bicycle. Due to the late search for treatment, element 21 was diagnosed with pulp necrosis and, therefore, underwent endodontic treatment using the Crown Down Technique and rehabilitation with a Splendor-SAP fiberglass pin. Later, elements 11 and 21 were restored with direct veneers in composite resin using the incremental technique. Conclusion: Rehabilitation with the installation of a fiberglass post associated with the incremental layering technique in composite resin is a good option for aesthetic rehabilitation as it allows the anatomical shape of the tooth with rich details in the layering of dentin and enamel, in addition to maximum preservation of the tooth structure. The results obtained reinforce the aesthetic and functional success with a significant impact on the patient's quality of life(AU)

ABSTRACT

Introduction: Proximal lesions that exceed the cement enamel limit (ACE) under the gingival margin complicate impressions and the adhesive technique. Compare the magnitude of micro infiltration between conventional resin and bulk fill resin in the cervical margin relocation technique. Materials and Methods: 48 samples of human teeth re-ceived two preparations: occluso-mesial (OM) and occluso-distal (OD) under LAC; first they received the cervical margin relocation technique (RMC) with bulk fill and conventional resin; then restored with semi-direct resinous inlays. Sample analysis: immersion in 50% colloidal silver nitrate solution, 24 hours, 37°C and cut mesiodistally. Observed under a stereoscopic magnifying glass to assess dye penetration and digitally photographed, analyzed with "Image J" software. Results: Sample of 96 cavities in two groups of 48 units; control group restored with conventional resin with incremental technique and study group restored with bulk fill resin, mono-incremental technique. Probabilistic sampling. No statistically significant diffe-rences in percentage of microinfiltrated area between Filtek™ Z250™ and Filtek™ Bulk Fill™ (p-value= 0.68). Discussion: Various studies show that the presence of marginal microinfiltration exist independent of: restorative technique, consistency, adhesive mechanism and polyme-rization technique. The research carried out is no exception, observing a similar degree for both systems. Conclusions: Results allow us to conclude that conventi-onal resin and bulk fill resin did not show significant differences in microleakage percentages for the RMC technique. Outside the study framework, bulk fill resins would have comparative advantages; better behavior against light in depths greater than 2 mm, less sensitivity to the "C" factor, and less clinical time.

ABSTRACT

Abstract Objectives This is a double-blind, split-mouth, randomized clinical study that aims to evaluate the influence of bulk-fill composite packaging presented in syringes (BSy) and capsules (BCa), and the effect of selective enamel etching (SEE) on the clinical performance of class I and II bulk-fill resin composite restorations after 24 months. Methodology A total of 295 class I or class II restorations were performed on 70 patients. One universal adhesive was applied in all restorations. SEE was used in 148 restorations and self-etching mode (SET) in 147 restorations. After the adhesive application, cavities were restored with Filtek Bulk-fill Posterior Restorative in syringes (BSy), Filtek One Bulk-fill in capsules (BCa), or Filtek Supreme Ultra in syringes with the incremental technique (In). All restorations were evaluated using the FDI criteria after one week and after six, 12, and 24 months. Kaplan-Meier survival analysis and Pearson's Chi-square test were used (α=0.05) for statistical analysis. Results After 24 months, 62 patients were evaluated and four restorations were lost due to fracture (one for SEEBSy, two for SEEIn, and one for SETIn). No significant differences in the fracture and retention rate were found between groups (p>0.05). SEE showed significantly fewer marginal adaptation defects than SET (p<0.05). BCa and BSy groups showed fewer marginal discrepancies compared to In (p<0.05). Restorations performed with BCa showed less color mismatch than BSy or In (p<0.05). Conclusion Although all restorations exhibited satisfactory clinical performance after 24 months of clinical service, the clinical behavior of class I and II restorations' improved when performed with a bulk-fill composite in capsules, mainly when associated with a universal adhesive applied with SEE.

ABSTRACT

Abstract Few long-term studies assess the discoloration induced by hydraulic calcium silicate-based cement on dental structures. In addition, as far as we know, no long-term study has assessed the discoloration induced by these cement on composite resin. Objective This in vitro study aimed to assess, during a period of two years, the discoloration potential of different hydraulic calcium silicate-based cements (hCSCs) on the enamel/dentin structure and composite resin restoration. Methodology A total of 40 enamel/dentin discs were obtained from bovine incisors, and 40 composite resin discs (10 mm in diameter × 2 mm thick) were fabricated. A 0.8 mm-deep cavity was made in the center of each disc and filled with the following hCSCs (n=10): Original MTA (Angelus); MTA Repair HP (Angelus); NeoMTA Plus (Avalon); and Biodentine (Septodont). An initial color measurement was performed (T0 - baseline). After 7, 15, 30, 45, 90, 300 days, and two years, new color measurements were performed to determine the color (ΔE00), lightness (ΔL'), chroma (ΔC'), hue differences (ΔH'), and whiteness index (WID). Results For enamel/dentin, the ΔE00 was significant among groups and periods (p<0.05). NeoMTA Plus had the greatest ΔE00. The NeoMTA Plus group had the greatest ΔE00 after two years for composite resin. Significant reduction in lightness was observed for all groups after two years (p<0.05). The most significant WID values were observed after 30 days for Biodentine (enamel/dentin) and MTA Repair HP groups (composite resin) (p<0.05). Conclusions The hCSCs changed the colorimetric behavior of both substrates, leading to greater darkening over time. The Bi2O3 in the Original MTA seems relevant in the short periods of color change assessment.

ABSTRACT

Objective@#To study the influence of procyanidins on the bonding strength of dentin bleached by carbamide peroxide to composite resin.@*Methods @#By applying different treatments to dentin bonding interfaces, 120 human third molars were randomly divided into 12 groups (n = 10): W group (no bleaching+deionized water), Wa group (no bleaching+deionized water+aging), WT1 group (no bleaching+5% procyanidins for 1 min), WT1a group (no bleaching+5% procyanidins for 1 min+aging), WT2 group (no bleaching+5% procyanidins for 5 min), WT2a group (no bleaching+5% procyanidins for 5 min+aging), C group (carbamide peroxide+deionized water), Ca group (carbamide peroxide+deionized water+aging), CT1 group (carbamide peroxide+5% procyanidins for 1 min), CT1a group (carbamide peroxide+5% procyanidins for 1 min+aging), CT2 group (carbamide peroxide+5% procyanidins for 5 min), and CT2a group (carbamide peroxide+5% procyanidins for 5 min+aging). The bond strength to composite resin was measured by universal mechanical testing machine, microstructure and the nanoleakages were measured by scanning electron microscope immediately or after the thermal cycling aging test.@*Results@#The immediate bond strength of the bleached groups pretreated with procyanidins for 1 min (P<0.001) and 5 min (P<0.001) was higher than that of Group C, and the difference was statistically significant. Meanwhile, there was no statistically significant difference between Group CT1 and Group CT2 (P = 1.000). After the thermal cycles, the bond strength of each group declined. The differences between Group W and Group Wa (P<0.001) and Group C and Group Ca (P<0.001) were statistically significant, but no significant differences between Group CT1 and Group CT1a (P = 0.052) or Group CT2 and Group CT2a (P = 0.053) were found. The main effects of “aging” (P<0.001), “bleaching” (P<0.001) and “procyanidins” (P<0.001) and the second-order interaction effects of “bleaching * procyanidins” (P = 0.008), “bleaching * aging” (P = 0.024), and “aging * procyanidins” (P<0.001) were statistically significant. SEM observations showed that the hybrid layers in Groups C, CT1 and CT2 were not clear, and the hybrid layers in Groups Ca, CT1a and CT2a were partially destroyed and disintegrated. Under backscattering mode, it was observed that there were a large number of silver nitrate particles in the hybrid layer of Group Ca, and the residual silver ions in the hybrid layer of Groups CT1a and CT2a were decreased. @*Conclusion@# Pretreatment with 5% procyanidins for 1 min can improve the immediate bond strength of dentin bleached by carbamide peroxide to composite resin and maintain bonding durability.

ABSTRACT

Abstract To evaluate stress distribution when applying vertical and tangential forces to 1mm thick occlusal veneers with different finish lines preparations, using the finite element method. One extracted third molar was prepared for occlusal veneers, firstly without any bevel. It was scanned in order to design two groups of lithium disilicate (G1A, G2A) and composite resin (G1B, G2B) occlusal veneers. Then, the third molar preparation was modified, beveling the finish line and it was subsequently scanned again to design the occlusal veneer groups with bevel (LD: G3A, G4A and CR: G3B, G4B). The four groups were subjected to different forces (400 N vertical and 900 N tangential). At 400 N, the non-beveled veneers showed slightly higher Von Mises stress values (G1A: 783 MPa and G1B 736.5 MPa) than the beveled veneers (G3A: 685.7 MPa and G3B: 675.8 MPa). However, when 900 N tangential forces were applied, the beveled occlusal veneers showed higher Von Mises stress values (G4A: 4297 MPa and G4B: 4133 MPa) than the non-beveled occlusal veneers (G2A: 2581.1 MPa and G2B: 3519.1 MPa). Furthermore, it was observed that the tissue under the occlusal veneers with bevel showed higher Von Mises stress values than the models without any bevel. Beveled and non-beveled occlusal veneers of lithium disilicate and composite resin presented similar stress distribution values when vertical forces of 400 N were applied; whereas with tangential forces of 900 N applied near to the finish line, the beveled groups presented notably higher stress values than the non-beveled groups. However, both finish line preparations presented adequate values for possible clinical performance.

ABSTRACT

Abstract This study investigated the effect of butylated hydroxytoluene (BHT) inhibitor on degree of conversion (DC), flexural strength (FS), flexural modulus (FM), Knoop microhardness (KH), microhardness reduction (HR), and consistency of experimental resin composites at different BHT concentrations: C0 (control-0%); C0.01 (0.01%); C0.025 (0.025%); C0.05 (0.05%); C0.1 (0.1%); and C0.5 (0.5%). For the consistency, the composites were tested immediately after being exposed to a dental chair headlight (0, 20, 40 and 60 s). Data concerning DC, FS, FM, KH, and HR were submitted to one-way ANOVA, while the consistency data was submitted to 2-way ANOVA; mean values were then compared (Tukey's test; α=0.05). The KH, FS and FM analyses showed no significant difference among the composites tested. For DC, C0 showed the highest mean value (74.2%) and differed only from C0.5 (67.2%). For HR, C0.5 showed the lowest mean (13.09%) value and differed from C0 (26.4%) and C0.01 (24.87). The consistency analysis showed no difference among C0.05, C0.1 and C0.5, considering 0 and 20 s of light exposure, while C0 (14.07 mm), C0.01 (13.97 mm), and C0.025 (14.18 mm) showed higher mean values at 0 s when compared to 20 s (12.67, 12.77 and 13.05 mm, respectivelly). Polymerization occurred within 40 s of light exposure for C0, C0.01, C0.025, and C0.05 and within 60 s for C0.1. In conclusion, the BHT concentrations had no significant influence on FS, FM and KH. The higher the BHT concentration, the longer was its handling time under light, with a significant improvement in the HR, but a decrease in DC. Therefore, BHT at 0.1% showed the best outcomes concerning all the BHT concentrations tested.

ABSTRACT

Introduction: The effect of oxygen on the surface of resins at the moment of photopolymerization creates a layer which is susceptible to pigments. The use of glycerin may block this effect. Objective: Determine whether there is a difference in the color stability of nanohybrid composite resin subjected to a coloring agent when glycerin is either applied or not applied. Methods: Sixty resin disks were distributed into Group 1 (without glycerin) and Group 2 (with glycerin). Photopolymerization was performed for 20 s with a Bluephase®N LED lamp. The first color measurement was taken with an Easyshase®V spectrophotometer, recording the values obtained for L* (luminosity), a* (red-green axis) and b* (blue-yellow axis). The second color measurement was taken after immersion of the samples for one month in Coca-Cola®, maintaining them in an incubator at 37ºC. Statistical data analysis was performed with Student's t test to determine the color stability changes occurring in the two groups and compare them. Results: Color stability changes were observed in all the resin samples. Mean distance between initial and final color was ΔE = 6.91 in Group 1 and ΔE = 3.74 in Group 2. A statistically significant difference was found between the two groups analyzed (p < 0.001). Conclusions: Glycerin is an effective substance to block the effect of oxygen on the resin surface, preventing the formation of an oxygen-inhibited layer and raising the color stability of the nanohybrid composite resin(AU)

ABSTRACT

Abstract The objective of this in vitro study is to examine the effect of antioxidants on the bonding strength of composite resin to bleached enamel. One hundred twenty non decay teeth were used in this study. Specimens were cut from the enamel cement junction. Then the specimens were randomly assigned into two groups-office bleaching and home bleaching. Bleaching antioxidants were applied to the first 12 subgroups. The specimens in the remaining 12 groups were kept in artificial saliva for 14 days and antioxidants were applied. Composite resin restoration was applied to all specimens. A cutting device was employed to obtain six specimens (0.9mm) from each tooth, and the microtensile bond strength (μTBS) test was performed. Data were analyzed using one-way ANOVA, the paired t-test, and Duncan's post hoc test (p=0.05). In office bleaching specimens, immediate antioxidant application increased the μTBS values (p0.05). Mean μTBS values in the groups to which antioxidants were applied two weeks after home bleaching were higher than those in the groups in which antioxidants were applied immediately. The use of antioxidants after bleaching may increase the MTBS value. The clinician should take this into account especially in immediate restoration applications after bleaching.

ABSTRACT

Context (Background): Resin composites are the most widely used material for restoring cervical defects. However, the high failure rate of these restorations is still a concern. Aims: The aim of this in vitro study was to evaluate, using microtomography (?CT), the interfacial gap and voids formation in Class V cavities in premolars restored with materials with lower polymerization shrinkage combined with different restorative techniques. Settings and Design: Cervical defects were created in 30 intact premolar and were randomly distributed to be restored by one of the following techniques (n = 6): Composite resin with two increments (CR), organic modified polymer (ORMOCER) with single (OR1) or two increments (OR2, or low viscosity bulk?fill composite resin with single (BF1) or two increments (BF2). Methods and Material: Each tooth was scanned before filling to determine the volume of interest (VOI) to be applied in the second ?CT after restoration and to control the cavity volume among the groups. In the ?CT after filling, the volume of interfacial gaps and voids was calculated for each group. Statistical Analysis: The groups were compared using one?way and Tukey HSD post hoc test (? = 0.05). Results: It was possible to identify higher gap formation in the OR1 group and higher void formation in CR group (P < 0.05). OR2 group showed better results than the group with one increment. BF2 showed the best filling capacity. Conclusions: It was possible to conclude that the material and the number of increments directly influenced the internal adaptation and voids formation of Class V restorations.
