ABSTRACT

Acupuncturists participating in consultation and treatment of multi-disciplinary has gradually become the general trend of acupuncture. This article analyzes the data obtained by acupuncturists who participated in the diagnosis and treatment of the emergency department for 9 months, investigates the cooperation mode with emergency doctors, communication skills with emergency patients, and indications for acupuncture and emergency treatment. Besides, the experiences are summarized, and how the acupuncturists deal with the "doctor-doctor relationship" and "doctor-patient relationship" when they "enter to other disciplines" is shared and the experiences and new ideas of acupuncture in integrating with other disciplines are provided, which will help to improve the service capacity of acupuncture.

ABSTRACT

OBJECTIVE: To analyze the current status of tumor-related occupational protection knowledge among medical staffs in comprehensive hospitals. METHODS: A stratified random sampling method was used to select 572 medical workers from 24 comprehensive hospitals as study subjects, including 289 nurses and 283 medical doctors. The Survey on Occupational Protection Knowledge and Management of Hospital Tumors was used to investigate the research subjects. RESULTS: i) The awareness status of the tumor-related occupational protection knowledge: the awareness rate of sharps injuries that should be included in occupational protection management was as high as 93.7%, while the awareness rate of standard operating procedures for occupational protection in working post was only 32.9%. ii) Evaluation on the management of tumor-related occupational protection in the hospital: most medical personnel think that medical occupational protection is very important to medical quality and safety, and they are not satisfied with personal protective equipment(PPE), various protective facilities and medical devices. The satisfaction scores of subjects in the nursing group on the use of PPE, the convenience of PPE, the reliable quality of PPE, the use of occupational health protection facilities, and the degrees of satisfaction with the occupational protective functions of medical devices and materials were lower than those in the medical group(P<0.05). iii) The awareness status on the knowledge of the management of tumor-related occupational protection in medical staffs of hospital: The awareness rate of items that the hospital′s prominent warning signs for medical waste places, equipment and materials was 89.9%, and the awareness rate of items that the contents of occupational safety and personal protection in the operating procedures of the workplace can meet the protection needs was only 38.1%. The awareness rates of understood that occupational exposure management department has regular inspections and supervision for related work in various departments, and can easily obtain occupational protection standard procedures and emergency treatment plans, the purchase information of work injury insurance, hospital′s prominent warning signs for places, equipment and materials were setting up in the nursing group were higher than that of the medical group(P<0.05). CONCLUSION: Hospitals should strengthen the related training on occupational protection operation specification and protective measures, improve the protection awareness of medical staffs, and make them actively use medical equipment and consumables with occupational protection functions, so as to reduce occupational exposure.

ABSTRACT

<p><b>OBJECTIVE</b>To summarize the 2491 acupuncture inpatient consultations in Qilu Hospital of Shandong University during 2014, and analyze the application of acupuncture in comprehensive hospital, hoping to provide direction and methods for clinical practice and scientific research of acupuncture.</p><p><b>METHODS</b>The consultation record and hospitalization information during 2014 in Department of Acupuncture-Moxibustion and Tuina, Qilu Hospital of Shandong University, were collected and summarized through Inpatient Information Management System. The distribution of consultation discipline and disease were analyzed.</p><p><b>RESULTS</b>Among all the consultations, 15 second-class disciplines were involved, and the number of neurology and surgery was 1399, accounting for 59.00% of all consultation. 111 types of diseases were found in consultation, mainly postoperative complications, cerebrovascular disease and arthropathy. The primary disease of consultations involved all the systems, mainly nervous system disease, musculoskeletal system disease, connective tissue diseases and genitourinary system diseases, accounting for 73.89% of all consultations. .</p><p><b>CONCLUSION</b>Acupuncture is closely related with clinical disciplines of neurology and surgery. The cooperation between acupuncture and pediatrics and oncology should be strengthened. Acupuncture has an advantage over treating postoperative complications and neuromuscular diseases.</p>

ABSTRACT

Objective To innovate management mode and enhance management efficiency for the clinical medical equipment in the comprehensive hospital. Methods The head nurses in the clinical departments were responsible for establishing archives and management lists for medical devices to realize three-level,bi-directional and interactive management with the assistance from the medical equipment engineer. Results The three-level, bi-directional and interactive management contributed to enhancing medical devices maintenance and safety and the communication between the departments of medical equipment management and clinical medicine. Conclusion The three-level, bi-directional and interactive management improves the efficiency,safety and benefit of medical devices as well as the clinical staffs'satisfaction.

ABSTRACT

Objective To analyze the distribution characteristics and risk factors of extended stay over 30 days at tertiary comprehensive hospitals, and explore methods of shortening average length of stay(LOS).Methods Homepage of all inpatients discharged from October 2014 to October 2015 of 31 tertiary comprehensive hospitals in Shanxi province were collected.39 234 cases of these inpatients stayed over 30 days and their case information was complete.A generalized linear model was used for risk factors analysis.Results With the extension of LOS, most of the inpatients with extended stay were found during their 30th-40th days, accounting for 50.21% of all and constituting the highest daily costs as well.The ratio of inpatients decreases with their LOS, and its distribution presents a positive skew in entirety.Distribution of inpatients over 30 days identified urban employees under medical insurance accounting for the highest ratio among all payment categories,and those from the department of orthopedics for the highest ratio among all departments, while those of diseases from injury and poisoning accounting for the highest ratio among all disease categories.The findings indicated ten important factors for extended stay, namely gender, medical payment category, ICD, inter-department transfer, nosocomial infection, and clinical pathway.Conclusions To strengthen management of specific departments and inpatients, focus on key process of medical quality, and accelerate medical insurance payment reform can effectively control extended stay.

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the current status of acupuncture consultation in Peking Union Medical College Hospital, which could provide references and suggestions for the development of acupuncture at comprehensive hospitals and the treatment of diseases.</p><p><b>METHODS</b>The source of departments, the number of patients, the number of consultation, treatment frequency, consultation reasons and western medicine diagnosis of 279 inpatients who received acupuncture consultation were analyzed.</p><p><b>RESULTS</b>A total of 26 departments, including department of internal medicine, department of surgery, gynecology department, etc. and emergency department, requested for acupuncture consultation, in which the request from neurology department was the highest. The distribution of diseases was quite wide, mainly gastrointestinal dysfunction, cerebrovascular diseases, urinary retention, hiccup and pain-related symptoms.</p><p><b>CONCLUSIONS</b>The indication of acupuncture is extensive, which could supplement the disadvantages of modern medicine and play a positive role in the adjuvant treatment of comprehensive hospital. It is suggested the communication and cooperation with other departments should be strengthened to promote development of acupuncture and improve the understanding and application of acupuncture for western medicine doctors.</p>

ABSTRACT

Objective To better understand the infection status of HIV in the patients seeking medical care in Peking Union Medical College Hospital.Methods The HIV detection data of the patients in the hospital from 2003-2014 were collected for a statistical analysis with software SPSS 19.0.Results A total of 715 421 patients were screened,and 1 012 (0.14%) patients were HIV positive,and HIV infection were confirmed in 776 (0.11%) patients by Western Blot testing.The detection rate of HIV infection increased from 0.05% in 2003 to 0.17% in 2014 (trend x2=66.83,P=0.000),and the increase during 2012-2014 was obvious.Of the 776 newly diagnosed HIV-infected individuals,631 (81.31%) were men and 145 (18.69%) were women.The percentage of the males infected with HIV increased from 50.00% to 90.26% (trend x2=58.41,P=0.000).The median age was 36 years (interquartile range:27-43),and the age group 18-50 years were mostly affected.In the 776 patients infected with HIV,634 (81.70%) were infected through sexual contacts,and the proportion of sexual transmissions increased with year (trend x2=126.38,P=0.000).The proportion of infected men who have sex with men (MSM) increased from 0% in 2003 to 53.90% in 2014 (trend X2=11.96,P=0.001),similar to the trend in western countries.The proportion of infected patients who were not married increased from 18.75% to 42.21% (trend x2=43.74,P=0.000).The top three source departments of HIV/AIDS cases were internal medicine (51.03%),emergency room (18.30%) and dermatology (13.53%).The proportion of the HIV/AIDS patients from department of gynecology and obstetrics declined from 18.75% in 2003 to 2.60% in 2014.No HIV/AIDS patients were detected in department of surgery,department of otorhinolaryngology,department of ophthalmology,department of stomatology and health examination center in 2003,but 14 cases (9.10%),11 cases (7.14%) and 4 cases (2.60%) were detected in these departments respectively in 2014.Conclusion The HIV detection rate increased with year in Peking Union Medical College Hospital,suggesting the necessity of strengthened HIV test in general hospitals.MSM are the population at high risk,to whom more attention should be paid.

ABSTRACT

Objective To explore the effect of bundle hand hygiene intervention in controlling healthcare-associated infec-tion(HAI)in a primary comprehensive hospital,so as to improve hand hygiene compliance and correct rate,and reduce AHI rate. Methods In January-June 2014,bundle hand hygiene intervention among health care workers (HCWs)in a hospital was performed,hand hygiene knowledge awareness rate,hand hygiene compliance and correct rate,hand sanitizer usage and HAI rate before and after intervention were compared. Results After performing intervention for six months,the awareness rate of hand hygiene knowledge(concept,significance,indications,methods,sanitizer use)of HCWs improved com-pared with before intervention (P<0.05);hand hygiene compliance and correct rate were significantly higher than before inter-vention (77.92% vs 49.78% ;76.47% vs 37.72% )(P<0.05). Hand sanitizer usage increased from 2.14mL/bed-day to 4.63 mL/bed-day ,HAI rate decreased from 1.97% to 1.54% (P<0.05).Conclusion Bundle hand hygiene intervention can improve HCWs’knowledge awareness,compliance and execution rate of hand hygiene,and effectively reduce HAI rate.

ABSTRACT

Objective To discuss the nurses'satisfaction degree with work and its influencing factors and analyze the difference of nurses'satisfaction degree with work with different background.Methods 398 nurses in a certain comprehensive hospital were investigated with Mueller/McCloskey(MMSS)satisfactory inventory. Results The nurses'general satisfaction degree with work was less than middle level with a score of (3.12±0.46).The satisfaction degree was statistically different in different dimensionality and nurses with different background. Conclusion The management department of hospitals should adopt positive measures to improve the nurses'satisfaction degree with work and prompt them to supply more high-quality and effective nursing services.

ABSTRACT

Along with the continuous deepening of system reformation of science and technology, the research of science and technology is significantly important. The investment in scientific research is increasingly enlarged by enterprises and national finance. The scientific research ftlnd tends to be increase obviously. Meanwhile. tlle research fund deposition has been showed gradually. which attracts the attentions of the scholars and the government. It is also awkard problem faced by the research manguagers. This paper analyzed the reasons for the occurrence of the research fund deposition in comprehensive hospitals and its disadVOJltages according to the actual condition of hospitals, and proposed some suggestions to prevent the occurfence of the research fund deposition. It could be valuable to further discuss on this issue in future.
