ABSTRACT

Las armas de aire comprimido son utilizadas desde hace muchos años con distintos usos que van desde el tiro recreativo hasta el control de plagas. Las más comunes son aquellas que disparan ''balines" o esferas metálicas que pueden llegar a tener capacidad de penetrar tejidos dependiendo del tipo de carga y de la longitud del cañón. Se presenta un caso clínico de un masculino que fue agredido con un arma tipo pistola con carga de CO2 que recibió un impacto a nivel facial y sufrió laceración en piel de labio y fractura dental complicada.


Air guns have been used for many years for a variety of purposes ranging from recreational shooting to pest control. The most common are those that shoot "pellets" or metallic spheres that can penetrate tissues depending on the type of load and the length of the barrel. A clinical case is presented of a male who was assaulted with a CO2-loaded pistol-type weapon, hit at facial level, and suffered laceration of the lip's skin and complicated dental fracture.

ABSTRACT

The pneumatic colorectal injury caused by high pressure compressed air are rare and can be fatal. Herein, we present a case of 45-year-old male who developed sudden onset of severe abdominal pain after cleaning the dust on his pants with high pressure compressed air gun dust cleaner. Emergent exploratory laparotomy was done which findings are a huge rectal perforation with multiple serosal and subserosal tear in sigmoid to splenic flexure of colon. Anterior resection with left hemicolectomy, and temporary transverse colostomy was performed. Postoperative course was uneventful. Recently, prognosis is generally favorable because of prompt diagnosis and emergent surgical management.

ABSTRACT

ABSTRACT We are reporting a case of colorectal injury caused by a jet of compressed air directed from a distance towards the anus. The patient mentioned that it happened accidentally while his colleague was cleaning his clothes using compressed air. The patient presented with acute abdominal pain and distension. A contrast CT study did not show any free air or leakage. The patient was treated conservatively, progressed well and was discharged from the hospital on the fourth day.


RESUMO Descrevemos um caso de lesão colorretal causada por um jato de ar comprimido direcionado para o ânus, a certa distância. O paciente mencionou que o ocorrido foi acidental, enquanto um colega estava limpando suas roupas com ar comprimido. O paciente se apresentou com dores abdominais agudas e distensão. Um estudo de TC contrastado não demonstrou ar livre, nem vazamento. O paciente foi tratado conservadoramente, teve boa evolução e recebeu alta hospitalar no quarto dia.

ABSTRACT

Objective To design an inflatable bed aerator to replace the traditional inflator pump.Methods The aerator used MCS-51 SCM micro control unit as the core component and compressed air as the air source,which was composed of the modules for main control,power supply,air pressure detection,liquid crystal display,button as well as real-time clock and alarming.Results The aerator gained the same efficacy as the traditional inflator pump,while the noise was decreased to the patient and medical staffs,which behaved particularly well for the bedsore prevention of the critical patient.Conclusion The aerator has simple structure,easy operation and high stability,which can be used for the inflation of two air cushions simultaneously and for timely alarming against failures,and thus is worthy promoting practically.

ABSTRACT

Compressed air can cause serious damage to internal organs. The stomach is an organ that is rarely perforated due to its elasticity. However, intestines are weaker and thinner compared to the stomach. A 40-year-old male came to the emergency room with severe abdominal pain due to dyspnea. The patient experienced abdominal pain right after his coworker shot compressed air into the patient's pants. The patient suffered from a rigid abdomen, and bed-side ultrasonography was carried out as soon as possible. Pneumoperitoneum was diagnosed by portable X-ray. After computed tomography, emergency paracentesis was carried out for decompression. After emergency paracentesis, the patient's symptoms and vital signs were stabilized. After the procedure, the patient had an emergency laparotomy.

ABSTRACT

The use of high-pressure air instruments has become more common. Consequently, there have been a number of cases of orbital emphysema caused by contact with high-pressure air. In this case, a 62-year-old male patient visited an emergency medical center after his left eye was shot by an air compressor gun that was used to wash cars. Lacerations were observed in the upper and lower eyelids of his left eye. Radiological examinations revealed orbital emphysema, optic nerve transection, pneumocephalus, and subcutaneous emphysema in the face, neck, shoulder, and mediastinum. Canalicular injury repair was performed, and the emphysema resolved. However, there was near-complete vision loss in the patient's left eye. Because most optic nerve transections occur after a severe disruption in bone structure, pure optic nerve transections without any injury of the bone structure, as in the present case, is extremely rare.

ABSTRACT

Objective To realize centralized supply of hospital compressed air to reduce repeate investment.Methods Air turned into compressed one after passing through air pressure pump, which undergoes preliminary cooling and drying by refrigerated drier, then the compressed air went into air tank for further cooling and storage, and finally was transmitted into hyperbaric oxygen chamber, ICU and operating room as desired.Results Centralized supply was realized for hospital compressed air with little technical modification and investment.Conclusion The compressed air of hyperbaric oxygen chamber is introduced into ICU and operating room and gains economic and social benefits, and thus is worth popularizing practically.

ABSTRACT

Colon barotrauma can be mostly caused by elevated intraluminal pressure. Air insufflation during colonoscopy procedure is the most common cause of iatrogenic colon barotrauma. Cat scratch colon can usually be seen in the mild type of colon barotrauma, and colon perforation can be seen in the severe type. We presently report a case of non-iatrogenic colon barotrauma caused by industrial compressed air. Multiple linear mucosal ulcers were noted in the recto-sigmoid colon, but the colon was not perforated. The patient was discharged without any further complications after conservative treatments.

ABSTRACT

PURPOSE: The authors experienced a case of orbital emphysema causing eye movement disorder following conjunctival tear without any orbital wall fractures after exposure to compressed air. CASE SUMMARY: A 28-year-old man's left periorbital area was injured while working with compressed air. There was marked lid swelling on the left side with palpable crepitus. The patient had an exotropia and hypertropia of the left eye. In addition, the extraocular movement of the left eye was restricted on upgaze without nausea and vomiting. On slit-lamp examination, conjunctival partial laceration and subconjunctival air bubbles adjacent to the conjunctival laceration could be seen. Orbital computed tomography showed air in the periorbital and retrobulbar region of the left eye and no evidence of orbital fracture. On examination 4 days after the primary repair of the conjunctival laceration, the patient completely recovered without any complication. CONCLUSIONS: Orbital emphysema following a conjunctival laceration caused by compressed air without any orbital wall fractures can cause restriction of eye movement.

ABSTRACT

Rupture of the colon caused by high pressure air is a rare injury. We present a case of this type of accident pointing out the clinical picture, diagnosis and treatment of these accidents, indicating how occur these ruptures, the location by order of frequency and the different surgical procedures.


La ruptura del colon causada por aire comprimido a alta presión es rara. A propósito de un caso se revisan los mecanismos de producción, tipos de roturas, clínica, diagnóstico y tratamiento de estos traumatismos.

ABSTRACT

A seaman died after a jet of compressed air directed to his anus while working with his colleagues in deck of the ship. He suffered from abdominal pain and distension, and died 5 hours later in the ship. Autopsy was performed and revealed rupture of the splenic flexure of the colon and partial seromuscular tearing in the rectosigmoidal region. We concluded that the cause of death was tension pneumoperitoneum by the pneumatic colonic rupture. The need for education of the workers handling compressed air appliances is emphasized.

ABSTRACT

OBJECTIVE: To verify the efficacy and safety of compressed air to produce pneumoperitoneum for laparoscopic surgery in pigs for a training program of residence. METHODS: Dalland pigs weighing 15-17kg underwent general anethesia and mechanical ventilation. They were divided in 3 groups: A - (38) the pneumoperitnoneum was established with an automatic CO2 insufflator, B - (7) as in A except the CO2 gas was changed by compressed air, and C - (11) abdomen insufflation was obtained with compressed air directly from hospital pipe network system. Intra-abdominal pressure in all groups was kept between 12 and 15 mmHg. The laparoscopic procedures performed were distributed proportionally among groups: 20 bilateral nephrectomy, 20 dismembered pyeloplasty and 16 partial nephrectomy. Arterial blood sampling for gasometry was obtained before and 2h after establishment of pneumoperitoneum in 5 pigs of group C. RESULTS: The cost of 25 4,5kg CO2 container used in group A was R$ 3,150.00 (U$ 1,050.00). The mean length time of surgeries in groups A, B and C were respectively: 181±30min, 196±39min e 210±47min (p>0.05). Respiratory alkalosis occurred in 3 out of 5 pigs of group C. No animal exhibited signs of gas embolism or died during surgery. CONCLUSION: The use of compressed air for laparoscopy in pigs was safe, reduced costs and did not require the use of an automatic gas insufflator.


OBJETIVO: Testar a eficácia e segurança do pneumoperitônio com ar comprimido para cirurgias videolaparoscópicas em porcos em treinamento de residência médica. MÉTODOS: Porcos da raça Dalland de peso variável de 15 a 17kg foram submetidos a anestesia geral e respiração controlada. Eles foram divididos em 3 grupos: A - 38 animais com pneumoperitônio feito com insuflador automático de CO2 usando este gás; B - 7 animais sujeitos ao mesmo procedimento exceto que o CO2 foi substituído por ar comprimido; e, C - 11 animais em que o pneumoperitônio foi feito com ar comprimido diretamente da rede hospitalar. Nos 3 grupos a pressão intra-abdominal foi mantida entre 12 e 14mmHg. Os procedimentos realizados foram distribuídos proporcionalmente nos 3 grupos: nefrectomia bilateral - 20, pieloplastia desmembrada - 20 e nefrectomia parcial - 16. Antes e 2h após o pneumoperitônio foi colhido sangue arterial para gasometria em 5 porcos do grupo C. RESULTADOS: Foram consumidos 25 torpedos de 4,5kg de CO2 a um custo total de R$ 3.150,00 no grupo A. A duração média da cirurgia nos grupos A, B e C foram respectivamente: 181±30min, 196±39min e 210±47min (p>0.05). Alcalose respiratória foi observada em 3/5 porcos testados do grupo C. Nenhum animal apresentou sinais de embolia gasosa ou faleceu durante o procedimento. CONCLUSÃO: O uso de ar comprimido para laparoscopias em porcos mostrou-se método seguro com redução de custos e tornou desnecessário o uso de insuflador automático.

There are many standards and recommendations for breathing air quality associated with respiratory protection equipment, but little has been done regarding the possible microbial contamination of medical air. The present study demonstrates quantitatively and qualitatively that pipelines might be incriminated as source of microbial contamination of compressed and synthetic air for medical use. Air samples were drawn into an especially pressure-resistant device and the bacterial and fungi contents were identified after growth on agar plates. The bacterial flora isolated from peripheral air outlets was virtually the same as that found in the central air-generating installations, consisting of a mixture of pathogens and normal skin bacteria. Several factors contributing to microbial contamination of medical air are mentioned and preventive measures are discussed.


Existem vários padrões e recomendações para a qualidade do ar respirável relacionado aos equipamentos de proteção respiratória, mas pouco tem sido feito em relação a uma possível contaminação microbiana do ar medicinal. O presente trabalho demonstra quantitativa e qualitativamente que as linhas de ar estão relacionadas à contaminação microbiológica do ar comprimido e ar sintético para uso medicinal. Amostras de ar foram coletadas por um equipamento especialmente resistente a pressão, e o conteúdo bacteriano e fúngico foi identificado após crescimento em placa. A flora bacteriana isolada tanto dos sistema periféricos de ar foi virtualmente a mesma encontrada nas instalações centralizadas, sendo uma mistura de patógenos e bactérias normais da pele. Vários fatores contribuintes para a contaminação microbiana do ar medicinal e medidas preventivas são discutidas.

ABSTRACT

We report two colorectal trauma patients whose rectosigmoid region was ruptured due to a jet of compressed air directed to their anus while they were playing practical jokes with their colleagues in their place of work. It was difficult to diagnose in one patient due to vague symptoms and signs and due to being stunned by a stroke of the compressed air. Both patients suffered from abdominal pain and distension, tension pneumoperitoneum and mild respiratory alkalosis. One patient was treated with primary two layer closure, and the other with primary two layer closure and sigmoid loop colostomy. Anorectal manometry and transanal ultrasonography checked 4 weeks after surgery, revealed normal anorectal function and anatomy. The postoperative courses were favorable without any wound infection or intraabdominal sepsis.

ABSTRACT

21 male adult goats were divided into the experimental group (11 goats) and control group (10 goats). The pressure-time of hyperbaric pretreatment was 700kPa-3min. The rates of the compressions and decompressions of hyperbaric pretreatments were l00kPa/min. The intervals between hyperbaric pretreatments and simulated divings which were repidly decompressed were 15min. The pressure-time of simulated diving was 600 kPa-60min. The rates of the compressions of the simulated divings were l00kPa/min and those of the decompressions were 60kPa/min. The results indicated that hyperbaric pretreatments might reduce the bubble formation and increased 5-HT concentrations which were caused by rapid decompression after simulated divings, and that other indexes such as morbility and mortality of decompression disease, platelet count of circulation, and bubble sound of blood flow on the precardium of the goats changed coincidently. These suggest that hyperbaric pretreatments could prevent goat's experimental decompression disease in some degree.
