ABSTRACT

OBJECTIVES: Falling in the elderly is considered a major cause of death. In recent years, ambient and wireless sensor platforms have been extensively used in developed countries for the detection of falls in the elderly. However, we believe extra efforts are required to address this issue in developing countries, such as Pakistan, where most deaths due to falls are not even reported. Considering this, in this paper, we propose a fall detection system prototype that s based on the classification on real time shimmer sensor data. METHODS: We first developed a data set, ‘SMotion’ of certain postures that could lead to falls in the elderly by using a body area network of Shimmer sensors and categorized the items in this data set into age and weight groups. We developed a feature selection and classification system using three classifiers, namely, support vector machine (SVM), K-nearest neighbor (KNN), and neural network (NN). Finally, a prototype was fabricated to generate alerts to caregivers, health experts, or emergency services in case of fall. RESULTS: To evaluate the proposed system, SVM, KNN, and NN were used. The results of this study identified KNN as the most accurate classifier with maximum accuracy of 96% for age groups and 93% for weight groups. CONCLUSIONS: In this paper, a classification-based fall detection system is proposed. For this purpose, the SMotion data set was developed and categorized into two groups (age and weight groups). The proposed fall detection system for the elderly is implemented through a body area sensor network using third-generation sensors. The evaluation results demonstrate the reasonable performance of the proposed fall detection prototype system in the tested scenarios.

ABSTRACT

PURPOSE: To describe the change of lacrimal gland volumes in Korean patients with thyroid-associated ophthalmopathy (TAO) via computed tomography (CT). METHODS: A retrospective review of CT images from 217 TAO patients and 135 control subjects was performed. The TAO patients were diagnosed between May 2005 and May 2014 and had a CT performed on initial presentation (330 orbital CT scans). These images were compared with 270 orbital CT scans from the control group, obtained between May 2013 and May 2014. An open source DICOM viewer was used to calculate the volume of the lacrimal gland. RESULTS: The mean volume of the lacrimal gland in TAO patients was 0.816 cm³ in the right orbit (standard deviation [SD], 0.048) and 0.811 cm3 in the left orbit (SD, 0.051), with no significant difference between right and left (p = 0.192). However, significant differences were observed between TAO patients and healthy individuals (p < 0.001). There was no significant difference between mean lacrimal gland volumes of males (0.812 cm³; SD, 0.037) and females (0.816 cm³; SD, 0.029) (p = 0.513). There was a negative correlation between gland volume and age in TAO patients (Pearson r = -0.479, p = 0.00). The subjective tearing (right: r = 0.244, p = 0.018; left: r = 0.226, p = 0.024), corneal superficial punctate keratopathy (right: r = 0.192, p = 0.040; left: r = 0.206, p = 0.036), and exophthalmometry (right: r = 0.182, p = 0.032; left: r = 0.180, p = 0.046) correlated with lacrimal gland volume. CONCLUSIONS: This study is the first to use CT images to calculate the lacrimal gland volume of Korean TAO patients. In TAO patients, the lacrimal gland volume was notably increased compared to control subjects. The lacrimal gland volume decreased with age, but there was no difference between gender and no difference between left and right. The lacrimal gland volume correlated with subjective tearing, corneal superficial punctate keratopathy and exophthalmometry.

ABSTRACT

Objective To evaluate the effectiveness and feasibility of web-based lifestyle Or behavior intervention on Chinese urban adults.Methods Six hundred and eighty-one adult subjects were rand omly assigned to the intervention group (n=341) or the control group (n=340).The intervention group was encouraged to visit a specified interactive web site at 1east once a month to submit self-report health diaries.and provided with individualized health promotion instructions and tailored counseling at the chat room or through email.The control group received routine medical services.The primary outcomes were changes in cigarette smoking,alcohol consumption,fruit or vegetable intake,and physical exercise duration at 6 month from baseline.Intention-to-treat method was used for data analysis.Results At 6 month,smoking cessation rate was 23.6% in intervention group and 4.6% in control group (X2=22.4,P<0.05),and daily alcohol consumption decreased by 28.6 g in intervention group compared to 6.1 g in control group (t=14.9.P<0.05).There Was no significant change in physical exercise duration for those with sub-optimal physical activities between the two groups.In contrast to the control group.mean systolic blood pressure,fasting blood glucose,and body mass index of the hypertensive,diabetic,or overweight/obese subjects in the intervention group showed statistically significant decrease.There was no significant change in serum cholesterol level for the participants with hypercholestemlemia in both groups.The attrition rate was 6.7% in the intervention group.Conclusion Web-based health management services may be helpful in promoting healthy lifestyle and behavior and enhancing the care of chronic conditions including hypertension,diabetes,overweight and obese.

Our multimedia center began its service as a internet Hospital in MIDAS Dongailbo, one of the major daily newspaper in Korea, on March 25th, 1997. The aim of this study is to examine the content and purpose of medical counseling on the web. The subjects consisted of 1,000 counsellings which were called 'Dongailbo Internet Hospital' for March, 25th, 1997 through October 30th, 1997. Many questioners wanted to know treatment principles, to make a diagnosis, and to know specific medical knowledges. The most common questions are as follows: general and unspecified, musculoskeletal, skin, digestive, respiratory, neurological problems.

ABSTRACT

Recently with the development in computer technology and its communication system, many changes have come to the medical field. For example, Picture Archiving and Communication System(PACS) has been used in the medicine experimentally. But this system needs high cost and instrument, so this system is not used in personal generally. Our hospital established the medical forum for health and medical counseling in Computer communication network called HITEL. We analysed the questions about urologic problem especially from Sep, 1994 to Jan, 1997. Most of questions are related to external genitalia and sexually transmitted diseases and most of users were in their 2nd, 3rd and 4th decades and male-predominant. In conclusion, medical counseling using PC communication is an effective method especially in urologic field.

ABSTRACT

Recent progress in medical informatics enable to use various medical regardless of time or place, to make a diagnostic and therapeutic plan, with increasing need about health of general population. There are many homepages on the web, which provide medical common senses, hospital information. and medical counseling on the web. Virtual Hospital, which is comprehensive medical information system on the web, has medical counseling program with various health information such as self-diagnosis, first-aid treatment information, drug information, Q and A, and so on. The aim of this study is to examine the content and purpose of medical counseling on the web. The subjects consisted of 277 counsels which visited 'Medical Counseling' in the Virtual Hospital for December 1996 through May 1997. Many questioners wanted to consult about their symptoms, to know prevention and treatment strategies, and to know specific medical knowledge. The most common questions are as follows: general and unspecified, digestive, musculoskeletal, and skin problems. These findings suggest medical counseling may be used to supplement the lack of direct medical Interviews with doctors.
