ABSTRACT

Eyes play a dominating role in the facial aesthetics. In the past decades, East Asian consider the westernized stereoscopic and double eyelid eyes as beautiful, as a result, double-eyelid blepharoplasty become the most commonly performed aesthetic plastic surgery continuing to gain popularity in East Asian. The authors summarized the important progress and innovative conclusion on upper eyelid anatomy, preoperative evaluation, double-eyelid blepharoplasty for Asian males as well as objective outcome assessment of double eyelid surgery in recent 10 years, which we hope could pave the way for surgeons to better understand, investigate and modify double eyelid surgery along with better serve the patients.

ABSTRACT

Objective To evaluate the clinical efficacy of topical fat redistribution in combination with shortening of upper eyelid levator muscle in patients with excessively wide palpebral fissure after double-eyelid operation.Methods Twenty patients with excessively wide palpebral fissure after undergoing double-eyelid operation were selected.The double-eyelid line was aesthetically re-designed,the skin or incisional scar between the original and new double-eyelid lines was excised,and the scarring adhesion among different tissues was fully isolated.The fat tissues within the orbital septum,deep orbit and anterior orbital septum combined with ROOF reconstruction were performed to block the adhesion tissues,shortening of levator muscle of the upper eyelid was equally conducted to enhance the cosmetic effect.Results Seventeen patients were satisfied with the cosmetic appearance following once double-eyelid operation.Two cases underwent twice operations due to different height between bilateral papebral fissures.One patient was unsatisfied with the cosmetic appearance.Conclusions Fat redistribution in combination with shortening of levator muscle of the upper eyelid can not only repair excessively wide palpebral fissure complicated with ptosis and upper eyelid depression,but also enhance the exposure rate of the cornea and increase the cosmetic appearance,which is consistent with contem porary aesthetic perception.

ABSTRACT

Objective@#Doubled blepharoplasty, which has a various types, is one of the commonest plastic surgery procedures. Incision method has a long-standing result but linear scars can be seen when eyes are closed. So it is necessary to improve this surgerical procedure. The aim of this report is to propose a novel palpebral margin incision technique which avoids scars in the upper eyelid.@*Methods@#15 patients undertook blepharoplasty with palpebral margin incision technique. All the patients in this study were healthy with no other serious diseases. The double-eyelid crease was formed after the operation.The aesthetic results were evaluated based on their satisfaction.@*Results@#All of the patients were satisfied with aesthetic results, and no significant complications occurred. And No obvious regression of the double fold took place.@*Conclusions@#This new method provides an effective way to achieve eyelid reconstruction without obvious scar in the upper eyelid which is of very important clinical significance and makes cosmetic surgery process take a step forward.

ABSTRACT

Objective@#In order to explore the method to correct double eyelids line scars and to present its therapeutic schedule, curative effects, pathogenesis and precautions.@*Methods@#Surgical incisions were designed based on preoperative communication, original scars location and skin elasticity. Scar contracture and synechia was relaxed thoroughly. The pretarsal orbicularis oculi muscle of the lower incision margin was fixed to the pretarsal fascia on the new designer location of double eyelid line to firm shape. Complete pretarsal layer tissue were joined layer by layer with interrupted suture. Finally, the skin was sutured closely by locking stitch technique.@*Results@#From November 2014 to September 2016, 583 eyes with unsatisfied double eyelid scars were treated. Patients were followed up for 3 months to 1 year with sustained aesthetic. There is no visible man-made crease in 549 eyes under eye-closed state. Less than 1 mm wide scars with smooth surface were showed on 19 eyelids. Mild depressed or more than 1 mm wide scars still remained in 15 eyes with improved appearance.@*Conclusions@#Unsatisfied appearance caused by double eyelids line scars must be paid highly attention in clinical setting. The appearance can be effectively improved by proper surgical procedures and appropriate suture and fixation.

ABSTRACT

OBJECTIVE: To investigate the effectiveness of combined three operations (rotated total upper eyelid skin flap, construction of double eyelid, and "Z" flap epicanthal plasty) for one stage defect repair after resection of xanthelasma palpebrarum with epicanthus. METHODS: Between December 2013 and December 2015, 12 female patients with large xanthelasma palpebrarum and epicanthus underwent rotated total upper eyelid skin flap, construction of double eyelid, and "Z" flap epicanthal plasty for one stage defect repair. The age ranged from 36 to 59 years (mean, 43 years). The course of disease was 3 to 16 years, with an average of 11 years. The initial resection was performed in 6 cases, second resection of residual xanthelasma palpebrarum in 4 cases, and 2 cases had recurrence after resection. The maximum diameter of xanthelasma palpebrarum was 0.5-1.3 cm (mean, 1.0 cm). According to CHE Junmin et al criterion, epicanthus was rated as mild in 7 cases, moderate in 3 cases, and severe in 2 cases. The blood lipid level was in normal range. RESULTS: Primary healing of incision was obtained, and the flaps survived in all patients; no complication occurred. Scar hyperplasia was found in 4 cases at 1 month after operation, and the comprehensive treatment of scar was performed. All patients were followed up for 3 months to 2 years, with an average of 1.5 years. Double eyelid effects were good, and no xanthelasma palpebrarum recurred. CONCLUSIONS: A combination of rotated total upper eyelid skin flap, construction of double eyelid, and "Z" flap epicanthal plasty is an effective operative procedure to repair defect after resection of xanthelasma palpebrarum with epicanthus; and better curve of double eyelid, better shape of endocanthion, and less tension of flap can be got.
