ABSTRACT

Objective To explore the effect of continuity education by WeChat public information platform on health education to advanced lung cancer patients undergoing treatment by implanting 125 1 radioactive particles guided by CT after discharge.Methods During August 2014 and August 2015,60 patients with advanced lung cancer admitted into the department of oncology of 3 military hospitals in Fujian Province were equally divided into control group and observation group by a simple random sampling method:the control group was given conventional discharge education and the observation group was treated continuity care by way of follow-up WeChat public information platform for more than 4 times for each patient.The two groups were compared in terms of physical function,satisfaction and rate of complications 6 months after discharge.Results The physical function of the observation group was significantly improved as compared to that the control group (P < 0.05).The satisfaction level was significantly improved as well (P <0.05).The rate of complications in experient group was less statistically significant (P < 0.05).Conclusion Continuity nursing with Watchat can prevent the occurrence of complications,relieve the pains of patients and improve patients' medical compliance as well as the quality of life.

ABSTRACT

Objective Understand the hospital surgical treatment of female infertility patients after discharge health needs,to provide evidence for continuity of the corresponding nursing measures.Methods After surgical treatment of hospital preparation of hospital,100 cases of infertility patients designed questionnaire,to understand the patient's health requirements.Results About 94% of patients had continuity care needs,The need for continuity care content in the top three:observation,the knowledge of the disease,mental health counseling.Continuity of care means demand is in the top three:We Chat public information push,add patients group of exchange of experience and telephone follow-up on a regular basis.Conclusion the hospitalized patients with infertility of the continuity of care after discharge and demand is high,enhancing health education for patients after discharge,the continuity of the individualized modern nursing measures,improve the patients discharged from hospital after conception rate and quality of life.

ABSTRACT

Objective To explore the influence of continuity care model on how to deal with medical waste of insulin injections among domestic patients Methods Consecutive diabetes patients using the home insulin injections were recruited as the the members of our Diabetes Education Classfrom September 2014 to March 2015. The home medical waste disposal was brought into the content of diabetes education;problems were intervened through the continuity care model. Using the questionnaire survey to investigate the cognitive and disposal of patients on medical waste, to compare the difference of results of questionnaire before and after the intervention. Results The cognitive rate of medical waste after intervention was 97.70%(212/217),while the cognitive rate before intervention was 3.69%(8/217), the difference was significant (χ2=383.63,P<0.01). Disposal of diabetes patients using the home insulin injections had improved after the continuity care, the rate of dealing with medical waste according to specification after the intervention was 91.24% (198/217),while the rate before the intervention was 3.23%(7/217), the difference was significant (χ2=337.26,P<0.01). Conclusions The continuity care model standardized the behavior of patients for disposal of medical waste, reduced the environment pollution of medical waste and decreased the spread of disease.

ABSTRACT

Objective To understand the continuous care needs and related services will of the women in puerperium, and analyze the relative factors of the continuity care of those discharged from maternity hospital.Methods A total of 207 delivery women who were discharged from a hospital in Chenghua District Maternal & Child Care Center in Chengdu city from September to December, 2014 were investigated with self-made questionnaires.Results As for newborn babies' continuity care service, what those delivery women care most were newborn babies' common disease prevention and care, and as for their own continuity care service, what they most need were maternal uterine instauration care.Enforcers for continuity of care services were more inclined to obstetricians and pediatricians, and the service forms tended to make telephone calls or do follow-up visits.Time frequency of the follow-up visits was inclined to 7 days once.Delivery women's educational degree (r=-0.216, P<0.01), place of residence (r=0.208, P<0.01), whether they would participate in pregnant Women school (r=0.192, P<0.01), pregnant women school number (r=-0.382, P<0.01),the newborn's tire(r=0.173, P<0.05) and maternal type(r=0.161, P<0.05) were factors that influenced the degree of maternal continued care needs.Conclusions The main factors which influence the continuity care in obstetrics are educational degrees, place of residence and the types of delivery women.As a result, different methods of continuity care service should be established to aim at different groups of people so as to promote the service quality in obstetrics in general to meet the needs of delivery women at all levels.

ABSTRACT

ObjectiveTo probe into the clinical application effect of continuity care model in stroke patients. Methods58 patients with stroke in our hospital during June 2008 to June 2011 were selected.According to block randomization method,the selected patients were divided into the study group and the control group with 29 patients in each group.The control group received routine care interventions,while patients in the study group was given routine care interventions plus continuity care model.Activities of daily living,compliance behavior,quality of life,nursing satisfaction degree and other related indicators of patients in two groups were comparatively analyzed. ResultsCompared with the control group,the activities of daily living of the study group at discharge did not show significant differences,while 1 month,3 months,6 months after discharge,the activities of daily living was significantly increased.Regular medication,exercise time,timely return visit,proper diet and other indicators of compliance behaviors were significantly improved.The quality of life score did not show significant differences at discharge,while 3 months after discharge,the quality of life score were significantly higher in the study group than that of the control group.The satisfaction degree of patients was significantly higher in the study group than that of the control group. ConclusionsApplication of continuity care model in stroke patients can improve activities of daily living,compliance behavior,quality of life,satisfaction degree with nurses.It's worthy of further clinical application and promotion.

ABSTRACT

BACKGROUND: The rapid increase in population of old people has been emphasized as a serious social problem even in medical world. Geriatric disease has special characters that can be mentioned as a non-contagious chronic degenerative disease. 'Regular Doctor System' can be considered as one of the efficient ways to care for it continuously. The purpose of this study was to promote the introduction of family doctor system gradually and care of geriatric disease through examination of old people's thinking about 'Regular Doctor System'. METHODS: The result of this study was based on the questionnaire survey on 292 old people(aged 65 and more, response rate 73%) at 2 welfare centers in Seoul, located at Yeongdeungpo-gu and Guro-gu, and 1 college for old people, for 2 weeks from May, 2003. The contents of surveywere consisted of many questions for understanding of old people's thinking about 'Regular Doctor System'. The response method was self-reported. RESULTS: Comparing with other age group surveys, this old people's survey has special character that it has higher affirmative answer rate for the question of 'Do you have a regular doctor?'. The old people's favorite natures of regular doctor were considerateness( 26.9%) and kindness(26.2%). And the most desirable service from their regular doctor was comprehensive health examination( 45.2%). The factors reflecting on selecting of doctor were separately questioned into two aspects, as factors related to doctor's behavior and the others. Among the factors related to doctor's behavior, the effect of medical treatment(22.7%) was top ranked and 'being aware of my disease'(17.3%), 'easy explanation'(15.5%) were followed. The other factors were medical specialty(24,1%), easy access(19.3%) and short waiting time(10.5%). The reasons for changing of doctor were 'to receive specialist's medical treatment'( 37.2%), 'no improvement of patient's condition'(13.1%) and 'distrust of diagnosis'(9.0%). CONCLUSION: The old people wanted considerate and kind attitude from their doctors. The factors, related to doctor's behavior that the old people wanted, were 'good effect of medical treatment', 'being aware of my disease well' and 'easy explanation'. The other factors they wanted were medical specialty, easy accessibility and short waiting time.

ABSTRACT

BACKGROUND: Family doctor registration program was proposed several years ago and is still in discussion. The success of this program depends on attitude and acceptability of people. This study was done to describe the attitudes of the people to the family doctor registration program. METHODS: We interviewed 657 adults who lived in Seoul, Bundang, llsan and Pyungchon with a preformed questionnaire. The questionnaire was pretested by five family doctors and trainee. RESULTS: About half of the respondents were favorable for family doctor registration program. The most importantly considered benefits of the program were continuity of care, telephone consultation and management of chronic disease. The most seriously considered drawbacks of the program were only single doctor available cost, and restriction of medical services. Over half of the respondents preferred internists as their treating doctor and family physician, pediatrician followed. About half of the respondents opposed on the fact that the family doctor was restricted to clinic-based practice. Half of the respondents intended to Participate in the program and the rate of intention to participate in the family doctor registration program was related to their household income and favorable attitude to wards the program and marginally related to the number in a household, haring a regular doctor, their interest in health. The demographie variables did not influence intention to participate in the program. CONCLUSIONS: Half of the respondents agreed to participate in the family doctor registration program, but ethers were negative to the principles of the program and cost sharing.
