ABSTRACT

Objective: To observe the efficacy of contralateral needling combined with Maitland ankle mobilization in the treatment of chronic ankle sprain. Methods: A total of 106 patients with chronic ankle sprain were included in the study. They were numbered according to their order of visit, with odd numbers assigned to the control group and even numbers assigned to the observation group, with 53 cases in each group. Patients in the control group were treated with Maitland ankle mobilization, while those in the observation group were given additional contralateral needling treatment. After treatment, we observed the indicators including, ankle circumference, range of motion (ROM) of dorsiflexion, ROM of plantar flexion, thickness of ankle ligaments, and the changes in Baird-Jackson ankle score, American Orthopaedic Foot & Ankle Society (AOFAS) ankle-hindfoot score, and visual analog scale (VAS) score to compare the efficacy between the two groups. Results: After treatment, the total effective rate of the observation group was 94.3% and was significantly higher than that of the control group (81.1%), indicating statistical significance (P<0.05). After treatment, the ankle circumference of both groups decreased, and the ROM of dorsiflexion and the ROM of plantar flexion increased (P<0.05). The changes in the observation group were more significant, indicating inter-group statistical significance (P<0.05). After treatment, the thickness of the anterior talofibular ligament, calcaneofibular ligament, and posterior talofibular ligament, as well as the VAS scores decreased in both groups; the changes in the observation group were more significant, indicating inter-group statistical significance (P<0.05). After treatment, the Baird-Jackson ankle score and the AOFAS ankle- hindfoot score increased in both groups, and the scores in the observation group were higher than those in the control group, indicating inter-group statistical significance (P<0.05). Conclusion: The efficacy of contralateral needling combined with Maitland ankle mobilization in the treatment of chronic ankle sprain is superior to that of Maitland ankle mobilization alone.

ABSTRACT

OBJECTIVE: To observe the effect contralateral needling of in improving pain, edema and limb dysfunction in stroke patients with shoulder-hand syndrome. METHODS: A total of 62 patients with post-stroke shoulder-hand syndrome were divided into a control group and an observation group, 31 cases in each one. The routine treatment with internal medicine and rehabilitation manipulation was adopted in the two groups. Additionally, the routine acupuncture treatment was used in the control group and the contralateral needling techniques was applied in the observation group. The needles were retained for 30 min. The treatment was given once a day for 5 days a week and consecutively for 4 weeks. Before and after treatment, the score of the modified Fugl-Meyer assessment scale (FMA scale), the score of the visual analogue scale (VAS), the score of the hand edema rating and the score of the modified Barthel index (ADL score) were evaluated. RESULTS: The total effective rate was 90.32% (28/31) in the observation group and was 67.74% (21/31) in the control group. The effective rate in the observation group was higher than that in the control group (P<0.05). VAS score, the score of the hand edema rating, FMA score and ADL score were obviously improved as compared with those before treatment in each group and the scores in the observation group were better than those of the control group (P<0.05). CONCLUSION: The contralateral needling therapy of acupuncture is effective for relieving pain and edema as well as improving the motor function of the affected limb in the patients with post-stroke shoulder-hand syndrome.

ABSTRACT

OBJECTIVE@#To observe the clinical effect on post-stroke oculomotor nerve palsy treated with the interaction of twelve meridian muscle regions and contralateral needling therapy.@*METHODS@#A total of 46 patients with post-stroke oculomotor nerve palsy were randomized into an observation group and a control group, 23 cases in each one. In the control group, the intramusclar injection of mecobalamine at the buttock region was given, 1 mL each time, once every two days, 3 times weekly. Besides, citicoline sodium capsules were prescribed for oral administration, 0.2 g each time, 3 times daily. In the observation group, on the base of the treatment as the control group, the interaction of twelve meridian muscle regions and contralateral needling therapy was supplemented. Acupoints on the health sides included Juliao (ST 3) and Hanyan (GB 4), acupoints on the affected side included Jingming (BL 1), Sibai (ST 2), Yangbai (GB 14), Cuanzhu (BL 2), Shangming (Extra), Sizhukong (TE 23), Tongziliao (GB 1) and bilateral Fengchi (GB 20), Quchi (LI 11), Pianli (LI 6), Waiguan (TE 5), Hegu (LI 4) were selected. The needles were retained for 30 min in each acupuncture treatment, once a day, 5 times weekly. The treatment for 4 weeks was required in the two groups. Before and after treatment, the score of cervical range of motion (CROM), pupil size, eye fissure width and eyeball mobility were observed in the patients of the two groups. The clinical effect was evaluated in the two groups.@*RESULTS@#After treatment, CROM scores and pupil size were reduced in the patients of the two groups (<0.05), and the values in the observation group were lower than the control group (<0.05). The eye fissure width and eyeball mobility were increased in the two groups (<0.05), the eye fissure width and the mobility of the muscles of rectus internus, inferior rectus and inferior oblique in the observation group were larger than the control group (<0.05). The effective rate was 82.6% (19/23) in the observation group, higher than 65.2% (15/23) in the control group (<0.05).@*CONCLUSION@#The interaction of twelve meridian muscle regions and contrallateral needling therapy effectively relieves diplopia, pupil dilation, narrow eye fissure and limited eyeball mobility in the patients with post-stroke oculomotor nerve palsy.

ABSTRACT

Objective To compare the clinical efficacy between contralateral needling and homolateral (the affected side) needling in treating acute-stage peripheral facial paralysis by using systematic evaluation. Method By computer and manual retrieval, literatures about clinical trials on contralateral needling in treating acute-stage peripheral facial paralysis published before March of 2017 were collected from China National Knowledge Infrastructure (CNKI), WanFang database, Vip database, China Biology Medicine disc (CBMdisc), Pubmed, Web of Science, EMbase and The Cochrane Library. By adopting the Cochrane systematic evaluation, the collected data underwent meta-analysis by using RevMen 5.3 software. Result A total of 276 articles were retrieved and 11 articles were finally recruited, including 761 patients with acute-stage facial paralysis, 394 cases treated by contralateral needling and 367 by homolateral needling. The meta-analysis showed that contralateral needling produced more significant effective rate and recovery rate than homolateral needling [effective rate: RR=1.06, 95%CI(1.01,1.11), Z=2.32, P=0.02; recovery rate:RR=1.53, 95%CI(1.31,1.78), Z=5.42, P<0.01]. Moreover, contralateral needling produced a better effect in improving facial nerve function, showing an advantage in ameliorating facial movement [HB: WMD=0.26, 95%CI(0.04,0.48), Z=2.34, P=0.02; FDIP: WMD=3.77, 95%CI(1.53,6.01), Z=3.30, P<0.01]. Conclusion Contralateral needling is worth applying in treating acute-stage peripheral facial paralysis. However, due to the small amount and unsatisfactoryquality of the recruited literatures in this systematic evaluation, multiple-centered randomized trials with high quality and large sample size are expected for further verification.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the differences in the clinical therapeutic effects on mild and moderate carpal tunnel syndrome between the combined therapy of regular acupuncture and contralateral needling technique at distal acupoints and the regular acupuncture.</p><p><b>METHODS</b>Sixty patients of mild and moderate carpal tunnel syndrome were divided into an observation group and a control group according to random number table, 30 cases in each one. Laogong (PC 8), Daling (PC 7), Neiguan (PC 6), Yuji (LU 10) and Hegu (LI 4) were selected on the affected side in the two groups. In the observation group, the reducing needling technique with lifting and thrusting was applied at first to Taichong (LR 3) and Zhongfeng (LR 4) on the opposite side and afterwards those on the affected side were stimulated. In the control group, acupuncture was given at the acupoints on the affected side. The even-needling technique was used at the acupoints on the affected side in the patients of the two groups. Acupuncture was given once every day, 30 min each time. The treatment for 10 days was as one session. The therapeutic effects were evaluated after 3 sessions. The changes in median nerve electrophysiology, Levine's carpal tunnel syndrome questionnaire, the patient-rated wrist evaluation (PRWE) and the evaluation of therapeutic effects were observed before and after treatment in the two groups.</p><p><b>RESULTS</b>The total effective rate was 90.0% (27/30) in the observation group, better than 70.0% (21/30) in the control group (<0.05). After treatment, the nerve sensory conduction velocity and the amplitude of median nerve were all increased in the two groups, indicating the significant differences (all<0.05). The increase in the observation group was more apparent than that in the control group (all<0.05). The symptom severity score (SSS) and function status score (FSS) were all reduced apparently after treatment in the two groups (both<0.05). SSS and FSS in the observation group were lower apparently than those in the control group after treatment (both<0.05). PRWE score after treatment was reducedapparently as compared with that before treatment in the two groups (both<0.05). The result in the observation group was lower than that in the control group after treatment (<0.05).</p><p><b>CONCLUSIONS</b>The combined treatment of regular acupuncture technique and the contralateral needling technique at distal acupoints achieves the definite therapeutic effects on mild and moderate carpal tunnel syndrome, apparently superior to the regular needling technique.</p>

ABSTRACT

Objective To observe the clinical efficacy of acupuncture plus contralateral needling in treating post-stoke myodystonia. Method Totally 120 patients with post-stroke myodystonia were randomized into group A, B, C and D, 30 in each group. Group A was intervened by yin-reducing and yang-tonifying needling method plus contralateral needling; group B was by ordinary acupuncture plus contralateral needling, group C was by yin-reducing and yang-tonifying needling alone, while group D by ordinary acupuncture. Clinical Spasticity Index (CSI) was observed before and after intervention. Result After treatment, the CSI scores (tendon reflex, muscle tension, episodic spasm scores and total score) were significantly changed in the four groups (P<0.01, P<0.05). The tendon reflex score, muscle tension score and total score in group A were significantly different from that in the other three groups after intervention (P<0.01,P<0.05). There was a significant difference in comparing the episodic spasm score between group A and D after intervention (P<0.01). There were significant differences in comparing the tendon reflex score, muscle tension score and total score between group B and D after intervention (P<0.05). The total score in group C was markedly different from that in group D after intervention (P<0.05). Conclusion Yin-reducing and yang-tonifying needling method plus contralateral needling is an effective method in treating post-stroke myodystonia.
