ABSTRACT

De forma a preservar o ímpeto evolutivo e o alcance da psicanálise, a tarefa da análise de controle é prevenir o estabelecimento da inércia que busca o menor denominador comum e a degeneração de ideias e práticas em evolução na direção de banalizações e preconceitos. A uma instituição psicanalítica é necessário manter padrões mínimos e simultaneamente preservar-se como ambiente vibrante e dinâmico para o contínuo desenvolvimento e regeneração da psicanálise enquanto estrutura viva e simbólica. Através de uma análise de controle, cada analista adquire um estilo analítico único, que não se refere a preferências e idiossincrasias egoicas, mas a uma singular e nova articulação subjetiva dos elementos preexistentes na estrutura tradicional. Sem a realização de uma análise pessoal, a análise da contratransferência e da identificação projetiva em supervisão pode operar como resistência à análise pessoal, essa última o mais importante fator para a autoautorização do analista dentro da organização psicanalítica. O analista não é autorizado pela supervisão ou pelo supervisor, mas pela experiência analítica pessoal com o inconsciente, o sintoma e seus efeitos pós-analíticos sob a forma do que Lacan nomeia de sinthoma. Lacan diz que, ao término da análise, o analisando ou futuro analista não se identifica com o analista, mas sim com seu sinthoma.


The task of control analysis is to preserve the evolutionary impetus and edge of psychoanalysis and prevent the inertia towards the lowest common denominator, and the degeneration of evolved ideas and practices into common assumptions and prejudices. A psychoanalytic organization needs to maintain standards while also preserving the organization as a vibrant and dynamic site for the continuing development and re-invention of psychoanalysis as a living symbolic structure. Through a control analysis every analyst acquires a unique analytical style that does not refer to ego preferences and idiosyncrasies, but to a singular and new subjective articulation of the pre-existing elements of a traditional structure. Without a personal analysis, the analysis of the countertransference and of projective identification in supervision can function as a resistance to the personal analysis which is the most important factor for the self-authorization of the analyst within a psychoanalytic organization. The analyst is not authorized by supervision or the supervisor, but by the personal analytical experience with the unconscious, the symptom, and its post-analytical effects in the form of what Lacan calls the sinthome. Lacan said that at the end of analysis, the analysand or the future analyst does not identify with the analyst, but rather with his/her sinthome.


Con el fin de preservar el ímpetu evolutivo y el alcance del psicoanálisis, la tarea del análisis de control es evitar el establecimiento de inercias que buscan el mínimo común denominador y la degeneración de ideas y prácticas en la evolución en la dirección de trivializaciones y prejuicios. Para una institución psicoanalítica es necesario mantener estándares mínimos y simultáneamente preservarse como un entorno vibrante y dinámico para el desarrollo continuo y la regeneración del psicoanálisis como una estructura viva y simbólica. A través de un análisis de control, cada analista adquiere un estilo analítico único que no se refiere a preferencias del ego e idiosincrasias, sino a una singular y nueva articulación subjetiva de los elementos preexistentes en la estructura tradicional. Sin realizar un análisis personal, el análisis de la contratransferencia y la identificación proyectiva en la supervisión puede operar como resistencia al análisis personal, este último el factor más importante para la auto autorización del analista dentro de la organización psicoanalítica. El analista no está autorizado por la supervisión o el supervisor, sino por la experiencia analítica personal con el inconsciente, el síntoma y sus efectos posanalíticos en forma de lo que Lacan llama sinthoma. Lacan dijo que al final del análisis, el analista o analista futuro no se identifica con el analista, sino con su sinthoma.


De façon à préserver l'élan évolutif et l'étendue de la psychanalyse, la tâche de l'analyse de contrôle est celle de prévenir l'établissement de l'inertie qui cherche le plus petit dénominateur commun et la dégénération d'idées et de pratiques en évolution dans la direction des banalisations et des préjugés. Il faut qu'une institution psychanalytique maintienne des étalons minimaux et simultanément se préserver en tant qu'une ambiance vibrante et dynamique pour le développement continu et régénération de la psychanalyse considérée comme une structure vive et symbolique. Par l'intermédiaire d'une analyse de contrôle, chaque analyste acquiert un style analytique unique qui ne se réfère pas à des préférences et des idiosyncrasies égoïques, mais à une nouvelle articulation singulière et subjective des éléments préexistant dans la structure traditionnelle. Sans mener une analyse personnelle, l'analyse du contre-transfert et de l'identification projective en supervision peuvent opérer en tant que résistance à l'analyse personnelle, cette dernière le plus important élément pour l'auto autorisation de l'analyste chez l'organisation psychanalytique. L'analyste n'est pas autorisé par la supervision ou par le superviseur, mais par l'expérience analytique personnelle du surmoi, du symptôme et de ses effets postanalytiques, sous la forme de ce qui Lacan nomme sinthome. Lacan dit que, à la fin de l'analyse, l'analysant ou le futur analyste ne s'identifie pas à l'analyste, mais à son sinthome.

Um marco essencial na formação psicanalítica é a supervisão de formação. A autora apresenta como e através do que podem se desenvolver a competência e a identidade, ou pode haver um efeito inibidor. Considera a relação emocional entre supervisor e supervisionando durante a supervisão de formação, permeada pelas próprias experiências de aprendizagem e didáticas, carregadas com material inconsciente e ancoradas no âmbito semipúblico do Instituto. Retoma a história da supervisão de formação, descreve a controvérsia quanto a teach or treat e mostra as teorias psicanalíticas mais utilizadas na supervisão. Constata que também nos contextos mais "racionais" se atualiza uma constelação e uma dinâmica relacional emocional, parcialmente inconscientes, entre o supervisionando e o supervisor.


Training supervision is an essential event in psychoanalytic training. This study reveals how and from what competence and identity can be developed, and also if there may be an inhibiting effect. At the same time, it is always about an emotional relationship between supervisor and supervised during the training supervision process, permeated with their own learning and didatics experiences, which are constituted of unconscious material and anchored in the semi-public scope of the Institute. In this paper, the history of training supervision is recapitulated, and its conflict related to the ''teach-or-treat'' controversy is approached. Moreover, a viewpoint on the most used psychoanalytic theories in the supervision process is introduced. Consequently, even in more 'rational' contexts, a constellation, as well as a partially unconscious, emotional relational dynamics between the supervised and the supervisor are always updated.


Un hito esencial en la formación psicoanalítica es la supervisión de la formación. Se presenta aquí cómo y a través de qué competencia e identidad puede desarrollarse, o puede surgir un efecto inhibitorio. Al mismo tiempo, es siempre una relación emocional entre supervisor y supervisionando durante la supervisión de la formación, influenciada por el propio aprendizaje y experiencias didácticas, cargada de material inconsciente y anclada en el ámbito parcialmente público del Instituto. En este artículo se retoma la historia de la supervisión del entrenamiento, se describe su conflicto con la controversia "teach or treat" y se presenta una visión de las teorías psicoanalíticas más utilizadas en la supervisión. De ello se deduce que también en los contextos más "racionales" siempre hay una constelación y una dinámica relacional emocional, parcialmente inconsciente, entre el supervisor y el supervisor.


Une borne essentielle dans la formation psychanalytique, c'est la supervision de formation. On présente ici comment et par quel moyen la compétence et l'identité peuvent se développer, ou il peut y avoir un effet inhibiteur. En même temps, il s'agit toujours d'une relation émotionnelle entre superviseur et supervisé pendant la supervision de formation, transpercée par ses propres expériences d'apprentissage et didactiques, chargées de matériel inconscient et ancrées dans le cadre semi-public de l'Institut. Dans cet article on reprend l'histoire de la supervision de formation, on décrit son conflit par rapport à la controverse "teach or treat" et on présente un panorama des théories psychanalytiques les plus employées en supervision. Le résultat, c'est que dans les contextes les plus « rationnels ¼ on actualise toujours une constellation et une dynamique relationnelle émotionnelle, partiellement inconsciente, entre le superviseur et le supervisé.

ABSTRACT

Background: Substantial improvement can be brought about in the hospital inventory and drug expenditure by inventory control techniques. These include ABC (always, better and control), VED (vital, essential and desirable) and ABC-VED matrix analysis. The objective of the study was to categorize the drugs based on cost and criticality and identify those which require stringent managerial control.Methods: ABC analysis according to drug expenditure, VED analysis according to criticality of the drugs,  ABC-VED matrix analysis to classify drugs into category I, II and III was done for drug store of a tertiary care teaching hospital of North India for the period of April, 2018 to March, 2019.Results: The total number of the drug items used by the drug store was 315. The total drug expenditure was Rs. 9,61,29,859. By ABC analysis, it was found that 15.24%, 22.54% and 62.22% items belonged to A, B and C category respectively, accounting for 70%, 20% and 10% of Annual Drug Expenditure (ADE). VED analysis showed that 31.11%, 60.32% and 8.57% were V, E, and D category items respectively, accounting for 30.44%, 57.12% and 12.44% of ADE. By ABC-VED matrix analysis, 42.86%, 52.38% and 4.76% items were category I, II and III items respectively, accounting for 78.91%, 20.15% and 0.94% of ADE.Conclusions: The study depicted the items belonging to category I which requires top managerial control, also the items belonging to categories II and III which require control by middle and lower managerial level respectively.

ABSTRACT

Background: An efficient inventory control system would help optimize the use of resources and eventually help improve patient care. Objectives: The study aimed to find out the surgical consumables using always, better, and control (ABC) and vital, essential, and desirable (VED) technique as well as calculating the lead time of specific category A and vital surgical consumables. Methods: This was a descriptive, record-based study conducted from January to March 2016 in the surgical stores of the All India Institute of Medical Sciences, New Delhi. The study comprised all the surgical consumables which were procured during the financial year 2014–2015. Stores ledger containing details of the consumption of the items, supply orders, and procurement files of the items were studied for performing ABC analysis and calculating the lead time. A list of surgical consumables was distributed to the doctors, nursing staff, technical staff, and hospital stores personnel to categorize them into VED categories after explaining them the basis for the classification. Results: ABC analysis revealed that 35 items (14%), 52 items (21%), and 171 items (69%) were categorized into A (70% annual consumption value [ACV]), B (20% ACV), and C (10% ACV) category, respectively. In the current study, vital items comprised the majority of the items, i.e., 73% of the total items and essential (E) category of items comprised 26% of all the items. The average internal, external, and total lead time was 17 days (range 3–30 days), 25 days (range 5–38) and 44 days (range 18–98 days), respectively. Conclusions: Hospitals stores need to implement inventory management techniques to reduce the number of stock-outs and internal lead time.

ABSTRACT

Partindo do trabalho como supervisores de estágio acadêmico, desenvolvido em uma clínica-escola do curso de graduação em psicologia, levantam-se questões pertinentes à condução de processos clínicos, fundados na perspectiva psicanalítica, nesses ambientes. É confrontado o lugar do saber e do sujeito na universidade e na psicanálise como elementos que fundam uma ética que é própria a esta - questionando, com isso, alguns efeitos aí produzidos sobre a inserção do aluno no campo psicanalítico e a possibilidade de sua transmissão pelo trabalho de supervisão. Privilegiando os efeitos suscitados pela imposição de um prazo para o encerramento dos atendimentos clínicos desenvolvidos no estágio, o posicionamento freudiano frente a esse dispositivo é tomado como paradigma da posição do analista frente aos impasses que lhe são apresentados no exercício de seu trabalho.


Supported by their practice as supervisors of Psychology graduating students, the authors bring up some questions related to psychoanalytical clinic at Universities. Two elements which fund psychoanalytical ethic are confronted: the place of knowledge and the place of subject at University and at Psychoanalysis. From that, it is questioned the insertion of the students in psychoanalytic field as well as the possibilities of its transmission be made though out an experience of "control - analysis". By focusing on the effects produced by the imposition of a deadline on clinical treatment, the authors took Freud's clinical directions as a paradigm of analyst's position in order to face some clinical difficulties.

ABSTRACT

OBJECTIVES: The aim of this study was to investigate the association between Korean ADHD patients and the l/s polymorphism of serotonin transporter(5-HTTLPR). METHODS: The study sample consisted of 189 Korean ADHD children diagnosed by Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version-Korean Version(K-SADS-PL), both parents of ADHD children, and 150 normal children. DNA were extracted from the blood of all samples, and genotyping was done. Based on the allele and genotype information, not only the case-control analysis between ADHD and normal children but also the family-based association test among ADHD children and their parents. Transmission disequilibrium test(TDT) were performed for family-based associated test(number of trio=113). The results of the clinical rating and neuropsychological tests were compared according to the l/s genotype of ADHD children. RESULTS: In case-control analysis, there were no statistically significant difference of l/s gene polymorphism between ADHD and normal children in various kinds of analysis condition. In family-based association study, TDT failed to detect linkage disequilibrium between l/s gene polymorphism and ADHD in whole ADHD families. However, in the families of ADHD inattentive type only(number of trio=23), l allele was transmitted more preferentially in the proband with ADHD even if the number of families was small(chi-square=4.57, p=.032). In the analysis of the results from the clinical scales and neuropsychological tests in ADHD children, the score of the Novelty- Seeking of ADHD children with l/l genotype was significantly lower than with the other genotypes(F=3.15, p=.047), and that of Self Transcendence was significantly higher(F=4.25, p=.017). CONCLUSION: The results of this study suggest there were no significant genetic association between the 5- HTTLPR gene polymorphism and Korean ADHD.
